Abstract

Fetal hydrocephalus is a congenital abnormality, which means the fetus is born with it, but babies and children can also experience it. Hydrocephalus pregnancy can trigger premature labor and can cause fetal distress. A 30 year old mother, G3P2A0, gestation age 34 weeks 4 days, was referred to Wonosari Hospital with diagnosis of Partus Prematurus Iminens (PPI), fetal hydrocephalus, and fetal distress. Based on the history of pregnancy examination at 24 weeks, the Biparietal Diameter (BPD) size of the fetal head was larger than the normal size, (5.94 cm), the fetus indicated ventriculomegaly, and on the third trimester ultrasound examination (32 weeks) the Biparietal Diameter (BPD) size is 9.49 cm so that the fetus is indicated for congenital hydrocephalus. The baby was born spontaneously, male, birth weight 2000 grams, body length 38 cm, head circumference 34 cm, chest circumference 24 cm, upper arm circumference 10 cm, born without crying, negative muscle tone, pale bluish skin color. From the results of this examination, it was confirmed that he was suffering from congenital hydrocephalus based on the physical appearance of the head, which was larger than the normal head size of premature babies in general, and dead..

Abstract

Latar belakang: Gestational Diabetes Mellitus (GDM) adalah kondisi intoleransi glukosa yang terdiagnosis selama kehamilan, sering kali muncul pada trimester kedua atau ketiga. Meskipun dapat menandakan diabetes tipe 1 atau 2 yang tidak terdeteksi sebelumnya, GDM umumnya disebabkan oleh intoleransi glukosa sementara terkait kehamilan. Wanita hamil dengan faktor risiko seperti obesitas, riwayat keluarga diabetes, atau melahirkan bayi besar perlu dicurigai mengalami GDM. Jika tidak ditangani, GDM dapat menyebabkan komplikasi serius bagi ibu dan janin, termasuk preeklamsia dan makrosomia. Di Indonesia, prevalensi GDM diperkirakan mencapai 1,9% hingga 5% dari total kehamilan. Penelitian ini bertujuan: untuk mengevaluasi manajemen GDM di Primary Health Centre wilayah Kota Yogyakarta dengan pendekatan deskriptif fenomenologi. Metode: Data dikumpulkan melalui wawancara semi-terstruktur dengan lima bidan yang memberikan pelayanan Kesehatan Ibu dan Anak. Hasil: analisis menunjukkan dua tema utama yaitu pendekatan perubahan perilaku dan strategi reorientasi pelayanan kesehatan masyarakat. Kesimpulan: Manajemen yang baik melalui deteksi dini dan penanganan yang tepat dapat meningkatkan kesehatan ibu hamil dan mengurangi risiko komplikasi. Saran: skrining awal dan pengaturan gaya hidup penting untuk dilakukan selama kehamilan. Kata Kunci: Bidan, Gestasional Diabetes Mellitus (GDM), Manajemen ABSTRACT Background: Gestational Diabetes Mellitus (GDM) is a condition of glucose intolerance diagnosed during pregnancy, often presenting in the second or third trimester. Although it can signify previously undetected type 1 or 2 diabetes, GDM is generally caused by pregnancy-related temporary glucose intolerance. Pregnant women with risk factors such as obesity, family history of diabetes, or giving birth to a giant baby need to be suspected for GDM. Unsolved GDM, lead to serious complications for both mother and fetus, including preeclampsia and macrosomia. In Indonesia, prevalence of GDM is estimated at 1.9% to 5% of total pregnancies. This study aimed to evaluate the management of GDM at Primary Health Centre in Yogyakarta City using a descriptive phenomenological approach. Methods: data were collected through semi-structured interviews with five midwives who provide Maternal and Child Health services. The results: of the analysis showed two main themes, namely the behavior change approach and the strategy of reorienting public health services. Good management through early detection and appropriate treatment can improve the health of pregnant women and reduce the risk of complications. Suggestions: early screening and lifestyle regulation is important to conduct during pregnancy. Key words: Midwife, Gestational Diabetes Mellitus (GDM), Management

Abstract

Background: The World Health Organization (WHO) estimates that 15% to 20% of all births worldwide are low birth weight (LBW) births, representing more than 20 million per year. LBW causes asphyxia, hypothermia, infection and growth and development disorders. This study aims to see the extent of the risk of growth and development disorders in children with a history of LBW. Method: Methods in this study used an observational retrospective cohort design. The affordable population in this study were children aged 1-2 years in the working area of Puskesmas Wates. For children who had a history of LBW samples taken by purposive sampling and for children who had a history of normal birth weight samples taken by accidental sampling with samples of 70 respondents. Data obtained from KIA book, KMS card and the results of measuring results development used KPSP. Data analysis used chi-square test. Results: 65.7% of children aged 1-2 years with a history of LBW experienced growth and development disorders, while 28.6% of children aged 1-2 years with a history of LBW experienced growth disorders and 31.4% experienced development disorders. Statistical tests obtained a p-value of growth = 0.002 and relative risk (RR) = 2.3 (95% CI 1.293-4.091). Statistical tests obtained a p-value of development = 0.004 and relative risk (RR) = 2.1 (95% CI 1.213-3.605). Conclusion: The conclusion of this study shows that children aged 1-2 years with a history of LBW are 2.3 times more likely to experience growth disorders and 2.1 times more likely to experience developmental disorders.

