Abstract

Background: In Indonesia, low back pain is a common health problem that can interfere with a person's productivity in carrying out activities due to the manifestations of the pain it causes. Acupuncture therapy is one of the complementary treatments that is quite effective in reducing the intensity of low back pain. This study aims to estimate the effectiveness of acupuncture therapy to reduce pain and improve quality of life in patients with low back pain.Subjects and Method: This study is a systematic review and meta-analysis with PICO, Population: Low back pain patients, Intervention: Acupuncture therapy, Comparison: No acupuncture therapy, Outcome: Pain reduction and quality of life improvement. It was measured using the Oswestry Disability Index. The article search process was carried out between 2006-2022 using 3 databases, including Google Scholar, PubMed, and Science Direct. The keywords used in the database search were “acupuncture” AND “oswestry disability index” AND “quality of life” AND “low back pain”. The inclusion criteria for full text articles using a Randomized Controlled Trial (RCT) study design by including the results of the study in the form of the number of respondents, the mean and standard deviation (SD) values, for articles using English and Indonesian. Articles that have met the requirements are analyzed using the RevMan 5.3 application.Results: There are 9 articles that meet quantitative requirements and are divided into 2 categories according to independent variables with a Randomized Controlled Trial (RCT) study design originating from Spain, Australia, South Korea, Brazil, Germany and England, which were carried out in a meta-analysis in this study. Meta-analysis of 7 articles showed estimates that acupuncture therapy was effective in reducing pain in LBP patients. Patients experienced a decrease in pain 0.32 units higher than those who were not given acupuncture therapy (SMD = 0.32; 95% CI= -0.46 to -0.17; p <0.001). Meanwhile, a meta-analysis of 9 articles showed the estimation of the effect of effective acupuncture therapy on improving the quality of life in LBP patients. Patients experienced an improvement in quality of life 0.52 units higher than those who were not given acupuncture therapy (SMD = 0.52; 95% CI= 0.22 to 0.81; p= 0.006).Conclusion: Acupuncture therapy is effective in reducing pain and improving the quality of life of LBP patients. Abstract

Background: Spinal anesthesia is an anesthetic method by injecting local analgesic drugs into the subarachnoid space in the lumbar region. The purpose of the study was to analyze the effect of health education with spinal anesthesia videos on anxiety in pre-anesthesia in the operating room of the Genteng Regional General Hospital Banyuwangi.Subjects and Method: A quasi experiment study was conducted in Genteng General Hospital, Banyuwangi, East Java, in August 2022. A sample of 15 patients who underwent surgery under spinal anesthesia was selected by incidental sampling. The dependent variable is anxiety. The independent variables were video educational intervention. Anxiety was measured by Anxiety Analog Scale (AAS). Other variables were collected by questionnaire. The difference test in the paired group was examined using paired t-test.Results: Anxiety level after intervention (Mean= 180; SD= 80.84) was lower than before (Mean= 305; SD= 75.12) with p<0.001.Conclusion: Video of spinal anesthesia intervention lower anxiety in pre anesthesia surgery.Keywords: anxiety, spinal video, pre anesthesia, spinal anesthesia, health educationCorrespondence: Katmini. Abstract

Background: Bipolar Disorder is a chronic disease with various variations in the course of the disease that are often unknown, misdiagnosed and even when diagnosed are often not adequately treated. The aim of the study was to analyze the effectiveness of KamiO on quality of life in Bipolar Disorder patients at dr. Iskak Tulungagung Hospital.Subjects and Method: Observational study with randomized control trial was conducted at dr. Iskak General Hospital, Tulungagung, East Java, from May 25 to August 25, 2022. The study population were 160 patients with bipolar disorder. A 114 sample was selected using the simple random sampling divided into (1) 57 sample in the treatment group and (2) 57 sample in control group. In this study, the independent variable was KAMIO. The dependent variable is medication adherence and quality of life. While the measuring instrument used is a questionnaire. Data analysis using Mann Whitney test.Results: Patients in the intervention group more adherent to visit medical treatment (Mean= 63.00) than control group (Mean= 52.00), with p= 0.039. Quality of life in bipolar disorder patients in the intervention group (Mean= 72.25) was higher than in the control group (Mean= 42.75) and it was statistically significant (p<0.001).Conclusion: Quality of life in the intervention group (kamio cards) is higher than in the control group.Keywords: kamio effectiveness, treatment adherence, quality of life, bipolarCorrespondence: Katmini. Abstract

