Abstract

Technology and information systems can help an organization to execute various activities accurately, punctually and in good quality, so that can increase the efficiency, effectivenesss and competitiveness of the organization. Success in implementing information systems can be determined by an evaluation of information systems that have been applied. The purpose of this study was to evaluate the application of information systems that have been implemented at Immanuel Hospital Bandung, using information systems success models developed by DeLone and McLean. This study was a descriptive study using cross sectional design. Data was obtained from questionnaire to employees and in depth interview to Director of Hospital, Head of Pharmacy Departement and Head of Technology and Information system departement. The research examined variable is user satisfaction to the accuracy, completeness, relevance, timeliness, auditability, and the reliability of pharmacy management information system at Immanuel Bandung Hospital. The data which have been collected from questionnaires then analyzed using qualitative analysis. The results of the study showed that the user satisfaction management information system in Immanuel Bandung Hospital were in medium category. The average user satisfaction ranges between 2.50 to 3.33 of 5 scales. The highest level of user satisfaction of information systems management was on timeliness aspects, auditability, accuracy and completeness. Relevance and reliability had the lowest satisfaction.Keywords : evaluation, pharmacy management information system, user perceptions.

Abstract

Diabetic ulcers and gangrene are the most frequent cases of diabetes that are hospitalized. Several studies in Indonesia showed that 17-23% of the mortality rate was due to ulcers or gangrene and 15-30% was due to amputation. Blood glucose control is an integral factor which affects the successful management of diabetic ulcers. Blood glucose control can be done by the management of pharmacologic therapy, either in the form of oral antidiabetics therapy, insulin, or a combination of both. This  was a nonexperimental research with descriptive evaluative design using  a retrospective data through the medical records of patients with a T2DM diabetic ulcers diagnosis. This research was conducted in PKU Muhammadiyah Yogyakarta hospital period January 2010-December 2011. Data were analyzed with descriptive analysis and chi-square evaluation on the correlation between the type of therapy and blood glucose levels, and the correlation between blood glucose levels and the clinical outcome of patients T2DM diabetic ulcers. The results showed that the usage pattern of antidiabetics in patients with T2DM in PKU Muhammadiyah Yogyakarta hospital were insulin (46.7%), oral hypoglycemic drugs (OHD) 18.1%, the combination therapy of OHD-OHD  (20.5%), the combination therapy of insulin-OHO (13.9%), and the combination therapy of insulin-insulin  (0.8%). The type of insulin which is mostly used was rapid acting insulin (67.9%), biguanid was mostly used for OHD (68.2%). As for the combination therapy, OHD-OHD that was mostly used was biguanid-sulfonylurea (94%), the combination of insulin and OHD that was majority used was insulin-biguanid, and the combination of insulin-insulin that was mostly used was long acting-rapid acting insulin (100%). The results of chi-square analysis showed that there were significant correlation (p = 0.016) between the type of therapy and blood glucose levels, and between blood glucose levels and diabetic ulcer clinical outcome (p = 0, 00).Keywords: diabetic ulcers, antidiabetics, blood glucose levels, clinical outcomes.

Abstract

Drug management in community health centers is conducted by the pharmacy section. Drug management problems in Sibela, Pajang and Nusukan health centers were that there were expired drugs and dead stock. The purpose of this study was to evaluate the efficiency of drug management in both use and distribution levels in Sibela, Pajang and Nusukan health centers. This research was conducted using retrospective and concurrent data. Data were collected in form of qualitative and quantitative data observations from existing document and interview with relevant officials. Then, the data were presented in tabular and textual description. The efficiency of drug management was measured at distribution and use stages using its indicators and compared with standard. The result showed that distribution and use of medicine in Sibela, Pajang and Nusukan health centers generally were not efficient. In Sibela and Pajang health centers, the level of drug availability were 53 and 68 days, respectively. The inefficiency of drug management in both health centers was shown in the percentage of expired drugs that accounts for 0.3% and 0.61%, and in the percentage of dead stock, that accounts for 4.6% and 20.40% respectively. In Nusukan health center, the level of drug availability was 40 days and there were no expired drugs. However, in Nusukan health center, the percentage of dead stock was 14.31% that was not efficient. Drug use in Sibela, Pajang and Nusukan health centers showed that the number of item of drugs per prescription was 3.17, 2.97 and 4, the percentage of generic drug per prescription was 91.85%, 93% and 88.10%, the demand of all medicines was fulfilled, the use of antibiotics for acute respiratory tract infection non-pneumonia was 1.87%, 0% and 1.20%, and the percentage of antibiotic use for non-specific diarrhea was 3.51%, 0% and 0.29, respectively.Keywords: medication management, indicators, efficiency, distribution, usage

