Abstract

Background: Digestive tract disease is still a health problem around the world, currently there are 2,544,084 cases of diarrhea in Indonesia in 2016. In the Central Java region, there are an estimated of 911,901 cases of diarrhea, while 95,635 cases of diarrhea have been handled. This study aims to estimate the magnitude of the effect of probiotics on the frequency and duration of diarrhea among toddler based on the results of a number of similar previous studies.Subjects and Method: This study was a systematic study and meta-analysis, with PICO as follows: Population= children under five. Intervention= Probiotics. Comparison= Not Probiotic. Outcome= Frequency and Duration of Diarrhea in toddlers. The articles used in this study were obtained from several databases including PubMed, Springer Link, Cochrane Database, Hindawi, Elsevier, JBBA, LWW, Google Scholar. The keywords for search­ing articles are as follows: "Probiotic", OR "fre­quency", OR "diarrhea", "kid", OR child "AND" Randomized Controlled Trial (RCT). The articles included in this study were full text articles with a randomized controlled trial study design. Articles were collected using PRISMA flow diagrams. Articles were analyzed using the Review Manager 5.3 application.Results: A total of 12 articles reviewed in this meta-analysis study originated from India, Canada, Indonesia, America, Iran. Studies showed that the effect of probiotics decreased the frequency (Standarized mean difference -0.32; 95% CI= -0.67 to 0.03 p= 0.070). The effect of probiotics reduced the duration of diarrhea in children under five (Standarized mean diffe­rence= -0.74; 95% CI= -1.60 to 0.12 p= 0.090).Conclusion: Probiotics reduced the frequency and duration of diarrhea in toddlers.Keywords: Probiotics, Frequency, Diarrhea, ToddlersCorrespondence: Wahyu Agustin Dwi Ciptaningtyas. Abstract

Background: Incision techniques that are of­ten used in cholecystectomy laparotomy are the Koc­her incision and midline incision. This study was carried out to compare the pain in the Kocher's in­cision and midline incision in patients with post­operative cholecystectomy laparotomy.Subjects and Method: : This study was an ob­ser­vational randomized controlled trial double sampling study conducted at the Department of Sur­gery Dr. Moewardi Hospital Surakarta in Octo­ber 2018 to February 2019. The sample of this study was 30 patients aged 18-65 years old who were diagnosed with symptomatic chole­lithiasis based on clinical, laboratory, and radio­logical tests. The samples were selected by simple ran­dom sampling. The dependent variable of this study was the pain. The independent variables were the Kocher and midline incision techniques. The pain was measured on a 24-hour post­ope­ra­ti­ve VAS scale, range 1-10. Data were analyzed by t-test.Results: There was no significant difference in pain level between the Kocher group (Mean= 2.33; SD=0.72) and the midline group (Mean= 2.20; SD=0.97) with p=0.192.Conclusion: There is no difference in pain level due to the Kocher incision technique and the mid­line incision technique.Keywords: pain, incision, kocher, midline, cholecy­stectomyCorrespondence: R. Abstract

Background: Non-Communicable Disease is a disease that is considered unable to transmit or spread from one person to another but is a cause of death globally, especially diabetes mellitus and obesity. According to WHO, the number of deaths due to PTM in 2016 was 40.5 million (71%) worldwide. Diet is  one of the risk factors for Diabetes Mellitus and Obesity. This study aims to analyze the effect of diet as a risk factor for diabetes mellitus and obesity.Subjects and Method: This study was a syste­matic review and meta-analysis using a cross-sectional design. The articles used in this stu­dy were obtained from several databases, including PubMed, Google Scholar, Spring­er­link, and ScienceDirect. The articles used in this study were those published from 2011-2020. The article search was carried out by consi­dering the eligibility criteria defined using the PICO model. P: adults, I: unhealthy eating patterns, C: healthy eating patterns, and O: Diabetes Mellitus and Obesity. The keywords to search for articles were "dietary pattern", "risk", "obesity", and "diabetes mellitus," and "adjus­ted odds ratio". The articles included in this study were full-text articles with cross-sectional study design. Articles were collected using PRISMA flow diagrams. Articles were analyzed using the Review Manager 5.3 application.Results: A total of 15 articles were reviewed in this study. The meta-analysis showed that an unhealthy diet increased the risk of Diabetes (aOR= 1.65; 95% CI= 1.29 to 2.11; p <0.001). The meta-analysis of 9 articles also showed that an unhealthy diet increased the risk of obesity (aOR= 1.42; 95% CI= 1.21 to 2.66; p <0.001). This meta-analysis combines primary studies from Swaziland, China, Ethiopia, Kenya, Ame­rica, Malaysia, Ghana, Romania, Nepal, Ire­land, and Korea.Conclusion: Unhealthy diet is a risk factor for diabetes mellitus and obesity.Keywords: dietary pattern, diabetes mellitus, obesity, cross-sectionalCorrespondence: Linda Wahyu Septiananwati. Abstract