Background: Thoracal spinal canal stenosis (SCS) that refers to multiple segments of the thoracic spine is a peculiar clinical condition. Various factors can cause thoracic SCS, including ossification of the ligamentum flavum (OLF). The aim of this study is to explain about the diagnosis for multiple segment thoracal spinal canal stenosis and management properly to prevent further complications.Case Presentation: A 58-year-old male came to Orthopaedic Outpatient Clinic in Prof. I.G.N.G Ngoerah Hospital, complaining back pain since one year prior to admission. Pain aggravated when he was bending his back. The patient was diagnosed with Thoracal Spinal Canal Stenosis Th 8-9, Th9-10, Th10-11. Patient underwent decompression, stabilization, fusion and biopsy surgery.Results: Patient underwent decompression, stabilization, fusion and biopsy surgery at RSUP Prof. I.G.N.G Ngoerah. In durante operation, already done laminectomy and eight pieces of pedicle screw was applied in Th8-Th11 (55mm x 40 mm Monoaxial on right side Th8, 55mm x 40 mm Polyaxial on left side Th8; 55mm x 40 mm Monoaxial on right side Th9, 55mm x 35 mm Monoaxial on left side Th9; 55mm x 40 mm Monoaxial on right and left side Th10; 55mm x 40 mm Monoaxial on right and left side Th11). There were no complications found in this patient after 4th day following surgery.Conclusion: The goal of surgical intervention for SCS is decompression by removing the calcific areas that are responsible for the spinal canal narrowing and subsequent cord compression. Although the diagnosis and choice of intervention can be difficult, early detection and management are critical to postoperative success. Abstract

Background: Upper trapezius myofascial pain syndrome is a muscle pain condition associated with sensory, motoric, or autonomic functions and generated by trigger point arising from hardened and stiff taut band of muscle fiber while being touched as well as palpated. One of the interventions to reduce the pain is by using acupuncture therapy. The study aims to analyze and estimate the effectiveness magnitude of acupuncture therapy toward the pain reduction in patients with upper trapezius myofascial pain syndrome.Subjects and Method: This was a systematic review and meta-analysis study using PRISMA diagrams. The article search was conducted based on the eligibility criteria using the PICO Model. population: patients with upper trapezius myofascial pain syndrome. Intervention: acupuncture therapy. Comparison: placebo acupuncture. Outcome: reduced pain. The articles used were from 6 databases, namely Pubmed, Google Scholar, Science Direct, Scopus, Spinger Link, and Hindawi Key words for article search were “acupuncture” OR “acupuncture therapy” AND “upper trapezius pain” OR “upper trapezius myofascial pain syndrome” OR “upper trapezius MPS” AND “Randomized Con¬trolled Trial” OR “RCT”. Inclusion criteria used of the study were full text articles with Randomized Controlled Trial study design, measure of association used was Mean SD. Articles were analyzed using the Review Manager 5.3 application.Results: The 9 articles reviewed were from Taiwan, China, UK, Turkey, Brazil, and Iran. The study result indicated that acupuncture therapy was effective in reducing pain in upper trapezius myofascial pain syndrome. Abstract

Background: Dementia is a common cause of morbidity and mortality in the elderly population. The elderly have a higher risk when exposed to COVID-19, especially if they have serious medical conditions such as dementia (senility). The purpose of this study was to determine the effect of dementia on the risk of mortality in COVID-19 patients based on previous primary studies.Subject and Method: This research is a systematic review and meta-analysis study using PRISMA diagrams. The article search was conducted based on the eligibility criteria using the PICO Model. P: COVID-19 patients, I: dementia, C: no dementia, O: mortality. The articles used were from 3 databases, namely Pubmed, Google Scholar and Science Direct. The search keywords for articles were as follows: dementia AND (death OR Mortality) AND COVID-19 OR Sars-Cov-2. The inclusion criteria in this study include full text articles with cohort study designs, full text available articles, the size of the relationship used is the adjusted Odds Ratio or Risk Ratio, the year the article used is in the 2019-2022 range and the research outcome is mortality. Articles were analyzed using the Review Manager 5.3 application.Results: There are 9 articles in this study from New York, United States, Spain, Korea with 2 articles, and England with 4 articles. The article was analyzed using the PRISMA flow diagram. The results of the study show that COVID-19 patients with dementia have a mortality risk of 2.02 times greater than COVID-19 patients without dementia. Abstract

