Abstract

Leprosy has not been eliminated in six provinces in Indonesia, one of which is Papua. Mappi Regency, Papua, contributed one thousand cases of leprosy in the last five years and 100 active cases in 2020. The high leprosy prevalence in Mappi Regency is still a mystery of the main cause. It is necessary to explore the causes of the high prevalence of leprosy in the region. The study aimed to explore socio-cultural factors and the role of health workers on the prevalence of leprosy in Mappi Regency, Papua, Indonesia. The study method was Qualitative ethnonursing by purposive sampling. This research was conducted in Mappi Regency. Data saturation was obtained from fourteen informants: four leprosy patients, four leprosy patients' families, three health workers, and three stakeholders in Primary Health Center Kepi, Kota 2, Senggo, and Health Office Mappi. Data retrieval and processing with Leininger's Ethnonursing analysis was assisted by Atlas.ti application. The study result showed there were seven emerging themes, including social and cultural: leprosy is considered like a common disease, changes in economic situation, affordability and geographical conditions, leprosy care by patients, families, and communities, suboptimal practices of health workers, limited role of government, budget, and logistics, and expectations for handling leprosy cases. The study concluded that social and cultural factors and the role of health workers contribute to the prevalence of leprosy in Mappi District, Papua, Indonesia. Strengthening the role of families, communities, health workers, and government policies is needed to support the recovery of leprosy patients and reduce the prevalence of leprosy.

Abstract

Introduction: Malaria vaccines show significant potential for disease control but face challenges, including declining efficacy over time and resistance to drugs and insecticides. Despite the initial success of vaccines like RTS,S/AS01, an integrated approach is still needed to address malaria’s complex transmission dynamics across endemic regions. This review examines the current status, challenges, and future prospects of malaria vaccine development, focusing on the impact of combining pre-erythrocytic vaccines and transmission-blocking drugs using mathematical modeling to enhance their effectiveness in reducing transmission. Methods: A systematic literature review was conducted using the PICO framework to evaluate the effectiveness of malaria vaccines in reducing disease transmission. The review included studies involving vulnerable populations, particularly children in African countries. It compared the effectiveness of malaria vaccines with other control strategies, such as insecticide-treated bed nets and antimalarial drugs. Five relevant primary journals were analyzed using databases like PubMed, Scopus, and Google Scholar. Results: Findings reveal that newly developed malaria vaccines have significant potential to reduce malaria cases. However, large-scale implementation and regular vaccination strategies are required for sustained effectiveness. Genetic studies highlight the emergence of resistance, emphasizing the need for adaptable vaccines and strategies. Conclusion: Malaria vaccines, while promising, face obstacles such as waning efficacy and resistance. A combined strategy involving vaccines and transmission-blocking treatments is essential for global malaria elimination. Further research and strategic implementation are needed to optimize vaccine effectiveness and overcome these challenges.

Abstract

Abstract

Papua recorded the highest number of malaria cases in Indonesia in 2021, with 275,243 individuals affected. Keerom Regency ranked second with 17,901 cases in 2020. Recurrent malaria infections in toddlers can lead to chronic nutritional disorders, anemia, and immune system impairments, which disrupt growth and increase the risk of stunting. Objective: To analyze the risk of malaria on stunting incidence among toddlers based on gender, age, history of fever, maternal knowledge, and malaria prevention practices in Keerom Regency. Method: This was a quantitative analytic study with a cross-sectional design. The study population consisted of toddlers aged 0–59 months who lived in Keerom Regency and had been diagnosed with clinical malaria, with or without stunting. A purposive sampling technique was applied. Data were collected through structured interviews using validated questionnaires administered to mothers, and anthropometric measurements of toddlers were taken to determine nutritional status based on height-for-age indicators. Data were analyzed using the chi-square test. Results: Showed that a history of fever, maternal knowledge of breastfeeding and supplemental feeding, and malaria prevention practices were significantly associated with stunting (p<0.05). Conclusions: Children with fever had a 90.6% stunting rate. Improving maternal knowledge and malaria prevention is essential to reduce stunting.
