Abstract

Microcirculation disorders are the focus of attention in cardiovascular disease research as they lead to serious cardiovascular complications. Nailfold videocapillaroscopy (NVC) is a non-invasive method that has great potential in exploring risk factors, clinical predictors and developing preventive measures for microcirculatory disorders in cardiovascular disease. This study was based on PRISMA guidelines, then adjusted with appropriate inclusion and exclusion criteria and the PICO framework. Risk of bias assessment using the Joanna Briggs Institute (JBI). There were 14 inclusion studies including cohort, case-control, cross-sectional, comparative and multicentre studies. The results found that the utilisation of NVC is proven to be used in the exploration of risk factors, predictors and preventive measures of microcirculatory disorders of several cardiovascular diseases namely primary, secondary and pulmonary hypertension with vasculitis. Risk factors include endothelial damage, arterial stiffness, and so on. Future predictors can be done in patients with microvascular abnormalities suspected of systemic sclerosis, Raynaud's symptom and hormonal disorders. Preventive measures that can be carried out further supporting examinations in confirming disease disorders and complications. With the utilisation of NVC, the public can utilise this technology as an early detection tool to prevent more serious cardiovascular complications. AbstrakGangguan mikrosirkulasi menjadi fokus perhatian dalam penelitian penyakit kardiovaskular karena mengakibatkan komplikasi serius bagi kardiovaskular. Nailfold videocapillaroscopy (NVC) merupakan metode non-invasif yang memiliki potensi besar sebagai dalam mengeksplorasi faktor risiko, prediktor klinis dan menyusun langkah prevetif terhadap gangguan mikrosirkulasi penyakit kardiovaskular. Penelitian ini didasarkan pada pedoman PRISMA, kemudian disesuaikan dengan kriteria inklusi dan eksklusi yang sesuai dan kerangka kerja PICO. Penilaian risiko bias menggunakan Joanna Briggs Institute (JBI). Didapatkan 14 studi inklusi termasuk studi cohort, case-control, cross-sectional, comparative dan multicenter study. Hasil penelitian menemukan bahwa pemanfaatan NVC terbukti dapat digunakan dalam eksplorasi faktor risiko, prediktor dan langkah preventif gangguan mikrosirkulasi dari beberapa penyakit kardiovaskular yakni hipertensi primer, sekunder dan pulmonal disertai vaskulitis. Faktor risiko yang didapatkan mulai dari kerusakan endotel, kekuan arteri, dan sebagainya. Predikor kedepannya dapat dilakukan pada pasien abnormalitas pada mikrovaskular yang dicurigai mengalami sistemik sklerosis, gejala raynaud dan gangguan hormon. Langkah preventif yang dapat dilakukan pemeriksaan penunjang lanjutan dalam mengkonfirmasi gangguan penyakit dan komplikasi. Dengan demikian pemanfaatan NVC, masyarakat dapat memanfaatkan teknologi ini sebagai alat deteksi dini untuk mencegah komplikasi kardiovaskular yang lebih serius.

Abstract

Polycystic Ovary Syndrome (PCOS) is one of the diseases that can cause infertility for women. Pharmacological therapy given is either metformin or clomiphene citrate while non-pharmacological is acupuncture. Both therapies have been shown to be beneficial in increasing pregnancy. Therefore, the purpose of this review is to determine the effect of the combination of acupuncture against metformin or clomiphene in increasing pregnancy rates. The method used was a literature review sourced from Google Scholar, Pub Med, Science Direct. Then identification, screening, adjustment of inclusion and exclusion variables were carried out and then selected titles, abstracts and contents so that only 6 articles were discussed. The results obtained that the combination therapy of acupuncture with metformin has better clinical effectiveness than clomiphene citrate because of minimal side effects and satisfactory outcomes in increasing pregnancy. Thus, it is hoped that further research will be carried out in the use of a combination of acupuncture therapy with metformin or clomiphene citrate. AbstrakPolycystic Ovary Syndrome (PCOS) merupakan salah satu penyakit yang dapat mengakibatkan infertilitas bagi wanita. Terapi farmakologi yang diberikan yaitu baik metformin atau clomiphene citrate sedangkan non farmakologi berupa akupuntur. Kedua terapi tersebut terbukti bermanfaat dalam meningkatkan kehamilan. Oleh sebab itu, tujuan tinjauan ini adalah untuk mengetahui efek kombinasi akupuntur terhadap metformin atau clomiphene dalam meningkatkan tingkat kehamilan. Metode yang digunakan berupa kajian literatur yang bersumber dari Google Scholar, Pub Med, Science Direct. Kemudian dilakukan identifikasi, skrining, penyesuaian terhadap variabel inklusi dan eksklusi lalu dilakukan seleksi judul, abstrak dan isi maka yang hanya dibahas berjumlah 6 artikel. Hasil yang didapatkan bahwasannya terapi kombinasi akupuntur dengan metformin memiliki efektifitas klinis yang lebih baik dibandingkan clomiphene citrate sebab efek samping yang lebih minim serta outcomes yang cukup memuaskan dalam meningkatkan kehamilan. Dengan demikian, diharapkan dilakukannya penelitian lebih lanjut dalam pemanfaatan kombinasi terapi akupuntur dengan metformin atau clomiphene citrate.

