Abstract

Latar Belakang Hipertensi adalah penyakit berbahaya namun demikian timbulnya penyakit dan akibat buruknya bisa dicegah. Penyakit ini sering ada tanpa memberikan gejala yang didasari penderita. ”Menjaga kesehatan lebih baik dari pada mengobati” merupakan anjuran yang belum dilaksanakan sepenuhnya oleh individu maupun masyarakat. Menjadi tugas tenaga kesehatan untuk terus memberi dan mengingatkan masyarakat untuk melaksanakan hal tersebut. Salah satu sarana untuk melaksanakan hal tersebut adalah melalui program pemeriksaan kesehatan rutin dan berkelanjutan. Tujuan Program ini yaitu untuk mendekatkan pelayanan kesehatan dengan masyarakat. Metode Pelaksanaan kegiatan di lakukan di Balai Kelurahan, salah satu keluharan di Kabupaten Malang. Peserta berjumlah 150 warga dari 11 RW yang tersebar diwilayah ini. Peralatan yang digunakan adalah: Sphygmomanometer, stetoscope, Timbang badan, alat ukur tinggi badan, kit pemeriksaan kimia darah (pemeriksaan gula darah sewaktu, lemak darah, asam urat), Media cetak (brosur) tentang anjuran hidup bersih dan menjaga kesehatan lebih baik dari mengobati. Hasil Sebanyak 72 (48%) peserta berusia 51-60 tahun. Sebanyak 48 peserta (32%) adalah penderita prehipertensi (Standar JNC 7). Hipertensi tahap 1 ditemukan pada 20 peserta (13,3%), hipertensi tahap 2 terdapat pada 57 peserta (38%). Empat peserta (2,7%) terdiagnosa Diabetes melitus. Kesimpulan Kegiatan ini diharapkan mendukung semua usaha untuk meningkatkan kesehatan jasmani masyarakat secara mandiri dengan terus menjaga kebersihan diri dan berperilaku positif untuk menjaga kesehatan dirinya terutama untuk menghindari penyakit hipertensi. Peserta diharapkan rutin kontrol dipusat layanan kesehatan primer sebagai bentuk upaya mewujudkan keluarga sehat dan sejahtera. Kata Kunci : Bina masyarakat, pemeriksaan kesehatan, pencegahan hipertensi.

Abstract

Rheumatoid arthritis (RA) is a joint disease, chronically cause permanent joint impairment leading to inability of daily life activities. Early diagnosis and preven-tive managements of RA are recommended to overcome the disease. The current drugs provide benefits for RA patients, unfortunately could not be used for long period and as prevention agents, due to the adverse effects.  Even though clinical and laboratory studies of pomegranate for osteoarthritis and RA had been done, the effects of the pomegranate peel on MMP-9, TNF-α, and IL-6 of RA are still unrevealed. TNF-α promotes inflammation process in RA and collaborates with osteoclasts to trigger osteoporosis. IL-6 shows negative effect on osteoblasts dif-ferentiation and MMP-9 stimulates cartilage degradation and inflammation mediat-ed by synovial fibroblast. This study measured TNF-α, IL-6, MMP-9, and arthri-tis score (AS) of RA rats treated with ethanolic extract of pomegranate peel (EPP) to evaluate its potency as a RA prevention agent. Male Lewis rats (three groups, five each), 200 g, received 80 mg, 160 mg, and 320 mg of EPP/rat respectively, in alternate day, within 60 days. On the 30th day, the rats were subcutaneously in-jected with 0,1 ml mycobacterium-complete Freund’s adjuvant (1 mg/mL) on plan-tar of the right hind paws to induce RA. Serum IL-6 and TNF-α were determined by ELISA.  Immunohistochemistry processed-synovial MMP-9 slices of ankle joints were evaluated by light microscope (400× magnification).  Arthritis score of Smit was used to determined AS. Data were analyzed by Kruskall Wallis, Mann Whitney U, and Pearson correlation test. p < 0.05 was significant. The EPP of 320 mg corrected serum TNF-α and IL-6, and synovial MMP-9 of RA rats (p < 0.05). No significant change was observed in arthritic score following the EPP treatment (p > 0.05). In conclusion, the results indicate the EPP may potential to be devel-oped as preventive agent of rheumatoid arthritis.

