Abstract

Di Indonesia, hampir 80% pasien skizofrenia mengalami relaps berulangkali. Dari hasil penelitian di tiga Rumah Sakit menunjukkan sekitar 70% pasien dengan skizoprenia yang dirawat inap merupakan pasien yang pernah dirawat sebelumnya atau merupakan pasien rawat ulang. Dari perspektif ekonomi, kekambuhan meningkatkan beban bagi pengeluaran negara. Disability Adjusted Life Years (DALYs), gangguan neuropsikiatri adalah penyebab utama ketiga dari tahun-tahun kehidupan cacat di Eropa  mencapai 15,2%. Penelitian ini bertujuan untuk mengembangkan dan melakukan uji aplikasi telenursing Me-Co Care. Penelitian dilakukan pada 23 respondent mewakili setiap domain aplikasi, diwilayah Kelurahan Cempaka Putih Timur, Jakarta Pusat selama tiga minggu penggunaan. Evaluasi efektifitas Aplikasi Me-Co Care dilakukan menggunakan kuesioner penggunaan aplikasi yang dikembangkan oleh tim peneliti. Terdapat tiga aplikasi yaitu aplikasi keluarga, pasien, kader dan perawat,. Setiap aplikasi terdiri atas menu-menu yang dapat di jalankan melalui handphone. Hasil penggunaan aplikasi selama tiga minggu menunjukkan 89%  Setuju dan 11% Tidak Setuju dalam impelmentasi dan penerapan aplicasi Me-Co Care. Tiga aplikasi yang telah dikembangkan mampu mengakomodir kebutuhan dimasyarakat dan sebagian besar pasien, keluarga, kader dan perawat mampu mengoperasikan aplikasi Me-Co Care dengan baik.

Abstract

The number of electronic health applications is increasing, including those that can be used for mental health. Previous studies showed that, use of several applications provides great benefits for mental health and greatly increases accessibility to mental health services. The objectives of this study were to assess needs and expectations of patients with mental disorder and a broad group of stakeholders with the expected end product being a well-functioning user-centered technology-based tool for mental disorders. The research was carried out by involving 80 patients with schizophrenia and 74 family’s caregivers in health services and local offices of health services in five cities of Indonesian capital city, Jakarta. This study was conducted through self-administered questionnaires and interviews where possible from December 2017 to July 2018. A descriptive statistic using proportion was used to present the data. The results indicated that patients needed information relevant to disease, patient care, check-up schedules and consultation with health workers. In addition, patients also wanted to get information on job vacancies due to many of them were unemployed and rehabilitation programs carried out by local health services. This study highlighted the needs to develop a mobile-based application for continuity of mental health services in the community.

