Abstract

Employee performance in hospitals has become a strategic concern amid increasing service demands, high workload, and declining public satisfaction. These challenges underscore the importance of empirically identifying the key determinants of employee performance to support evidence-based managerial interventions. Understanding how organizational and motivational factors influence performance is essential for strengthening hospital service quality and institutional sustainability. This study aimed to analyze the effects of competency development, leadership style, work facilities, and work motivation on employee performance at hospital. A quantitative study with a cross-sectional design was conducted. Data were collected from 210 hospital employees using a stratified random sampling technique to ensure proportional representation across work units. The data were analyzed using Partial Least Squares Structural Equation Modeling (PLS-SEM) to examine both direct and total effects among variables within the proposed structural model. The analysis revealed that leadership style was the most dominant determinant of employee performance, with a total effect value of 0.497 (p = 0.000). Competency development emerged as the second strongest determinant, with a total effect of 0.338 (p = 0.000). Work motivation also had a statistically significant contribution to employee performance, with a total effect of 0.220 (p = 0.002). In contrast, work facilities demonstrated the smallest total effect on performance (0.142; p = 0.006). These findings indicate that leadership quality and employee competency levels exert stronger influences on performance compared to structural factors such as facilities. As conclusion, employee performance is primarily determined by leadership quality and competency development, with work motivation playing a supportive role, while work facilities have a comparatively limited influence. A performance determinant framework grounded in motivation and organizational dynamics provides a robust foundation for designing hospital performance improvement policies.Keywords: hospital management; employee; competency development; leadership style; work motivation; performance

Abstract

First-stage labor pain is a significant challenge in maternity nursing care because it is multidimensional and can increase stress, prolong labor, and reduce maternal comfort if not optimally managed. A nursing theory approach is an important basis for safe, holistic, and appropriate non-pharmacological interventions for the mother. This study aims to illustrate the urgency of effective pain management, identify nursing theories used in non-pharmacological interventions, and analyze their effectiveness and existing research gaps. This study is a literature review, with articles searched through PubMed, Scopus, ScienceDirect, ProQuest, CINAHL, Google Scholar, and Garuda based on publications from 2019–2024. A total of 15 articles met the criteria and were analyzed thematically. The review results indicate that frequently used theories include Gate Control, Comfort Kolcaba, Adaptation Roy, Self-care Orem, Caring Watson, and Transcultural Leininger. Various non-pharmacological interventions such as massage, acupressure, cold compresses, hydrotherapy, aromatherapy, music therapy, guided imagery, breathing techniques, progressive muscle relaxation, the use of birth balls, and partner support have been shown to help reduce pain, reduce anxiety, and increase maternal comfort and satisfaction. However, most research focuses on short-term effects and has not evaluated the application of nursing theory in the context of clinical practice. This study concluded that non-pharmacological interventions based on nursing theory are consistently effective in reducing first-stage labor pain and increasing maternal comfort.Keywords: labor pain; nursing theory; non-pharmacological interventions; maternity nursing care ABSTRAK Nyeri persalinan kala I merupakan tantangan penting dalam asuhan keperawatan maternitas karena bersifat multidimensional dan dapat meningkatkan stres, memperpanjang kala persalinan, serta menurunkan kenyamanan ibu bila tidak ditangani secara optimal. Pendekatan teori keperawatan menjadi dasar penting bagi intervensi nonfarmakologis yang aman, holistik, dan sesuai kebutuhan ibu. Studi ini bertujuan untuk menggambarkan urgensi manajemen nyeri yang efektif, mengidentifikasi teori keperawatan yang digunakan dalam intervensi nonfarmakologis, serta menganalisis efektivitas dan kesenjangan penelitian yang ada. Studi ini merupakan tinjauan literatur, dengan pencarian artikel melalui PubMed, Scopus, ScienceDirect, ProQuest, CINAHL, Google Scholar, dan Garuda berdasarkan publikasi tahun 2019–2024. Sebanyak 15 artikel memenuhi kriteria dan dianalisis secara tematik. Hasil review menunjukkan bahwa teori yang sering digunakan meliputi Gate Control, Comfort Kolcaba, Adaptasi Roy, Self-care Orem, Caring Watson, dan Transcultural Leininger. Berbagai intervensi nonfarmakologis seperti pijat, akupresur, kompres dingin, hidroterapi, aromaterapi, terapi musik, guided imagery, teknik pernapasan, relaksasi otot progresif, penggunaan birth ball, dan dukungan pasangan terbukti membantu menurunkan nyeri, mengurangi kecemasan, serta meningkatkan kenyamanan dan kepuasan ibu. Namun, sebagian besar penelitian masih berfokus pada efek jangka pendek dan belum banyak mengevaluasi penerapan teori keperawatan dalam konteks praktik klinis. Studi ini menyimpulkan bahwa intervensi nonfarmakologis berbasis teori keperawatan secara konsisten efektif dalam menurunkan nyeri persalinan kala I dan meningkatkan kenyamanan ibu.Kata kunci: nyeri persalinan; teori keperawatan; intervensi nonfarmakologis; asuhan keperawatan maternitas

