Abstract

This research aims to determine the factors that influence tuberculosis infection in children with close contact with adult pulmonary tuberculosis sufferers. This research is an analytical observational study with a consecutive sampling method at the Glugur Darat Health Center in November 2024 until January 2025. Bivariate analysis using chi-square to determine whether there is a relationship between the independent and dependent variables. The type of test used is the Fisher Exact and Kruskal Wallis Test. There is no multivariate test because there are no significant variables in the bivariate test. In total, in 42 samples there were 10 children with positive Mantoux tests. Of the 23 female children there were 7 (30.4%) with positive results and of the 19 male children there were 3 (15.8%) positive results. Of the 39 children with a history of BCG immunization, there were 10 (25.6%) with tuberculosis infection. Of the 37 children with parents whose income was <UMR, 10 children (27%) had tuberculosis infection. 37 children whose parents had junior high school education, 9 people (24.3%) had tuberculosis infection. 10 children aged 1-5 years and 6-10 years, there were 2 (20%) each with tuberculosis infection. 26 children aged 11-18 years, there were 6 people (27.3%) with tuberculosis infection. There was no significant relationship between age, gender, immunization status, nutritional status, parental income and parental education level in this research.

Abstract

Drug-resistant epilepsy (DRE) is found in up to one-third of individuals with epilepsy who have received appropriate therapy and this condition causes significant child morbidity and mortality. Epilepsy that appears at an early age is at higher risk of developing DRE. Objective to assess the effect of age on the characteristics of seizure severity in children with DRE. This study used a cross-sectional design involving 36 DRE patients aged 2-18 years at the Child Neurology Polyclinic, Adam Malik Hospital Medan, from September to October 2024. Seizure severity characteristics were assessed using the Global Assessment of the Severity of Epilepsy (GASE) questionnaire instrument. The categorical data are presented in the form of proportions and the effect of age on seizure severity characteristics was analyzed using the chi-square test. There were 36 children with DRE in this study, predominantly aged ≥ 10 years. Bivariate analysis showed a significant effect of age in children with drug resistant epilepsy only on the disruption of activity experienced by children with a p value = 0.036 (p <0.05). Most patients aged ≥ 10 years did not experience activity disorders or experienced mild activity disorders (73.9%), while in patients aged <10 years, most patients experienced severe activity disorders (61.5%). The age of children with drug resistant epilepsy have a significant effect on daily activity disorders.

Abstract

Pulmonary dysfunction in children with thalassemia can be restrictive, obstructive, or diffusion impairment. Elevated serum ferritin levels are a cause of pulmonary dysfunction in transfusion-dependent thalassemia patients. The aim of this study was to assess  relationship between serum ferritin levels and pulmonary dysfunction in transfusion-dependent thalassemia patients.  This cross-sectional study was conducted on 44 transfusion-dependent thalassemia children between May and June 2025 at the Thalassemia Clinic of Adam Malik Hospital, Medan. Demographic characteristics were obtained from interviews and medical records. Lung function was assessed using a spirometer. Associations were analyzed using Shapiro Wilk, Kruskal-Wallis test, and multinomial logistic regression test. A total of 17 children (38.6%) had normal lung function, while 27 children (61.3%) had impaired lung function. Restrictive impairment was present in 24 children (54.5%), and obstructive impairment in 3 children (6.8%). Analysis showed significant differences in serum ferritin levels and splenomegaly in relation to impaired lung function, with p-values of <0.001 and 0.013, respectively. Each 1 ng/mL increase in ferritin levels increased the likelihood of developing restrictive and obstructive pulmonary dysfunction by 0.2% (OR = 1.002; 95% CI: 1.001–1.003). However, gender, hepatomegaly, and duration of transfusion did not show significant differences.

Abstract

Tuberculosis (TB) in children presents a major diagnostic challenge due to the paucibacillary nature of the infection and difficulties in sputum collection. As an alternative, blood cell ratios such as Neutrophil-to-Lymphocyte Ratio (NLR), Monocyte-to-Lymphocyte Ratio (MLR), and Neutrophil-Monocyte-to-Lymphocyte Ratio (NMLR) have been proposed as hematological biomarkers that can support the diagnosis of TB in children. This study aimed to evaluate the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy of NLR, MLR, and NMLR in establishing the diagnosis of TB in children at RS Adam Malik. A retrospective observational study was conducted using data from the TB DOTS medical records of children aged 0–18 years during 2024. Hematologic parameters (neutrophils, monocytes, lymphocytes) and the ratios NLR, MLR, and NMLR were analyzed and compared with clinical and bacteriological TB diagnosis (TCM/BTA). Of the 219 children who met the inclusion criteria, NLR showed a sensitivity of 82.3%, specificity of 78.7%, PPV of 84.9%, NPV of 75.3%, and accuracy of 80.8%. MLR showed a sensitivity of 80.8%, specificity of 75.3%, PPV of 82.7%, NPV of 72.8%, and accuracy of 78.5%. NMLR showed a sensitivity of 76.9%, specificity of 66.3%, PPV of 76.9%, NPV of 66.3%, and accuracy of 72.6%. These ratios were significantly associated with TB diagnosis (p < 0.05). The most common clinical symptoms found in positive cases included persistent cough, prolonged fever, weight loss, and night sweats. NLR, MLR, and NMLR demonstrate good diagnostic value in supporting the diagnosis of TB in children. NLR showed the most optimal results, followed by MLR and NMLR. These hematological ratios can be used as quick, inexpensive, and easily accessible diagnostic aids, especially in facilities with limited bacteriological testing capabilities.