Abstract

The government has arranged regulations to establish the implementation of pharmaceutical care standard by pharmacists, such as Kepmenkes No. 1027/2004 about Standards of Pharmaceutical Care in Pharmacy. The government also issued a higher level of regulation, that is PP. 51/2009 about Pharmaceutical Practice. This study is aimed to observe the implementation for pharmaceutical care standard by pharmacist in Sleman Regency and determine the indicators of pharmacy services which have (not) been implemented by the pharmacist. Non experimental-quantitative and descriptive analysis were used in this study. The population were pharmacists practicing at the pharmacy in Sleman Regency. There were 35 pharmacists as sample, being determined by proportional random sampling. The data was collected by using questionnaire which was filled in by respondents, includes seven indicators of the implementation of pharmaceutical care based on PP No. 51/2009 and Kepmenkes No. 1027/2004. The data analysis was interpreted into categories, that are those with the percentage score of 81-100% as good category, 61-80% as adequate category and ≤ 60% as lacking category. The results showed the most optimum implemented indicator from the seven indicators according to the ranking, as follows indicator of workforce with the score of 95.24%, Supporting Factors of 91.10% and drug management of 81.90%. Those three indicators were categorized as good. While indicator of drug services with the score of 78.70%, administration of 77.62%, communication, information and education of 76.80% and evaluation of service quality 71.29%, were categorized as adequate. Among the 35 respondents, the results of the evaluation of standards implementation based on all indicators was 65.71% categorized as good category, 31.43% as adequate and only 2.86% as the lacking ones.Keywords : Implementation, Pharmaceutical Care, Pharmacy, Sleman

Abstract

Medical service companies are getting more competitive so companies have to always improve the service quality. The Pharmacy Unit of Yakkum Emanuel Hospital which is a supporting medical unit is demanded to give an optimal service to their patients. The recapitulation of the complaint box in February 2011 showed that 50% of patients (10 patients) complain on waiting time; 30% of patients (6 patients) complain on the hot air in the waiting room; and 20% of patients (4 patients) complain about when the patients will get their medicine. Regarding those issues, this research aimed to analize the level of patient satisfaction on the service quality at pharmacy unit of Yakkum Emanuel Hospital. This research was conducted with descriptive method. The subjects of this research were outpatient in Yakkum Emanuel Hospital that met the inclusion criteria. The number of the sample was 480 patients that taken with purposive method of sampling. The data were obtained using quetionaire about the quality of service based on SERVQUAL model (referring to tangibles, reliability, responsiveness, assurance, and empathy dimensions) and the data were mapped in cartesian diagram. The result shows that there were negative gap values between the patients’ expectation and perception. The highest gap value was on the assurance dimension followed by responsiveness, reliability, empathy, and tangibles dimensions. In general, patients felt dissatisfied with the performance of the management of the Pharmacy Unit of Yakkum Emanuel Hospital Purwareja-Klampok. Based on the Cartesian Diagram mapping, the management of the Pharmacy Unit of Yakkum Emanuel Hospital should pay more attention to the Assurance and Responsiveness dimensions.Keywords: service quality, satisfaction, SERVQUAL.

Abstract

People often consider that generic drugs are inferior drugs. Knowledge holds big roles in consumer decision making process, including in purchasing generic drugs. Students of health sciences got different understanding compared to students of non health sciences about to knowledge of generic drugs. Therefore, this study aimed to get better understanding and compare product class knowledge, perceived risk, and attitude towards generic drugs in students of health sciences versus students of non health sciences. The population of this non experimental study was Gadjah Mada University students. Sampling technique was using purposive sampling. Samples were UGM students of health sciences and students of non health sciences who met inclusion criteria. Data were collected using questionnaires and analyzed using path analysis and Mann-whitney. Results of this study showed that there was significant negative influence between objective knowledge and perceived risk (standardized coefficient beta value = -0.368), subjective knowledge and perceived risk (standardized coefficient beta value = -0.485), prior experience and perceived risk (standardized coefficient beta value = -0.485), and perceived risk and attitude toward generic drugs (standardized coefficient beta value = -0.489). There were difference between health students and non health students on their product class knowledge, perceived risk and attitude towards generic drugs, which health students got higher product class knowledge and attitude towards generic drugs, while non health students got higher perceived risk compared to health students. Keywords: product class knowledge, perceived risk, attitude towards generic drugs, generic drugs.

Abstract

Systemic lupus erythematosus (SLE) is a multisystem autoimmune disease characterized by fluctuating disease activity. SLE treatment in Indonesia is still limited on how to control clinical features and complications that may occur, using glucocorticoids, NSAIDS, antimalarial agents, and cytotoxic drugs. The objectives of this study were to identify treatment patterns and the degree of appropriate treatment among SLE patients. A retrospective study was carried out using medical record data of adult patients with SLE diagnoses in the inpatient department of X Public Hospital Yogyakarta during January-December 2011. Appropriate descriptive analysis was used. The results showed that the most used drugs in SLE pharmacologic therapy were topical antiinflammatory agents 56%, glucocorticoid methylprednisolone 47%, cytotoxic mycophenolate mofetil 17%, antimalarial chloroquine 9%, and NSAID diclofenac sodium 6% while the most common treatment pattern was with glucocorticoid as monotherapy (35%). Abstract

