Abstract

The palm oil mill industry has many risks to occupational safety and health. There are still many work accidents that occur resulting in losses for the company. Therefore, it is necessary to do a risk analysis wit importance index that assesses risk by considering the frequency index and severity index. Risk assessment is carried out by experts namely mill manager, mill assistants, and foreman. Then the risk identification is done based on the cause and effect of the risk using the Bow Tie method. Risk assessment indicators use risk indicators from palm oil mills. From 54 risk indicators, there is 3 significant risk, namely, employees exposed to hot iron (stew operation), employees inhaled /exposed to chemicals (WTP operation), noise (engine room operation). And there are 2 high risks, namely employees exposed to steam to hot steam (stew operation), employees exposed to fire bursts (boiler operation). The causes of significant and high risks are leaking pipes, opening the stew doors when high pressure, stew doors are not tightly closed, employee negligence, employees do not obey work instructions, high air pressure, lack of noise absorbent, material, there are worn, engine parts. The impact of significant and high risk burns to the skin, material loss, shortness of breath, skin irritation, communication, disorders, damage to hearing. Control of significant and high risk using a complete PPE, providing evacuation tools.

Abstract

This research discusses the analysis of the consequences of gas dispersion, fire, and explosion at SPPBE PT Aroma Jaya Sejati. This study uses a semi-quantitative method that uses secondary company data and direct observation. The data is then analyzed using ALOHA software to determine the extent and impact of gas dispersion, fire, and explosion due to leakage of LPG storage tanks. The results of research in scenario A, scenario A propane dispersion yellow zone reaches a distance of 262 meters, with a propane concentration of 5,500 ppm, the orange zone of propane dispersion scenario A reaches a distance of 133 meters, with a propane concentration of 17,000 ppm. The red zone of propane dispersion reaches a distance of 89 meters, with a propane concentration of 33000 ppm. In scenario B, the yellow zone of propane dispersion reaches a distance of 72 meters with a propane concentration of 5500 ppm, the orange zone reaches a distance of 34 meters with a propane concentration of 17000 ppm, the red zone reaches a distance of 23 meters, with a propane concentration of 33,000 ppm. Meanwhile, jet fire scenario A, for 39 minutes with total burning propane of 46,085 kg with a maximum fuel rate of 3740 kg/minute. The maximum length of the flame reaches 37 meters. Scenario B, a jet fire occurs for 1 hour with total burning propane of 15,088 kg with a maximum fuel rate of 256 kg/minute. The maximum length of the flame reaches 10 meters. Conclusion of propane dispersion in scenario A and B, it is found that the safe distance for propane dispersion is after 262 meters, while the results of jet fire modelling with scenario A and scenario B show that the safe distance for jet fire is after 133 meters.

Abstract

behaviour-based Safety is a tool for observing behaviour to create safe behaviour and prevent work accidents. This study aims to evaluate the behaviour-based Safety program in the observation card program at PT Penajam Banua Taka Harbor. The National Safety Council (NSC) (2011) conducted research which resulted in 88% of the causes of work accidents due to unsafe behaviour, 10% due to unsafe conditions and 2% of unknown causes. PT Denso Indonesia's work accident rate has also decreased by 66.67–88.89% so that BBS activation in the company can be prevented and reduce the incidence of work accidents. Based on preliminary study data, it was obtained that 30% of employees were mature about safety-based programs and 60% reported unsafe action figures. Employees are not yet committed to reporting unsafe action findings and unsafe conditions with the observation card program. This causes the objectives of the behaviour-based Safety Program to prevent work accidents have not been optimally achieved. This research is descriptive quantitative. The population are an employee of the Operational Department of PT Pelabuhan Penajam Banua Taka, the amount of sample is 37 respondents. The sampling technique used is simple random sampling. In the input element, the Sub-element of Critical Safety Determination Behaviour has the highest effectiveness value, which is equal to 78.05%, while the training has the lowest effectiveness value, which is 73.94%. The Observation and Feedback Process element has the highest effectiveness value is the implementation of observations, which is 76.75%. meanwhile, Field / Equipment / System Modification has the lowest effectiveness value, which is 70.27%.

