Abstract

Globally and in Indonesia, diabetes mellitus (DM) is mainly type 2. When outpatient care fails to control blood glucose, hospitalization treatment becomes necessary. This is in line with the implementation of the referral system in the era of the National Health Insurance. This study aims to analyze the utilization of hospitalization among patients diagnosed with Type 2 DM (T2DM). A cross-sectional design was applied using sample data from BPJS Kesehatan Year 2024, representing 2023 visit records. All patients admitted with T2DM were included. Inpatient utilization measured by admission frequency, intervals, length of stay, referral sources, diagnostic consistency, discharge status, and comorbidities among T2DM patients. Descriptive analysis was performed on patient characteristics and hospitalization utilization patterns. In 2023, there were 480 admissions involving 417 patients, mean interval between hospitalizations was 69 days. The length of hospital stay increased with the presence of complications. Most referrals came from hospitals, 66% of primary diagnoses matched the admission diagnosis, and hypertension was the most common comorbidity. Most discharge status was recovered. Recommendations include enhancing continuity of care and the referral back program through system integration to mitigate T2DM readmission risks.

Abstract

Abstract: Claim files that are returned / pending can result in a decrease in hospital income due to a mismatch in service costs with the number of claims billed. The results of submitting inpatient claims at Sentra Medika Cibinong Hospital in September-November 2023 were 327 (10%) pending files out of 3,250 files billed. The purpose of the study was to determine the effect of coding accuracy on the results of submitting BPJS Health inpatient claims. With a quantitative approach and cross sectional design. The population was all inpatient claim files, a sample of 300 claim files with the Accidental Sampling technique. Data analysis using chi square test and observation data collection. The results showed inaccurate coding files 116 (38.7%) and accurate coding 184 (61.3%) and pending files 49 (16.3%) and not pending 251 (83.7%). While the results of the chi square test obtained a P-value of 0.002 <0.05 OR value of 2.733 It was concluded that there was an influence between coding accuracy on the results of submitting BPJS Health inpatient claims at Sentra Medika Cibinong Hospital. Furthermore, inaccurate coding accuracy files are 2.7 times more likely to be pending than accurate files. Accurate coding files were pending 20 (10.9%) and not pending 164 (89.1%), inaccurate coding files were pending 29 (25%) and not pending 87 (75%). Suggestions for conducting regular evaluations of cases that have the potential to experience pending claims. Keyword: Coding Accuracy, Submission Results, Pending Claims Abstrak: Berkas klaim yang dikembalikan/pending dapat mengakibatkan penurunan pendapatan rumah sakit akibat ketidaksesuaian biaya layanan dengan jumlah klaim yang ditagihkan. Hasil pengajuan klaim rawat inap di RS Sentra Medika Cibinong pada bulan September–November 2023 Sebanyak 327 (10%) berkas pending dari 3.250 berkas yang ditagihkan. Tujuan penelitian untuk mengetahui pengaruh akurasi koding terhadap hasil pengajuan klaim rawat inap BPJS Kesehatan. Dengan pendekatan kuantitatif dan desain cross sectional. Populasi seluruh berkas klaim rawat inap pada bulan Maret 2024, sampel 300 berkas klaim dengan teknik Accidental Sampling. Analisa data menggunakan uji chi square dan pengumpulan data observasi. Hasil penelitian menunjukan berkas koding tidak akurat 116 (38,7%) dan koding akurat 184 (61,3%) serta berkas terpending 49 (16,3%) dan tidak terpending 251 (83,7%). Sedangkan hasil uji chi square diperoleh nilai P-value 0,002 < 0,05 nilai OR 2,733 Maka disimpulkan ada pengaruh antara akurasi koding terhadap hasil pengajuan klaim rawat inap BPJS Kesehatan di RS Sentra Medika Cibinong. Selanjutnya berkas akurasi koding tidak akurat berpeluang 2,7 kali akan terpending dibandingkan berkas yang akurat. Berkas koding akurat yang terpending 20 (10,9%) dan tidak terpending 164 (89,1%), berkas koding tidak akurat yang terpending 29 (25%) dan tidak terpending 87(75%). Saran melakukan evaluasi secara berkala terhadap kasus yang berpotensi mengalami pending klaim. Kata kunci: Akurasi Koding; Hasil Pengajuan; Pending Klaim