Abstract

Abstract Background: The condition of pregnant women with chronic energy deficiency (CED) could cause bad influence on mothers and children such as delivering infant with low birth weight (LBW). In 2020 the proportion of CED in Surakarta was 4,16% and experienced increase compared to 2019. The highest LBW case in Surakarta was obtained in work area of Pajang Pajang Health Center with case proportion 2,8%. The CED status is included into maternal factor of LWB because of insufficient nutrition of mothers and infants.This study aim to find out the status of CED on pregnant women towards the risk of LBW. Method: Analytic observational research with case control design retrospective approach using secondary data of Pajang Public Health Center in 2020. The research samples consisted of case sample with total sampling technique and control sample comparison 1:2 until obtained case sample 25 and control sample 50. Results: The result of chi-square showed that p-value 0.005 and odd ratio = 4,935. Conclusion: Pregnant women with CED has risk of 4,935 times higher to give birth with LBW compared to pregnant women with no CED.

Abstract

Anemia pada kehamilan merupakan masalah kesehatan global yang berdampak terhadap ibu dan janin, dengan prevalensi tinggi di negara berkembang. Salah satu intervensi yang digunakan adalah suplementasi zat besi, namun keterbatasan kepatuhan dan efek samping memerlukan alternatif terapi berbasis herbal. Sauropus androgynus (daun katuk) dikenal kaya akan zat besi dan senyawa bioaktif yang berpotensi meningkatkan kadar hemoglobin. Studi ini bertujuan untuk menganalisis efektivitas pemberian Sauropus androgynus terhadap peningkatan kadar Hb pada ibu hamil melalui pendekatan meta-analisis. Data diperoleh dari pencarian sistematis terhadap database Google Scholar, PubMed, dan Researchgate dari tahun 2021-2025, dengan kriteria inklusi studi eksperimental, populasi ibu hamil, serta intervensi menggunakan Sauropus androgynus. Sebanyak 10 artikel memenuhi kriteria dan dianalisis menggunakan software RevMan 5.4. Hasil meta-analisis menunjukkan bahwa pemberian Sauropus androgynus secara signifikan meningkatkan kadar hemoglobin (SMD=1.59; 95% CI= 0.99-2.18; p<0.001).

Abstract

Infant mortality remains a major public health challenge in Indonesia, with rates still exceeding the Sustainable Development Goals (SDGs) target. Digital health innovations, such as TeleCTG, have been introduced to improve accessibility and quality of maternal care by enabling remote fetal monitoring. This study aimed to evaluate the effectiveness of TeleCTG in reducing neonatal morbidity and mortality. A retrospective cohort design was conducted using secondary data from Puskesmas Pakutandang, involving 78 pregnant women at ?28 weeks of gestation or with risk factors. The exposure group consisted of 34 mothers who underwent TeleCTG examinations, while the non-exposure group included 44 mothers who received routine antenatal care without TeleCTG. Infant outcomes, including mortality and Apgar scores at 1/5/10 minutes, were analyzed using Fisher’s Exact Test and logistic regression with a 95% confidence interval. The results showed a significant association between TeleCTG use and reduced infant mortality (p=0.033; RR 1.158, 95% CI 1.030–1.302), while no significant effect was observed on Apgar scores <7 at 1/5/10 minutes (p=0.065). In conclusion, TeleCTG has the potential to lower infant mortality and support maternal health services, although further research with intrapartum monitoring is warranted to strengthen evidence of its effectiveness.

Abstract

Background: Prostate cancer is one of the most common malignancies among men worldwide. Prostate-specific antigen (PSA) testing and prostate biopsy are commonly used for diagnosis. However, these methods may be invasive and can lead to delayed detection in some cases. This study aimed to evaluate the expression of hsa-miR-21-5p in urine samples from prostate cancer patients compared with healthy individuals to assess its potential as a non-invasive biomarker.Methods: This observational study used a cross-sectional design and was conducted in Yogyakarta, Indonesia, between October and December 2018. Expression profiling of hsa-miR-21-5p was performed using quantitative real-time polymerase chain reaction (qRT-PCR) in urine samples obtained from 12 prostate cancer patients and 8 healthy controls. Relative expression levels were analyzed using GenEx software and calculated using the Livak (2^−ΔΔCt) method.Results: The age of prostate cancer patients ranged from 60 to 83 years, whereas the healthy control group had a mean age of 53.88 ±2.59 years (range 50–58 years). A slight decrease in hsa-miR-21-5p expression was observed in  urine samples from prostate cancer patients compared with healthy controls (fold change=0.7; p>0.05).Conclusion: Expression of hsa-miR-21-5p shows a tendency toward downregulation in urine samples of prostate cancer patients, however, no statistically significant difference is identified. Further studies with larger sample sizes are required to clarify its potential role as a non-invasive biomarker for  prostate cancer detection.

