Abstract

Left untreated or not properly managed odontogenic infection can cause spreading into facial spaces. Severe oral and maxillofacial infection can spread systemically and cause sepsis.1 Sepsis can induce unfavorable condition for the kidney, which is Acute Kidney Injury (AKI).2,3 The aim of this paper is to report a rare case of an oral and maxillofacial  infection with sepsis-induced AKI and upper airway obstruction which have high mortality rate. The standard treatment consists of proper monitoring of vital function, fluid resuscitation, drainage of accumulated purulence, empirical use of antibiotics chosen to cover the spectrum of potential pathogens until culture results are available, and supplemental oxygen.1,4 This is a case report of  a 48 year old female patient with oral and maxillofacial infection suffers sepsis-induced AKI and upper airway  obstruction. This patient was diagnosed as right submandibular abscess  which has extended into right buccal space, with sepsis, stage I AKI, and upper airway obstruction due to retropharyngeal abscess. We managed this patient with oxygenation, fluid resuscitation, antibiotics and analgesic administration, incision and drainage, and also extraction of mandibular right third molar. Tracheostomy, incision and drainage of retropharyngeal abscess were supposed to be performed by ENT department, but the patient refused. However the patient still survived. In case of oral and maxillofacial infection, the involvement of distant organ should be investigated, because this conditions result in high mortality rate. Early diagnosis and prompt treatment are important and able to lead to a better survival.

Abstract

Objective: Gnathoschisis is the most common congenital craniofacial abnormality in patients with cleft lip and/or palate (CL and/or P). The panoramic radiograph is one means to establish a diagnosis and treatment plan with two-dimension photograph. The cone-beam computed tomography (CBCT) is one means of supporting the maxillofacial to get a three-dimension photograph. The aim of this study was to determine the difference in the results of the use of panoramic photos and CBCT in evaluation before and after gnathoplasty surgery.Material and Methods: This is an observational descriptive research with cross-sectional study method. All samples that meet the suitable criteria are put in this research. The data gained by collecting status card at medical record installation about photo panoramic application in gnathoschisis patients was then compare to the examination with CBCT.Results: The result performed shows that there is a significant difference between panoramic and CBCT picture. The CBCT provide better evaluation about teeth age and segment position of jaw curve from the cleft size. While the panoramic photo provides a widely covered area to observe facial and teeth bone. Conclusion: The CBCT results are more accurate in evaluating gnathoplasty surgery than panoramic photo; however, exposure to the radiation should be considered.

Abstract

Recurrent aphthous stomatitis (RAS) is the common lesion in oral cavity. The etiologyof RAS remains unclear. The trigger factors are genetic, traumatic, abnormalimmunology, digestive diseases, hormonal disturbance, HIV, stress, infection andnutrition deficiency. This is a case report of a patient with minor, multiple, pain, whitecolor ulcer with erythema halo on lips palatum and tongue mucous. One day beforethe ulcers erupted, prodromal symptoms had occurred such as subfebris, malaise, anddizziness. The patient was 19-year-old, male, undergraduate student and with historyof ulceration every month. The clinical diagnosis was primary herpeticgingivostomatitis. The differential diagnosis was RAS. The complete blood count(CBC) result indicated that the patient suffered anemia. He was provided withbenzydamin HCl gargle and oral supplement contain ferrous, vitamins, and minerals. It can be concluded that anemia can be a trigger factor of RAS. Therefore, it isimportant for the dentist to know the clinical signs and manifestations of anemia inoral cavity in order to provide an appropriate treatment.

Abstract

Acrylic resin denture base has the properties absorbing that affecting physical and mechanical properties. One of the physical properties of acrylic resin is surface roughness. The aim of the study was to find out the roughness effect on heat cured acrylic that was immersed in alkaline peroxide and 75% celery (Apium graveoens L) extract as a disinfectant solution. The study was a true experimental and posttest with control group designed with a rectangular shape size 65 x 10 x 3.3 mm based on the ISO standard 1567, six samples were used for alkaline peroxide, celery extract 75% and aquadest group for 5 and 15 days. A Surface Roughness Tester was used for the surface roughness changes observation. The statistical test used One-way ANOVA and post hoc Bonferroni. The results of this study showed the value of roughness on 5 days for alkaline peroxide (1.51 µm) is greater than celery extract (0.36µm) and aquadest (0.30 µm). The soaking for 15 days in alkaline peroxide (1.52 µm) is greater than 75% celery extracts (0.38 µm) and aquadest (0.34 µm). Alkaline peroxide caused higher roughness value of heat cured acrylic resin than 75% celery extract.