Abstract

Triglyceride (TG) is a simple and the main lipid of the daily diet. Nowadays, TG is emerging as an independent risk factor for cardiovascular disease, and increasing clinical data that indicate a high level of this simple lipid in serum may alert and play a role in liver impairment. Red yeast rice (RYR) reduces serum TG in human and animals. The rice is widely used as a natural inhibitor of HMG-CoA reductase to correct serum cholesterol level. Bioactive components of the rice are well known to have antioxidant properties. This study examined whether RYR protects hepatocytes by evaluating the serum AST-ALT, HDL-Cholesterol (HDL-C), TG levels, and the number of liver foam cells in hypertriglyceridemic rats. Twenty male Wistar rats were grouped into 5. Four groups received a high-fat diet (HFD), 40 g/animal/day for 60 days to induce hypertriglyceridemic condition. Along with the HFD treatment, three groups received 108, 54, and 27 mg/kgBW/day of RYR, respectively. Two other groups received standard and only HFD diet, respectively. Intracardiac blood was collected for measuring AST and ALT using AST or ALT activity assay Kit, respectively, serum TG and HDL-C by Enzymatic Caloric Test.  Hematoxylin-Eosin–stained 4µmm thick slices of liver tissues were prepared to count foam cell number by a light microscope with 400x magnificence. Data were analyzed using the Kruskal Wallis continued by the Mann Whitney U test. The p values of < 0.05 were considered to be significant. The present study found that 108, 54, and 27 mg/kgBW/day of RYR significantly decreased serum TG, HDL-C, AST, and ALT compared with those of hypertriglyceridemic rats receiving no RYR. The decreasing levels of the parameters were in relation to the doses of RYR. The doses of 108 and 54 mg/kgBW/day resulted in complete recovery of the liver tissues suffered from steatosis (p < 0.05). The RYR dose of 108 mg/KgBW/day completely corrects the serum HDL-C level. In conclusion, red yeast rice may have a potency to protect hepatocytes injuries due to hypertriglyceridemia.

Abstract

Chronic hyperglycemia is one of the signs of human type-2 diabetes mellitus due to insulin resistance and depletion. This study aimed to evaluate hexane, ethanolic, and aqueous leaf extracts of Urena lobata as antihyperglycemic agent. Design of this study was a post-test only control group using 25 male Sprague-Dawley rats divided into 5 groups. High fructose diet (HFD) ad libitum and single dose of intraperitoneal streptozocin (STZ) (25 mg/kgBW) were administered to induce diabetes in rats. Three groups of the diabetic rats orally received 500 mg/kgBW of only a type of leaf extract of U. lobata for 4 weeks. Insulin serum levels were measured using enzyme-linked immunosorbent assay (ELISA) method. Size, shape, and density of the islet cells were observed by light microscope. Blood glucose level and the area under curve (AUC) of serial oral glucose tolerance test (OGTT) were measured. The U. lobata leaf extracts of three types of solvent all increased serum insulin level and improved β cells condition, also decreased the AUC of the OGTT series in diabetic rats. Values were compared with untreated diabetic rats (p<0,05). The aqueous leaf extract of U. lobata was the best in increasing insulin serum level, recovering islet cells condition, and correcting blood glucose level. The hexane extract showed poor results when compared to the other soluble agents.

Abstract

Backgound. iNOS enzyme was secreted by immunocompetent cells during pathogenesis of diseases involving inflammation. The enzyme catalized a reaction resulting in nitric oxide radical (NO˙) in large amount. The radical reacts with superoxide (O2 ̅ ) to yield peroxynitrite ( ONOO ̅ ) which are highly reactive and toxic to the tissues. Kayumanis (C.burmanii) was considered rich with compounds overcoming iNOS secretion. This is the initial study of kayumanis potency in reducing iNOS expression. Diabetes is one of many diseases with high level of serum iNOS and was choosen for the study.Purpose. To study kayumanis potency in reducing serum iNOS of a diabetic conditions.Methode.5 groups of rats, each 5, four groups of which were intraperitoneally injected by aloxan at dose of 150 mg/Kg BW to induce hyperglycemia. The another group recieved no treatment. Three groups of diabetic rats were treated with C. Burmanii each day at dose of 0. 5 ml, 1 ml, and 2 ml respectively for 14 days. The remained diabetic group was untreated. Serum iNOS were measured by ELISA. iNOS data were statistically analyzed using ANOVA followed by LSD test (p ≤ 0.05).Results. C.burmanii dose of 1 ml reduced serum iNOS of diabetic rats (p ≤ 0.05). The two other doses of C.burmanii did not reduced the enzyme (p ≥ 0.05).Conclusion. C. Burmanii may have potention of reducing iNOS serum in diabetic condition.Key words: iNOS;Cinnamomum burmanii; diabetes; aloxan

