Abstract

Patient adherence to anti-tuberculosis (OAT) medication is a critical factor in the success of tuberculosis (TB) treatment. Various elements may influence this adherence, one of which is family support. This study aimed to examine the relationship between the level of family support and patient adherence to OAT consumption in the working area of the Plumbon Public Health Center. A quantitative method with a retrospective approach was employed. The sample consisted of 24 TB patients (total population). Data were collected using a questionnaire that had been tested for validity and reliability. The hypothesis test using the chi-square method revealed a significant relationship between family support and medication adherence among TB patients (p-value < 0.05). These findings indicate that family support plays an essential role in enhancing patients’ compliance with TB treatment. Therefore, family-based interventions may serve as an effective strategy to support the success of tuberculosis control programs.

Abstract

Cardiovascular disease remains the leading cause of death in Indonesia, yet epidemiological and administrative data are often inaccurate due to misclassification. Misclassification can occur at the clinical level (misdiagnosis), diagnostic level (limitations in biomarkers, ECG, or imaging), and administrative level (incorrect ICD-10 coding). This article systematically reviews the evidence on forms, causes, and impacts of misclassification of heart disease patients in Indonesia using the PRISMA framework, and identifies strategies for mitigation. Literature searches were conducted in PubMed, Scopus, Google Scholar, and national portals (Ministry of Health, PERKI, Riskesdas) using keywords related to misclassification of cardiovascular disease in Indonesia. From 512 articles identified (2012–2025), 41 were fully screened, and 11 met the inclusion criteria. Data were synthesized narratively due to heterogeneity among studies. Misclassification was most frequently observed in three domains: (1) clinical—heart failure misdiagnosed as COPD, (2) diagnostic—reliance on a single biomarker without imaging verification leading to misdiagnosed acute coronary syndromes, and (3) administrative—incorrect ICD-10 coding. National surveys such as Riskesdas 2018 and SKI 2023 also highlighted discrepancies in prevalence estimates. Misclassification of heart disease in Indonesia is a multidimensional issue impacting epidemiological accuracy, clinical quality, and financing systems. Strengthening diagnostic algorithms, training ICD-10 coders, and implementing integrated digital audits are essential to reduce misclassification rates.

Abstract

Post sectio mothers face greater breastfeeding obstacles than normal delivery, this occurs due to surgical pain, limited mobility, difficulty in breastfeeding positions, delays in early breastfeeding initiation can worsen the condition of flat nipples. This condition can interfere with the success of exclusive breastfeeding. The purpose of the study was to evaluate the effectiveness of nipple puller exercises in improving nipple eversion and the quality of breastfeeding attachment in postpartum sectio caesarean mothers with flat nipples. The research design used a case study with a nonpharmacological intervention-based approach to maternity nursing care conducted prospectively to evaluate the effectiveness of nipple puller exercises using the modified syringe method according to standard procedures, which were performed on three postpartum mothers on days 1 - 4 of the year for 2–3 times a day in 4 days of treatment. Data collection methods included interviews, direct observations, documentation of nipple shape changes, attachment success and effective breastfeeding ability. Evaluation was carried out through observation of changes in putting conditions, the success of baby attachment, the ability of the baby to breastfeed effectively, and the comfort of the mother while breastfeeding. The results showed that all three patients experienced significant improvements in nipple eversion and improvement in attachment quality after consistent Nipple Puller exercises. All mothers reported increased breastfeeding comfort and reduced barriers in the lactation process. Changes are characterized by the appearance of adequate nipple protrusion and an improvement in the quality of the baby's attachment, characterized by effective suction, the absence of significant pain in the mother, and a reduced incidence of detachment of attachment during breastfeeding. In conclusion, the Nipple Puller (Syringe Method) exercise is effective as a simple, safe, cheap and easy-to-apply intervention to help overcome flat nipples in postpartum caesarean section mothers in supporting the success of lactation, so it has the potential to be recommended in the practice of lactation services in health facilities.

Abstract

One educational medium considered effective in improving health knowledge is the booklet. Booklets are printed media that present information concisely and clearly, complemented by visual illustrations, making them easily understood by target groups with varying educational levels. This study aimed to determine the effect of using a pregnancy danger signs booklet on the knowledge level of pregnant women at the Sungai Ambawang Community Health Center in 2025. This study used a quantitative pre-experimental design with a one-group pretest–posttest design. The sample consisted of 32 pregnant women selected using a total sampling technique. The intervention consisted of health education using a pregnancy danger signs booklet, accompanied by a health worker. Knowledge data were collected through a questionnaire that had been tested for validity and reliability, and the data were analyzed using a Paired Sample t-test. There was a significant increase in the level of knowledge of pregnant women after being given the pregnancy danger signs booklet intervention, with a p-value of 0.001 (p < 0.05) and a mean difference of 1.44. It can be concluded that the use of the pregnancy danger signs booklet has a significant impact on increasing maternal knowledge.

Abstract

Type 2 diabetes mellitus is a chronic disease that requires long-term management, including the implementation of a balanced diet. A balanced diet helps maintain blood glucose levels and prevent complications. The success of diet adherence is greatly influenced by family support, which includes emotional, instrumental, informational, and appraisal support. Lack of family support can become a barrier to patients’ compliance with healthy eating patterns. This study aims to determine the influence of family support on compliance with a balanced diet in patients with type 2 diabetes mellitus. This study used a qualitative descriptive method with a case study approach involving five families of patients with type 2 diabetes mellitus. Data were collected through in-depth interviews and direct observation of family support and patient dietary compliance. Of the five families studied, three families demonstrated positive support in the form of assistance with providing healthy food, monitoring meal schedules, and offering motivation. Patients in these families showed high adherence to a balanced diet. Meanwhile, the other two families did not provide sufficient support—they were less involved in dietary management and lacked understanding of the importance of a balanced diet. This resulted in low dietary compliance among those patients. Family support plays an important role in the successful implementation of a balanced diet in patients with type 2 diabetes mellitus. Families that are actively involved in dietary support contribute to better adherence and help prevent complications.

