Abstract

Tujuan: Mengetahui aktivitas SOD, katalase, dan GSH-PX limfosit, serta kadar MDA plasma penderita Candidiasis Vagina yang diintervensi VCO yang diperkaya dengan Zn selama 2 bulan. Rancangan/rumusan data: Penelitian eksperimental dengan rancangan acak lengkap. Analisis data menggunakan anova single faktor, dilanjutkan dengan uji Duncan bila terdapat signifikansi. Tempat: Sampel sekret diperoleh dari RS Sinar Kasih, tempat prostitusi (Gang Sadar), Puskesmas, dan praktik dokter di Purwokerto. Pengujian aktivitas enzim antioksidan dan kadar MDA di laboratorium Klinik Mediko Purwokerto. Bahan dan cara kerja: Penelitian dilakukan mulai Oktober 2006 sampai April 2007. Sebanyak 30 responden dipilih berdasar kriteria seperti berikut, sekret vagina mengandung C. albicans lebih dari 105 cfu/ml, sukarela, sehat, bersedia menandatangani informed consent, dan tinggal di Purwokerto. Responden dibagi dalam 3 kelompok; kelompok A sebanyak 10 orang diintervensi dengan VCO yang diperkaya Zn dengan dosis 2 sdm/hari; kelompok B sebanyak 10 orang diintervensi dengan dosis 1 sdm/hari; dan kelompok C sebanyak 10 orang sebagai kontrol. Pengambilan sampel darah dilakukan 3 kali yaitu pada baseline, 1 dan 2 bulan setelah intervensi. Uji aktivitas enzim antioksidan dan kadar MDA menggunakan spektrofotometer. Hasil: Setelah 2 bulan intervensi, aktivitas enzim SOD meningkat dari 87,32 menjadi 348,70 unit/mg protein (p=0,00016), katalase meningkat dari 20,42 menjadi 40,12 Unit/mg protein (p=0,031); demikian pula glutation peroksidase limfosit juga meningkat dari 5,77 menjadi 10,64 μmol/gr protein (p=0,036). Sebaliknya kadar MDA plasma menurun dari 1.858,86 menjadi 1.214,36 pmol/ml (p=0,019). Kesimpulan: Intervensi VCO yang disuplementasi dengan Zn kepada penderita Candidiasis Vagina berpotensi sebagai antioksidan. Selanjutnya, mereka disarankan mengkonsumsi suplemen dengan dosis 1 sdmx1. Abstract

Objective: To compare serum Anti Müllerian Hormone (AMH) levels in infertile women with and without endometriosis, and to determine the mean levels of serum AMH in every stage of endometriosis. Method: We performed a cross-sectional study. Sixty-eight subjects who have undergone laparoscopy and fulfilled both inclusion and exclusion criteria are recruited consecutively. They are divided into two groups, namely group with endometriosis and without endometriosis. Blood samples are taken from each subject before laparoscopy, where serum AMH levels are then measured. The difference in mean levels of each group are tested with Mann-Whitney test. Result: The mean levels of serum AMH were significantly lower in the endometriosis group than those in the group without endometriosis (2.30 1.8 ng/ml vs 3.75 2.13 ng/ml; p=0.005). Using Kruskal-Wallis test, it was found that there was a statistically significant difference among endometriosis groups based on the severity of endometriosis. Abstract

Objective: To investigate and compare serum profile of CA-125, erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) in various stages of endometriosis. Methods: Forty endometriosis patients’ blood samples were taken prior to laparoscopic surgery for various indications. Diagnosis of endometriosis was confirmed by laparoscopy. Independent T test methods were used for statistical analysis. ROC analysis was examined for each variable. Results: The mean level of CA-125 serum was 12.0 vs. 36.3 U/ml (p < 0.05); ESR serum was 11 vs. 12.8 mm/hour (p > 0.05); CRP serum was 0.7 vs. 0.3 (p > 0.05) for endometriosis stage I-II and stage III-IV, respectively. The optimal cut off point to discriminate endometriosis stage I-II and III-IV was 16.9 U/ml with sensitivity 83% and specificity 81%. Conclusion: The CA-125 serum level was increased for moderatesevere compare to minimal mild endometriosis patient. Abstract

Objective: To determine the Ca-125 level in pre- and post-operative of endometriosis and its correlation to endometriosis stage and severity of dysmenorrhea. Method: This was a cross-sectional study design conducted at Dr. Wahidin Sudirohusodo dan some affiliated hospitals. We took the patients undergoing laparoscopy or laparotomy consecutively. Abstract

Objective: To compare the effectivity of N-acetylcysteine with Betamethasone in fetal lung maturation. Methods: This research was a double blind randomized clinical trials in women threatened by preterm delivery (28 to 34 weeks of gestation). Results: There were 90 subjects randomly assigned to N-acetylcysteine group (n=30), betamethasone group (n=30), and the control group (who had not been given a tocolytic, n=30). Fetal lung maturation was assessed with the p. The values for lung maturation at random were 5 foams. After being tested with the Tapp, there were significant differences between the 3 groups of this study based on analysis of variance (ANOVA) (p = 0.001). The average foam on the N-acetylcysteine was 4.8 ± 1.3 while in the group Betamethasone, was 3.2 ± 1.0, and the average amount of foam in the control group was 5.5 ± 1.6. The end point of the Tapp were mature and immature, which in the N-acetylcysteine group there were 21 subjects (70.0%) with mature lung, Betamethasone groups 28 subjects (93.3%) with mature lungs, whereas in the control group, there were 15 subjects (50.0%) with mature lung. Conclusion: Betamethasone was more effective than N-acetylcysteine for lung maturation in women threatened with preterm delivery. Abstract

