Abstract

Infant hospitalization in the neonatal intensive care unit adversely affects both infants and their parents. Efforts can be developed to minimize the negative impact by applying family-centered care. The first step is to identify the needs of parents. Therefore, this literature review aims to identify the needs of parents with critically ill infants in the neonatal intensive care unit. Articles were collected through EBSCOhost, PubMed, Science Direct, and Scopus electronic database using the keywords critically ill infants, family-centred care, needs of parents, and neonatal intensive care unit were searched for qualitative and quantitative studies. The articles reviewed were only articles with full text, written in English, and published from 2015 to 2024. It found 925 articles on infant care in the neonatal intensive care unit, only 10 articles explicitly related to the needs of parents. Needs of parents with critically ill infants, including the needs for (1) positive acceptance and caring attitude of nurses towards infant and parents; (2) accurate information, effective communication, and involvement of parents in decision-making regarding the condition of the infant; (3) feel confident and believe that infant gets the best care; (4) have adequate contact with an infant; (5) involved in caring for the infant with the guidance and supervision of nurses; and (6) comfortable neonatal intensive care unit environment. According to the six needs, each parent has a different priority. Identifying the needs of parents in the neonatal intensive care unit can allow nurses to integrate these needs into family-centered care so that parents can meet these needs, get satisfaction, and improve the quality of life of infants

Abstract

Abstract

Background: Ventilator-associated pneumonia (VAP) remains a leading cause of morbidity and mortality among patients in intensive care units (ICUs). Nurses play a central role in preventing VAP through consistent implementation of evidence-based prevention bundles. Although VAP bundles are widely recommended, suboptimal adherence among nurses continues to contribute to high VAP incidence rates, highlighting the need for strategies that improve compliance. Objective: This scoping review aimed to identify and synthesize interventions designed to enhance nurse compliance with VAP bundle implementation in ICU settings. Methods: A scoping review was conducted using the Population–Concept–Context (PCC) framework, focusing on nurses, interventions to improve VAP bundle compliance, and ICU settings. Electronic databases—including PubMed, Cochrane Library, ProQuest, EBSCOhost, ScienceDirect, ClinicalKey, SAGE Journals, and Google Scholar—were searched for full-text articles published in English between 2016 and 2021. Study selection followed the PRISMA framework. Data were extracted, analyzed, and synthesized narratively. Results: From 945 identified records, eight studies met the inclusion criteria. The reviewed literature identified educational and training programs, experiential learning strategies, interprofessional collaboration, and real-time compliance monitoring dashboards as effective interventions to improve nurse adherence to VAP bundles. Conclusion: Multifaceted interventions, particularly continuous education supported by supervision and monitoring, are effective in enhancing nurse compliance with VAP bundle implementation. Sustained organizational support is essential to improve adherence and reduce VAP incidence in ICU settings.

Abstract

Care for children in the Pediatric Intensive Care Unit (PICU) is a stressful experience not only for the child but also for parents. The complex PICU environment, the use of advanced medical equipment, and the child’s critical condition can affect parents’ emotional, psychological, and spiritual well-being. Therefore, a deep understanding of parents’ experiences during their child’s care in the PICU is essential as a foundation for implementing family-centered care (FCC). During PICU hospitalization, parents strive to balance anxiety and hope, with clear and empathetic communication from healthcare professionals identified as a primary support need. This study aimed to explore parents’ experiences during their child’s care in the PICU. A qualitative descriptive design was employed. The participants consisted of five parents (three mothers and two fathers) whose children were treated in the PICU. Data were collected through in-depth interviews and analyzed using thematic analysis. The analysis identified six main themes: strong emotional reactions when the child was first admitted to the PICU; parents’ feelings of role loss during the child’s hospitalization; nurses as a source of safety and reassurance; hope and spirituality as coping strategies; parents’ desire to be more involved in their child’s care; and unmet parental needs during PICU care, particularly related to information, emotional support, and orientation to PICU routines. Spirituality emerged as an important parental need within the Indonesian cultural context and played a significant role in parents’ coping processes. In conclusion, parents’ experiences during their child’s care in the PICU are complex and shaped by interactions with healthcare providers, fulfillment of informational needs, involvement in care, and emotional and spiritual support. These findings highlight the importance of implementing holistic and culturally sensitive family-centered care in PICU nursing practice.
