Abstract

Workload is one of the critical factors influencing nurses’ productivity in hospital settings. Excessive workload may lead to fatigue, stress, and decreased concentration, ultimately compromising service quality and patient safety. At Pemangkat Regional General Hospital, Sambas Regency, the imbalance between the number of nurses and patients, combined with limited resources, contributes to increased workload and reduced productivity. This study aims to analyze the effect of workload on the work productivity of nurses. This study employed a quantitative approach with a cross-sectional design. A total of 172 nurses were selected through simple random sampling. The workload variable was measured using the Copenhagen Psychosocial Questionnaire III (COPSOQ III), while work productivity was measured using the Indonesian version of the Individual Work Performance Questionnaire (IWPQ). Primary data were collected through self-administered questionnaires, and data analysis was performed using IBM SPSS Statistics version 25. Statistical tests included univariate, bivariate, and multivariate analyses with a 95% confidence level. The Chi-Square test was used to examine the relationship between variables, and linear regression analysis was conducted to determine the influence of workload on productivity. The findings showed that 59.3% of nurses experienced a high workload, and 51.7% were categorized as unproductive. Statistical analysis indicated a significant relationship between workload and nurse productivity (p = 0.011). The Prevalence Ratio (PR) value of 2.232 (95% CI: 1.200–4.152) revealed that nurses with high workloads are 2.2 times more likely to be unproductive than those with lower workloads. High workload negatively affects nurses’ performance by increasing fatigue, psychological strain, and reducing motivation. Effective workload management through adequate staffing, equitable scheduling, and supportive leadership are crucial to maintain nurses’ well-being and optimize productivity. Workload has a significant affects nurse productivity at Pemangkat Regional General Hospital. Implementing workload control strategies is recommended to sustain productivity and service quality.

Abstract

Blood pressure is one of the parameters used as a reference for the success of donor selection. Blood pressure is the pressure pumped by the heart against the artery walls. Acceptance criteria in donor selection regarding blood pressure are a systolic pressure of prospective donors of 90-160 mmHg, with a diastolic pressure of 60-100 mmHg. This study aims to determine the characteristics of donors who experienced rejection due to high blood pressure. The research design used was descriptive quantitative. The population in this study was all donors who were rejected due to high blood pressure with a total sampling technique. Data were collected using a tally sheet. Data were obtained from secondary data documentation studies from the management information system application (Hero Blood) and analyzed using frequency distribution and percentage of the characteristics of rejection due to high blood pressure. The results showed that rejection of prospective donors due to high blood pressure was most common in the age group 46-55 years (38.49%), male donors (82.57%), blood type O (40.94%), and private employees (54.85%). These findings emphasize the importance of early detection of blood pressure, especially in male donor candidates aged over 46 years. For further research, it is important to detect factors that influence the incidence of hypertension in potential donors.

Abstract

The experiences of nurses in assisting the elderly require considerable attention and time, particularly in fulfilling meal-related needs. Aging is accompanied by a decline in physiological functions, which can affect the ability of the elderly to eat independently. The purpose of this study was to explore nurses’ experiences in assisting older adults with eating activities. This study employed a descriptive qualitative design with a phenomenological approach. A total of eight nurses were selected using purposive sampling technique. The research was conducted in seven nursing homes located in East Java Province. Data were collected through in-depth interviews guided by structured interview questions and supported by field notes. The data were analyzed using Colaizzi’s method with the aid of the QDA Miner Lite application tool. Four major themes emerged from data analysis: 1) Assessment of the Eating Ability of the Elderly in Nursing Homes, 2) Nurses’ Experiences in Feeding the Elderly including determinants such as type of food, eating frequency, and meal times, 3) Incidence of choking among the elderly caused by swallowing disorders, age-related factors and body positioning, 4) Feeding challenges among the elderly including decreased appetite, food selectivity, dissatisfaction with meals, texture modification needs, food mixtures, slow eating pace and the need  for additional snacks. A comprehensive understanding of nurses’ experiences in assisting the elderly individuals during feeding activities is essential for improving appetite and maintaining adequate nutritional intake among older adults.