Background: The increase in Covid-19 cases in Indonesia as well as the hospital length of stay is unpredictable. The degree of lung damage in Covid-19 largely determines the severity of the disease, a serial chest X-ray using Brixia score can be used to assess changes in pulmonary parenchymal damage and could predict the length of hospitalization. This study aims to analyze the correlation of Brixia score with the length of hospitalization in Covid-19 patients.Subjects and Method: This cross-sectional study was conducted at Sumber Waras Hospital from November 2021 until January 2022. The samples are Covid-19 confirmed patients who were treated at Sumber Waras hospital. Independent variable is Brixia score and dependent variable is length of hospitalization. Brixia's score assessment from two serial chest X-rays was divided into 4 categories: low to low, low to high, high to low, and high to high. All the categories were analyzed using Kruskall-Wallis method then continued with Mann-Whitney U test. The multivariate variable was analyzed using linear regression.Results: There are 190 samples, the median score of Brixia at the beginning of treatment is 4 (range 0-18), and the median score of Brixia on serial examination is 6 (range 0-18). Patients in low to low category had median 10 days length of hospitalization, low to high category and high to low category had median 11 days, and high to high category had median 8 days. There was no statistically significant correlation between Brixia score and length of hospitalization (p= 0.377).Conclusion: There is no significant relationship between the median Brixia score changes with the length of hospitalization of covid-19 patients.Keywords: Covid-19, hospitalized, Brixia score changes, length of stayCorrespondence: Widiastuti Soewondo. Abstract

Background: A stroke is an attack on the brain that occurs when the blood supply to part of the brain is blocked so that part of the brain becomes damaged and dies because it does not get nutritional intake. Cognitive exercise activities can maintain optimal blood flow and improve the delivery of nutrients to the brain. The study aims to analyze the effect of cognitive exercise activity on the independence ability in post-stroke patients.Subject and Method: The study was a meta-analysis with PICO as the following: Population: post-stroke patients. Intervention: cognitive exercise activity. Comparison: no cognitive exercise activity. Result: independence ability. The articles used in this study were obtained from three databases, namely Google Scholar, PubMed, and ScienceDirect. The keywords for searching for the articles were "cognitive exercise activity" AND "cognitive ability" AND "post-stroke". The included articles were full-text English versions with a randomized control trial study design from 2012 to 2022. The article selection was conducted using the PRISMA flow diagram. The articles were analyzed using the Review Manager 5.3 application.Results: A total of 9 case studies from Europe (Oxford, Sweden, United Kingdom), South America (Brazil), North America (Texas, Jamaica), Asia (Korea), and Africa (Egypt) were selected for systematic review and meta-analysis. It was discovered that cognitive exercise activity of 0.36 units increased independence ability in post-stroke patients compared to not obtaining cognitive exercise activities. Abstract

Background: The most frequent primary malignant bone tumor in children is osteosarcoma. Patients with osteosarcoma are currently treated with a combination of neoadjuvant and adjuvant chemotherapy as well as surgery, either limb-sparing surgery or limb-ablation surgery. How to manage the remaining bone growth during the limb-sparing procedure in youngsters is one of the challenges. Unfortunately, amputation is still many perform for OS patients. The purpose of this study was to inform and educate doctors about the value of early OS diagnosis and treatment in pediatric patients.Case Presentation: We presented case from a male 16 years old Balinese complaint of pain on her left knee 12 months ago. The pain was continuous and got worse at night. The patient went to a traditional masseuse and was given topical herbal ointment. Three months later, a lump appeared on her knee at the size of a small ball and getting bigger. We did a review slide of the previous biopsy sample by the pathology of anatomy in our hospital. The result came out as osteosarcoma with osteoblastic cells. We performed to amputated the limb-affected tumor following wound care routinely controlled.Results: Intraoperatively, we performed wide excision, without patellar tendon, medial collateral ligament, and cruciate ligament preserve. We amputated the limb-affected tumor. The patient routinely controlled to our outpatient clinic every week. Unfortunately, the patient was died after 3 weeks of admission.Conclusion: To reduce the risk of misdiagnosis or delayed diagnosis of osteosarcoma, every source of knee pain with a mass in pediatric age groups should be thoroughly explored with a high index of suspicion for malignant lesions. Abstract

Background: Coronavirus 2019 or COVID-19, caused by the new SARS-CoV2 virus, has become a pandemic and attacked more than 200 countries, including Indonesia. The most comorbid disease in COVID-19 patients was hypertension. Chest radiography can predict prognosis and mortality in COVID-19 cases and one of the methods that can be used for chest radiographic assessment is the Brixia Score, specifically designed for COVID-19 patients to measure and analyze the severity of lung abnormalities in patients with COVID-19. This study aimed to find the correlation between the mortality of COVID-19 patients with hypertension and chest radiography using the Brixia Score treated in the intensive care unit (ICU) of Dr. Moewardi Hospital.Subjects and Method: This study used an analytic observational study with a cross-sectional approach. The subjects were 84 COVID-19 patients with hypertension who met the exclusion and inclusion criteria. The independent variable is chest radiography and dependent variable is mortality. Sampling was collected by purposive sampling and the data obtained were analyzed using the contingency coefficient test.Results: There was a significant correlation between the mortality of COVID-19 patients with hypertension and chest radiography as assessed using the Brixia Score (p<0.001).
