Abstract

The importance of breastfeeding has strong evidence. Complementary therapy that can increase milk production is to increase comfort and body fitness by using herbal steam baths. This is local wisdom that is still preserved in Indonesia as a therapy for postpartum mothers with water vapor. Another similar alternative is endorphin massage, which is a light touch therapy on the neck, back and arms that can trigger the release of endorphins that affect milk production. The purpose of this study was to estimate that the combination of the two methods would be more effective in influencing breast milk production. This research design is a quasi-experimental. The population in this study were postpartum mothers at the Independent Practice Midwife in Surabaya with a total sample of 40 which were divided into treatment and control groups, using purposive sampling technique. The treatment group was given intervention with a combination of herbal steam baths 2x/week for 5-20 minutes and endorphin massage 2x/week for 20 minutes, while the control group was not given. Each group was given a pre-test and post-test to determine the volume of breast milk production with indicators of increased baby weight, increased frequency of urination and defecation. Statistical test using Mann Whitney and Shapiro Wilk. The results showed that there was a significant effect of the combination of herbal steam baths and endorphin massage on increasing breast milk production with a value of α =0,000 atau α <0,05. There was a significant difference in milk production in the treatment group compared to the control group with a value of α =0,000 atau α <0,05. Conclusion: By doing a combination of herbal steam baths and endorphin massage regularly, it can increase milk production.

Abstract

Background: Congenital diaphragmatic hernia (CDH) is a disorder where abdominal contents protrude into the chest. Congenital diaphragmatic hernias are seen in 1 of every 2000-4000 live births. Mortality is predominantly due to the development of pulmonary hypoplasia which is thought to be due to the mass effect on the developing lung.Case Report:Mrs. Y, a 32-year-old controlled for antenatal care at the fetomaternal polyclinic M Djamil hospital. She had no complaints, the patient was referred from another hospital with a suspected fetal diaphragm hernia. The patient is known to be 38-39 weeks term pregnancy, from ultrasound examination found herniation of abdominal contents into the thoracic cavity and. the liquor was excessive and the doppler study was normal. The ultrasound showed normal growth and corresponded to 38 week Then the patient was planned for caesarean section and a hernia repair surgery. The neonate was admitted to the neonatal intensive care unit (NICU). He was shifted to the pediatric surgeon with ventilator support after 48 h and was undergoing surgery. There was a posterolateral left sided defect (BOCKDALEK type) of around 22–24 mm. The intestines were reduced to the abdomen and the closure was done. Immediately after the surgery, the child was admitted back to the NICU and the further postnatal course was uneventful.Discussion and Conclusion: CDH is a malformation of the diaphragm due to failure of closure of the pleuroperitoneal canal at around 10 weeks leading to herniation of abdominal contents into the thoracic cavity leading to compression of lung tissues at pseudo glandular stage. CDH is often ultrasound diagnosed before birth [120, 121]. The intestine and/or the liver may be in the thorax and the lungs are small. US scan allows detailed assessment of the heart. The survival depends on the contralateral lung volume, Ninety percent of cases are due to posterolateral defect which is also called Bochdalek hernia—80% are left sided, 15% are right sided and 5% are bilateral. Herniated viscera leads to pulmonary hypoplasia and pulmonary hypertension. Prognosis depends on the gestational age at diagnosis (earlier the diagnosis, likely larger the defect), associated anomalies, contralateral lung volume, pulmonary vasculature (Doppler), cardiac function, liver herniation and gestational age at delivery

Abstract

This study aimed to provide knowledge for midwives to deepen their understanding of their clients when working closely with women during the perinatal period through an integrative review of ‘fear of childbirth’, and to clarify the research topics to be addressed in the future. Methods: This study used an integrative review (IR) method in addition to bibliometric analysis methods to classify and analyse the studies. Review questions (RQ) included 1. ‘what is the status of research efforts (number of papers, location, etc.) on “fear of childbirth”?’, 2. ‘what are the screening methods and prevalence of “fear of childbirth”?’, and 3. ‘what can be learned from the findings of these studies?’Findings: RQ1: The number of papers published gradually increased after 1981 and has been increasing since 2000. Studies conducted in Scandinavia accounted for more than half of the total studies. Conversely, only five studies were conducted in Japan. RQ2: The Wijma Delivery Experience Questionnaire is a screening scale for fear of childbirth developed in Sweden, which has been translated and utilised in multiple languages. In meta-analyses worldwide, the prevalence of fear of childbirth has been reported to range from 3.7% to 43%. RQ3: Fear of childbirth leaves a negative impression of past childbirth experiences on women’s psychological well-being and subsequently leads to post-traumatic stress disorder. Intervention studies have shown that prenatal education can help couples overcome their fear of childbirth. However, the effectiveness of intervention studies that have examined these psychological aspects has not yet been clarified. Conclusion: The prevalence of fear of childbirth varies considerably, possibly owing to cultural differences. Research on fear of childbirth has reported negative effects on the postpartum period and on subsequent pregnancy and childbirth. Practising women-centred care may help prevent this fear.

Abstract

Menstruation can cause various problems including pain during menstruation or what is commonly known as dysmenorrhea. Dysmenorrhea can interfere with daily activities, including school activities for adolescents. Dysmenorrhea can occur 2-3 years after menarche. The average duration of menstruation is 3-6 days, followed by reduced blood loss from the 2nd day to the end. This causes women to feel pain on the 2nd day of their cycle due to the unmaximized detachment of the endometrium. Puberty is an important phase of life, followed by development and growth. Good knowledge will shape behavior that supports maximum growth and development. Dysmenorrhea is the number one reason why female students are absent from school. Absences lead to missing information and an impairment. The aim of this study is to determine the relationship between menstrual length and menstrual cycle with dysmenorrhea in female students. This study uses a quantitative method with a cross-sectional approach. The sample in this study consisted of 106 female students drawn by the total sampling method. Inclusion criteria were female students with menarche, and exclusion criteria were PCOS. Analysis using the Chi-Square test. The results showed that the prevalence of female students with dysmenorrhea was 71.1%. The results of the bivariate analysis test showed that there was no significant association between menstrual cycle (p value = 1.000) and menstrual length (p value = 0.852) with dysmenorrhea.