Abstract

Alzheimer's disease (AD) is one of the common health problems faced by the elderly, which can affect cognitive abilities and brain function. Currently available therapies are not very effective in managing AD in some people, so there is still a need for other treatment innovations in improving the QoL of patients. So this innovation presented with the aim to exploring the effectiveness of gamma sensory stimulation in preventing white matter atrophy and myelin loss, and its impact on cognitive decline. This research is a literature review sourced from several internet search engines. The review conducted through several stages, including identification, screening, and fulfillment inclusion and exclusion criteria, and finally resulting in 7 articles. The use of this device as therapy in geriatric patients with AD has been clinically tested and shown positive results in overcoming white matter atrophy and myelin loss in geriatric patients with AD. By improving these conditions, the device has the potential to slow the progression or even reduce symptoms experienced by patients, such as cognitive decline. Thus, based on the findings of this study, gamma stimulation could be a potential complementary therapy for Alzheimer's patients, thereby improving their quality of life. AbstrakPenyakit alzheimer (AD) merupakan salah satu masalah kesehatan yang sangat sering dihadapi oleh lansia (usia 60 tahun keatas), yang dapat mempengaruhi kemampuan kognitif serta fungsi otak. Terapi yang tersedia saat ini belum begitu efektif dalam mengelola AD pada beberapa orang, sehingga masih diperlukan inovasi pengobatan lainnya dalam meningkatkan kualitas hidup pasien. Sehingga inovasi ini dihadirkan dengan tujuan untuk mengeksplorasi efektivitas stimulasi sensorik gamma dalam mencegah atrofi materi putih dan kehilangan mielin, serta dampaknya terhadap penurunan kognitif. Penelitian ini merupakan kajian pustaka yang bersumber dari beberapa mesin pencari internet. Kajian dilakukan melalui beberapa tahap, meliputi identifikasi, penyaringan, dan pemenuhan kriteria inklusi dan eksklusi, sehingga menghasilkan 7 artikel yang memenuhi kriteria inklusi. Penggunaan alat ini sebagai terapi pada pasien geriatri dengan AD telah teruji secara klinis dan menunjukkan hasil yang positif dalam mengatasi atrofi materi putih dan kehilangan mielin pada pasien geriatri dengan AD. Dengan memperbaiki kondisi ini, alat ini berpotensi memperlambat progresi atau bahkan mengurangi gejala yang dialami pasien, seperti penurunan kognitif. Sehingga, berdasarkan temuan kajian ini, stimulasi gamma bisa menjadi terapi pelengkap yang potensial untuk pasien Alzheimer, sehingga meningkatkan kualitas hidup pasien.

Abstract

Purpose: Cardiovascular disease (CVD) continues to be the primary cause of global morbidity and mortality, predominantly attributable to modifiable risk factors such as unhealthy diets, physical inactivity, and so on. While behavioral interventions are critical, the effective implementation of prevention strategies is often hindered by resource limitations. The integration of community-based strategies with mobile health (mHealth) technologies presents a promising approach for scalable, personalized risk mitigation. This review evaluates the efficacy of community-integrated mHealth inter- ventions in reducing CVD risk factors. Methods: We searched multiple databases for randomized controlled trials (RCTs) published over the last 10 years. Fourteen RCTs (n = 9,862 participants) met inclusion criteria, with interventions combining mHealth tools and community components. The risk of bias was assessed using the Cochrane RoB 2.0 tool, and meta-analyses were conducted using RevMan 5.4. Results: mHealth and community have demonstrated considerable efficacy in diminishing various cardiovascular risk factors. The primary mechanisms include enhanced adherence to a healthy lifestyle, continuous monitoring, and improved access to health-related information. The results of the meta-analysis are directly proportional to the findings, which significantly reduce the risk factors for heart disease: healthy dietary patterns (p < 0.0001), physical activity (p = 0.04), BMI (p = 0.002), systolic blood pressure (p = 0.002), and diastolic blood pressure (p = 0.02). However, total cholesterol and fasting blood sugar did not have significant results, namely (p=0.23) and (p=0.77). Conclusion: Community-integrated mHealth is an effective strategy for addressing CVD risk factors.
