Abstract

Background: The increasing elderly population poses its own challenges, particularly in the field of health. The elderly tend to experience degenerative diseases and often have multiple chronic conditions. This can lead to the use of multiple medications (polypharmacy). Drug interactions occur when one medication affects the activity of another. These interactions can be pharmacokinetic or pharmacodynamic. The Beers Criteria is a tool used to detect Potentially Inappropriate Medications (PIMs) and identify drugs that may pose greater risks than benefits for elderly patients aged ≥ 65 years. Methods: This research uses descriptive method and is an observational study with a cross-sectional approach, measuring variables by collecting data through patient geriatric questionnaire. It aims to evaluate medication use in geriatric patients who may have drug interactions at Cilacap Regional General Hospital. Results: This research shows that female patients are more numerous, with 53 individuals (53.53%), while male patients amount to 46 individuals (46.46%). Patients in the age range of 60-74 years are 94 individuals (94.94%), and those in the age range of 75-90 years are 50 individuals (5.05%). Patients receiving <5 medication therapy are 57 individuals (57.57%), while those receiving >5 medications are 42 individuals (42.42%). The most commonly used medication among geriatric patients is candesartan, with 46 prescriptions (9.82%). The research results indicate that the medications falling under Potentially Inappropriate Medications (PIMs) according to the 2023 Beers criteria are mostly in category 1, totaling 81 medications (50.63%). Conclusion: The occurrence of Potentially Inappropriate Medications (PIMs) in geriatric patients at the outpatient department of Cilacap Regional General Hospital during the period of July-August 2024, based on the Beers Criteria 2023, shows that the largest percentage is attributed to glimepiride with 25 cases (17.606%). This indicates that prescribing PIMs to geriatric patients still involves potentially inappropriate drugs, necessitating attention from healthcare professionals to ensure safe and effective prescribing for this population.

Abstract

Toner formulation is created using a combination of glycerin and propylene glycol. Glycerin can activate transglutaminase activity in the stratum corneum, increase the corneocyte maturation process and reduce scaling in xerotic skin. Propylene glycol is a penetration enhancer so it is usually used as a solvent and carrier for substances that are not soluble in water. As part of the formulation developement, the product is formulated as a toner. This research aims to determine the optimum composition of the toner formula with variation in the concentrations of glycerin and propylene glycol to achieve the optimal formula. This research uses the Simplex Lattice Design method. The evaluation tests carried out were organoleptic, specific gravity, viscosity, pH, transferred volume and hedonic tests. The organoleptic test results are white with a liquid texture and a distinctive aroma of tea tree oil. The specific gravity results meet the range between 1.01-1.1. The panelists' hedonic test results preferred the color and texture/shape of the tea tree oil toner, while the aroma preferred the comparison toner. Optimizing the optimum formula resulted in 12% glycerin and 8% propylene glycol with predicted values of pH 5.91, viscosity 3.70, and volume transferred 95.84. The optimum formula shows an average replication of 1.2 and 3 in the pH test of 5.84, viscosity of 3.76 and volume transferred 96. The results of the optimum formula and predicted values were analyzed using the One Sample T-Test, and the results showed that there were no significant differences. (p>0.05) namely pH test 0.981; viscosity 0.899; and the volume moved was 0.408. The conclusion of this research is that the tea tree oil toner preparation obtained an optimum formula with a ratio of 12% glycerin and 8% propylene glycol.

Abstract

One of the plants that can be used as a natural medicine is the tea tree plant (Melaleuca alternifolia Cheel) which is generally known as the tea tree. The tea tree plant contains essential oils which are useful as antimicrobial and anti-inflammatory. Another plant that can be used as a natural ingredient is the fennel plant (Foeniculum vulgare Mill) which contains essential oils which are useful as antimicrobials. In order to develop the preparation, a telon oil preparation was formulated. The use of additional ingredients in the formulation of telon oil preparations, namely VCO and Olive Oil as vegetable oils which function as solvents for essential oils, can certainly affect the physical properties of telon oil preparations, so a method is needed to find an optimum formula that meets the good physical properties of the preparation. This research aims to determine the optimum formula composition of telon oil preparations with variations of VCO and Olive Oil using the Simplex Lattice Design method by looking at the results of evaluation tests of the physical properties of telon oil preparations which include organoleptic tests, homogeneity tests, pH tests, specific gravity tests, viscosity tests and displaced volume test. The results of organoleptic tests, homogeneity, pH, specific gravity, viscosity and transferred volume have met the requirements. Analysis using SLD showed that the optimal preparation formula for baby telon tree tea oil (Melaleuca alternifolia Cheel) was obtained with a VCO and Olive Oil composition of 78.4% and 19.6% with a specific gravity response value of 0.906 gr/ml, viscosity 36.84 cp and volume transferred 57.33 ml. In the hedonic testing of baby telon oil preparations, tree tea oil (Melaleuca alternifolia Cheel) with variations of VCO and olive oil from the optimum formula in terms of color was preferred more by the panelists.