Abstract

Abstract: Hospitals are required to minimize errors made by medical and non-medical staff to build patient trust in the services provided. Data show that patient visits at Jakarta Islamic Hospital Pondok Kopi from January to August 2023 reached 36,188 with an average of 4,523 visits per month, with the lowest number recorded in April 2023. This study aims to describe the influence of outpatient trust in doctors’ services on their interest in repeat visits. The research employed a quantitative approach with a cross-sectional design, involving 100 non-BPJS outpatients at the Outpatient Unit, with data collected through questionnaires. Data were analyzed using a Simple Logistic Regression test. The findings indicate that patients trust doctors’ services and are interested in returning for subsequent visits. Bivariate analysis revealed that outpatient trust in doctors’ services simultaneously had a significant effect on repeat visit interest, as shown by the wald test result (f count 11.170 > f table 1.984467) with a significance level of 0.001 < 0.05. Thus, patient trust plays a crucial role in fostering loyalty through repeat visits at Jakarta Islamic Hospital Pondok Kopi. Keyword: Patient Trust, Doctor Services, Repeat Visits, Hospital, Outpatient Abstrak: Rumah sakit dituntut untuk meminimalkan kesalahan tenaga medis maupun nonmedis agar tercipta kepercayaan pasien terhadap pelayanan yang diberikan. Data menunjukkan kunjungan pasien di Rumah Sakit Islam Jakarta Pondok Kopi pada Januari–Agustus 2023 sebanyak 36.188 orang dengan rata-rata 4.523 kunjungan per bulan, di mana angka terendah terjadi pada April 2023. Penelitian ini bertujuan menggambarkan pengaruh kepercayaan pasien rawat jalan terhadap pelayanan dokter dalam meningkatkan minat kunjungan ulang. Penelitian menggunakan pendekatan kuantitatif dengan desain cross sectional, melibatkan 100 responden pasien non-BPJS di Unit Rawat Jalan, dan data dikumpulkan melalui kuesioner. Analisis dilakukan menggunakan uji Regresi Logistik Sederhana. Hasil penelitian menunjukkan bahwa pasien menaruh kepercayaan pada pelayanan dokter dan berminat melakukan kunjungan ulang. Analisis bivariat mengungkapkan kepercayaan pasien secara simultan berpengaruh signifikan terhadap minat kunjungan ulang, dibuktikan dengan hasil uji wald (f hitung 11.170 > f tabel 1.984467) pada tingkat signifikansi 0.001 < 0.05. Dengan demikian, kepercayaan pasien menjadi faktor penting dalam membangun loyalitas melalui kunjungan ulang di Rumah Sakit Islam Jakarta Pondok Kopi. Kata kunci: Kepercayaan Pasien, Pelayanan Dokter, Minat Kunjungan Ulang, Rumah Sakit, Rawat Jalan