Abstract

Medical records are crucial documents in healthcare that must be managed appropriately and according to standards. This study aims to determine the effect of the timeliness of storing medical record files on the efficiency of patient services at the Tanjung Community Health Center, Brebes Regency. A quantitative analytical design with a cross-sectional approach was employed. The study population consisted of all outpatient medical records from the internal medicine polyclinic and patients using services at the health center. The sample comprised 66 observed medical records and 30 patient respondents selected through purposive sampling. Data were collected using observation sheets and questionnaires, and analyzed using the Chi-Square test with a significance level of α = 0.05. The results showed a significant effect between the timeliness of medical record storage and patient service efficiency (p = 0.035). In conclusion, timely management of medical records significantly contributes to increased patient care efficiency. This study is expected to provide a basis for health center management to improve their medical record storage system and for medical record staff to be more disciplined in returning files within the required timeframe.

Abstract

Gout arthritis is a condition that can interfere with a person's daily activities. Individuals with this condition will experience joint pain during movement, swelling, stiffness in the fingers when moved, and eventually, the person may become unable to carry out normal activities. The World Health Organization (WHO) reported that 34.2% of people worldwide experienced gout in 2020.Pain is an unpleasant sensory and emotional experience resulting from tissue damage and is subjective in nature. Chronic pain is constant or intermittent pain that persists over a period of time. Health education is a planned and systematic learning process aimed at increasing knowledge, a wareness, and skills. This study is an intervention-type research, in which the researcher conducts actions or treatments (interventions) on subjects to observe the impact. The participants involved were two clients or two families (two cases) with the same nursing problem and medical diagnosis, namely clients experiencing chronic pain due to gout. The assessment was carried out from May 12 to 28, 2025. From the assessment results, Ny. Sn had a uric acid level of 8.0 mg/dL and Ny. Sy had 8.7 mg/dL. The nursing diagnosis identified based on problem screening was: Chronic Pain Related to the Family’s Inability to Care for Sick Family Members, with a score of 5. The planned interventions were General Objective and Specific Objectives 1–5, with the expectation that the family would be able to recognize the problem, make decisions, provide care, modify the environment and diet, and utilize health care facilities. During the evaluation stage, the planned actions were carried out for both clients and the problems were resolved.

Abstract

Refraction examination is one of the main pillars of optical services, playing a crucial role in determining the accuracy of refractive error diagnosis and the provision of appropriate visual correction. The implementation of refraction examination not only reflects the clinical competence of optometry personnel but also serves as an indicator of the overall quality of optical service management. Inaccurate examinations may lead to visual discomfort, reduced productivity, and decreased patient trust in optical services. This study aims to analyze the implementation of refraction examinations as part of optical service quality management, emphasizing procedural accuracy, compliance with standard operating procedures (SOPs), and their impact on patient satisfaction and loyalty. This study employed a mixed-methods approach. Quantitative data were collected through questionnaires administered to 100 patients from five optical service units in the Jakarta area, while qualitative data were obtained through in-depth interviews with 15 optometrists and 5 optical managers. Quantitative analysis was conducted using multiple linear regression, and qualitative data were analyzed thematically to identify quality management patterns and barriers to implementation. The results indicate that the accuracy of refraction examinations, clarity of communication regarding examination results, and compliance with SOPs significantly affect patient satisfaction (p < 0.05), contributing 42%, 28%, and 18%, respectively. Optical units with internal quality monitoring systems and regular training demonstrated higher service consistency. The main barriers identified include limited service time, variations in SOP understanding, and limited examination equipment.

Abstract

Research Objective: To describe the epidemiological characteristics, identify risk factors, and recommend control measures for a hepatitis A outbreak at X Islamic Boarding School, Kebumen District, Central Java, Indonesia, in 2022. Methodology: A descriptive study combined with a case-control design was conducted from February 12 to March 17, 2022, involving 1,319 students. A case was defined as a student presenting with symptoms such as fever, fatigue, nausea, vomiting, headache, loss of appetite, abdominal pain, diarrhea, joint pain, and jaundice. Laboratory confirmation was performed using IgM anti-HAV testing. A case-control study with a 1:2 ratio (39 cases and 77 controls) was conducted using purposive sampling. Data were collected through structured interviews to assess demographic characteristics and behavioral risk factors. Bivariate analysis was performed using the chi-square test, and multivariate analysis was conducted using logistic regression. Results: A total of 212 students developed symptoms (attack rate: 16.1%), of whom 45 were laboratory-confirmed cases. The highest attack rates were observed among females (17.5%) and students aged less than 15 years (50.5%). Four behavioral risk factors were significantly associated with infection: not washing hands with soap before eating (OR = 3.26; 95% CI: 1.415–7.541), sharing food from a single container (OR = 3.94; 95% CI: 1.749–8.889), sharing or borrowing eating utensils (OR = 2.96; 95% CI: 1.324–6.648), and frequent snacking (OR = 3.08; 95% CI: 1.293–7.350). Water samples from regular wells and the canteen’s drinking water tested positive for Escherichia coli contamination. The epidemic curve indicated a common-source outbreak pattern. Conclusion: The hepatitis A outbreak was confirmed through epidemiological investigation and laboratory testing. Contaminated water sources and poor hand hygiene practices were identified as the primary transmission routes. The implementation of water treatment, health education, and improved sanitation facilities is essential to prevent future outbreaks in congregate settings.
