Abstract

Accurate triage is fundamental to emergency nursing because it determines patient prioritization and influences clinical outcomes. Although previous studies have identified motivation, workload, subjective norms, and clinical competence as factors associated with triage performance, these determinants have largely been investigated independently. Evidence regarding their combined influence on triage accuracy remains limited. This study aimed to develop and evaluate an integrated model examining the effects of work motivation, workload, subjective norms, and clinical competence on triage accuracy among emergency nurses. A cross-sectional analytic study was conducted among 101 emergency nurses from three hospitals in Karo District, Indonesia. Participants were selected using stratified sampling. Data were collected using validated questionnaires to measure work motivation, workload, and subjective norms, while clinical competence and triage accuracy were assessed through structured observation. Multiple linear regression analysis was performed using SPSS to determine the relative contribution of each predictor to triage accuracy. The integrated regression model significantly explained 76.3% of the variance in triage accuracy (R² = 0.763; F = 34.395; p = 0.003). Work motivation (B = 0.619; p 0.001), workload (B = 0.455; p = 0.022), subjective norms (B = 0.776; p = 0.015), and clinical competence (B = 0.256; p 0.001) were all significant predictors of triage accuracy. Overall, nurses with higher motivation, stronger subjective norms, and better clinical competence demonstrated greater triage accuracy, whereas excessive workload was associated with poorer performance.Conclusion: Triage accuracy is influenced by the combined effects of behavioral, organizational, social, and competency-related factors. These findings support the implementation of integrated interventions, including competency-based training, supportive leadership, motivation enhancement, and effective workload management, to strengthen emergency nurses' triage performance and improve patient safety.

Abstract

Healthcare workers frequently experience high workloads, emotional demands, and organizational pressures that may reduce work engagement. Although job crafting has been associated with positive work outcomes, the mechanism through which organizational trust influences this relationship remains insufficiently understood, particularly in healthcare settings. This cross-sectional study was conducted at X Hospital Surabaya between 2 March 2026 and 12 March 2026. The study involved healthcare workers (nurses, doctors, and midwives) selected using purposive sampling. Data were collected using validated questionnaires measuring job crafting, organizational trust, and work engagement. Structural Equation Modeling Partial Least Squares (SEM-PLS) with 5,000 bootstrap resamples were generated to analyze the data. Job crafting significantly influenced organizational trust (?=0.585; p0.001). Organizational trust significantly influenced work engagement (?=0.725; p0.001). However, the direct effect of job crafting on work engagement was not significant (?=0.035; p=0.649). The indirect effect through organizational trust was significant (?=0.424; p0.001), indicating full mediation. Organizational trust fully mediates the relationship between job crafting and work engagement. Strengthening organizational trust is essential for translating proactive work behaviors into sustainable engagement among healthcare workers

Abstract

Transmitted infections (STIs) and unintended pregnancies. This study examined sociodemographic, geographic, and behavioral factors, including mobile phone ownership, associated with risky sexual behavior among Indonesian women. A cross-sectional analysis was conducted using data from 30,875 women aged 15–49 years who participated in the 2017 Indonesian Demographic and Health Survey (IDHS). Survey-adjusted multivariable logistic regression was used to identify independent correlates. Compared with women aged 15–20 years, those aged 36–40 years (AOR = 1.47, 95% CI: 1.24–1.78) and ?41 years (AOR = 1.36, 95% CI: 1.16–1.59) had higher odds of risky sexual behavior. Higher educational attainment was also associated with increased odds, including primary education (AOR = 2.11, 95% CI: 1.06–4.22), secondary education (AOR = 2.70, 95% CI: 1.35–5.42), and higher education (AOR = 2.59, 95% CI: 1.28–5.25). Women living in the middle (AOR = 1.72, 95% CI: 1.41–2.10) and western regions (AOR = 1.49, 95% CI: 1.24–1.79), those reporting consistent condom use (AOR = 3.31, 95% CI: 2.93–3.72), those with knowledge of HIV testing (AOR = 1.30, 95% CI: 1.15–1.46), and those owning a mobile phone (AOR = 1.20, 95% CI: 1.04–1.40) were also more likely to report risky sexual behavior. These findings support age-specific, regionally tailored, and digitally supported sexual health interventions to reduce STI risk among Indonesian women.

