Abstract

Leukemia menduduki urutan sepuluh besar penyakit kanker terbanyak yang umumnya diderita oleh anak-anak. Pengobatan yang paling sering digunakan pada leukemia adalah kemoterapi. Kemoterapi pada leukemia dibagi menjadi beberapa fase, fase induksi termasuk salah satu fase kemoterapi paling singkat dan intensif. Agen kemoterapi yang digunakan dalam fase induksi umumnya memiliki potensi efek samping berupa gangguan fungsi hati. Penelitian ini bertujuan untuk menganalisis hubungan pemberian kemoterapi terhadap fungsi hati pada pasien anak dengan leukemia yang menjalani fase induksi di RSUP Haji Adam Malik Medan pada tahun 2021−2023. Penelitian ini merupakan penelitian analitik perbandingan dengan desain cross-sectional. Sampel penelitian ini adalah pasien anak dengan leukemia di RSUP Haji Adam Malik, Medan, tahun 2021−2023 sesuai dengan kriteria inklusi dan eksklusi. Pengambilan data menggunakan rekam medis dengan uji statistik Wilcoxon. Dari 100 pasien anak penderita leukemia, nilai mean dan standar deviasi kadar enzim aspartate aminotransferase (AST) sebelum dan sesudah pemberian kemoterapi fase induksi adalah 30,21±23,235 dan 37,11±25,223; serta kadar enzim alanine transaminase (ALT) memiliki rerata sebesar 48,37±62,824 dan 61,48±64,301. Hasil uji Wilcoxon antara pemberian kemoterapi fase induksi dengan parameter fungsi hati AST dan ALT menunjukkan nilai p<0,05. Terdapat hubungan yang bermakna antara pemberian kemoterapi fase induksi dengan nilai parameter fungsi hati AST dan ALT pada pasien anak dengan leukemia di RSUP Haji Adam Malik, Medan, tahun 2021−2023.

Abstract

Background: High morbidity and mortality rates in pediatric patients indicate the necessity of a Pediatric Early Warning Score (PEWS) capable of early clinical deterioration identification and providing an opportunity for healthcare professionals to promptly manage patients. This study aimed to serve as foundational data and as an evaluative tool to enhance the proficiency of healthcare workers in assessing patient deterioration. Methods: This study utilized a descriptive method with a cross-sectional design and consecutive sampling technique who were treated from January to December 2022.   Results: there were 119 samples, the majority of gender was male (54.6%), the majority of the age group was 1-4 years old (37.8%), and the majority of the disease group was infection (31.1%). Both male and female majority had green PEWS (54.6%) and (45.4%), the majority of patients with an age range of 1 month to 5 years had red PEWS, the majority of infectious disease group had green PEWS, and the majority of patients with green PEWS had discharge outcome.   Conclusion: This study provides an overview of characteristics in pediatric patients and shows the majority of pediatric emergency department patients come with non-emergency conditions.

Abstract

Background: Pressure injury is a frequently occurring health issue in healthcare environments, especially within hospitals. The risk is increased in hospital settings where patients are frequently confined to beds or wheelchairs. Objectives: This study evaluated the prevalence of pressure injury in regular inpatients at H. Adam Malik Hospital, Medan, from 2020 to 2022. Methods: This study employs a descriptive design with a cross-sectional research approach, describing all variables using secondary data from patient medical records. Results: The research findings indicate that the most common age group is 41-60  years old (28,4%), with females comprising the majority (52,9%). The primary diagnoses related to the occurrence of pressure injury is neurological disorders (25.5%). The most frequent location of injury is the buttocks region (45.1%), and the most prevalent stage of injury is stage III (36.3%), with the most common management approach being debridement (43.6%). Conclusion: In H. Adam Malik Hospital, Medan, from 2020 to 2022, pressure injury was mostly found at ages 41-60 years old, in women, the most prevalent underlying primary disease was neurological disorders, on the buttocks region, at stage III, and the most common management approach being debridement. Latar Belakang: Ulkus Dekubitus merupakan masalah kesehatan yang sering terjadi di lingkungan perawatan kesehatan, terutama di rumah sakit. Risiko ini meningkat di rumah sakit yang pasiennya sering terbaring di tempat tidur atau kursi roda. Tujuan: Mengetahui gambaran Ulkus Dekubitus pasien rawat inap reguler di RSUP H. Adam Malik Medan pada tahun 2020 - 2022. Metode: Penelitian ini merupakan studi deskriptif dengan rancangan penelitian cross-sectional yang seluruh variabel dilihat menggunakan data sekunder berupa rekam medis pasien. Hasil: Penelitian ini menunjukkan bahwa kelompok usia yang paling banyak adalah usia 41-60 tahun (28,4%), dengan mayoritas adalah perempuan (52,9%). Diagnosis utama yang berhubungan dengan terjadinya Ulkus Dekubitus adalah gangguan neurologis (25,5%). Lokasi ulkus yang paling sering adalah daerah bokong (45,1%), dan derajat ulkus yang paling banyak terjadi adalah stadium III (36,3%), dengan penatalaksanaan yang paling banyak dilakukan adalah debridemen (43,6%). Kesimpulan: Di RSUP H. Adam Malik Medan pada tahun 2020 hingga 2022, Ulkus Dekubitus umumnya dijumpai pada usia 41-60 tahun, wanita, diagnosis utama gangguan neurologis, lokasi bokong, derajat III, dan tata laksana debridemen.

Abstract

Mosquitoes are the primary vectors for disease transmission in tropical regions such as Indonesia, which has high humidity levels that support their reproduction. Household environmental conditions play a crucial role in determining the presence and density of mosquito larvae as disease vectors. The Medan Denai subdistrict is a densely populated area experiencing an increase in Dengue Hemorrhagic Fever (DHF) cases. This study analyzes the relationship between household environmental conditions and mosquito larvae density. A survey was conducted on 300 homes divided into three groups. Environmental condition data were collected through direct observation, while the presence of larvae was analyzed using the House Index (HI), Container Index (CI), and Breteau Index (BI) to calculate the Density Figure (DF). Results showed that 24% of homes tested positive for larvae, primarily in dispenser containers. A total of 61% of homes had unclean environmental conditions. The highest DF of 5.3 was found in the 1–100 home group, which falls under the high-risk category for disease transmission. Chi-Square analysis revealed a significant association between environmental conditions and larval density (p = 0.045). These findings underscore the importance of improving environmental conditions as a strategy for mosquito vector control.
