Abstract

Objective: To compare the effectiveness of rectal misoprostol administration to the amount of blood loss in 4th stage of labor at high risk pregnancy. Method: We recruited all pregnant women with high risk criteria including anemia, age  35 years old, or the number of pregnancies  4. Women would deliver appropriate to standard procedure of normal delivery. After that, we gave 400-mcg tablet of misoprostol rectally in treatment group. To count the amount of blood loss during 4th stage of labor, we put the underpad. Result: The amount of blood loss in 4th stage of labor in the treatment group was 201.1 (SD 80.3) ml less than the control group (285.9 (SD 93.2) ml). The result showed that the administration of 400-mcg misoprostol tablet rectally immediately after the birth of the placenta at high risk pregnancy impacted significantly to reduce the amount of blood loss in 4th stage of labor (p

Abstract

Objective: To know the change in ferritin level in second and third trimester of pregnancy. Methods: This study was a cohort study. The level of ferritin was estimated in the second trimester of pregnancy and examined again in the third trimester of pregnancy. Data processed and analyzed by statistically. Result: The number of sample in this study was 30 patients. From this study, we found that the Ferritin level in second to third trimester of pregnancy was decreased in 28 patients (93.3%) and increased in 2 patients (6.7%). This result showed that ferritin serum level in second and third trimester would significant difference (p < 0.001). This different can be showed in media ferritin serum level in second trimester higher than third trimester (31.10 ng/ml > 22.20 ng/ml). Conclusion: There was a significant decrease in the level of ferritin from the second to third trimester. This could be caused by the occurence of hemodilution reaching its peak at 28-32 weeks of pregnancy, increasing the need of iron intake. This could also be caused by an imbalanced diet of the mother. Abstract

Objective: To report the profile of patients with Hypertension in Pregnancy (HIP) at Sanglah Hospital Denpasar from 2009 to 2010. Method: A retrospective descriptive study from all HIP patients at Sanglah Hospital Denpasar, Bali during January 2009 to December 2010. Result: The prevalence of HIP at Sanglah Hospital was 9.32%, which consisted of 1.82% gestational hypertension, 0.19% chronic hypertension, 1.36% mild preeclampsia, 4.70% severe preeclampsia, 0.43% superimposed preeclampsia, and 0.82% eclampsia. From all of HIP cases, we found that the majority were nulliparous (47.23%), primipaternal (53.1%), and had hypertension at term (51.9%). Most of the cases were found at maternal age > 35 years (14.62%), followed by age < 20 years (13.47%). Most of them also had Ante Natal Care (ANC) frequency ≥ 4 times (69.09%), and had their ANC done by a midwife (52.76%) and followed with an obstetrician (40.81%). The majority of the cases were referral cases (62.39%) mostly by midwives (22.45%). From all of the cases, we found that total preterm labor was 35.14%, perinatal mortality was 9.32% and maternal mortality was 1.16%. Conclusion: The prevalence of HIP at Sanglah Hospital was higher than previous years. Most of them had already had ANC done by health care providers, who were mostly midwives and obstetricians. Therefor, the quality of ANC seems to need an improvement by having an earlier referral system, so cases can be treated earlier. Abstract

Tujuan: Untuk mendapatkan lesi prakanker maupun penderita kanker serviks dalam stadium dini dengan metode inspeksi visual asam asetat. Rancangan/rumusan data: Studi potong lintang. Tempat: Puskesmas Sukmajaya, Abadijaya, Bhakti Jaya, Villa Pertiwi dan Kalimulia, Kotib Depok, Jawa Barat. Subjek, pasien, peserta: Sampel adalah ibu yang berusia 25 - 64 tahun dan berdomisili di Kecamatan Sukmajaya, Depok. Penelitian dilakukan pada September - November 2005. Yang melakukan pemeriksaan IVA adalah bidan dari puskesmas masing-masing yang sudah dilatih sebelumnya. Responden yang IVA positif dilanjutkan dengan pemeriksaan kolposkopi dan atau leep. Semua responden mendapat penyuluhan dan leaflet tentang kanker serviks. Hasil: Dari 3331 responden yang diperiksa IVA diperoleh 2956 negatif, 369 positif (bercak putih) dan 6 positif (suspek kanker). Pada minggu berikutnya semua responden yang IVA positif (375 orang) diundang pada puskesmasnya masing-masing untuk diperiksa ulang oleh dokter ginekolog dari RSCM, namun yang datang hanya 188 orang (50%). Dari 188 tersebut ditemukan IVA positif hanya 33 orang (17,5%). Pada minggu berikutnya ke 33 orang tersebut diundang untuk pemeriksaan lanjutan (kolposkopi dan/atau leep), namun yang memenuhi undangan hanya 21 orang (63,6%). Pada akhirnya ditemukan 1 kasus kanker serviks stadium II B, 1 kasus karsinoma in-situ dan 3 kasus displasia ringan. Kesimpulan: Untuk menghindari hasil positif palsu sebaiknya pelatihan bidan dilakukan lebih intensif dan untuk menghindari responden lost to follow up sebaiknya pemeriksaan IVA dan pemeriksaan lanjutan dilakukan pada hari yang sama. Abstract

Objective: To compare the effects of Intra cervical and paracervical block with 1% lidocaine for pain management in curettage of incomplete abortion. Method: This study is a prospective study with Randomized Control Trial approach. The samples were 52 pregnant women with gestational age of less than 20 weeks, which diagnosed as having an incomplete abortion and a procedure using any local anesthetic technique. T test was used to calculate the mean VAS score and standard deviation for each group. Fisher Exact test was used to assess the relationship between variable characteristics and the local anesthetic technique. Result: The use of local anesthesia using intra cervical block technique for pain management in incomplete abortion with curettage proved to be more effective in lowering degree of pain than paracervical block techniques. Conclusion: Intracervical block technique as a local anesthetic technique is simpler and relatively safer than paracervical block. This technique can be used extensively in Department of Obstetrics and Gynecology Medical Faculty, Hasanuddin University for pain management in curettage. Abstract

Objective: To externally validate EFI in an Indonesian population. Methods: A prospective study was performed in 59 reproductive aged women diagnosed with both endometriosis and infertility who underwent laparoscopic surgery in Dr. Cipto Mangunkusumo and Carolus Hospital during the period of December 2012 to June 2016. Subjects were followed up for two years. Results: Statistical analyses suggested that younger age, infertility duration  3 years, higher LF score, and AFS score below 71 were significantly associated with pregnancy (p&lt;0.001, p&lt;0.001, p=0.035, p&lt;0.001, respectively). Pregnant subjects had significantly higher median EFI score compared to the not pregnant group [EFI score 7 (5-9) vs 4 (1-8), p&lt;0.001)]. Conclusion: Higher EFI score was significantly associated with higher pregnancy rate. Abstract

Objective: To analyze the differences of calcium serum and calcium urine level in preeclampsia and normal pregnancy, and to analyze the correlation between calcium serum and calcium urine level with blood pressure. Method: This study is a cross sectional study with 44 women with preeclampsia and 45 women with normal pregnancies, that meet our inclusion criteria. The samples were obtained from Dr. Hasan Sadikin Hospital and six satellite hospitals from June to September 2011. The comparison of mean calcium serum and calcium urine level of the preeclampsia group was calculated using Mann-Whitney test, and the correlation between calcium serum and calcium urine level and preeclampsia were calculated using Rank Spearman correlation test. Result: The result of the characteristic test in two groups of study shows that both groups are homogenic and comparable.