Abstract

The effect of prone position for increasing oxygenation in patients with COVID-19: A systematic reviewBackground: Prone position is known to reduce mortality in intubated non-COVID-19 patients with moderate to severe acute respiratory distress (ARDS).Purpose: To determine the effect of prone position for increasing oxygenation in patients with COVID-19 by a systematic reviewMethod: Criteria of patients with COVID-19  and pneumonia but not with severe respiratory distress (ARDS) increasing oxygen saturation, preventing the use of HFNC breathing apparatus, and preventing the need for intubation. Search systematically from 5 database s medical (PubMed, Proquest, Embase, Medline, Cinhal ) and 1 manual (Google) performed until 12 October 2020, from 5 databases and 1 manual (Google) found 5 articles that met the inclusion criteria, and 122 patients were included for the final analysis. Results: The severity of disease and the need for intubation decreased by 65,6% in patients.  The prone position to increase oxygenation, prevent disease severity, intubation / ICU, and mechanical ventilation to see the effect of the prone position on increasing oxygen saturation.Conclusion: Prone position indicates increased oxygenation of patients with respiratory disease related to COVID-19. The severity of the disease and the need for intubation decreased by 65.5% in the patients. For this reason, the need for the prone position in COVID-19 patients with pneumonia before falling into ARDS, the provision of the prone position was done randomlyKeywords: Prone Position; Patient; COVID-19; OxygenationPendahuluan: Prone posisi diketahui dapat menurunkan angka kematian pada pasien non-COVID-19 yang diintubasi dengan gangguan pernapasan akut sedang sampai berat (ARDS). Namun literature yang meneliti tentang efek posisi prone pada pasien COVID-19 masih kurang.Tujuan: Untuk mengetahui pengaruh posisi tengkurap terhadap peningkatan oksigenasi pada pasien COVID-19 dengan tinjauan sistematisMetode : Kriteria pasien COVID-19 dengan pneumonia tetapi tidak yang mengalami gangguan pernafasan berat (ARDS) sehingga meningkatkan satursi oksigenasi, mencegah penggunaan alat bantu napas HFNC, dan mencegah kebutuhan intubasi. Pencarian sistematis dari 5 databases medis (PubMed, Proquest, Embase, Medline, Cinhal) dan 1 manual (Geogle) dilakukan hingga 12 oktober 2020, dari 5 databases dan 1 manual (Geogle) ditemukan 5 artikel yang memenuhi kriteria inklusi, dan 122 pasien dilibatkan untuk analisis akhir.Hasil: Sebagian besar pasien berusia diatas 50 tahun dengan dominasi jenis kelamin laki-laki (67%), peningkatan saturasi oksigenasi 81/122 (66,4%), respiratori rate < 30 x/menit (60%), tidak diintubasi 78/119 (65,5%), dan penurunan jumlah kematian 77/85 (90,6%) dari penelitian. Komplikasi yang berhubungan dengan pemberian posisi prone yang dilaporkan dalam study literature 9% discomfort, 4% worsening oxygenation, 2% coughing,  5 deaths (9%) [1], 42% backpain, 17% tolerated <1 h, 17% required intubation within 72 h, Tetapi menurut study literature Damarla (2020) dan Schifino G, at al (2020) tidak ditemukan komplikasi intubasi dan kematian. Pemberian prone posisi untuk peningkatan oksigenasi, mencegah derajat keparahan penyakit, tindakan intubasi/ICU dan ventilasi mekanik untuk melihat efek prone posisi terhadap peningkatan saturasi oksigenSimpulan: posisi prone menunjukkan peningkatan oksigenasi pasien penyakit pernapasan terkait COVID-19. Derajat keparahan penyakit dan kebutuhan intubasi menurun 65,5% pada pasien. Untuk itu perlunya posisi prone pada pasien COVI-19 dengan pneumonia sebelum jatuh ke acute respiratory distress (ARDS), pemberian posisi prone ini dilakukan secara randomisasi.

Abstract

Background: Posttraumatic stress disorder (PTSD) occurs as triggered or exposure to traumatic events. Psychotherapies have been proven to be effective and superior for people with PTSD. Several psychotherapies have been developed with different approaches. Yet the application of psychotherapy is rarely found and provided by professionals to those who need it in Indonesia. Purpose: This study aimed to figure out the application of PTSD psychotherapies for people with PTSD based on studies conducted in Indonesia. Methods: The review was reported following the PRISMA statement for scoping reviews. A systematic screening was performed in CINAHL, Cochrane library, Embase, Portal Garuda, PubMed, Scopus, as well as manual searches without language and date restrictions. The quality of the study was determined based on the risk of bias. Cochrane risk of bias 2.0 (RoB 2.0) and MINORS were used to evaluate the risk of bias of RCT and quasi-experimental studies, respectively. Results: Five RCTs and four quasi-experimental studies published from 2008 to 2022 (n=465) were employed in this review. Three types of therapies were conducted for people with PTSD; five CBT studies, two EMDR studies, and two SHAT studies, performed in one day up to six weeks, one to 15 sessions, and 30 to 60 minutes for each session. All studies evaluated PTSD as the primary outcome, while the most measured secondary outcomes were depression and anxiety symptoms. Conclusions: CBT was the most frequent therapy for Indonesian people with PTSD, followed by EMDR and SHAT. The application of therapies varied in frequency, duration, length of therapy, and component. Further research on the implementation of various types of psychotherapy for people with PTSD will be required.

Abstract

Background: One death can cause nine bereavements. Considering the impact of human loss, studies on grief are crucial for better bereavement care services.   Purpose: This study examined studies on grief conducted in Indonesia. Indonesian grief research mapping will identify study gaps and suggest further research.   Methods: The reporting for a scoping review adhered to the PRISMA (preferred reporting elements for systematic review and meta-analysis) statement for a scoping review. A systematic screening was performed in MedLine, Embase, CINAHL, Web of Science, Scopus, and PubMed, as well as manual searches via Google Scholar, Portal Garuda, and relevant studies without language and date restrictions. The data retrieved in the literature were analyzed descriptively, and the themes were developed using NVivo software. Results: Based on the 18 included studies, the types of grief studies in Indonesia can be divided into pre- and after-loss grief areas. Regarding the themes of the studies that have been conducted in Indonesia, five themes can be derived, related to (1) study purposes, (2) cause of grief, (3) population, (4) concept being studied, and (5) outcomes being studied. Conclusion: There is significant room for growth in the grief research field in Indonesia. It will be vital for researchers in Indonesia to perform additional studies on pre-loss grief, other studies connected to interventional trials, and further studies examining variation in the outcomes.

