Abstract

Background: Malaria remains a significant public health concern, particularly in endemic regions, is frequently associated with anemia in children. Accurate hemoglobin measurement is crucial for diagnosing anemia, especially in resource-limited settings. This study compares the accuracy of SD Biosensor Standard G6PD Analyzer with Hemocue HB301 System for hemoglobin measurement among children in malaria-endemic areas. Objective: To determine the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), positive likelihood ratio (LR+), negative likelihood ratio (LR-), pretest odds, post-test odds, and overall accuracy of the SD Biosensor Standard G6PD Analyzer compared to the Hemocue HB301 System. Method: This cross-sectional diagnostic study involved 325 children aged 6-18 years in Kualuh Leidong District. Secondary data were obtained from a previous study conducted in August 2024, using a total sampling technique. Result: The mean hemoglobin level measured by SD Biosensor Standard G6PD Analyzer (14,22±2,21 mg/dL) was higher than that measured by Hemocue HB301 System (13,45±1,63 mg/dL), with significant difference (p=0,001). The prevalence of anemia detected by SD Biosensor and Hemocue was 12,3% and 11,4%, respectively. The SD Biosensor Standard G6PD Analyzer demonstrated a sensitivity of 70,27%, specificity of 95,14%, PPV of 65%, NPV of 96,14% and an overall accuracy of 92,31%. Conclusion: The SD Biosensor Standard G6PD Analyzer exhibited relatively low sensitivity but high specificity compared to Hemocue HB301 System in measuring hemoglobin levels among children in malaria-endemic areas. Further research is needed to compare both devices against the gold-standard automated hematology analyzer and to identify the factors influencing measurement accuracy. Abstract

Pneumonia remains one of the leading causes of morbidity and mortality among children, particularly in developing countries. Rapid assessment of disease severity is essential for appropriate clinical management. Procalcitonin (PCT) is recognized as a biomarker of systemic bacterial infection; however, its role in determining the severity of pediatric pneumonia is still debated. This study aimed to analyze the correlation between procalcitonin levels and pneumonia severity in pediatric patient. This was an observational analytic study with a retrospective cross-sectional design using consecutive sampling. Data were collected from medical records of 102 pediatric patients aged 1 month to 18 years who were diagnosed with pneumonia throughout 2024. PCT levels were compared between children with severe pneumonia and those with severe pneumonia accompanied by danger signs. Statistical analysis included the Mann-Whitney test to assess differences in PCT values and ROC curve analysis to determine the optimal cut-off, sensitivity, specificity, and accuracy. The majority of patients (71.6%) presented with severe pneumonia with danger signs. PCT levels differed significantly between groups (p < 0.001). ROC analysis identified an optimal cut-off value of ≥1.06 µg/L, yielding a sensitivity of 82.2%, specificity of 79.3%, positive predictive value of 90.9%, negative predictive value of 63.9%, and overall accuracy of 81.37% (AUC = 0.874). There is a significant association between procalcitonin levels and the severity of pneumonia in children. Procalcitonin may serve as a valuable early biomarker for assessing disease severity and guiding clinical decision-making in pediatric pneumonia, particularly in healthcare settings in North Sumatra.

Abstract

Pulmonary dysfunction in children with thalassemia can be restrictive, obstructive, or diffusion impairment. Elevated serum ferritin levels are a cause of pulmonary dysfunction in transfusion-dependent thalassemia patients. The aim of this study was to assess  relationship between serum ferritin levels and pulmonary dysfunction in transfusion-dependent thalassemia patients.  This cross-sectional study was conducted on 44 transfusion-dependent thalassemia children between May and June 2025 at the Thalassemia Clinic of Adam Malik Hospital, Medan. Demographic characteristics were obtained from interviews and medical records. Lung function was assessed using a spirometer. Associations were analyzed using Shapiro Wilk, Kruskal-Wallis test, and multinomial logistic regression test. A total of 17 children (38.6%) had normal lung function, while 27 children (61.3%) had impaired lung function. Restrictive impairment was present in 24 children (54.5%), and obstructive impairment in 3 children (6.8%). Analysis showed significant differences in serum ferritin levels and splenomegaly in relation to impaired lung function, with p-values of <0.001 and 0.013, respectively. Each 1 ng/mL increase in ferritin levels increased the likelihood of developing restrictive and obstructive pulmonary dysfunction by 0.2% (OR = 1.002; 95% CI: 1.001–1.003). However, gender, hepatomegaly, and duration of transfusion did not show significant differences.