Abstract

Abstract: Workload is the accumulation of tasks and responsibilities that must be carried out by employees or staff in accordance with the targets set by their supervisors within a specific period of time. The transition from a manual system to an electronic system has the potential to affect the workload of Medical Record and Health Information (MRHI) personnel. This study aims to measure the workload of MRHI staff after the implementation of Electronic Medical Records (EMR) in the Medical Records Department, using job analysis and workload analysis methods based on Regulation of the Ministry of Administrative and Bureaucratic Reform (KEMENPAN RB) No. 1 of 2020. This is a descriptive study with a purposive sampling approach, involving 10 out of 34 MRHI staff with a D3-level education. Data were collected through interviews and direct observation.The results show that the workload for new outpatient registration is 4 minutes, returning outpatients 3 minutes, inpatients 13.4 minutes, and self-registration via APM 5 minutes—requiring 16 staff. Assembling takes 10 minutes per medical record, requiring 2 staff; outpatient coding takes 1 minute and inpatient coding 3.5 minutes per record, requiring 3 coders. Filing and distribution take 6 and 5 minutes per record respectively, requiring 6 staff. Retention and digitization of active and inactive records take 10 minutes per record, requiring 5 staff. Reporting requires 15,240 minutes in total, with 3 reporting staff. Analysis of EMR completeness requires 15 minutes per record, needing 2 expert MRHI staff, while EMR monitoring requires 3 expert MRHI staff. The total workforce required is 35 skilled MRHI personnel and 5 expert MRHI professionals. Keyword: Electronic Medical Records, Workload, Medical Record and Health Information, Job Analysis Abstrak: Beban kerja merupakan akumulasi tugas dan tanggung jawab yang harus dilaksanakan oleh karyawan atau petugas sesuai dengan target yang telah ditentukan oleh atasan dalam periode waktu tertentu. Transisi dari sistem manual ke sistem elektronik berpotensi mempengaruhi beban kerja Perekam Medis dan Informasi Kesehatan (PMIK) Penelitian ini bertujuan untuk mengukur beban kerja PMIK pasca penerapan RME di Instalasi Rekam Medis menggunakan metode analisis jabatan dan analisis beban kerja sesuai Peraturan KEMENPAN RB No. 1 Tahun 2020. Penelitian ini merupakan penelitian deskriptif dengan pendekatan purposive sampling, melibatkan 10 orang dari total 34 petugas di instalasi rekam medis dengan pendidikan D3 PMIK. Pengumpulan data dilakukan melalui wawancara dan observasi. Hasil pengukuran menunjukkan bahwa beban kerja pendaftaran rawat jalan pasien baru 4 menit,pasien lama 3 menit untuk pasien rawat inap 13,4 menit sedangkan APM 5 menit dibutuhkan sebanyak 16 petugas, Beban kerja assembling 10 menit/BRM dibutuhan 2 tenaga assembling, Beban kerja koding pasien rawat jalan 1 menit , koding rawat inap 3,5 menit/BRM di butuhkan 3 tenaga koding, beban kerja filling 6 menit/ BRM dan distribusi 5 Menit/BRM memerlukan 6 petugas, Beban kerja petugas retensi dan alih media berkas rekam medis in akif dan aktif 10 menit/BRM dan dibutuhkan 5 tenaga, Beban kerja pelaporan 15.240 menit kebutuhan tenaga pelaporan 3 petugas , beban kerja analisa kelengkapan rekam medis elektronik 15 menit/RME di butuhkan 2 tenaga PMIK ahli dan monitoring rekam medis elektroni k sebanyak 3 tenaga PMIK ahli, total kebutuhan tenaga PMIK adalah 35 tenaga PMIK terampil dan 5 orang PMIK ahli. Abstract

One of the factors that plays an important role in maintaining the security of EMR data is the compliance of medical record officers with applicable security procedures. However, in using EMR there are several obstacles such as a lack of skills in using technology so that officers have a dependent attitude in carrying out their duties and limited training provided. This study aims to determine the influence of officer attitudes towards compliance with maintaining the security of electronic medical record data at Tarakan Regional Hospital. The type of research used is quantitative research with a cross-sectional design. The population in this study were all 44 medical record officers. Data collection used a questionnaire with a Likert scale. The univariate data analysis technique used the three box method and bivariate data analysis using the Chi Square test. The results showed that overall the average index of all officer attitude indicators fell into the high (positive) category. In the compliance variable, the average index of all indicators fell into the high (compliant) category. The results of the Chi Square statistical test showed a significant influence between officer attitudes towards compliance with maintaining the security of EMR data at Tarakan Regional Hospital (p-Value 0.000 <0.05).
