Abstract

Background: acute Coronary Syndrome patients often return to the hospital due to relapses that occur after they are declared cured. Secondary prevention programs are an important part for ACS patients to treat and stop the disease process and prevent them from complications and disability. Nurses have an important role in carrying out secondary prevention, especially for ACS patients to train patients to live a healthy lifestyle.Purpose: the purpose of this review study was to map and explore the implementation of secondary prevention in acute coronary syndrome patients initiated or coordinated by nurses.Methods: this study used scoping review design by including all full-text primary studies written in English and published in the last 10 years from 7 sources including EBSCO-hosted Academic Search Complete, PubMed, ScienceDirect, Sage Journals, Taylor and Francis, ProQuest, and Google Scholar. All study results were extracted manually using the tabulation method and analyzed thematically.Results: nurses can support secondary prevention through LDL reduction and lifestyle modifications. LDL reduction involves lipid-lowering medications, the NAILED-ACS approach, and Nursing Coordinated Care (NCC). Lifestyle modifications include promoting healthy habits, ensuring medication adherence, counseling for smoking cessation, and implementing programs for weight loss and physical activity.Conclusion: secondary prevention initiated or coordinated by nurses in the form of LDL reduction intervention and lifestyle modification program have been shown to be effective in reducing the risk of recurrent ischemia in patients with acute coronary syndrome.

Abstract

Latar Belakang: Tindakan histerektomi dapat menimbulkan nyeri karena adanya perlukaan yang dapat menyebabkan ketidaknyamanan. Oleh karena itu, diperlukan upaya untuk mengurangi nyeri tersebut, salah satunya adalah effleurage massage. Tujuan: Mengetahui efektivitas effleurage massage dalam mengurangi nyeri post histerektomi atas indikasi kista endometrium bilateral dan mioma uteri. Metodologi: Metode yang digunakan adalah case report dengan pendekatan asuhan keperawatan berdasarkan penemuan kasus saat pelaksanaan profesi ners stase maternitas di Ruang Marjan Bawah RSUD Guntur Garut, tanggal 07–10 Desember 2023. Intervensi effleurage massage diberikan selama 5 menit dalam 3 hari dengan gerakan sirkuler menggunakan permukaan satu tangan di abdomen. Nyeri dievaluasi menggunakan Numeric Rating Scale (NRS) dengan skala 0 (tidak nyeri) sampai 10 (nyeri berat). Deskripsi kasus: Perempuan 40 tahun didiagnosa kista endometrium bilateral dan mioma uteri sehingga dilakukan histerektomi. Setelah histerektomi, pasien mengeluh nyeri di area perut dan tidak menjalar ke area lain, terasa ‘nyut-nyutan’ serta terasa seperti disayat. Skala nyeri 8 dari 10 (nyeri berat) dan terasa terus-menerus. Hasil: Terdapat penurunan skala nyeri dari 8 (nyeri berat) menjadi skala 1 (nyeri ringan). Pembahasan: Penurunan intensitas nyeri setelah effleurage massage terjadi akibat stimulus nyeri yang diblokade oleh adanya stimulus sentuhan dalam perjalanannya menuju cortex cerebri sehingga terjadi perubahan persepsi nyeri. Kesimpulan: Effleurage massage mampu meringankan nyeri post histerektomi akibat perjalanan stimulus sentuhan yang lebih cepat ke otak dibandingkan stimulus nyeri sehingga terjadi perubahan persepsi nyeri menjadi perasaan nyaman. Pada penelitian selanjutnya, effleurage massage dapat dilakukan pada pasien dengan kondisi sama dan karakteristik berbeda untuk membandingkan keefektifannya dalam mengurangi nyeri post histerektomi.