Abstract

AbstractBackground. Hyperglycemia was reported to harm liver and induce metabolic inbalance. The tree bark of Cinnamomum burmannii (C. burmanii) was potential as anti-hiperglycemic agent. The bark was rich with secondary metabolites that may lead to possess other advantages for diabetes.Objectives. To study the effect of aqueos extract of C.burmannii (AEC) on liver and metabolic condition by measuring necrotic hepatocytes, serum urea and uric acid levels in alloxan-induced diabetic rats (AIDRs).Methods. Three groups of fasted rats (free allowed to water) were intraperitoneally injected with 150 mg/Kg BW in 1 ml normal saline 6 hours prior to intraperitoneal injection of 2 ml 10% glucose to induce AIDRs, the animals were allowed to 5% glucose solution for 24 hours afterward. The AIDRs were treated with 0.5 ml, 1ml, and 2 ml of AEC respectively for 2 weeks. Hematoxyline-Eosine-stained liver slices were observed for necrotic hepatocytes by light microscope. Serum urea and uric acid levels were measured by Glutamate dehydrogenase and Uricase-PAP methods respectively. Data were analyzed by ANOVA followed by LSD test, p values of < 0.05 were considered to be significant.Results.The given doses of AEC lowered urea and uric acid serum levels of AIDRs (p = 0.000) which approximated to the non diabetic ratss serum levels (p > 0.05). Except the smallest dose, the remaining doses of AEC reduced necrotic hepatocytes number in AIDRs (p = 0.000).Conclusion.Aqueos extract of C.burmannii protected hepatocytes and corrected serum urea and uric acid levels of alloxan-induced diabetic rats.Keywords: Cinnamomum burmanii; diabetes; necrotic hepatocytes; serum urea: serum uric acid.

Abstract

Fatty liver is an accumulation of lipid drops exceeding 5% of the total amount of liver, which can increase the risk of nonalcoholic fatty liver disease (NAFLD). NAFLD is associated with dyslipidemia, which the treatments for both condition are in common. Jati Belanda leaves, Kemuning leaves, Murbei leaves and Bangle rhizome (JKMB) are often used alone or in combination to improve dyslipidemia, but their effect on the liver is unknown.This study used an experimental post test control group method where 25 rats were acclimatized for 2 weeks. After that, the rats were divided into 5 groups including KN : normal feed, KP : DTLM feed, D1 : DTLM+ JKMB extract dose 189 mg/200gbb, D2 : DTLM+ JKMB extract dose 378 mg/200gbb, D3 : DTLM+ JKMB extract dose 756 mg /200gbb and treated for 12 weeks. JKMB extract was obtained in a ratio of 8: 3: 3: 7. After that, the rats were sacrificed and the liver tissue was stained with Hematoxylin-Eosin staining. HE preparations were performed to calculate the percentage of liver fat at 400x magnification with 5 fields of view per slide.The results of the normality (0.200> 0.05) and homogeneity test (0.558> 0.05) were found to be normal. The ANOVA test was found to be significantly different (0.000< 0.05). The Post Hoc test found that the negative control was significantly different from the positive control (0.000 < 0.05). Positive control did not differ significantly with treatment doses D1 (0.094>0.05), D2 (0.186>0.05) and D3 (0.734>0.05).From the results of the research above, it can be concluded that the combination of extracts of Jati Belanda leaves, Kemuning leaves, Murbei leaves and Bangle rhizome extracts could not reduce the percentage of fatty liver.