Increase of public interest in using herbal medicine causes increase in the number of herbal medicine producers and the competition will be harder. Marketing concept states that companies have to satisfy their consumers in order to gain profits. The satisfied consumers will be loyal to certain products or companies, and they will buy the products again in the future. The more important things is that they will carry out word of mouth activity about the products to people around them. The aims of this research were to know the influence of consumers’ satisfaction on their loyalty, the influence of their satisfaction on their word of mouth behavior, and the influence of their loyalty on their word of mouth behavior. The subjects of this research were the consumers of An Nuur’s herbal medicine in Yogyakarta. This research was conducted by survey with questionnaire as an instrument. The samples were taken with non probability sampling design with purposive sampling procedure. The sample consisted of 65 respondents and was taken from November 2011 to May 2012. The criteria of the respondents were they who had used An Nuur’s herbal medicine at least three times, bought it from stores in Yogyakarta, aged at least 17, and agreed to be subjects of this research. Then, the data were analyzed using linear regression analysis to know the relation between variables. The hypothesis was tested using t test. The result of this research showed that consumers’ satisfaction had significant positive influence on their loyalty. The highest consumers perception was the easiness in consuming An Nuur’s herbal medicine, while the lowest consumers perception was the easiness in purchasing An Nuur’s herbal medicine. Consumers’ satisfaction had significant positive influence on their word of mouth behavior. Consumers’ loyalty had significant positive influence on their word of mouth behavior. Consumers’ loyalty had greater influence than their satisfaction on their word of mouth behavior.Keywords: Satisfaction, loyalty, word of mouth, An Nuur.

Abstract

Performance  illustrates the achievement level of an activity, program or policy that is implemented in order to realize the goals, objectives, mission and vision of the organization as stated in the formulation of organization’s strategy. High performance is a necessity for an organization to be sustainable. The objective of this research was to determine the influence of organizational design, leadership style and work climate toward employee performance in PT. Kimia Farma Apotek Yogyakarta. The design of this study was cross sectional and analytical observation. Data were collected using a closed questionnaire. The samples were selected by census method. The number of population available as respondents was 160 people, but only 121 of them completed the questionnaire. The result of multiple linier regression analysis showed that there was a positive and significant impact between organizational design, leadership style, work climate and employee’s performance (the calculated F value (14.342) > F table (2.68) and p = 0.000) with the effective contribution of 26.9%. This mean that the organizational design, leadership style, and work climate influence the employee’s performance. Partially, based on the results of the t-test analysis, the design of the organization had a positive and significant impact on employee’s performance (value (2.057) > t table (1.658) and p = 0.042) with the effective contribution of 8.2%. This means that the more effective the organizational design, the higher the employee’s performance. Furthermore, the results of the t-test analysis showed that leadership style had a positive and significant impact on employee’s performance (t count value (2.354) > t table (1.658) and p = 0.020) with the effective contribution of 6.1%. It means that the more effective leadership style, the higher the employee’s performance. The results of the t-test analysis showed that work climate had a positive and significant impact on employee’s performance (t count value (4.782) > t table (1.658) and p = 0.000) with the effective contribution of 19.9%. This means that the more favorable work climate, the higher the work performance of employees.Keywords: The organizational Design, Leadership Style, Work Climate, Employee’s Performance.

Abstract

The appropriate choosing of antibiotics to treat typhoid fever will effectively decrease the cost of care. The objectives of this study was to describe typhoid fever theraphy, the appropriate choosing of antibiotic, the total hospitalization cost of typhoid fever and the appropriateness between the Jamkesmas patient’s cost of hospitalization and the Indonesian Case Base Group’s (INA CBGs). The research was descriptive analytical research. The data were obtained through medical records of typhoid fever patients who were hospitalized in General Hospital of Yogyakarta from 1st October, 2009 up to 31st September, 2011.  Patient’s were also observed to analized the use of antibiotics, while the patiens invoices were also analyzed to calculate the total hospitalization cost of typhoid fever. The statistical analisys that were used to analyzed the total cost of hospitalization in order to know the factor that influence that cost were the T-Test, Oneway Anova and Kruskal Wallis with the confidence level of 95%. Moreover, the one sample T-test was also used to compare the cost of hospitalization and the lenght of stay. The result of this research showed that 50 out of  86 of the total respondents were male. The age of that respondents were mostly 6-12 years old accounts for 35%. The majority of respondents were also categorized as general patient (41.86%) and 60.47% of the respondents were hospitalized in third class room. Moreover , most of the respondents were hospitalized for 5-10 days. The appropriateness of antibiotics use in child-age patients was 91.86% while in adults patient was 100%. The average of total cost of typhoid fever care for VIP room patients was IDR. 3,076,700.78 while for Askes patients the cost was IDR. 2,698,632.20. Factors that influence the cost of care were age (P = .013), lenght of stay (P = .001), class of service (P = .000), and kinds of medical service (P = .002).  The total cost of care and lenght of stay of Jamkesmas patients agreed with the INA CBGs.
