Abstract

Objective: Recurrent intraoral herpes (RIH) is one of the common oral diseases that causes painful ulcers. Mouthwashes are widely used for prevention and treatment of various oral and dental diseases. Chlorhexidine oral rinses may be useful for controlling intraoral Herpes simplex virus type 1 (HSV-1) infection, related virucidal effects and also inhibition of viral replication. This case report describes the effectiveness of chlorhexidine gluconate mouthwash therapy in patient with RIH. Case Report: A 50-year-old male patient complained of ulcers at lip and tongue since 10 days before, the patient had applied triamcinolone oral paste to the lesion but it did not improved. Oral ulcers frequently repeated and appeared in different places. Intraoral examination showed multiple ulcers at buccal mucosa and dorsum of tongue, as well as scrapable white plaques on tongue. The laboratory examination showed IgG Anti HSV 1 was reactive. RIH was made as the diagnosis, based on clinical and laboratory examination. Results: After 1 month therapy of 0.2% chlorhexidine gluconate mouthwash, vitamin B12, and folic acid the oral lesions were improved. Conclusion: The chlorhexidine mouthwash is effective for RIH therapy in reducing pain and accelerating wound healing.

Abstract

Objective: This case report discussed the clinical picture of SLE patients with oral manifestations related to thrombocytopenia. Two-Case Reports: The first patient was consulted from the Pediatric department, a 13-year-old-girl complaint oral canker sores about 2 weeks before hospitalization, accompanied by fever and difficulty in eating. The second patient was an 18-year-old-woman consulted from the Internal Medicine department with complaints of bleeding gums and lips before hospitalization. Blood tests in both patients showed a decrease in value of hemoglobin, hematocrit, erythrocytes, leukocytes and thrombocytes. Results: Systemic Lupus Erythematosus was diagnosed with Anti-ds DNA examination for the first patients, while the second one did with ANA test. Management of oral lesions for the first patient, given a solution of dexamethasone by sterile gauge to compress lips, gargling with 0.2% chlorhexidine gluconate, vitamin B12, folic acid, and petroleum jelly for her lips. The second patient gave an 1% ferracrylum mouthwash, 0.9% NaCl compresses on the lips, 0.1% chlorhexidine gluconate and a smear thin layer of petroleum jelly on her lips. The condition of the two patients improved after two weeks so that they could gradually consume solid food and be allowed to go home. Conclusion: In-depth history, complete examination, comprehensive multidisciplinary approach related to manifestations of SLE has a significant effect in reducing the severity of the disease due to oral lesions and improving quality of patients life.

Abstract

Objective: Oral candidiasis is the most opportunistic infection in HIV/AIDS patient. Neoplasms may also frequently be found in the oral cavity of HIV/AIDS patient and may indicate the progression of HIV infection. Case Report: A 56-year-old complaint of yellowish-white spots in oral cavity and swelling in the palate accompanied with shortness of breath and difficulty in eating and swallowing. Various medication was used to relieve the complaints but there were no improvements. He claimed a weight loss of 10 kg in last three months. Physically showed pale and weakness. Intraorally, diffuse pseudomembranous was found throughout the oral cavity to pharynx, and an exophytic mass with red color at the palate area extending to oropharynx. Direct mycology test, complete blood count examination and HIV rapid test were conducted. Results : Topical antifungal and antiseptic mouthwash was prescribed after direct mycology test result was positive for Candida albicans, a reactive anti-HIV examination with immunochromatography confirmed the patients was HIV and referral to head and neck oncologist for the Kaposi’s sarcoma. In second visit, two days after therapy, the pseudomembrane slightly reduced, but complaints of tightness still felt. Unfortunately, the patients passed away after second visit and have not yet received therapy from head and neck oncologist. Conclusion: Oral candidiasis may be the oral manifestation of HIV/AIDS which may accompanied by oral malignant lesions. Due to the late management of the oral candidiasis and oral malignant lesion, may result in poor prognosis for the treatment of the patient

Abstract

Objective: This case report aims to present the management of crown fractures with pulpal exposure caused by traumatic injury and a condition called gummy smile or altered passive eruption (APE), through a multidisciplinary approach. Case Report: A 24-year-old male patient came to the Dental Conservation Clinic RSGM Prof Soedomo FKG UGM, complained of broken teeth and pain in the upper front tooth due to an accident. #12 and #21 experienced horizontal discontinuity to the dentine surface and some lines on the labial surface and #11 underwent a fracture with exposure of the pulpal tissue. The patient wants to take care of broken teeth and improve his smiles. Results: A treatment plan was formulated, which consisted of a stepwise treatment including endodontic treatment, crown lengthening, prefabricated glass-fiber post followed by a core build-up, and finally, direct composite to achieve patient satisfaction. Conclusion: The careful planning of a multidisciplinary approach such as endodontics, periodontics, and direct restorations can treat patients with complex aesthetic complaints relating to problems with the tooth and its soft tissues, and immediate restoration for providing a fast, predictable, economical, yet satisfactory result to improves patient self-confidence.