Abstract

Diabetic peripheral neuropathy (DPN) is one of the most common chronic complications of diabetes mellitus and a major contributor to foot ulceration, lower-extremity amputation, and reduced quality of life. Early identification of modifiable risk factors is essential to improve preventive strategies and reduce disease burden. The study aims to identify factors associated with diabetic peripheral neuropathy among patients with diabetes mellitus. A cross-sectional study was conducted among 120 patients with diabetes mellitus. This study uses an accidental sampling technique. Diabetic peripheral neuropathy was assessed using the Michigan Neuropathy Screening Instrument (MNSI), which consists of an objective foot examination, a 128-Hz tuning fork test, and a 10-g monofilament test. Knowledge level was examined using modified questionnaire Diabetic Foot Knowledge Scale. Bivariate associations were analyzed using the chi-square test, and variables with significant associations were entered into a multivariable binary logistic regression model to identify independent predictors. Statistical significance was set at p 0.05. Among the participants, 48 (40.0%) had diabetic peripheral neuropathy. Bivariate analysis showed significant associations between neuropathy and blood glucose level (p = 0.002), diabetes knowledge (p = 0.021), and duration of diabetes (p = 0.017), whereas age, smoking status, and medication adherence were not significantly associated. In the multivariable analysis, blood glucose level remained the strongest independent predictor of neuropathy (adjusted odds ratio [AOR] = 0.220, 95% confidence interval (CI: 0.080–0.608; p = 0.003), followed by diabetes knowledge (AOR = 0.423, 95% CI: 0.190–0.945; p = 0.036) and duration of diabetes (AOR = 0.405, 95% CI: 0.182-0.901; p = 0.027). Blood glucose level, diabetes knowledge, and duration of diabetes were independently associated with diabetic peripheral neuropathy. These findings highlight the importance of optimizing glycemic control, strengthening diabetes education, and implementing routine neuropathy screening to facilitate the early identification of high-risk patients and support preventive diabetes care.

Abstract

Patient safety incident reporting is a critical component of healthcare quality improvement. However, underreporting remains a persistent challenge, influenced by both individual and organizational factors. This study aimed to examine the effect of patient safety competence and the nursing work environment on patient safety incident reporting and to analyze the moderating role of the nursing work environment in this relationship. A cross-sectional quantitative correlational study was conducted among nurses from three hospitals in Malang. The study population consisted of 370 nurses. A minimum sample size of 190 nurses was calculated using proportional quota sampling based on the proportion of eligible nurses in each participating hospital, with an anticipated 10% dropout rate. A Total of 224 eligible nurses completed an online questionnaire (Google Forms). Data were collected using three validated and reliable Indonesian versions of the instruments: PES-5, H-PEPSS and ROCAES. Data analysis was performed using Smart PLS 4 to examine direct effect and to test the moderating role of nursing work environment. Patient safety competence had a positive and significant effect on patient safety incident reporting (? = 0.182, p = 0.009). The nursing work environment also had a positive and significant direct effect on patient safety incident reporting (? = 0.309, p = 0.002). Furthermore, the nursing work environment functioned as a quasi-moderator, meaning that it had both a significant direct effect on patient safety incident reporting and a significant interaction effect with patient safety competence. These findings indicate that competence is more effectively translated into reporting behavior within supportive organizational environments. Both individual competence and organizational context are essential determinants of patient safety incident reporting. Healthcare institutions should integrate systematic competency development with efforts to create supportive, non-punitive work environments to enhance reporting practices and improve patient safety outcomes.

Abstract

Adolescent mental health remains a significant public health concern. The 2022 Indonesia National Adolescent Mental Health Survey reported that approximately 34.9% of Indonesian adolescents experienced mental health problems, whereas only a small proportion accessed counseling services. This study aimed to examine changes in adolescent mental health scores following the implementation of peer group counseling as part of the Healthy School Movement. A quantitative pre-experimental study with a one-group pre-test–post-test design was conducted among 30 students at MAN 1 Banyuwangi from August to October 2025. Participants were selected using stratified sampling based on age and grade level. Mental health was measured before and after the intervention using the Mental Health Inventory questionnaire. Descriptive analysis was used to present the distribution of mental health categories, while a paired-sample t-test was performed to examine differences between pre-test and post-test scores. The results showed that the proportion of adolescents in the poor mental health category decreased from 53.3% before the intervention to 26.6% after the intervention. Conversely, the proportion in the moderate mental health category increased from 43.3% to 66.6%, while the proportion in the good category increased from 3.3% to 6.6%. The mean mental health score increased by 11.47 points after peer group counseling, and the difference was statistically significant (t = ?4.852; p 0.001). The estimated effect size was large (Cohen’s d = 0.89), indicating a substantial within-group improvement. Peer group counseling was associated with improved adolescent mental health scores and may be considered a supportive school-based strategy within the Healthy School Movement. However, because this study involved a small sample and did not include a control group, the findings should be interpreted cautiously.