Background: Corona Virus Deases 2019 (COVID-19) is an infectious disease caused by the corona virus. COVID-19 is currently a challenge for diabetes patients. Diabetes mellit­us predisposes to a very severe disease course and increases the risk of death from COVID-19. This study aims to estimate the average mag­ni­tude of the effect of diabetes mellitus comor­bi­dity on the risk of death of COVID-19 patients.Subjects and Method: This was a systematic review and meta-analysis. This research was conducted using secondary data that will be conducted by searching and selecting data from the results of clinical trials conducted around the world. Search for articles through syste­matic and comprehensive databases from Pub­med, Google Scholar, Springer Link. The ana­lysis in this study was carried out using the Review Manager 5.3 software. The results of the analysis are the effect size, heterogeneity and then the study model. Variations between studies are used to determine the analysis model which is divided into two models, namely the fixed effect model (FEM) and the random effect model (REM).Results: Based on the results of the analysis, there was high heterogeneity between experi­ments (I2= 85%; p= 0.04), so that the Random Effects Model (REM) was used. COVID-19 patients with comorbid diabetes mellitus were 2.17 times more likely to die than patients without comorbid diabetes mellitus (aOR 2.17; 95% CI 1.04 to 4.54).Conclusion: Diabetes mellitus co-morbidity increases 2.17 times the risk of death in COVID-19 patients compared to patients without dia­betes mellitus comorbidity.Keywords: COVID-19, Comorbidity, Diabetes Mellitus, DeathCorrespondence: Utin Ilma Agni Kun'ain. Abstract

Background: Epilepsy is one of the oldest neurological disease requiring long-term treat­ment. Long-term use of certain anti-epileptic drugs (AED) will strongly induce cytochrome-P450 (CYP) enzyme resulting increase of homo­cysteine associated with risk of cerebrovascular disease and depression. The purpose of this study was to determine relationship between serum level of ApoA, ApoB, and C-Reactive Protein (CRP) with depression in epileptic patients receiving AED therapy.Subjects and Method: Respondents of this cross-sectional study were epileptic outpatients in neurology clinic Dr. Moewardi Hospital, Surakarta receiving AED therapy and met inclusion and exclusion criteria. Depression condition was evaluated by Hamilton Depress­ion Rating Scale (HDRS). Serum level of ApoA, ApoB, and CRP of each respondent were mea­sured from vein blood. Data were analyzed by a multiple linear regression.Results: There were 51 epileptic patients with proportional ratio between men and women (1.2:1) with age range from 18 to 59 years old. The average length of AED 92.67 months (Mean= 92.67; SD= 91.58). Average of HDRS score was 8.49. Meanwhile, average of ApoA level was 86.14 mg/dL, average of ApoB level was 47.57 mg/dL, and average of CRP level was 0.18 mg/dL. Serum level of ApoA, ApoB, and CRP increased depression (HDRS score) in epileptic patients.Conclusion: There was a statistically signi­fi­cant relationship between serum level of ApoA, ApoB, and CRP with depressive condi­tion of epileptic patients receiving long-term AED treatment.Keywords: ApoA, ApoB, C-Reactive Protein, depression, epilepsyCorrespondence: Diah Kurnia Mirawati. Abstract

Background: Over the past 50 years, laparoscopy has evolved from a limited gynecological surgical procedure that is only used for diagnosis and tubal ligation to become the main surgical tool used for many gynecological indications. Many studies have shown that laparoscopy is safer, more affordable, and has a shorter recovery time than laparotomy. This study aimed to report the management of laparoscopic surgery in intraoperative subcutaneous emphysema.Case Presentation: The subject of the study was a 37-year-old woman with adenomyosis and chocolate cyst of sinistra with a surgery plan for resection of adenomyosis and per laparoscopic cystectomy with the physical status of the ASA II Plan GAET. The surgery was carried out on October 15, 2018, with surgery for 5 hours. At the intraoperative, subcutaneous emphysema was found from the diaphragm to the thorax. In this condition, the peritoneal insufflation was reduced to 10-12 mmHg. The head down was returned to a position that made it easier for the patient's condition to reduce the occurrence of decreased lung compliance. The patient's postoperative condition was good, the hemodynamic was stable, her breath was spontaneous, and the saturation was 99% with oxygen supplementation via Nasal Kanul 3 lpm. The subcutaneous emphysema gradually disappeared 24 hours postoperatively. Conclusion: Subcutaneous emphysema may occur in the laparoscopic procedures. The close monitoring during surgery and the expertise of the anesthesiologist in diagnosing and intervening is crucial in controlling this condition.Keywords: subcutaneous emphysema; laparoscopy; intraoperative interventionCorrespondence: RTH. Abstract