Abstract

Objective: Marsupialization is therapy for ranulas but sometimes in determining the diagnosis with epidermoid cysts like to be wrong. Epidermoid cysts in the mouth, especially at the base of the tongue, have clinical features similar to those of ranula. In the operating durante we can find the difference. For that we need a more careful examination and in determining the diagnosis of epidermoid cysts with ranula. Methods: a 16-year-old male has a lump under the tongue since the age of 2.5 years as big as a marble. Since 1 month ago the lump was getting bigger and causing the tongue to be bitten frequently, difficulty speaking and pain relapsing in lumps. We perform therapy with biopsy excision where previously we will marsupialization. Results: Epidermoid cyst found during surgery previously diagnosed with ranula Conclusion: Precisely distinguishing epidermoid and ranular cysts requires a complete clinical examination and interview with the patient. So that surgical therapy such as marsupialization and biopsy excision can be selected

Abstract

Objective: Cemento-ossifying fibroma is a rare benign lesion of the head and neck region. Only a few children had this lesion. Case Report: A 12-years old man came to the RSCM hospital in the oral and maxillofacial department. He complains of a lump in the left cheek since 4 years ago. The patient was referred to RSCM Hospital with pathological anatomy result was cemento ossifying fibroma. A recent complaint was nasal congestion on the left side, neither fluid nor blood discharge from mouth and nose. Result: Cemento-ossifying fibroma can occur in children 12 years old, and radiological and histopathological appearance Conclusion: A critical approach towards the clinical, radiological, and histopathological features is the main subject for the best result.

Abstract

Objective : A panfacial fracture is a fracture that involves the mandible, maxilla , and zygomatic areas. Panfacial fractures may also be accompanied by the involvement of naso orbito-ethmoid (NOE) and frontal bone fractures. Open reduction internal fixation (ORIF) is one of the treatments for midface reconstruction. Case Report: 19-year-old female patient came with complaints of minor head injuries, Lefort 2 Fracture + Compression Orbital Fracture, Bilateral Zygoma Arcus Fracture . The patient was subjected to Open Reduction Internal Fixation(ORIF) for midface and orbital reconstruction under general anesthesia. Application of midface plate in regio zygoma sinistra , in regio zygoma dextra section, and glabella. After placing the midface plate, the wound was sutured. Results: Surgical management for panfacial trauma using Open Reduction Internal Fixation (ORIF) result in the unification of the midface bones and reestablishing facial deformity. Conclusion: The main goal of panfacial fracture management is to restore the occlusal connection so that other parts of the structure can be repaired.

Abstract

Objective: Keratocystic odontogenic tumor (KCOT) is a benign intraosseous tumor of odontogenic origin, making up the third most common odontogenic cyst. KCOTs arise from the remnants of dental lamina or the basal cells of the overlying oral epithelium. KCOTs are characterized by an aggressive behaviour with a relatively high recurrence rate, especially if they are associated with syndromes. Treatment of KCOTs include marsupialization, saucerization, Methods: A 12-year-old child patient was referred to the Specialist Clinic of Oral and Maxillofacial Surgery, Cipto Mangkusumo Hospital. On physical examination, facial asymmetry, a fixated swelling and lump in right maxilla from the second molar to canine with tenderness were observed. A complete history was taken and clinical examination followed by complete enucleation with sulcular flap from 12 region until 18 regions was performed. The diagnosis was maxillary dentigerous cyst based on histopathological result after the operation. Results: The medical history was not significant and patient was in a generally good condition with no fever, nausea, pain, or difficulty in breathing. Extra oral examination showed facial asymmetry and visible lump in the right region of the maxilla. Conclusion: The diagnosis of maxillary KCOT expanding to maxillary sinus can be achieved with adequate datas; history, clinical, and histopathological examinations. Careful surgical approach and proper treatment decision is crucial to avoid complications according to individual case of each patient, especially in children.

Abstract

Objective: The use of open reduction and internal fixation with resorbable plate is the best management for complex pediatric mandibular fractures as the potential growth and development of the mandibular. The most common resorbable plate in Indonesia is ®inion plate. Case Report: A 6 year old girl fell off her bicycle on a downhill road 30 minutes before the hospital admission. Comminuted basal triangle fracture of left mandibular parasymphysis from 3DCT imaging was found. Open reduction and internal fixation with resorbable plate and monocortical screws was performed from extraoral and intraoral approach. Conclusion: Management of complex pediatric parasymphysis fracture require ORIF with plating on inferior border of mandibular and using monocortical screws to avoid any damage to nearby tooth buds. The resorbable plates give good outcome and advantage in pediatric patients as their potential growth and development of the mandibular.

Abstract

Objective: A local flap, including an advancement flap, is useful for covering large skin defects on the head and neck. The incision of the advancement flap is carefully made to cover the main defect as efficiently as possible. Methods: A 34 years old female patient was referred to Department of Oral and Maxillofacial Surgery, Cipto Mangunkusumo National Central General Hospital and had a mandibular resection, extraction of tooth 45, and left condyle disarticulation, followed by fistulectomy, debridement, and squesterectomy. For closing the defect, a local advancement flap was made, the flap was incised, reduced, and pulled to wraparound the main site of the defect. Result: The local advancement flap combined with Burrow’s triangle design, result in complete closure of the defect and also has a maximum cosmetic appearance. Conclusion: Unipedicle-pedicle advancement flaps involve advancement by taking benefit of the skin elasticity, the advancement flap is also done by making an incision of the Burrow-triangles design subsequently bringing the skin laterally to level the length of the flap and the adjacent wound margin.