Abstract

Hypertension has become one of the leading causes of death due to its potential to cause systemic complications. Individuals diagnosed with hypertension must undergo regular treatment and medication because they have a high risk of recurrence. One of the factors influencing this is intrinsic factors such as positive emotions. The purpose of this study is to analyze positive emotions in recurrence symptoms in hypertensive. The study design employed a correlational analysis with a cross-sectional approach. The study population consisted of 98 hypertensive in Karangbong Village, Pajarakan, Probolinggo. The sample size was 79 individuals who met the inclusion and exclusion criteria. The sampling technique used was purposive sampling. Inclusion criteria included patients with hypertension for more than 3 years, with a history of hypertension with systolic blood pressure ? 120 mmHg and diastolic blood pressure ? 80 mmHg. Exclusion criteria included patients with hypertension who had visual and hearing impairments, and complications such as diabetes mellitus, heart disease, and stroke. The positive emotion instrument used the Dispositional Positive Emotions Scale (DPES) questionnaire and the hypertension recurrence symptom questionnaire. Data analysis used the Spearman rank test. The findings of the study indicated that most respondents had positive emotions (68%) and hypertension recurrence symptoms (66%). There was a significant correlation between positive emotions and recurrence symptoms in hypertensive patients with a p value of 0.001. Hypertensive patients are prone to recurrence symptoms due to low positive emotions. Respondents are encouraged to manage their emotions to prevent the onset of hypertension recurrence symptoms. Future research is recommended to incorporate psychotherapeutic interventions, such as peer support groups, to further explore their potential impact on the outcomes.

Abstract

Stroke is the leading cause of death and disability throughout the world, with a higher burden in low- and middle-income countries, including Indonesia. As the elderly population grows, identifying factors linked to stroke is essential for prevention and policy-making. This study aims to examine the association between sociodemographic, behavioral, and clinical factors and stroke among Indonesians aged ? 60 years using data from the 2023 Indonesian Health Survey (Survei Kesehatan Indonesia/SKI). This study employed a cross-sectional design using secondary data from a nationally representative survey. A total of 97,339 older adults were included in the analysis. Stroke status was based on self-reported physician-diagnosed status. Independent variables included age, sex, education, marital status, employment, residence, hypertension, heart disease, smoking, and alcohol consumption. Data were analyzed using complex survey analysis with sampling weights, utilizing descriptive statistics and Chi-square tests to examine associations between variables. Analyses were conducted using IBM SPSS Statistics version 27.0.1, adjusting stratification and clustering in the survey design. The prevalence of stroke was 3.2%. Significant associations were found between stroke and age, gender, education, occupation, place of residence, hypertension, heart disease, smoking habits, and alcohol use (all p0.05). Stroke was more common among individuals with hypertension, heart disease, lower educational attainment, those who were unemployed, and those living in urban areas. Stroke in older Indonesians is strongly influenced by modifiable factors, particularly hypertension and lifestyle behaviors. Targeted prevention, early detection, and lifestyle interventions are critical to mitigating the stroke burden in Indonesia’s aging population.

Abstract

This study aimed to evaluate the impact of employee performance appraisal systems on healthcare service quality in hospitals. A systematic literature review was conducted by analyzing 12 peer-reviewed articles published between 2020 and 2025, selected from prominent academic databases such as PubMed, Scopus, and ScienceDirect. These articles were reviewed based on their relevance to the relationship between performance appraisals and healthcare quality. The review identified that well-structured performance appraisal systems significantly contribute to enhancing employee motivation, which directly influences the overall quality of healthcare services. Transparent, objective, and fair appraisal systems, when implemented effectively, help align employee goals with organizational objectives, leading to better patient care. However, the study also highlighted several challenges, including bureaucratic barriers, a lack of evaluator training, and cultural misalignment, which hinder the successful implementation of these systems in hospitals. Additionally, the review found that performance appraisals, when conducted periodically with constructive feedback, encourage continuous professional development and improve service delivery. The study concluded that for a performance appraisal system to be truly effective, it must be ongoing, incorporate clear communication, and be adaptable to the hospital’s organizational culture. Future research should focus on longitudinal studies to assess the long-term effects of performance appraisals on healthcare quality. Furthermore, combining qualitative and quantitative methods may provide a deeper understanding of the complex interactions between performance appraisals and employee performance in healthcare settings.