Background: Insomnia is a disease that has a negative impact on the health and well-being of the sufferer. Insomnia can occur in anyone, both men and women with various ages of vulnerability and in individuals with various socioeconomic backgrounds. This study aims to examine the effectiveness of acupuncture in insomnia patients.Subjects and Method: Meta-analysis was performed with PICO which is presented as follows: the population in this study was insomnia patients aged 16-85. The intervention is in the form of acupuncture therapy. The com­parison used was a sham/ placebo acupuncture. The outcome was insomnia. The meta-analysis study was applied to this study with electronic data sources: Clinical Key, Google Scholar, MEDLINE/ PubMed, Science Direct, Scopus. The article used is a full-text article with a randomized control trial (RCT) study design. There were 9 articles used in this study with a sample size of 649 people who were divided into two groups (372 people in the acupuncture group and 277 people in the sham / placebo acupuncture group). Articles were analyzed using the Review Manager 5.4 application. The results of this study aim to determine the Standardized Mean difference (SMD) and hete­rogeneity of the study sample.Results: There is high heterogeneity between one experiment and another (I2= 94%; p <0.001) so that what is used is the Random Effect Model (REM). The provision of acu­puncture therapy was able to reduce the degree of insomnia with the Standardized Mean Diffe­rent (SMD) of 0.98 compared to sham acu­puncture (SMD= -0.98; 95% CI= -1.71 to -0.25; p = 0.008).Conclusion: The provision of acupuncture therapy was able to reduce the degree of insomnia with the Standardized Mean Different (SMD) of 0.98 compared to sham acupuncture.Keywords: Acupuncture, InsomniaCorrespondence: Kurnia Eka Putri. Abstract

Background: Traumatic fingertip injuries are the most common injuries of the hand. Several million estimated visits to primary care pro­vi­ders and the emergency department occur each year. But the handling is often not done pro­per­ly and comprehensively. Reconstruction of the fingertip must be done as soon as possible in the acute stage, resulting in better outcomes in the future. Aim of the surgery was to restore as much finger length, preserve sensation finger­tip properties and achieve normal nail growth, and cosmetics appearance as well.Case presentation: Three cases of emergency patients with fingertip injury treated at the hospital in the acute setting. All of these pati­ents experienced work injuries and were imme­diately taken to the emergency department.Results: The debridement and skin coverage reconstruction were carried out. Abstract

Background: Incidence of septic arthritis in children less than 5 years old about 5 from 100,000 cases every year and adult above 64 years old about 8.4 cases every 100,000 people every year. The diagnosis of the chronic septic arthritis, especially in hip joint is still challenging, even for skilled doctor in musculo­skeletal. Delayed diagnosis and inadequate treatment can lead to permanent joint damage and impairment.Case presentation: In this case we discuss about clinical improvement of chronic septic arthritis in a 4 year old girl with hip pain that had been felt since 2 months before admission. Physical examination, laboratory examination, and ultrasonography examination was done and the diagnosis had been made.Results: A surgical debridement and syno­vec­tomy was done with addition antibiotic therapy. The patient not only felt the pain was relieved but also can return into good function of hip joint.Conclusion: Diagnosis with proper early treatment is expected to reduce permanent joint loss and improve the clinical outcome for the patient.Keywords: septic arthritis, hip, synovectomy, surgical debridementCorrespondence: Pamudji Utomo. Abstract

Background: Osteoarthritis (OA) is the most common joint condition and is charac­terized by progressive erosion of the articular cartilage. Based on data from Basic Health Research (2018) the prevalence of joint disease in Indonesia is recorded at around 7.3% and osteoarthritis (OA) or arthritis is a common joint disease. As the two main non-surgical methods, physiotherapy and acu­puncture have been shown to be safe and effective in the therapy of osteoarthritis.Subjects and Method: This was a syste­matic review and meta-analysis study. The articles used in this study were obtained from several databases including PubMed, Google Scholar, and Springer Link. The articles used in this study were those published from 2000-2020. The article search was carried out by considering the eligibility criteria defined using the PICO model. The popu­lation in the study were patients with osteo­arthritis complaints with intervention in the form of acupuncture therapy, the comparison was the sham acupuncture outcome in the form of pain reduction. The keywords for finding articles are as follows: "Acupuncture" OR "Osteoarthritis" AND "Randomized Con­trolled Trials". The articles included in this study are full text articles with an obser­vational study design. Articles were collected using PRISMA flow diagrams. Articles were analyzed using the Review Manager 5.3 application.Results: A total of 9 articles were reviewed in this study.
