Abstract

The availability of adequate facilities and the quality of healthcare services are crucial determinants of patient satisfaction, particularly in outpatient care. H. Padjonga Daeng Ngalle Regional General Hospital still faces several infrastructural and service challenges, such as ceiling leaks during the rainy season and limited seating in waiting areas, which may reduce patient comfort. This study aimed to analyze the influence of facility quality and healthcare service quality on outpatient satisfaction at H. Padjonga Daeng Ngalle Regional General Hospital in 2025. A quantitative approach with a cross-sectional design was employed, involving 362 randomly selected respondents. Data were collected through a validated and reliable questionnaire and analyzed using the chi-square test with SPSS software. The results showed a significant relationship between facility quality (p = 0.000; PR = 2.36; 95% CI: 1.78–3.12) and healthcare service quality (p = 0.000; PR = 2.91; 95% CI: 2.04–4.15) with outpatient satisfaction (p < 0.05). These findings indicate that both facility and service quality substantially influence outpatient satisfaction. Therefore, hospital management should prioritize improving physical facilities, service systems, and healthcare personnel competence to enhance patient satisfaction sustainably.

Abstract

The Standardised Inpatient Care Class (KRIS) is a national policy within the National Health Insurance (JKN) programme in Indonesia that mandates hospitals to fulfil 12 technical criteria to ensure equitable inpatient services for all BPJS Kesehatan participants. This study evaluated the institutional readiness of Baubau City Regional General Hospital in implementing KRIS as mandated by Presidential Regulation (No.59/2024). A convergent mixed-methods design was employed, involving four key informants with direct authority over the implementation process. Data were collected through in-depth interviews, readiness assessments based on the 12 KRIS criteria, direct field observations, and document reviews. Qualitative and quantitative data were analysed separately and integrated during interpretation using SWOT and fishbone analyses. The findings indicated that the hospital had not yet achieved readiness to implement KRIS, particularly in infrastructure standards, clinical-support facilities, and service system requirements. Major barriers included limited regional fiscal capacity, disparities in governance authority between local and centrally managed hospitals, and weak inter-unit coordination. These constraints may affect policy implementation effectiveness and equitable service delivery. The study concludes that Baubau City Regional General Hospital remains institutionally unprepared for full KRIS implementation. A phased PDCA (Plan–Do–Check–Act) approach is recommended to strengthen infrastructure readiness, coordination mechanisms, and institutional implementation capacity.

Abstract

Burnout Syndrome refers to a condition in which the body is completely exhausted, both physically and mentally. According to data from the International Labour Organization (ILO) in 2021, nearly 2 million workers die each year due to work-related accidents caused by fatigue. Of these, 71.43% of workers experienced mild fatigue before starting work. After work, 100% of workers experienced fatigue, with 11.43% reporting mild fatigue, 42.86% moderate fatigue, and 45.71% severe fatigue. It is noted that the number of contract employees at Bhayangkara Hospital Makassar is 448, with 7 employees having resigned, which accounts for 1.52%. The aim of this study is to determine the effect of workload and work stress on employee Burnout at Bhayangkara Hospital Makassar in the year 2024. This study used a quantitative research method with a cross-sectional study design. The total population in this study was 448 individuals, with a sample of 211 respondents. Data collection was conducted using questionnaires, and the analysis techniques included univariate, bivariate, and multivariate analysis. The results showed that workload had a significant effect on Burnout (p = 0.000), work stress had a significant effect on Burnout (p = 0.000), and there was a simultaneous effect of workload and work stress on Burnout, where the calculated F-value (7.363) was greater than the F-table value (3.04), with a significance value of 0.000 < 0.05.Conclusion: There is a simultaneous effect of workload and work stress on employee Burnout at Bhayangkara Hospital Makassar.

Abstract

Hospital Management Information Systems (SIMRS) serve as a critical foundation for ensuring the accuracy of National Health Insurance (JKN) claims under the Indonesian Case Based Groups (INA-CBGs/INA-DRG) payment system. However, limited SIMRS integration quality combined with the policy shift toward Standard Inpatient Class (KRIS) implementation pursuant to Presidential Regulation No. 59 of 2024 potentially increases the risk of data conflicts and claim fraud in hospitals. This systematic literature review aimed to examine the role of SIMRS in potential fraud within INA-CBGs/INA-DRG claims during KRIS implementation. A systematic literature review using descriptive qualitative analysis was conducted across PubMed/Medline, ScienceDirect, Wiley Online Library, Cochrane Library, and Google Scholar. From 205 initially identified articles, five primary studies were included. Significant weaknesses were found in SIMRS integration, data quality, and claim verification that increased fraud risk. BPJS Kesehatan data indicated potential fraud amounting to IDR 866.8 billion was successfully prevented in 2023. A negative tariff disparity averaging IDR 3,036,855.37 (36.66%) was identified for Caesarean section cases. Fingerprint-based verification technology reduced fraud to 0.01% in one facility. An integrated and reliable SIMRS supported by verification technology plays a strategic role in preventing INA-CBGs/INA-DRG claim fraud and supporting effective KRIS implementation.