Abstract

Self-medication refers to the practice by which individuals use medicines obtained without a physician’s prescription to treat their symptoms or pain. However, errors in medication use remain common in self-medication practices; therefore, pharmacists play an important role in providing drug information. This study aimed to analyze the factors influencing the provision of drug information for self-medication by pharmacists. A cross-sectional study was conducted involving 100 pharmacists in Pekanbaru, Indonesia. Data were collected through participant observation and analyzed using t-tests and ANOVA. The analysis showed that the p-values for each candidate determinant were as follows: gender = 0.287, work experience = 0.014, secondary employment = 0.222, age = 0.022, education = 0.191, weekly working hours = 0.000, and daily working hours = 0.000. It can therefore be concluded that the determinants of pharmacists’ provision of drug information in self-medication are age, work experience, and working hours.Keywords: pharmacist; drug information provision; self-medication

Abstract

Pregnancy-related discomfort is associated with decreased quality of life and an increased risk of complications extending to the time of delivery. One complementary midwifery service that serves as a promotive and preventive non-pharmacological intervention to reduce such discomfort is prenatal yoga, which is influenced by factors including knowledge, family support, and the role of healthcare professionals. This study aimed to analyze the determinants of pregnant women’s participation in prenatal yoga activities at an independent midwifery practice. A cross-sectional design was employed, involving 24 pregnant women who attended antenatal care. Data were collected using a structured questionnaire and analyzed using Chi-square test. The results indicated that the prevalence of participation in prenatal yoga was relatively high, at 70.8%. The p-values for each determinant were as follows: knowledge (p = 0.001), family support (p = 0.020), and the role of healthcare professionals (p = 0.007). It can therefore be concluded that knowledge, family support, and the role of healthcare professionals are positive determinants of pregnant women’s participation in prenatal yoga.Keywords: knowledge; family support; role of healthcare professionals; prenatal yoga

Abstract

Nurses working in hospital emergency departments are healthcare professionals at high risk of experiencing distress due to intense work pressure, emotional burden, and unpredictable working conditions. This situation may affect nurses’ mental and physical health as well as their performance. Therefore, effective coping mechanisms are required to manage distress, maintain well-being, and ensure the quality of healthcare services. This study aimed to analyze the role of coping mechanisms in addressing distress experienced by nurses in hospital emergency departments. The study employed a systematic literature review approach. Articles were searched through the Google Scholar and ScienceDirect databases using the keywords coping mechanism, distress, nurse, and hospital emergency unit. From a total of 48,117 articles identified, selection was conducted based on the following inclusion criteria: (1) articles published in English or Indonesian, (2) published between 2020 and 2025, and (3) discussing coping mechanisms and distress among emergency department nurses. The selection process consisted of identification, eligibility screening, and full-text review. The final results yielded 12 articles that met the criteria and were analyzed based on research design, methods, measurement instruments, and the alignment between study objectives and findings. From these 12 articles, various coping strategies were identified as effective in reducing distress, including coping-related training and education, psychological interventions, mindfulness, spiritual coping, social support, and social behavioral changes. Several interventions also focused on organizational improvements and the development of compassion skills. In conclusion, coping mechanisms play an important role in reducing distress among emergency department nurses through both individual- and organizational-oriented interventions. The implementation of continuous training and support is essential to strengthen nurses’ coping capacity, create a healthy work environment, and improve the quality of hospital services.Keywords: coping mechanism; distress; nurses; emergency department