Abstract

Aims:  The study aimed to determine the effect of bridging exercise on muscle strength and body balance in stroke patients at national brain center hospital Prof. DR. Mahar Mardjono East Jakarta. Methods: This study used experimental design aimed to test the application of bridging exercise to body balance and muscle strength in stroke patients. This study was conducted in one group with pre and post-test design. This study was conducted at National Brain Center hospital Prof. DR. Mahar Mardjono East Jakarta. The bridging exercise was conducted at 2-weeks, with light tp moderate intensity, three times a week, with 30 to 45 minutes every session. Results: The result of the statistical test with the Paired Samples t-Test P value resulted in 0.001. These result mean that bridging exercise has a significant effect on muscle strength in stroke patients with a mean difference of -0.63 (CI: -0.91-0.34) and a significantly improved the balance of patients with a mean difference of 17 (CI 12.1-21.9) and a high magnitude of effect (1.69). Conclusion: These result mean that bridging exercise has a significant effect on muscle strength in stroke patients.

Abstract

Aims:  This Evidance Based Nursing Practice (EBNP) is to increase language production in patients with non-fluent aphasia post stroke. Methods: This EBNP method uses pre and post to see an increase in language production by assessing token test scores. By calculating the number of samples G-Power version 3.1.9.7 as many as 7 respondents. Data processing using Jamovi. Results: The analysis obtained using the t-test showed that the token test score experienced a significant change before and after the administration of intonation melody therapy (pValue <0.001), while the outcome of an increase in language production showed a significant change after administration of intonation melody therapy (pValue 0.896) with an average mean change mean 11.4 (95% CI: 6.28 -16.6) and moderate effect size (Cohen's d: 0.205). Conclusion: provision of melodic intonation therapy is proven effective in increasing language production in patients with post stroke non fluent aphasia.

Abstract

Aims: Stroke is the second leading cause of death and disability globally, characterized by sudden neurological deficits. Pressure ulcers, caused by prolonged bed rest, can be managed non-pharmacologically through immobilization and routine tilting to prevent decubitus. To compare the effectiveness of the lateral position with an inclination of 30 and 90 degrees on the risk of decubitus in stroke patients, in this study the control group was tilted 90 degrees and the intervention group was tilted 30 degrees. Method: The sampling method is non-probability sampling with purposive sampling, a sample of 22 patients. Results: Data analysis using Reapeted Measurement Anova resulted in a value in the experimental group resulting in a sum of square value of 24.9, f value 14.3, P < 0.001, while in the control group a sum of square value of 76.5, f value 26.8, P < 0.001 was obtained, so it can be concluded that the lateral position with an inclination of 90 degrees and 30 degrees has the same effect, namely providing a significant influence on reducing the risk of decubitus. Conclusions: The study reveals that stroke patients aged 56-65 have a higher risk of pressure ulcers, with low-weight patients at higher risk. Early lateral position intervention and nutrition can significantly reduce the risk of decubitus, highlighting the importance of early wound healing.

Abstract

This his study aims to overcome anxiety in CHD patients who will undergo percutaneous coronary intervention (PCI). The method used was random sampling with a sample of 12 CHD patients who were going to undergo PCI. This sample size was obtained from calculations using G-Power version 3.1.9.4. The research results showed that the characteristics of the participants in the EBNP were an average age of 48.4 years (12.3%), male 9 (75.0%), Muslim 7 (58.4%), high school education 5 (41.7%), married status 11 (91.7%), BPJS health insurance for retired military personnel 5 (41.7). The results of the T-Test analysis or comparison test of state anxiety inventory and trait anxiety inventory before and after being given music therapy showed a P value of 0.000. In conclusion, music therapy has a significant effect on reducing anxiety in patients with coronary heart disease who will undergo PCI or percutaneous coronary intervention.