Abstract

Community satisfaction is an important indicator in evaluating the quality of health services. One of the factors influencing satisfaction in village health services is the performance of village nurses who provide direct care through Posyandu activities. This study aimed to analyze the effect of village nurse performance on community satisfaction at Posyandu services in Tangkil Village. This study used a quantitative observational design with a cross-sectional approach. The population consisted of all Posyandu members in Tangkil Village, totaling 467 people. A sample of 215 respondents was selected using proportional random sampling. The independent variable was village nurse performance, while the dependent variable was community satisfaction level. Data were collected using structured questionnaires and analyzed using ordinal regression with a significance level of 0.05. The results showed that most respondents assessed village nurse performance as good (54.88%). Community satisfaction was also mostly categorized as very satisfied (54.88%). Statistical analysis indicated a significant effect of village nurse performance on community satisfaction (p = 0.000). In conclusion, better village nurse performance significantly increases community satisfaction in Posyandu services. Improving responsiveness, promotive efforts, preventive services, and communication skills is recommended to enhance public trust and participation in community health programs.

Abstract

Pending claims remain a challenge in the implementation of the INA-CBGs-based payment system within Indonesia’s National Health Insurance (JKN) program, resulting in administrative and financial burdens for hospitals. This study aimed to analyze the patterns of pending BPJS claims in cardiac arrest cases using K-Modes clustering and Random Forest classification. A quantitative analytical study with a cross-sectional design was conducted using 177 cardiac arrest medical records submitted for BPJS claims in 2024. The study population consisted of all cardiac arrest claim records, and samples were selected using simple random sampling. Inclusion criteria included complete medical records and finalized claim verification outcomes. Data were collected using a structured data extraction form from medical records and claim documents. K-Modes clustering was applied to identify claim patterns, while Random Forest was used to classify claim status and determine the relative importance of predictor variables. Model performance was evaluated using classification accuracy and feature importance measures. The analysis identified distinct claim clusters characterized by differences in diagnosis, medical interventions, and claim outcomes. Random Forest achieved an accuracy of 89.93% in classifying claim status. Cluster membership, final diagnosis, and medical interventions were identified as the most influential variables associated with pending claims. These findings demonstrate that combining K-Modes clustering and Random Forest classification can effectively identify patterns of pending BPJS claims in cardiac arrest cases and support hospital management by improving claim submission strategies and service quality. Future studies are recommended to use larger multicenter datasets and incorporate additional clinical and administrative variables to enhance model performance and generalizability.

Abstract

rescue programs is becoming increasingly widespread, serving as a vital component in the chain of survival for cardiac arrest cases. However, the effectiveness of training programs for rescuers often faces challenges regarding skill acquisition and retention. Collaborative learning has emerged as an educational trend that emphasizes social interaction and shared responsibility within small groups to accomplish tasks or solve problems. The purpose of this study was to determine the effect of collaborative learning in efforts to increase knowledge of AED use in BLS (Basic Life Support). This study employed a pre-experimental, one-group pre-test/post-test design. The study population consisted of 58 nursing students from a vocational college in Kediri, selected via total sampling. The knowledge of Automated External Defibrillators use in Basic Life Support were gathered using a questionnaire and analyzed with the Wilcoxon Signed-Rank Test. The results demonstrated a statistically significant improvement in knowledge levels (p-value = 0.000 0.05), with the mean score increasing from 1.76 in the pre-test to 2.40 in the post-test. Furthermore, the calculated effect size was -0.67 indicating that collaborative learning has a large effect on improving participants knowledge. This study was reported in compliance with the STROBE guidelines. A recommendation for future research is to integrated the collaborative approach with technology-based media, such as video-guided training or interactive CD-ROMs.

Abstract

Radiology turnaround time (TAT) is a crucial indicator of hospital service effectiveness, as delays in reporting can hinder clinical decisions, prolong queues, and disrupt patient flow. This systematic review examined how radiology workflows are measured, where bottlenecks occur, and which interventions improve TAT. The review followed PRISMA 2020 and searched PubMed, Scopus, Web of Science, Google Scholar, and Cochrane Library for hospital-based studies published from January 2019 to February 2026. Eligible studies reported TAT indicators, workflow stages, bottlenecks, or process-improvement interventions. Eighteen studies were included and synthesized narratively because TAT definitions and outcome measures were heterogeneous. Most studies measured TAT from image acquisition completion to preliminary, final, or signed reports, but several also used order-to-start, completion-to-PACS, time-to-view, and critical-result communication indicators. The main delays occurred during scheduling/preparation, image transfer, interpretation, reporting, and final communication. Lean redesign, protected emergency workflows, worklist optimization, dedicated emergency radiologists, 24/7 coverage, dashboard monitoring, and well-governed digital or AI tools reduced TAT and improved patient flow, including emergency department length of stay and time to surgery. Indonesian evidence remains limited and needs phase-based TAT standards, routine timestamp audits, and multi-site evaluation. Hospitals should prioritize standardized measurement before expanding technology-based interventions.
