Abstract

Latar Belakang : Pandemi COVID-19 menjadi masalah global. Di Indonesia angka prevalensi COVID-19 terus meningkat. COVID-19 merupakan penyakit yang disebabkan oleh virus SARS-CoV-2. SARS-CoV-2 menggunakan ACE2 sebagai reseptornya untuk masuk ke dalam sel. Maka dari itu, penelitian ini bertujuan untuk melihat hubungan karakteristik berupa usia, jenis kelamin dan gejala dengan kejadian COVID-19 di Sumatera Barat Metode : Case Control Study dengan sampel berupa spesimen swab nasofaring Laboratorium Pusat Diagnostik dan Riset Penyakit Infeksi (LPDRPI) yang diisolasi dan diperoleh isolat RNA virus. Isolat yang diperoleh kemudian diamplifikasi dengan metode qRT-PCR. Hasil : Tidak ada hubungan antara karakteristik jenis kelamin dengan kejadian COVID-19 dimana nilai P value yaitu 0,485 dan lebih besar dari 0,05 (Ho ditolak). Sedangkan terdapat hubungan antara karakteristik usia dan gejala terhadap kejadian COVID-19 dimana nilai P value yaitu 0,000 dan 0,036 yang lebih kecil dari 0,05 (Ho diterima). Kesimpulan : Laki-laki maupun perempuan memiliki probabilitas yang sama untuk terinfeksi COVID-19. Kelompok usia <50 tahun lebih berisiko terinfeksi COVID-19 daripada kelompok usia ≥50 tahun. Lebih banyak orang tanpa gejala daripada orang dengan gejala yang terinfeksi COVID-19.

Abstract

Gastroesophageal reflux disease (GERD) and erosive esophagitis (EE) are common gastrointestinal disorders typically managed with proton pump inhibitors (PPIs). Although effective, PPIs have certain limitations, prompting the development of a new class of acid-suppressing agents known as potassium-competitive acid blockers (P-CABs). These agents offer the potential for faster and possibly more effective treatment compared to PPIs. This literature review aims to evaluate the efficacy, safety, and potential cost-effectiveness of P-CABs versus PPIs in the treatment of GERD and EE, based on five recent clinical trials. This review includes randomized controlled clinical trials investigating four types of P-CABs: vonoprazan, linaprazan glurate, fexuprazan, and tegoprazan, based on five source articles. The findings indicate that all P-CABs demonstrate efficacy that is comparable to or greater than that of PPIs, with a faster onset of action, favorable safety profiles, and promising cost-effectiveness, particularly in severe cases of EE. These results suggest that P-CABs are a promising alternative therapy for GERD and EE, supported by strong clinical evidence across various treatment scenarios. This review offers a novel contribution by summarizing five recent clinical trials on four P-CAB agents. Addressing the lack of comparative evaluations on their efficacy, safety, and cost-effectiveness, especially in PPI-resistant GERD and EE cases, this review supports more informed clinical decision-making.

Abstract

Anak dengan cerebral palsy (CP) membutuhkan perawatan jangka panjang dan dukungan dari orang tua, terutama ibu sebagai caregiver utama. Di Kota Padang, Sumatera Barat, Yayasan Rumah Gadang Cerebral Palsy (Raga CP) menjadi wadah pendampingan anak dengan cerebral palsy, tapi layanan yang tersedia masih memiliki keterbatasan. Kegiatan pengabdian kepada masyarakat ini bertujuan untuk memperkuat peran ibu dalam perawatan secara holistik anak dengan cerebral palsy melalui pendekatan edukasi komprehensif berbasis komunitas. Metode pelaksanaan terdiri dari lima tahap, yaitu studi pendahuluan, identifikasi masalah, pengembangan program, penerapan program, dan evaluasi program menggunakan model evaluasi Kirkpatrick pada level 1 (reaction) dan level 2 (learning). Edukasi melibatkan tim multidisiplin dari Fakultas Kedokteran Universitas Andalas dan disampaikan melalui ceramah interaktif, diskusi, dan praktik relaksasi. Hasil evaluasi menunjukkan peningkatan pengetahuan peserta serta respon positif terhadap keseluruhan pelaksanaan kegiatan. Kegiatan pengabdian kepada masyarakat ini menunjukkan bahwa edukasi holistik berbasis komunitas dapat dilakukan untuk penguatan peran ibu dalam perawatan anak cerebral palsy serta kemitraan dengan komunitas dalam menjangkau sasaran secara tepat dan berkelanjutan.

Abstract

Polycystic ovary syndrome (PCOS) is a common endocrine disorder with complex reproductive and metabolic consequences that are often not fully addressed by symptom-oriented management strategies. Increasing evidence shows that metabolic dysfunction, particularly insulin resistance and hyperinsulinemia, plays a central role in the pathophysiology of PCOS and contributes to both reproductive and systemic abnormalities. Objective: This study aimed to review and synthesize recent evidence on the metabolic basis of PCOS, with particular emphasis on the role of insulin resistance and hyperinsulinemia as key mechanisms underlying metabolic and reproductive disturbances. Method: This study used a narrative review design by analyzing recent peer-reviewed literature related to insulin signaling disturbances, mitochondrial dysfunction, oxidative stress, inflammatory pathways, and emerging therapeutic approaches in PCOS. The selected studies were examined descriptively to identify major pathophysiological patterns and current treatment perspectives. Findings: The review found that insulin resistance and hyperinsulinemia are major drivers of PCOS across various phenotypes, including non-obese women. Mitochondrial dysfunction and chronic inflammation were also shown to aggravate metabolic and reproductive impairments. In addition, the findings indicate that improvements in metabolic parameters do not always correspond to normalization of reproductive hormones, highlighting the heterogeneous and multifactorial nature of PCOS. Implications: These findings have important implications for clinical practice by supporting the development of personalized and mechanism-based management strategies that integrate metabolic profiling with reproductive goals. Such an approach may improve the effectiveness of long-term treatment and patient outcomes. Originality: The originality of this review lies in its integrative metabolic–reproductive framework, which brings together recent mechanistic insights and emerging therapeutic perspectives to reconceptualize PCOS as a systemic metabolic–reproductive disorder rather than a purely reproductive condition.