Abstract

Hazardous and toxic materials and waste have a negative impact on human safety, human health, environmental protection. More than 26 million types of chemical compounds circulating in the world will cause dangerous substances and toxic substances, while in Indonesia there are almost 5.000 species. According to the Central Statistics Agency of the KALTIM Province, the number of cars is 184.566 units, and a large number of vehicles will certainly also increase the use of oil and oil changes, where used oil is included in the dangerous substances and toxic substances category. For motor vehicle repair shops serving oil changes, it is called a producer of dangerous substances and toxic substances and is required to store dangerous substances and toxic substances. According to data from PT. Serasi Mitra Mobil (June 2019), can produce B3 liquid waste of used oil by 30 litres per month. The research objective is to determine the storage of hazardous and toxic waste case studies at PT. Serasi Mitra Mobil in Balikpapan. This research was conducted by direct observation and in-depth interviews related to the storage of hazardous and toxic waste. Based on direct observation and in-depth interviews, PT. Serasi Mitra Mobil as a produces of dangerous substances and toxic substances are left to other parties. Dangerous substances and toxic substances included in the appropriate packaging but not labelled and symbol, also not placed in a separate building. The necessary repairs consist of having dangerous substances and toxic substances facilities in the form of buildings, tanks or containers, slime, waste piles, waste impoundment, hazardous and rain protected buildings and rain and sun, having lighting, ventilation, drainage, drainage basins, waste buildings dangerous substances and toxic substances must be adhered to by dangerous substances and toxic substances, having emergency equipment.

Abstract

ISO 45001: 2018 was published on 12 March 2018 to replace OHSAS 18001: 2007 so that many lecturers, students, workers and management teams of an organization are interested in studying it. One of them was the management team of company X who attended the ISO 45001 training held by consultant Y in Yogyakarta. This study aims to determine whether the persuasive communication provided by the communicator to the communicant is effective or not. The research methodology used is descriptive statistics. The results showed that the management team of company X had understood the messages given by the communicators. It can be concluded that ISO 45001 persuasive communication runs effectively.

Abstract

The purpose of this study is to provide a description of passive fire protection material specifications that are suitable and suitable for pipe rack protection, pipe support and hanger pipe support hotline hydrocarbons and working methods of application of epoxy intumescent material. The research method used is the applied research method with a case study approach to the difficulty of handling sub-standard conditions at the Balikpapan I refinery. The research was conducted for 6 months and an observation period of 2 years. The results showed that the epoxy intumescent material specifications could be applied well and safely with an application thickness of 8.13 mm for a 1.5-hour fire rating according to API Standard 2218 and covered insurance findings at the Balikpapan refinery 1. The success of the intumescent epoxy application replaced the fireproofing material based on cementitious this can be replicated to overcome similar problems at other process unit refineries that experience similar problems.

Abstract

This study aims to determine the relationship of heat temperature and age with work fatigue in the area of Punggur Final Disposal Of 2019. This research uses observational analytic research using a cross-sectional approach using quantitative methods. Is a study that studies the relationship between risk factors (independent) with effect factors (dependent), the number of samples in the study amounted to 30 workers. The study was conducted in the area of Punggur Lake in Batam City. The study was measured by using a questionnaire and measuring instruments using the QUEStempº32 Thermal Environment Monitor (measuring hot work environment) ISBB / WBGT results were obtained. Test analysis using the chi-square test. The results of this study obtained the value of P-value for heat, and age less than 0.05. This means that Ho in Reject there is a significant relationship between the factors of Heat Temperature, Age With Work Fatigue. Conclusion There is a Significant Relationship between Heat Temperature and Age on Work Fatigue in workers in the Telaga Punggur TPA area in Batam City in 2019. Suggestions are expected management of Telaga Punggur TPA Providing adequate drinking water and providing sufficient rest periods for workers pedestrians also try to cross to use sidewalk facilities on the other side that are not used to sell traders who have the risk of conflict between pedestrians and vehicles at the crossing point.