Objective: To determine the effect of exercise during pregnancy in reducing oxidative damage marked by decrease of malondialdehyde (MDA) and 8-hydroxy-2-deoxy-guanosine (8-OHdG) levels. Method: A number of 66 pregnant women were recruited in this study and grouped to two groups, i.e. 30 of them as control group and the rest as treatment group. Pregnancy exercise was performed to all 36 pregnant women from 20 weeks gestation on treatment group. The exercise was performed in the morning for about 30 minutes, twice a week until fullterm. Blood sample was taken for MDA and 8 OHdG level at the beginning of research, at 20 weeks of gestation, and at intial delivery or fullterm. Student’s t-test was applied to compared the difference between treatment and control group with 5% significant value. Result: This study reveals that there were significant decrease of MDA and 8-OHdG level amongs treatment and control groups (p < 0.05). Conclusion: Light exercise started from pregnancy age 20 weeks will decrease MDA and raised 8-OHdG levels compared to control group without exercise. Abstract

Objective: To asses ovarian reserve (OR) is important in infertility management that will undergo Assisted Reproductive Technology (ART) programs. using some methods, eg. physical measurements: ovarian volume, Antral follicle Count (AFC); Biomarker assay, indirect and direct methods, Day 3 FSH, Anti-Mullerian Hormone (AMH), Inhibin B. Method: Literature review in published studies which assessment the Ovarian Reserve. Conclusions: Assessment of ovarian reserve (OR) is very important to predict the outcome of ART programs. There were several methods to assessing the OR, but none as a single predictor. The best candidate for a single biomarker is AMH. Abstract

Tujuan: Menemukan metoda diagnostik sederhana dalam mendeteksi vaginosis bakterial (VB) dalam kehamilan dengan menentukan sensitivitas, spesifisitas, nilai duga positif dan negatif, rasio kemungkinan dan derajat kesesuaian pemeriksaan pH dan LEA (leukosit esetrase) vagina dengan menggunakan dipstick dibandingkan pewarnaan Gram. Tempat: RSUPN Dr. Cipto Mangunkusumo dan RS Bersalin Budi Kemuliaan, Jakarta. Bahan dan cara kerja: Wanita hamil pada usia kehamilan 16-24 minggu. Pemeriksaan kemudian dilanjutkan pengambilan apusan lendir vagina dan dilakukan pemeriksaan pH vagina dan kadar LEA dengan menggunakan dipstick Uriscan (Lampiran IV). Sebagai baku emas dilakukan pewarnaan Gram untuk menilai adanya infeksi VB menggunakan skor Nugent. Bila dari penilaian mikroskopis didapatkan skor Nugent 7-10, maka sampel dinyatakan sebagai VB positif dan dilakukan analisis selanjutnya. Hasil yang didapat dari pemeriksaan dipstick Uriscan dibandingkan dengan hasil dari pewarnaan Gram. Hasil: Penelitian sejak Mei-Agustus 2006, didapatkan 80 subjek penelitian yang sesuai dengan kriteria. Subjek berusia 20-38 tahun dengan rerata usia 27,84 ± 4,46 tahun, 47,5% adalah primigravida. Usia kehamilan sebagian besar dalam kelompok 16-20 minggu, dengan rerata usia kehamilan 19,98 ± 2,58 minggu. Keluhan keputihan dijumpai pada 41 orang. Pada penelitian ini didapatkan sebanyak 32,5% (n=26) subjek dengan VB positif. Pada pemeriksaan dipstick didapatkan sensitivitas pemeriksaan LEA vagina adalah 42,3%, spesifisitas 61%, nilai duga positif 34,3% dan nilai duga negatif 68,7%. Derajat kesesuaian 55% dengan nilai kappa 0,032. Sensitivitas pemeriksaan pH vagina dalam mendeteksi VB sebesar 61%, spesifisitas 79%, nilai duga positif 59%, dan nilai duga negatif 81%. Pada kurva ROC pH didapatkan nilai AUC (Area Under Curve) 0,70 yang berarti akurasi pemeriksaan pH cukup baik dalam membedakan kelompok VB positif dan yang bukan, sementara pemeriksaan LEA menunjukkan akurasi yang buruk (AUC = 0,51). Dengan menggunakan uji chi-square didapatkan adanya hubungan yang bermakna (p=0,001) antara pH vagina dengan kejadian VB, namun didapatkan hubungan yang tidak bermakna (p=0,46) antara LEA vagina dengan hasil pemeriksaan Gram. Kesimpulan: Pemeriksaan pH dan LEA vagina dengan dipstick dapat digunakan untuk mendeteksi VB secara cepat dan sederhana dalam klinik. Pemeriksaan pH vagina memiliki sensitivitas yang lebih baik dibandingkan LEA vagina. Namun dibandingkan pewarnaan Gram, sensitivitas dan spesifisitas pemeriksaan ini masih belum memuaskan. Derajat kesesuaian antara kedua pemeriksaan ini juga kurang baik, sehingga pemeriksaan LEA vagina masih belum dapat menggantikan kedudukan pemeriksaan Gram dalam mendeteksi VB secara akurat.
