Abstract

Diabetes mellitus adalah gangguan metabolik kronis yang dapat menyebabkan disfungsi hati dan perubahan histopatologi. Penelitian ini bertujuan untuk menyelidiki pengaruh pemberian ekstrak daun sirsak (Annona muricata L.) terhadap fungsi hati dan gambaran histopatologi hati pada tikus putih galur Wistar jantan yang mengalami diabetes mellitus. Penelitian ini menggunakan desain eksperimental dengan tikus Wistar jantan yang diinduksi diabetes melalui injeksi streptozotocin. Tikus dibagi menjadi kelompok kontrol dan kelompok perlakuan, di mana kelompok perlakuan diberikan berbagai dosis ekstrak daun sirsak. Fungsi hati dinilai dengan mengukur kadar enzim hati dalam serum, termasuk AST (aspartate aminotransferase) dan ALT (alanine aminotransferase). Jaringan hati juga dikumpulkan untuk analisis histopatologi. Hasil penelitian menunjukkan bahwa ekstrak daun sirsak secara signifikan meningkatkan fungsi hati dengan menurunkan kadar AST dan ALT serum pada tikus diabetes dibandingkan dengan kelompok kontrol. Analisis histopatologi mengungkapkan adanya pengurangan kerusakan seluler dan perbaikan integritas struktural pada jaringan hati kelompok perlakuan. Temuan ini menunjukkan bahwa ekstrak daun sirsak memiliki efek hepatoprotektif dan manfaat terapeutik potensial dalam mengelola disfungsi hati yang terkait dengan diabetes mellitus. Penelitian lebih lanjut direkomendasikan untuk mengungkap mekanisme yang mendasari efek tersebut serta mengevaluasi keamanan dan efektivitas ekstrak daun sirsak dalam pengaturan klinis.

Abstract

Abstract— According to WHO (World Health Organization), anxiety disorders affected 266 million people globally in 2019, or 5.3% of the population. As the second most common mental disorder after depression, anxiety impacts 284 million people and significantly contributes to the disease burden worldwide. Meanwhile, regional prevalence rates are 19.2%, 16.2%, 9.0%, and 11.8% in North America, Europe, Africa, and Southeast Asia, with particularly 57.5% and 68.8% rates among pregnant women in Bali (2020) and Semarang (2023), respectively. In this context, ginger (Zingiber officinale), traditionally used for various ailments, contains phenol such as shogaol, zingerone, gingerol, and 6-shogaol, which have antioxidant and anti-inflammatory properties for reducing anxiety. Therefore, this research aims to analyze the phenol contents of ginger using UV-Vis spectrophotometry in the Faculty of Pharmacy, University of North Sumatra. The results show that there is a strong correlation of 0.9794 between gallic acid and phenol content. Additionally, phenol levels are reported with different extraction methods including 43.2653 mgGAE/g (4.326%) with water, 62.7680 mgGAE/g (6.27%) with 70% ethanol, and 129.7456 mgGAE/g (12.97%) with 96% ethanol. The highest phenol content in organic red ginger using 96% ethanol suggests the potential to reduce anxiety by counteracting free radicals.

Abstract

Hypertension is one of the leading causes of morbidity and mortality in the world. The Chronic Disease Management Program (PROLANIS) by BPJS Health aims to improve the quality of life of patients with hypertension. However, the level of patient participation is still low. The Health Belief Model (HBM) is used to understand the factors that influence the health behavior of hypertensive patients in following PROLANIS. Objective of this research is to analyze the relationship between variables in the HBM, such as perceived susceptibility, severity, benefits, barriers, self-efficacy, and cues to action, with hypertension patients' participation in PROLANIS at Puskesmas Kampung Baru. This study used a cross-sectional design with total sampling technique, involving 295 hypertensive patients who were members of PROLANIS. Data were collected through a validated HBM questionnaire. Data analysis was performed with Spearman's Rho correlation test. The results showed significant associations between Health Belief Model constructs and hypertensive patients’ participation in PROLANIS. Perceived susceptibility and self-efficacy showed the strongest correlations, while perceived severity, benefits, and barriers showed weak correlations. Cues to action were not significantly associated. Health Belief Model explains PROLANIS participation among hypertensive patients. Perceived susceptibility and self-efficacy are key determinants, indicating the importance of risk awareness and confidence-building interventions. AbstrakHipertensi merupakan salah satu penyebab utama morbiditas dan mortalitas di dunia. Program Pengelolaan Penyakit Kronis (PROLANIS) yang diselenggarakan oleh BPJS Kesehatan bertujuan untuk meningkatkan kualitas hidup pasien hipertensi. Namun, tingkat partisipasi pasien masih rendah. Model Keyakinan Kesehatan (HBM) digunakan untuk memahami faktor-faktor yang mempengaruhi perilaku kesehatan pasien hipertensi dalam mengikuti PROLANIS. Tujuan penelitian ini adalah menganalisis hubungan antara variabel dalam Model Keyakinan Kesehatan (HBM), seperti persepsi kerentanan, keparahan, manfaat, hambatan, kepercayaan diri, dan dorongan untuk bertindak, dengan partisipasi pasien hipertensi dalam PROLANIS di Puskesmas Kampung Baru. Studi ini menggunakan desain cross-sectional dengan teknik sampling total, melibatkan 295 pasien hipertensi yang menjadi anggota PROLANIS. Data dikumpulkan melalui kuesioner HBM yang telah tervalidasi. Analisis data dilakukan dengan uji korelasi Spearman's Rho. Hasil menunjukkan hubungan yang signifikan antara konstruksi Model Keyakinan Kesehatan dan partisipasi pasien hipertensi dalam program PROLANIS. Persepsi kerentanan dan kepercayaan diri menunjukkan korelasi terkuat, sementara persepsi keparahan, manfaat, dan hambatan menunjukkan korelasi lemah. Isyarat tindakan tidak menunjukkan hubungan yang signifikan. Model Keyakinan Kesehatan menjelaskan partisipasi dalam PROLANIS di kalangan pasien hipertensi. Persepsi kerentanan dan efektivitas diri merupakan determinan kunci, menunjukkan pentingnya kesadaran risiko dan kepercayaan diri.

Abstract

Hypertension, a condition characterized by persistent elevated blood pressure, often places individuals at risk of developing complications that can adversely impact their quality of life. One notable complication associated with hypertension is cognitive dysfunction. A study published in the Journal of Alzheimer's Disease (2020) demonstrated that individuals with long-standing hypertension exhibited an elevated risk of developing cognitive impairment and Alzheimer's disease. Preliminary research suggests that approximately 12.5 percent of hypertensive deaths may be attributable to underlying factors. Aims: This study aims to examine the cognitive deficits in patients with hypertension and summarise research on the relationship between hypertension and cognitive impairment. This analytical research study with a cross-sectional design, total sample of 84 individuals who suffer from hypertension, the ages of 60 and 79 years old. The sample selected using purposive sampling, instruments used in this study were medical record and Mini Mental State Examination (MMSE) questionnaire. The data was analyzed univariate and bivariate with the Chi Square statistical test. The MMSE cognitive function test revealed that out of 84 individuals with hypertension, 67.85% had normal cognitive function, while 32.14%) moderate cognitive impairment. The sample for the research consisted of 40 male individuals (47.61%) and 44 female individuals (52.38%). Hypertension can affect a person's cognitive function which has an impact on quality of life.