Abstract

PT. XYZ is one of the companies engaged in the palm oil processing industry which often experiences high occupational accident caused by an inadequate K3 management system (SMK3), this case if left unchecked can be detrimental to the company decreased productivity as well as material losses. Costs that must be incurred by the company for each work accident case that occurs are quite large, namely RP. 59.555.539.00 in 2018. By looking at potential hazard and handling work accidents that have not been going well, this company requires hazard identification assessment of risk and Opportunities (HIARO) is one of the latest methods based on ISO 45001. The concept of HIARO takes into account the management of work, social factors (including workload, work hours) leadership, culture in the organization. The result of the study using the HIARO method of the sterilizer station after data processing for January-December 2018 total accident of 34 cases, for a total Frequency Rate value of 1167,56, for the value of Severity Rate of losses workdays of 26 days in 2018, for a total Severity Rate of 0,94. For Boiler station the total accident is 32 case, for a total Frequency Rate value of 995,86, for the value of Severity Rate of lost workdays is 42 days in 2018, for a total Severity Rate of 1,5. This figure shows that K3 Management is not good, so the writer gives a proposal that is to provide special PPE for Sterilizer and Boiler stations such as, Earplug, Handy Talkie, Heat Resistant Glove, Coverall Nomex, Doing Five Minute For Safety.

Abstract

Law Number 1 of 2009 concerning Aviation Article 136, "the transportation of special and dangerous goods must meet the requirements of aviation safety and security". Good handling of air cargo, especially dangerous goods, needs to be supported by personal knowledge, handling and transportation facilities, and optimal standard procedures. The research objective was to determine the knowledge of personnel in handling dangerous goods. The research approach used quantitative methods with a total of 30 respondents. The validity of all items of Variable X and Variable Y is declared valid because the r-count is greater (>) r-table. Each Cronbach's alpha value for variable X is 0.808 and variable Y is 0.781. For the reliability value of variable X is stated to be very strong and the reliability value of variable Y is in a strong category. Relationship knowledge of personnel in dangerous good handling is 0.744. In conclusion, there is a strong positive relationship of 0.744 between the knowledge of personnel in handling dangerous goods at PT Angkasa Pura Logistik Balikpapan.

Abstract

Cleaning of pipelines is necessary to ensure that the pipeline remains in a condition that is suitable for use, does not contain impurities that can affect the product or affect the condition of the pipe. The research objective is to identify the K3 hazards and risk assessment of the pig acceptance process using Hazop’s techniques, as well as identify them. This study used a qualitative observational approach by assessing the 6 Node P&ID diagrams in the pigging process. Assessment using the Hazops table. The results showed that the first node of Pig Barrel 20 "MOL (HMC) identified 2 critical deviations, namely: less pressure and less flow; 5 Causes, and 5 Consequences, 2nd Node Pig Barrel 12 "MOL (Condensate) identified 2 critical deviations, namely: Less Pressure and Less Flow; 6 causes, and 5 consequences, the 3rd node of the Sump Tank (T-2080) identified 3 critical deviations, namely: High level, No pressure of the pump, and More sand; 5 Causes, 6 Consequences, 4th Node Close Drain (V-915) identified only 1 (one) critical deviation, namely: High Level; 3 Causes, 4 Consequences. The 5th node of the First Stage Separator HMC (V-860) identified 4 deviations, namely: High Level, High Pressure, Low Level, Low Pressure; 11 Causes, 15 Consequence, 6th Node First Stage Separator Condensate (D-6250) identified 5 (three) deviations, namely: High Level of Condensate, High Pressure, Low Level of condensate, Low Pressure, High Level of Water; 13 Causes, 19 Consequences. The conclusion in this study is that there are 17 deviations in the pigging work process and there are 5 hazards with low risk, 12 hazards with moderate risk, 4 hazards with high risk.
