Abstract

Latar Belakang : Menurut WHO sekitar 80% dari penderita osteoartritis berusia lebih dari 65 tahun. Penyakit ini menyebabkan nyeri dan disabilitas pada penderita sehingga mengganggu aktivitas sehari-hari. Tujuan : Mengetahui apakah terdapat korelasi antara usia, jenis kelamin, obesitas, diabetes mellitus dengan tingkat keparahan penyempitan celah sendi osteoartritis menurut Kellgren Lawrence Pasien Di RSUD Waled Tahun 2019-2022. Metode : Penelitian observasional dengan desain Cross sectional dari hasil foto rontgen pasien osteoartritis tahun 2019-2022 dan data rekam medis pasien osteoarthritis di Rawat jalan Poliklinik orthopedi, Saraf dan Penyakit dalam. Analisis statistik dilakukan dengan uji korelasi spearman dan Regresi logistik dengan aplikasi Software. Hasil : Hasil analisis didapatkan p = 0,012 (p<0,05) r=0,228 pada usia dengan penyempitan celah sendi, jenis kelamin dengan penyempitan celah sendi didapatkan p = 0,004 (p<0,05) r=0,259, obesitas dengan penyempitan celah sendi didapatkan p = 0,043 (p<0,05) r=0,184 dan diabetes mellitus dengan penyempitan celah sendi didapatkan p = 0,220 (p<0,05) r=0,112. Dengan hasil regresi logistik didapatkan variable yang berpengaruh yang paling besar dengan penyempitan celah sendi terdapat pada variabel usia karena nilai Exp(B) (0.112). Kesimpulan : Terdapat hubungan bermakna antara usia, jenis kelamin dan obesitas dengan penyempitan celah sendi dan tidak memiliki hubungan yang bermakna antara diabetes melitus dengan penyempitan celah sendi. Kata Kunci : osteoartritis, usia, jenis kelamin, obesitas, diabetes melitus, penyempitan celah sendi. ABSTRACT Introduction : According WHO, about 80% of people with osteoarthritis are over 65 years old. This disease causes pain and disability in the sufferer so that it interferes with daily activities. Purposes : Knowing whether there is a correlation between age, gender, obesity, diabetes mellitus and the severity of osteoarthritis joint space narrowing according to Kellgren Lawrence Patients at Waled Hospital in 2019-2022. Methods : Observational study with a cross-sectional design based on X-rays of osteoarthritis patients in 2019-2022 and medical record data of osteoarthritis patients at the Outpatient Polyclinic of Orthopedics, Neurology and Internal Medicine. Statistical analysis was carried out using the Spearman correlation test and logistic regression with software applications. Results : The results of the analysis obtained p = 0.012 (p <0.05) r = 0.228 at age with narrowing of the joint space, gender with narrowing of the joint space obtained p = 0.004 (p <0.05) r = 0.259, obesity with narrowing of the joint space obtained p = 0.043 (p<0.05) r=0.184 and diabetes mellitus with joint space narrowing obtained p = 0.220 (p<0.05) r=0.112. With the results of logistic regression, it was found that the variable that had the greatest influence on the narrowing of the joint space was found in the Age variable because of the Exp(B) value (0.112). Discussion : There is a significant relationship between age, gender and obesity with narrowing of the joint space and has no significant relationship diabetes melitus with narrowing of the joint space.

Abstract

Background: Fractures are projected to become the fourth leading cause of death by 2030. A fracture is a discontinuity in the bone that is often accompanied by damage to the surrounding tissues. The healing process of fractures is influenced by various factors, the greater the damage to the bone and surrounding tissues, the longer the healing time. In cases of tibia and fibula fractures, the most commonly used surgical procedure in Indonesia is Open Reduction and Internal Fixation (ORIF) using Plates and Screw. Aims: To compare of healing time in closed fractures of the tibial diaphysis and tibial-fibula after ORIF Plate and Screw at Waled regional public hospital. Methods: This research uses a cross sectional design. A total of 77 samples were obtained using total sampling. Statistical tests use Mann Whitney. Results: There were 37 patients with closed fractures of the tibia diaphysis and 40 patients with closed fractures of the tibia-fibula diaphysis. The healing time for closed tibial diaphyseal fractures was ≤ 24 weeks in 12 patients (32.4%), while > 24 weeks was 25 patients (67.6%). In patients with closed fractures of the tibia-fibula diaphysis within ≤ 24 weeks and > 24 weeks, the number was the same, namely 20 patients (50.0%). In the Mann-Whitney test, a significance value of 0.032 (p<0.05) was obtained, indicating a significant difference in the healing time between closed fractures of the tibial diaphysis and tibia-fibula after ORIF Plate and Screw at Waled Regional Hospital, with an average healing of 44.69 weeks for closed fractures of the tibial diaphysis, whereas for fractures closed tibia-fibula diaphysis 33.74 weeks.   Conclusion: The average healing time for closed tibial diaphyseal fractures is longer than for tibia-fibula.
