Abstract

Background: E-cigarettes are increasingly popular among adolescents, driven by massive advertising on the internet, especially through social media. These advertisements often display modern and attractive images without including sufficient information about health risks. Social media influencers also play a role in strengthening the appeal of e-cigarettes to adolescents. The purpose of this study is to analyze the relationship between exposure to e-cigarette advertising on the internet and smoking behavior among adolescents in Indonesia.Method: This study used a cross-sectional design with an online survey conducted over three months, from March to June 2024. A total of 625 adolescents aged 14–19 years participated in this study. Respondents reported exposure to e-cigarette advertising on the internet, which was analyzed by age, gender, social media used, and smoking status. Data were analyzed using logistic regression to see the relationship between advertising exposure and smoking behavior.Result The results showed that adolescents who were more frequently exposed to e-cigarette advertising were 2.3 times more likely to try e-cigarettes than those who rarely saw these advertisements. Instagram and YouTube were the main platforms where adolescents were exposed to e-cigarette advertisements. In addition, 50% of respondents reported seeing influencers using e-cigarettes, and 40.2% of them were interested in trying them. Most e-cigarette advertisements were also found not to include clear health warnings. This study confirms that exposure to e-cigarette advertisements on social media plays a significant role in increasing smoking interest and behavior in adolescents. Therefore, stricter regulations are needed for e-cigarette marketing on the internet, including supervision of the role of influencers and the obligation to include health warnings in every advertisement. In addition, broader health education is needed to increase adolescent awareness of the risks of e-cigarettes.

Abstract

Perkembangan media digital meningkatkan akses remaja terhadap informasi kesehatan, namun juga memperbesar risiko paparan hoaks kesehatan melalui media sosial. Rendahnya literasi kesehatan digital menyebabkan remaja rentan mempercayai dan menyebarkan informasi kesehatan yang tidak valid. Kegiatan pengabdian kepada masyarakat ini bertujuan meningkatkan kemampuan remaja dalam memverifikasi informasi kesehatan digital melalui pelatihan berbasis praktik. Kegiatan dilaksanakan pada siswa kelas XI dan XII di salah satu SMA Kota Malang dengan melibatkan 29 peserta. Metode pelaksanaan menggunakan pendekatan edukatif partisipatif melalui edukasi interaktif, simulasi kasus hoaks kesehatan, praktik verifikasi informasi menggunakan sumber resmi, diskusi kelompok, dan mini-proyek pembuatan media edukasi digital. Evaluasi dilakukan menggunakan pre-test, post-test, dan kuesioner kepuasan peserta. Hasil kegiatan menunjukkan peningkatan rata-rata nilai peserta dari 56,3 menjadi 90,1 setelah intervensi, dengan seluruh peserta (100%) mencapai nilai di atas standar ketuntasan. Peserta juga menunjukkan peningkatan kemampuan dalam mengidentifikasi sumber informasi terpercaya dan melakukan verifikasi informasi kesehatan digital. Kegiatan ini menunjukkan bahwa pelatihan berbasis praktik verifikasi informasi efektif dalam meningkatkan literasi kesehatan digital remaja serta berpotensi menjadi model edukasi preventif terhadap penyebaran hoaks kesehatan di lingkungan sekolah.   Abstract  The development of digital media has increased access to health information among adolescents, but on the other hand, it has also increased the risk of spreading health hoaxes through social media. Low digital health literacy skills make adolescents vulnerable to receiving and spreading invalid health information. This community service activity aims to improve adolescents' ability to filter and verify health information through digital health literacy training. The activity was held at School in Malang involving 29 students in grades 11 and 12. The implementation method used a participatory educational approach through interactive counseling, case simulations, health information verification practices, group discussions, and mini-projects on creating digital educational media. The activity was evaluated using pre- and post-tests, as well as a participant satisfaction questionnaire. The results showed an increase in the average participant score from 56.3 in the pre-test to 90.1 in the post-test. All participants (100%) achieved scores above the completion standard after the activity. The satisfaction evaluation also indicated that participants found the activity beneficial and helped improve their critical thinking skills regarding health information on social media. Health information verification training has proven effective in improving adolescents' digital health literacy and can be an educational strategy to prevent the spread of health hoaxes in schools.