Abstract

The quality of nursing care in hospitals is influenced by multiple organizational and psychological factors, particularly the implementation of nursing management and the presence of burnout syndrome among nurses. Effective nursing management ensures coordinated care delivery, while burnout may reduce professional performance and service responsiveness. This study aimed to analyze the implementation of nursing management and the effect of burnout syndrome on the quality of nursing care. A cross-sectional design was employed involving 98 respondents. Data were collected using structured questionnaires, and hypotheses were tested using multiple linear regression analysis to examine the simultaneous and partial effects of nursing management and burnout syndrome on nursing service quality. The analysis demonstrated that nursing management had a significant effect on the quality of nursing care (p = 0.027; t = 2.242). Burnout syndrome also showed a significant effect on nursing service quality (p = 0.005; t = 2.899). These findings indicate that both organizational management practices and psychological conditions significantly influence service quality. As conclusion, nursing management implementation and burnout syndrome are the predictors of nursing care quality. Strengthening managerial practices and addressing nurse burnout are essential strategies to improve hospital service quality.Keywords: nursing management; burnout syndrome; service quality; nurses

Abstract

Depression has been shown to decrease cognitive, emotional, and physical function, thus impacting the quality of life of the elderly. Conversely, social support plays an important role as a protective factor that can reduce the risk of depression and improve the well-being of the elderly. The purpose of this study was to analyze the relationship between depression and social support with the quality of life of the elderly living in nursing homes. This study was a literature review with a search of articles from PubMed, ScienceDirect, Scopus, ProQuest, SpringerLink, and Google Scholar. The keywords used were depression, social support, quality of life, elderly, and nursing home. The selected articles were published between 2019 and 2024 with respondents from elderly in nursing homes. A total of 12 articles met the inclusion criteria and were analyzed narratively and thematically. The review results showed consistent findings that depression was significantly negatively associated with the quality of life of the elderly, with a decrease in scores of up to 30–50% in elderly with moderate-severe depression. Conversely, social support was positively associated with the quality of life, both directly and through a reduction in depressive symptoms. Emotional and family support were reported as the most influential factors. Cross-national studies show similar patterns, although the intensity of the influence varies across cultural and social contexts. These studies conclude that depression is a major risk factor for declining quality of life in older adults, while social support plays a protective role.Keywords: elderly; nursing homes; depression; social support; quality of life ABSTRAK Depresi terbukti menurunkan fungsi kognitif, emosional, dan fisik sehingga berdampak pada penurunan kualitas hidup lansia. Sebaliknya, dukungan sosial berperan penting sebagai faktor protektif yang mampu menurunkan risiko depresi sekaligus meningkatkan kesejahteraan lansia. Tujuan dari studi ini adalah untuk menganalisis hubungan antara depresi dan dukungan sosial dengan kualitas hidup lansia yang tinggal di panti werdha. Studi ini adalah telaah literature review dengan pencarian artikel dari PubMed, ScienceDirect, Scopus, ProQuest, SpringerLink, dan Google Scholar. Kata kunci yang digunakan adalah depression, social support, quality of life, elderly, dan nursing home. Artikel yang dipilih adalah publikasi 2019–2024 dengan responden lansia di panti werdha. Sebanyak 12 artikel memenuhi kriteria inklusi dan dianalisis secara naratif serta tematik. Hasil telaah menunjukkan konsistensi temuan bahwa depresi berhubungan negatif signifikan dengan kualitas hidup lansia, dengan penurunan skor hingga 30–50% pada lansia dengan depresi sedang-berat. Sebaliknya, dukungan sosial berhubungan positif dengan kualitas hidup, baik secara langsung maupun melalui penurunan gejala depresi. Dukungan emosional dan keluarga dilaporkan sebagai faktor paling berpengaruh. Studi lintas negara menunjukkan pola yang serupa, meskipun intensitas pengaruh bervariasi sesuai konteks budaya dan sosial. Studi ini menyimpulkan bahwa depresi menjadi faktor risiko utama penurunan kualitas hidup lansia, sementara dukungan sosial berperan sebagai faktor protektif.Kata kunci: lansia; panti werdha; depresi; dukungan sosial; kualitas hidup

