Abstract

Lower back pain is the most common form of discomfort during pregnancy experienced by mothers, especially in the second and third trimesters. Hormonal changes, weight gain, and biomechanical adaptations of the body are the main factors causing this complaint. The impact is not only pain but also disrupts daily activities, sleep quality, and increases stress. Pharmacological management is not always safe, so non-pharmacological interventions are more recommended. One potential method is pregnancy massage with stimulation of the BL40 (Weizhong) acupressure point, which works to relax muscles, improve circulation, and stimulate the release of endorphins. This study used a quasi-experimental design with a pretest–posttest without a control group. The sample consisted of 30 pregnant women in their second and third trimesters who experienced lower back pain and received antenatal care at the Sujiati Nur Elys TPMB, Dlanggu District, Mojokerto Regency. The intervention was given three times a week, with pain measured using the Numeric Rating Scale (NRS). The results showed that before the intervention, the majority of respondents experienced severe pain (53.3%), while after the intervention, none experienced severe pain. A total of 26.7% of respondents had no pain, 53.3% had mild pain, and 20% had moderate pain. The Wilcoxon test showed p = 0.000 (p < 0.05), indicating a significant difference before and after the intervention. Thus, pregnancy massage at the BL40 point has been proven effective in reducing lower back pain in pregnant women.

Abstract

First stage labor pain is a physiological process that can cause physical and psychological stress for mothers giving birth, so it requires effective and safe non-pharmacological treatment. Objective: This study aims to determine the effect of a combination of counterpressure therapy and lavender aromatherapy on reducing the intensity of first stage labor pain. Method: This type of study used a quasi-experimental design with a one group pre-test and post-test design approach. Sampling was carried out using an accidental sampling technique with a total of 25 mothers giving birth in the first stage. Measurement of pain intensity used a Visual Analogue Scale (VAS) or Numeric Rating Scale (NRS). Bivariate data analysis used the Wilcoxon Signed Ranks Test. Results: The characteristics of respondents showed that the majority were in a healthy reproductive age of 20–35 years (80%) and were primiparous (60%). Before the intervention, most respondents experienced severe pain (72%) with an average value (mean) of 7.24 ± 1.15. After being given a combination of counterpressure therapy and lavender aromatherapy, pain intensity decreased to moderate pain (52%) and mild pain (44%) with an average value of 4.12 ± 0.98. The results of the Wilcoxon statistical test showed a p-value <0.001 (p <0.05). Conclusion: The combination of counterpressure therapy and lavender aromatherapy had a significant effect on reducing the intensity of pain in the first stage of labor. This combination therapy can be recommended as one of the non-pharmacological independent midwifery care interventions to reduce labor pain.

Abstract

The increasing rate of cesarean section deliveries contributes to a higher risk of postoperative pain and delayed mobilization among postpartum mothers, which may negatively affect both physical and psychological recovery. Inadequately managed pain and limitations in early mobilization can prolong the recovery period, increase the risk of complications, and reduce the quality of life of postpartum mothers. This study aimed to analyze the correlation between the implementation of early mobilization and pain management and the quality of recovery among postpartum mothers following cesarean section. This study employed a quantitative approach using an observational analytic design with a cross-sectional method. The study subjects were postpartum mothers following cesarean section who were hospitalized at Gatoel Hospital, Mojokerto. Data were collected using standardized instruments to assess pain intensity (Numeric Rating Scale), implementation of early mobilization, and maternal recovery quality. Data analysis was performed using bivariate analysis (Chi-Square test) and multivariate analysis (Ordinal Logistic Regression) to determine the most dominant relationship between early mobilization, pain management, and recovery quality. The results demonstrated a significant relationship between early mobilization and recovery quality (p = 0.001; p < 0.05). Likewise, a significant relationship was found between pain management and recovery quality (p = 0.003; p < 0.05). Ordinal logistic regression analysis revealed that early mobilization was the most dominant factor influencing the quality of recovery among postpartum mothers following cesarean section, with an odds ratio (OR) of 5.21. These findings indicate that early mobilization directly contributes to the restoration of physiological functions and enhances patient independence during the hospitalization period. Early mobilization and pain management are important factors influencing the quality of recovery among postpartum mothers following cesarean section. Early mobilization was identified as the most dominant determinant and should therefore become a primary focus of evidence-based nursing care at Gatoel Hospital, Mojokerto.

Abstract

Lactation problems during the early postpartum period, such as delayed onset of lactation and breast engorgement, frequently trigger excessive neonatal weight loss and increase the risk of exclusive breastfeeding failure, which predisposes infants to stunting. This study aimed to analyze the effectiveness of the Woolwich Massage intervention on increasing breast milk intake and infant weight gain among postpartum mothers. Method:This quasi-experimental study using a non-equivalent control group design with pre-test and post-test involved 26 postpartum mothers in the Gebangmalang village and Tambakagung village, selected through purposive sampling (13 in the intervention group and 13 in the control group). Breast milk intake was measured using the test weighing method, and infant weight was recorded before and after a 5-day intervention period. The Wilcoxon Signed Rank Test showed that the intervention group experienced a significant increase in mean breast milk intake by 28.9 ml and a significant mean infant weight gain of 190.8 grams, with respective significance values of p-value = 0.001 (p<0.05). Woolwich Massage effectively stimulates the lactiferous sinuses mechanically and hormonally to trigger prolactin release and optimize breast emptying. This complementary therapy is proven effective in optimizing early neonatal nutritional fulfillment and supporting ideal physical growth.

Abstract

Hypertension is a major public health issue requiring continuous control efforts through health promotion at the primary care level. The effectiveness of interventions is heavily influenced by the alignment of communication strategies and media with the target audience's characteristics. This study aimed to analyze the sociodemographic characteristics of hypertension patients as a basis for planning health promotion media at Puskesmas Bambanglipuro. A quantitative descriptive design was used. The sample consisted of 100 hypertension patients registered at Puskesmas Bambanglipuro, selected through purposive sampling. Variables analyzed included gender, age, marital status, education level, occupation, monthly income, family history, comorbidities (diabetes mellitus), and health insurance ownership. Data were analyzed using univariate and descriptive bivariate methods with the Chi-Square test (Continuity Correction) to observe the difference in educational proportions based on age groups. The majority of hypertension patients were female (74.0%) and dominated by the elderly group aged  ≥ 60 years (78.0%). Most respondents had a low level of education (67.0%), earned below the regional minimum wage (91.0%), and worked in the informal sector. The Chi-Square test showed a statistically significant difference in education level proportions between the elderly and adult groups (p-value = 0.029). All respondents (100%) had a family history of hypertension, and the vast majority owned health insurance (94.0%). The characteristics of hypertension patients, dominated by the elderly with low education and literacy levels that are statistically significantly lower, require a specific educational approach. Health promotion media planning at Puskesmas Bambanglipuro should emphasize a simple visual approach, interpersonal communication supported by visual aids, and active family involvement to optimize the utilization of health insurance in hypertension control.