Peserta berjumlah 150 warga dari 11 RW yang tersebar diwilayah ini. Peralatan yang digunakan adalah: Sphygmomanometer, stetoscope, Timbang badan, alat ukur tinggi badan, kit pemeriksaan kimia darah (pemeriksaan gula darah sewaktu, lemak darah, asam urat), Media cetak (brosur) tentang anjuran hidup bersih dan menjaga kesehatan lebih baik dari mengobati. Hasil Sebanyak 106 (70,7%) peserta berusia lebih dari tahun. Sebanyak 48 peserta (32%) berusia 51-60 tahun. Sejumlah 48 peserta (32%) adalah penderita prehipertensi (Standar JNC 7). Hipertensi tahap 1 ditemukan pada 20 peserta (13,3%), hipertensi tahap 2 terdapat pada 57 peserta (38%). Empat peserta (2,7%) terdiagnosa Diabetes melitus. Kesimpulan. 70,7% (106) peserta berumur lebih dari 50 tahun. 82, 60% (124) peserta terdeteksi memiliki hipertensi dan prehipertensi. Kegiatan ini direspon baik oleh masyarakat ditandai dengan kehadiran 150 peserta.

Abstract

Abstrak: Prostaglandin E2 (PGE2) adalah senyawa yang akan meningkat pada jaringan inflamasi dan berkontribusi memodulasi nyeri. Delima (Punica granatum L.) dilaporkan memiliki potensi anti-inflamasi dan anti-nyeri neurogenik. Studi pustaka sistematis (SPS) ini mempelajari potensi anti-inflamasi ekstrak dan senyawa polifenol delima terhadap penurunan kadar PGE2 pada sel makrofag RAW264.7. Metode: Studi pustaka sistematis. Data penelitian dikumpulkan dari PubMed Central, PubMed, dan Google Scholar berdasarkan kata kunci pomegranate atau Punica granatum, inflammation, PGE2, dan sel makrofag RAW264.7. Delapan artikel memenuhi kriteria inklusi dan ditetapkan untuk ditelaah. Hasil: Ekstrak kulit dan bunga delima serta senyawa polifenol dalam buah dan kulit delima terbukti mampu menurunkan kadar PGE2 pada sel makrofag RAW264.7. Aktivitas anti-inflamasi ekstrak kulit dan bunga delima serta senyawa polifenol dalam buah dan kulit delima menurunkan kadar PGE2 melalui hambatan ekspresi Toll-Like Receptor 4 (TLR4), hambatan aktivasi jalur pensinyalan Mitogen-Activated Protein Kinase (MAPK), hambatan aktivasi faktor transkripsi Nuclear Factor-Kappa B (NF-κB), serta hambatan ekspresi dan regulasi enzim COX-2. Data menunjukkan bahwa senyawa polifenol delima menghambat sintesis PGE2 secara dependen. Kesimpulan: Senyawa polifenol delima mampu menurunkan kadar PGE2 pada sel makrofag RAW264.7.Kata kunci: Delima, Inflamasi, PGE2, Sel Makrofag RAW264.7

Abstract

Introduction: Interleukin-6 (IL-6) is one of the citokynes released by macrophage and patologic cells it has a main role in mediating inflammation. Pomegranate (Punica granatum l.) contain polyphenol such as punicalagin, ellagic acid, urolithin, gallic acid, and delphinidin which has high anti-inflammation activity. Research done this far is the ability of pomegranate to decrease IL-6 level in only one disease. This systematic literature review reports the effect of pomegranate’s polyphenol in decreasing IL-6 level on macrophage and other pathologic cells by in vitro method focus at inflammatory disease that includes alzheimer’s, inflammatory bowel disease, cancer, metabolic disease and degenerative disease such as  rheumatoid arthritis, osteoarthritis, osteoporosis, and atherosclerosis.Method: Systematic literature review. Data were colected from Google Scholar and PubMed Central. The keywords used are “punica granatum,  inflammation, interleukin-6”. 19  papers which fullfilled the inclusion criteria reported the effect of pomegranate (Punicagranatum l.) to IL-6.Result: Active substance like punicalagin, ellagic acid, gallic acid, urolithin A/B, granatin A/B and delphinidin from the flower, seed, skin, mesocarp (inner skin) and all parts of pomegranate are reported to decrease the interleukin-6 (IL-6) level in diseases involving inflammation patophysiology, which include inflammatory disease such as inflamatory bowel disease and alzheimer’s, cancer, metabolic disease and degenerative disease include rheumatoid arthritis, osteoarthritis, osteoporosis, and atherosclerosis.Keyword: Punica granatum, Inflammation, Interleukin-6, In Vitro