Abstract

Continuity of care in midwifery refers to comprehensive services provided continuously from pregnancy, childbirth, postpartum, breastfeeding, newborn care, and family planning. This study aimed to evaluate the application of oxytocin massage for postpartum mothers within the framework of continuity of care. The study employed a case report design using continuous midwifery management based on scientific evidence and was conducted from August to November 2024. The subject was a pregnant woman at 37 weeks of gestation who was followed until participation in postpartum family planning. The results of care were reported descriptively. During pregnancy, several problems were identified, including sleep disturbance, back pain, overweight status, mild anemia, and Braxton Hicks contractions. Intrapartum care focused on pain management using a birth ball. During the postpartum period (day 12 postpartum), insufficient breast milk production was identified. The intervention provided was oxytocin massage to improve milk production and flow. After the intervention, a significant improvement was observed, indicated by a questionnaire score increasing from 3 (poor milk flow) to 8 (adequate milk flow). In newborn care, weight loss was identified at 12 days of age, and education and counseling on infant nutrition were provided. For family planning care, Mrs. F chose an implant contraceptive method. All identified problems were successfully managed. As conclusion, continuity of care in midwifery combined with the application of oxytocin massage was effective in addressing maternal and neonatal problems, particularly in improving breast milk production and supporting successful breastfeeding outcomes.Keywords: continuity of care; breastfeeding; postpartum period; oxytocin massage

Abstract

Aplastic anemia is a hematological disorder that has complex physiological and psychosocial impacts on patients, thus requiring a holistic nursing care approach. The Callista Roy Adaptation Model conceptualizes individuals as adaptive systems who respond to various stimuli through coping mechanisms in order to achieve equilibrium. This scientific study aimed to describe the application of the Roy Adaptation Model in nursing care for a patient with aplastic anemia accompanied by gastrointestinal bleeding. The study employed a case report design using the nursing process approach, including assessment, diagnosis, planning, implementation, and evaluation based on Roy’s four adaptive modes: physiological–physical, self-concept, role function, and interdependence. The results of nursing care indicated that the management of focal stimuli in the form of bleeding, as well as contextual and residual stimuli influencing the patient’s psychological condition, facilitated more positive adaptive responses. Nursing interventions in the physiological–physical mode contributed to the stabilization of the patient’s condition, while supportive approaches, therapeutic communication, and education in the self-concept mode helped reduce anxiety and improve self-acceptance. Support for role function and interdependence through family involvement strengthened the patient’s adaptation process during treatment. In conclusion, the Roy Adaptation Model is effective as a conceptual framework for nursing care in patients with aplastic anemia, as it integrates physiological, psychological, and social aspects comprehensively.Keywords: aplastic anemia; Roy adaptation model; holistic nursing care; case report; anxiety

Abstract

Invasive breast carcinoma is a malignant neoplasm with metastatic potential influenced by various histopathological parameters, including tumor budding (TB). TB has emerged as a potential indicator of tumor aggressiveness; however, differences in assessment criteria may affect its clinical relevance. This study aimed to evaluate the association between TB grade and lymphovascular invasion (LVI) as well as lymph node involvement in invasive breast carcinoma using three different assessment criteria. A cross-sectional study was conducted on 44 cases of invasive breast carcinoma that met the inclusion criteria. TB was assessed at the invasive front using the International Tumor Budding Consensus Conference (ITBCC) 2016 criteria, as well as the criteria proposed by Usta et al. and Manimaran et al. LVI and lymph node status were determined through histopathological examination. The associations between variables were analyzed using the chi-square test or Fisher’s exact test, as appropriate. Tumor budding grade based on one standardized assessment system demonstrated a significant association with both LVI and lymph node involvement. In contrast, the other two grading approaches did not show statistically significant associations. As conclusion, within the context of this study, the ITBCC 2016 criteria appear to be more representative in reflecting tumor invasive activity in invasive breast carcinoma. These findings support the potential role of tumor budding as an additional indicator of tumor aggressiveness and highlight the importance of standardizing its assessment method in invasive breast carcinoma.Keywords: invasive breast carcinoma; tumor budding; lymphovascular invasion; lymph node involvement
