Abstract

ABSTRACT Citronella plants (Cymbopogon nardus L) contained active compounds such as citrononeal, citronellol, geraniol. Citronella has been known to have antibacterial, antioxidant and aromatherapy properties. Solid soap is used as a means to cleanse the body and skin from bacteria and protected against exposure to free radicals that have a negative effect on skin health. Solid soap has the benefit of cleaning and brightening the skin, was easy to use and has a relatively affordable price. The research aimed to make solid soap innovations using citronella ethanol extract ingredients. The extraction method used is maceration using 70% ethanol. The preparation of citronella extract solid soap consists of 3 formulas namely FI (2.5%), FII (5%), FIII (7.5%). Physical properties tests carried out on solid soap preparations are organoleptic test, pH test, irritation test, homogeneity test, foam height test, and favourability test. Data analysis in this study was descriptive. Data were obtained from the average observation results of the physical evaluation test, specifically organoleptic test, pH test, irritation test, homogeneity test, foam height test, and favourability test, test in accordance with the requirements of the SNI standards that have been set. The results showed that FI (2.5%), FII (5%) showed that they met the requirements of solid soap preparations such as organoleptic test, homogeneity test, pH test, high foam test, irritation test and favourability test. The difference in concentration in the solid soap formula of citronella ethanol extract had an effect on the physical characteristics of the product. The best formulation of some concentrations of citronella (C. nardus L) ethanol extract solid soap is in formula I with an extract concentration of 2.5%.

Abstract

Prevalensi hipertensi di Indonesia tahun 2018 berdasarkan hasil pengukuran pada penduduk usia 18 tahun sebesar 34,1%. Studi sebelumnya oleh Hussain menunjukkan bahwa hanya 9% orang dewasa dengan hipertensi yang mencapai tekanan darah terkontrol di Indonesia. Pengobatan hipertensi di Indonesia bergantung pada pelayanan kesehatan primer. Namun untuk mencapai tekanan darah terkontrol masih menjadi tantangan di Indonesia. Penelitian ini bertujuan untuk mengetahui gambaran penggunaan obat antihipertensi yang sering digunakan sebagai di Puskesmas Kotagede II.  Jenis penelitian merupakan penelitian deskriptif studi survei yang bersifat retrospektif dengan rancangan cross sectional.  Sampel penelitian adalah data rekam medis pasien hipertensi di Puskesmas Kotagede II ada bulan November sampai Desember tahun 2020 yang berjumlah 184.  Teknik sampling secara Total Sampling. Data diolah secara persentase dan disajikan dalam bentuk tabel.  Berdasarkan penelitian diperoleh karakteristik pasien hipertensi di Puskesmas Kotagede II yang dominan berdasarkan jenis kelamin adalah perempuan sebesar 114 (62%), berdasarkan umur adalah 61-70 tahun sebesar 70 (38,0%). Penggunaan obat antihipertensi terapi tunggal lebih banyak digunakan daripada terapi kombinasi yaitu sebesar  90,22%. Terapi tunggal yang paling banyak diresepkan adalah amlodipine 5 mg sebanyak 81 kali peresepan, terapi kombinasi yang paling banyak diresepkan adalah amlodipine 10 mg dan hidroklortiazid yaitu 12 kali peresepan. Jenis obat antihipertensi yang paling sering digunakan adalah amlodipine 5 mg diresepkan  sebanyak 82 kali (40,59%). Golongan obat antihipertensi yang banyak diresepkan adalah CCB (Calcium Channel Blocker) sebesar 159 kali (70,71%) meliputi amlodipin 5 mg, amlodipin 10 mg dan nifedipin. Frekuensi cara penggunaan obat antihipertensi yang paling sering adalah 1 x 1 (satu kali sehari satu minum obat).

Abstract

Hypertension and diabetes mellitus are the non-communicable diseases with the highest prevalence in the Special Region of Yogyakarta. The use of herbal medicines (obat bahan alam, OBA) as complementary therapy has grown among chronic disease patients, including in Program Pengelolaan Penyakit Kronis (PROLANIS) participants, yet OBA profiles and their potential drug interactions remain poorly characterized in this population. This study described OBA use profiles among hypertensive and diabetic PROLANIS participants at primary health centers in Yogyakarta City. A descriptive cross-sectional study was conducted at five primary health centers (Puskesmas) in Yogyakarta City from January to May 2025. A total of 160 respondents (hypertension n=115, diabetes mellitus n=45) were enrolled through consecutive sampling, and data were collected via structured face-to-face interviews and analyzed descriptively. Respondents were predominantly female (70.0%), aged 65 years or older (48.8%), and housewives (52.5%). In the hypertension group, the most commonly used OBAs were kunyit asam (28.7%), beras kencur (20.9%), and ginger (20.0%); in the diabetes mellitus group, kunyit asam (26.7%), turmeric (22.2%), and jahe (20.0%) predominated. Most OBAs were taken as liquid preparations (80.0%), sourced from tukang jamu (45.0%) or prepared at home (44.4%). Friends and relatives were the primary information source (68.1%), while only 3.1% of respondents received information from healthcare providers. Cross-tabulation of the 47 respondents currently using OBA revealed amlodipine combined with kunyit (turmeric) as the most frequent drug-herb combination, with pharmacokinetic interaction potential via CYP3A4 inhibition. Perceived therapeutic benefit was reported by 75.6% of respondents, and 77.5% intended to continue using OBA. OBA use is widespread among PROLANIS participants, and combinations carrying pharmacokinetic interaction risk, notably kunyit with amlodipine or angiotensin receptor blockers, and hypoglycemic OBAs with sulfonylureas and insulin, warrant systematic monitoring. Pharmacist-led OBA counselling at primary health facilities should be strengthened.