Abstract

Cigarette smoking is a major contributor to health problems worldwide and is highly prevalent in Indonesia, especially among young adult males. Nicotine in cigarettes disrupts lipid metabolism, leading to increased triglyceride levels and dyslipidemia, which are closely related to the double nutrition problem, characterized by abnormalities in the lipid profile. Smoking induces these abnormalities, indicating the need for antioxidant administration. This study aims to investigate the effect of Polysaccharide Peptides from Ganoderma lucidum on lowering triglycerides lipid profile level in active smokers with dyslipidemia. This experimental study used a pretest-posttest control group design involving 28 active smokers aged 18-21 years with dyslipidemia. Subjects were assigned to either a polysaccharide peptide (PsP) therapy group, receiving PsP extracted from Ganoderma lucidum, or a control group. Fasting serum triglyceride concentrations were measured before and after treatment. Statistical analyses used paired and unpaired t-tests to assess triglyceride level changes. The results showed a significant lowering of triglyceride levels in the PsP-treated group (p= 0.006). While the difference before and after PsP administration reached marginal statistical significance (p= 0.059), clinical improvement in triglyceride profiles was observed compared to controls. In conclusion, Ganoderma lucidum-derived PsP lowers triglyceride lipid profiles in smokers with dyslipidemia. These findings support the use of PsP as a therapeutic herbal intervention to improve lipid metabolism disturbed by smoking-induced inflammation.

Abstract

Temper tantrums are a common behavioral challenge during early childhood development. Managing this behavior is important to support children’s emotional and social development. One approach that has been explored to address this issue is the use of cooperative games, which emphasize collaboration and shared goals among children. This study examined the effect of cooperative games and art therapy on temper tantrum reactions in preschool-aged children at Ulul Albab Kindergarten, Jombang. Through a quasi-experimental method with a pretest-posttest control group design, Data were collected from 78 children as a population. The sampling technique used Stratified Random sampling obtained 69 research samples by applying the proportional strata formula. The research instrument used the Temper Tantrum Rating Scale. The research sample was divided into 3 groups, namely the cooperative game play intervention group, the art therapy intervention group and the control group with 23 students as respondents in each group. After the cooperative game intervention was carried out for three weeks, statistical analysis with a paired t-test proved the effect of cooperative game therapy with a significant value (p = 0.000) and art therapy with a significant value (p = 0.001) while the Welch's ANOVA test showed a poshoc value (p = 0.032) Cooperative game play intervention was more effective than art therapy. These findings suggest that cooperative games are effective in reducing temper tantrum reactions. Therefore, nurses should introduce and recommend this approach to parents as a useful strategy for managing temper tantrum behavior.

Abstract

Cognitive decline is a common health problem experienced by older adults and can affect their independence and quality of life. This issue is also aligned with the Sustainable Development Goals (SDG 3: Good Health and Well-Being), which highlight the importance of improving health and well-being across all age groups. Non-pharmacological cognitive stimulation, such as Jigsaw Puzzle Therapy (JPT), is one of the interventions that aims to help maintain brain function. Objective: To describe the implementation and outcomes of Jigsaw Puzzle Therapy (JPT) in an older adult with cognitive impairment at Griya Lansia Ciparay. This case report employed the nursing care process approach involving a 74-year-old female older adult. Cognitive function was assessed before and after the intervention. JPT was administered daily for seven consecutive days using picture-based puzzles with a low level of difficulty. An improvement in cognitive ability was observed, indicated by an increase in cognitive function scores from 15 to 20 after the intervention. The most notable improvements were seen in time and place orientation as well as basic calculation skills. This case report shows that seven sessions of Jigsaw Puzzle Therapy were associated with improvements in the client’s cognitive scores. JPT may serve as a simple and feasible activity to provide cognitive stimulation for older adults in long-term care settings.

Abstract

Electronic Medical Records (EMR) as a form of healthcare digitalization, aim to improve the effectiveness, efficiency, and quality of healthcare. However, the transition from paper-based systems remains limited in many countries and continues implementation challenges. review aims to identify facilitators and barriers of EMR implementation among healthcare workers in hospitals settings. The study design used was a scoping review. A comprehensive search was conducted using the PCC (Population-Concepts-Context) strategy, using terms related to factors influencing the implementation of EMR by healthcare workers in hospitals, in four databases (ProQuest, ScienceDirect, PubMed, and Google Scholar) for studies from 2020 - 2025. Article selection was performed using the PRISMA-ScR guidelines. A total of twenty-eight studies met the inclusion criteria. The findings indicate that facilitators and barriers of EMR implementation emerge from the interaction between staff-related, organization, and technological factors. Key facilitators include digital literacy, organizational training support, and system quality. Major barriers involve low staff readiness, insufficient organizational support, and technology quality and complexity issues. Most studies conducted in low- and middle-income countries (LMICs) reported staff-related factors as facilitators and barriers of EMR implementation. Thus, effective implementation strategies need to be designed contextually through the simultaneous strengthening of human resource capacity, organizational support, and technological quality.