Abstract

Background Jakarta is one of the most polluted cities in Southeast Asia, with particulate matter 2.5 (PM2.5) levels frequently exceeding international standards. Exposure to PM2.5 during pregnancy has been linked to adverse fetal outcomes, including intrauterine growth restriction and low birth weight (LBW). Objective To analyze an association between PM2.5 concentration and the incidence of LBW in Jakarta Province, Indonesia. Methods A retrospective cross-sectional study was conducted using secondary data from community health centers (Puskesmas) located within a 5-km radius of Air Quality Monitoring Stations (AQMS) in five regions of Jakarta: Bundaran Hotel Indonesia (HI), Kelapa Gading, Jagakarsa, Lubang Buaya, and Kebon Jeruk. Results A total of 648 LBW cases and 11,498 normal birth weight (NBW) cases recorded in 2020–2021 were included through total sampling. PM2.5 concentration data were obtained from the Jakarta Environmental Agency. Data were analyzed using univariate, bivariate (Pearson’s correlation), and simple linear regression tests. A significant positive correlation was found between PM2.5 concentration and LBW incidence (r=0.885; P=0.049) in 2020, whereas the 2021 data showed no significant correlation (r=0.871; P=0.055). Regression analysis revealed that every one-µg/m³ increase in PM2.5 concentration was associated with a 0.091% rise in LBW cases. Areas with higher pollution levels, particularly Lubang Buaya and Kebon Jeruk, showed the highest prevalence of LBW. Conclusion There was a significant positive correlation between PM2.5 air pollution and the incidence of LBW in Jakarta, particularly in areas with high levels of pollution, based on 2020 data. These findings underscore the importance of implementing public health interventions and enhancing air quality management to safeguard maternal and neonatal health.

Abstract

Managing hypertension becomes significantly more complex when diabetes mellitus co-occurs, often leading to poorer clinical outcomes and higher healthcare costs. This research investigated the prevalence of diabetes comorbidity among hypertensive patients at advanced referral health facilities and its relationship with patient age. Using a cross-sectional approach, we analyzed 5,505 records from the 2025 Indonesian National Health Insurance (BPJS Kesehatan) sample data for patients presenting with a primary diagnosis of hypertension. The overall prevalence of comorbid diabetes was 9.3% (510 cases). Within this cohort, 57.6% (3,172) were non-elderly (<60 years) and 42.4% (2,333) were elderly (?60 years). Diabetes prevalence was 9.6% in the elderly group and 9.0% in the younger demographic. The Pearson Chi-Square analysis confirmed no significant association between age category and diabetes (p = 0.459). These findings suggest that the risk of comorbid diabetes among referred hypertensive patients remains consistent regardless of age. Therefore, we recommend universal endocrine screening for all hypertensive referrals to prevent delayed diagnosis and improve targeted interventions, rather than relying on age-based screening protocols.

Abstract

Purpose: Digital health innovations like telecardiotocography (TeleCTG) are promoted to strengthen maternal services in primary care, yet their diffusion and effectiveness within Indonesia’s health system remain underexplored. This study analyzed the diffusion of TeleCTG and maternal service outcomes at a primary health center. Methods: A concurrent embedded mixed- methods design was used. Qualitative data were obtained through in-depth interviews with nine stakeholders and were analyzed thematically using Rogers’ diffusion of innovation framework. Quantitative data were obtained from a retrospective cohort of 78 pregnant women (34 TeleCTG exposure; 44 standard care). Bivariate analysis used Chi-square or Fisher’s exact test, and multivariable analysis used a risk-ratio framework with log-binomial or modified Poisson regression, with robust variance estimates when sparse data limited convergence. Results: Four themes emerged from the qualita- tive analysis: innovation characteristics, communication channels, time, and social system. TeleCTG was perceived as advantageous for real-time monitoring and referral support, but its diffusion remained constrained by dependence on governance and policy. Quantitatively, no statistically significant adjusted association was observed between TeleCTG use and infant mortality, cesarean section, low APGAR score at 5–10 minutes, or planned early referral. Sparse events limited estimation for some outcomes, suggesting that TeleCTG functioned more as a service-support tool than as a determinant of measurable clinical outcomes in this sample. Conclusion: Health system readiness, not technology alone, drives TeleCTG adoption. Primary care managers should integrate TeleCTG workflows; district authorities should streamline procurement; and national policymakers should issue adoption guidelines.