Abstract

Seiring dengan bertambahnya usia, akan berpengaruh terhadap kondisi fisik seseorang. Berbagai keluhan kesehatan dirasakan oleh lansia yang diakibatkan oleh adanya penurunan fungsi tubuh dan biasanya berkaitan dengan adanya penyakit degeneratif serta sindrom metabolik yang dialami seseorang. Pencegahan dan identifikasi dini penyakit degeneratif dan sindrom metabolik menjadi keuntungan utama dari pemantauan kesehatan, serta diperlukan untuk menekan perkembangan penyakit dan dapat digunakan untuk mendasari manajemen pengobatan. Pengobatan secara farmakologi dan non farmakologi dapat digunakan untuk mengatasi kesehatan pada lansia. Adanya obat sisa atau obat persediaan di rumah tangga merupakan kemungkinan yang sering terjadi, obat tersebut harus dapat dikelola dengan baik sehingga diperlukan edukasi kepada para lansia. Tujuan dilakukannya kegiatan pengabdian masyarakat ini adalah supaya lansia bisa memahami terakit penyakit degeneratif baik dari penyebab, pencegahan maupun penanganan serta pengobatan yang benar dan pengelolaan yang baik agar tidak terjadi salah penggunaan obat, penggunaan yang berlebih maupun obat kadaluarsa. Kegiatan diawali dengan pemeriksaan tekanan darah, gula darah sewaktu dan asam urat. Kemudian diberikan edukasi dengan media ceramah mengenai penyakit degeneratif dan pengelolaan obat (DAGUSIBU). Peserta kegiatan diberikan kuesioner pre-test dan post-test untuk mengukur pengetahuan mengenai penyakit degeneratif dan ketrampilan pengelolaan obat sebelum dan setelah mendapatkan materi. Berdasarkan hasil analisis menggunakan uji wilcoxon dari 20 peserta yang mengisi kuesioner secara lengkap menunjukkan nilai asymp.sig (2 tailed) 0.01, yang berarti kurang dari 0.05 dan memberikan kesimpulan bahwa terdapat perbedaan pengetahuan dan pemahaman peserta kegiatan sebelum dan sesudah diberikan sosialisasi.As age increases, it will affect a person's physical condition. Various health complaints are felt by the elderly due to a decrease in body function and are usually related to the presence of degenerative diseases and metabolic syndrome experienced by a person. Prevention and early identification of degenerative diseases and metabolic syndrome are the main benefits of health monitoring, and are needed to suppress disease progression and can be used to inform treatment management. Pharmacological and non-pharmacological treatments can be used to manage health in the elderly. The existence of residual drugs or drug supplies in households is a possibility that often occurs, these drugs must be managed properly so that education is needed for the elderly. The purpose of this community service activity is so that the elderly can understand degenerative diseases both from the causes, prevention and handling as well as correct treatment and good management so that there is no wrong use of drugs, excessive use or expired drugs. The activity began with checking blood pressure, blood sugar and uric acid. Then education was given with lecture media about degenerative diseases and drug management (DAGUSIBU). Participants were given pre-test and post-test questionnaires to measure knowledge about degenerative diseases and drug management skills before and after the educational program. Based on the results of the analysis using the Wilcoxon test of 20 participants who filled out the questionnaire completely showed an asymp.sig (2 tailed) value of 0.01, which means less than 0.05 and provides a conclusion that there is a difference in knowledge and understanding of activity participants before and after being given socialisation.
