Abstract

Moderate head injury is a neurological condition that carries a high risk of serious complications, particularly increased intracranial pressure (ICP), thus requiring appropriate and comprehensive nursing management. A patient with moderate head injury post-craniotomy requires intensive care to prevent further neurological damage. This research aims to analyze nursing care in patients with neurological system disorders due to moderate head injury post-craniotomy through the application of a 30° head-up intervention to reduce signs of increased intracranial pressure in the ICU. The method used was a descriptive analytical method with a case study approach on Mr. O, involving data collection through observation, physical examination, interviews, documentation review, and literature review. The results of the analysis indicate that the consistent and controlled application of the 30° head-up position can help improve hemodynamics, increase the level of consciousness, and reduce signs of increased intracranial pressure. This intervention also supports improved patient comfort and prevents further complications. Therefore, the 30° head-up position can be recommended as an effective non-pharmacological intervention in the nursing care of post-craniotomy patients with moderate head injury in critical care settings.

Abstract

During challenging periods such as the COVID-19 pandemic, the role of nurse managers in applying core management functions becomes increasingly vital. Despite this, there remains a lack of research exploring how these functions are implemented in crisis settings. This qualitative study, using a phenomenological approach, involved nurse managers from hospitals in South Sulawesi Province that provided inpatient care for COVID-19 patients. Participants were selected through purposive sampling, and data were collected via semi-structured interviews. Thematic analysis revealed three main themes and eight subthemes that reflect the lived experiences of nurse managers during the pandemic. The findings indicate that management practices were necessarily flexible, evolving in response to the unpredictable nature of the pandemic. The organizing function was particularly prominent, as nurse managers had to address urgent issues such as staffing shortages and the reconfiguration of treatment spaces. The study also highlights the importance of strengthening nurse managers’ skills in cross-sector collaboration and coordination, which proved essential in navigating the complexities of pandemic response. These insights offer practical guidance for nurse managers and healthcare institutions in preparing for future crises, emphasizing the need for adaptable management strategies and enhanced interprofessional cooperation.

Abstract

Chronic Kidney Disease (CKD) is a chronic condition characterized by a progressive and irreversible decline in kidney function, which can lead to various systemic complications. Reduced kidney function causes fluid retention and activation of the renin–angiotensin–aldosterone system, thereby increasing cardiac workload. In the long term, this condition can result in congestive heart failure (CHF), which further triggers an increase in pulmonary capillary hydrostatic pressure and leads to pulmonary edema in the alveoli. CKD patients who develop CHF with pulmonary edema require management beyond pharmacological therapy, namely through non-pharmacological nursing interventions such as the application of the high fowler position and pursed-lip breathing exercises. The purpose of this study is to determine the effectiveness of applying the high fowler position and pursed-lip breathing on improving respiratory status in CKD patients complicated by CHF with alveolar pulmonary edema. The method used is a case study with a nursing process approach, including assessment, determination of nursing diagnoses, planning, implementation, and evaluation. Data were collected through interviews, observation, physical examination, and documentation and literature review. The results of nursing care showed improvement in respiratory status after the application of the high fowler position and pursed-lip breathing exercises, indicated by decreased respiratory rate, reduced dyspnea complaints, increased oxygen saturation, and the disappearance of additional breath sounds. It is concluded that the application of the high fowler position and pursed-lip breathing exercises contributes to the improvement of respiratory status in CKD patients complicated by CHF with alveolar pulmonary edema.

Abstract

An estimated 10.6 million people fall ill with tuberculosis worldwide. Based on data from the 2023 Global Tuberculosis Report published by the WHO, Indonesia ranks second in the world in terms of the highest number of tuberculosis cases. In 2022, Indonesia accounted for about 10% of the total global TB cases, with an estimated 969,000 new cases. Data from the Gorontalo Provincial Health Office shows that Gorontalo ranks 15th in Indonesia with a prevalence of 68%. There are 6,949 patients. Gorontalo Regency has the highest number of patients, with 2,310 people as of 2025. Data from the Gorontalo Regency Health Office shows that Limboto District has the highest prevalence of pulmonary tuberculosis patients. Currently, various efforts have been made in Gorontalo, including health education, but researchers feel that there are still groups of people who need special treatment for several reasons, including the low level of education and knowledge of the community. Therefore, researchers are interested in studying the extent of the community's knowledge about tuberculosis so that the government can take strategic steps to increase public knowledge about tuberculosis. The purpose of this study was to determine the level of knowledge about pulmonary tuberculosis in Bulota Village, Limboto District, Gorontalo Regency. The method used in this study was a quantitative descriptive method to identify the community's knowledge about pulmonary tuberculosis. The results obtained in this study show that out of a total of 50 respondents, 19 people (38%) were in the good category, 27 people (54%) were in the adequate category, and 4 people (8%) were in the poor category. Thus, the knowledge of the community in Bulota Village, Limboto District, Gorontalo Regency is mostly in the adequate category. With these results, there is a need for new breakthroughs in the form of methods that are more interesting and closer to the community to increase public knowledge so that it improves to good.

Abstract

School-age children are vulnerable to the impact of disasters due to their lack of understanding and knowledge about earthquake preparedness. To increase children’s disaster preparedness knowledge, an attractive learning media smart boxes can be employed. This study aims to analyze the effect of smart box education on students’ knowledge of earthquake disaster preparedness. The research design was a quasi-experiment with two groups and a pre-post-test control group. The population consisted of 180 elementary school students, selected by cluster random sampling, with 130 students involved in the study (65 in the intervention group and 65 in the control group). Data were collected using an earthquake preparedness knowledge questionnaire. The intervention group received smart box education, while the control group received lecture-based education. The pre- and post-tests were conducted on the same day. The study took place on February 27, 2025, at Madrasah Ibtidaiyah Muhammadiyah Sedayulawas in the Brondong District of the Lamongan Regency. The data were then analyzed using Wilcoxon and Mann-Whitney tests (α ≤ 0.05). The results showed that 60% of students had poor knowledge before receiving smart box education while after receiving smart box education, 58.5% of students had good knowledge. Providing smart box education and lecture media increased students’ knowledge about earthquake disaster preparedness (p=0.000), with an average score of 25.77 in the intervention group and 4.77 in the control group. Smart box education proved to be more effective than lectures in increasing students’ knowledge of earthquake preparedness because smart boxes encourage critical thinking and hands-on learning, whereas lectures only require students to listen.

Abstract

Pain management in the elderly is a complex challenge, particulatly in a non-pharmacological context. This study aims to determine pain management in elderly patients undergoing hemodialysis at Prof. Dr. H. Aloei Saboe Hospital, Gorontalo City. The design of this study uses a quantitative approach, using a descriptive method. The population in this study were patients undergoing hemodialysis related to Non-Pharmacological Pain Management in all elderly undergoing hemodialysis at Prof. Dr. H. Aloei Saboe Hospital with a total of 46. The sampling technique used purposive sampling with a total of 31 samples. The results of the study showed that with the title Non-Pharmacological Pain Management in Elderly Patients Undergoing Hemodialysis at Prof. Dr. H. Aloei Saboe Hospital, Gorontalo City. H. Aloei Saboe Gorontalo City can be concluded that non-pharmacological pain management with the majority in the sufficient category amounted to 26 respondents (83.8%), the good category amounted to 3 respondents (9.7%) and the less category amounted to 2 (6.5%). The conclusion in this study of non-pharmacological pain management in the elderly is that most are in the sufficient category (83.8%), this study is expected to contribute to the development of more effective pain management strategies for the elderly
