Abstract

The novel coronavirus infection (COVID-19), now a worldwide public health concern is associated with varied fatality. Patients with chronic underlying conditions like hypertension have shown worst outcomes. Hypertension is a multifactorial disease and a public health burden, as it is a risk factor for diseases like stroke, coronary artery disease, and heart failure. As one of the most important public health problems in the 21st century, hypertension affects more than one billion individuals worldwide. Hypertension is a common comorbidity in COVID-19 patients. However, the association of hypertension with the severity and fatality of COVID-19 remain unclear. Hypertension is a major risk factor regarding the severity and fatality of COVID-19, and the RAAS plays an important role in COVID-19 infection since SARS-CoV-2 can lead to a dysregulation of the system by reducing the ACE2 expression. Hypertension may also affect the prognosis of COVID-19 illness. The severity rate of COVID-19 in hypertensive patients was much higher than in non-hypertensive cases. A systematic literature search was conducted using several databases on studies that assess hypertension and outcome in COVID-19. Composite of poor outcome, comprising of mortality, severe COVID-19, acute respiratory distress syndrome (ARDS), need for intensive care unit (ICU) care and disease progression were the outcomes of interest. Authors independently reviewed some titles, abstracts and read full-texts of articles to identify potentially eligible studies to decide about final inclusions. This systematic review was conducted based on the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) Statement. We used six different databases: Pubmed, Clinical Key, Science Direct, Cochrane Library, Springer Link and Oxford Academic. A total of 2731 articles from the databases were identified, we got three articles that fulfill the criteria. We excluded the other studies by reading the title, abstract, and full text assessment. Hypertension was associated with the severity and mortality of COVID-19 illness. Hypertension was also independently associated with a significantly increased risk of critical COVID-19 and inhospital mortality of COVID-19.

Abstract

Vitamin D deficiency (VDD) is a common problem in reproductive-aged women and has become a major public health problem worldwide. Adverse health outcomes such as preeclampsia, low birthweight, neonatal hypocalcemia, poor postnatal growth, bone fragility, and increased incidence of autoimmune diseases have been linked to low vitamin D levels during pregnancy and infancy. A systematic literature search was performed on September 6th – 21st, 2022 using respected database. The identified articles were screened using the chosen eligibility criteria. Literature selection was performed with rest time restriction from January 1, 2017, and September 22, 2022 and restricted to only published in English articles. Three papers were evaluated in full-text after duplicates were removed. Tammo et al, Mansur et al and Moralez et al were the three studies that remained following full-text screening utilizing eligibility criteria and quality assessment showing a significant proof regarding this review. Vitamin D deficiency had wide variety of effects in pregnancy as it may increased risk of multisystem disorders in pregnancy such as preeclampsia, low birth weight in neonates, insulin resistance, gestational diabetes mellitus (GDM), and a higher risk of primary cesarean delivery. Optimal level of 30 ng/mL or more before conception and throughout the entire pregnancy with different dosages available, avoiding levels below 20 ng/ml.

Abstract

Immunization is one of the most successful, most accepted and proven cost-effective public health preventive interventions in the world and has saved 2 to 3 million children from immunization-preventable diseases.  The COVID-19 outbreak and the effort being made to overcome the outbreak could cause negative impacts on the utilization of basic immunization services. During the outbreak, basic immunization coverage rate in various regions had decreased. This article conducted a comprehensive and systematic literature search in electronic databases (PubMed, ScienceDirect, Sage Journal, SpringerLink, ClinicalKey, and Oxford Academic) with articles published in the last 5 years and adjusted according to inclusion and exclusion criteria. This systematic review use PRISMA (Preferred, Reporting Items for Systematic Review and Meta-analysis). There were 2 studies that met the final inclusion criteria. The results showed a significant reduction of basic immunization coverage happen especially during the early period of pandemic.  Six themes are identified related to the decrease: 1) Covid-19 lockdown; (2) motivation for childhood immunization; (3) availability of biosafety procedures and personal protective equipment (PPE); (4) service adjustments and guidelines during the pandemic; (5) the availability of vaccines; and (6) the resilience of the immunization program in view of COVID-19. Childhood routine immunization coverage rate significantly decreased during the early period of the pandemic.  Interventions for providers and parents is required to ensure adequate catch-up of delayed/missed immunizations in order to prevent potential outbreaks of vaccine-preventable diseases.

Abstract

Grade II eyelid burns are burns that reach the epidermis and part of the dermis of the palpebrae and can have impaired wound healing. TGF β1 has the broadest spectrum of action, affecting all cell types involved in all stages of wound healing. TGF β1 increases rapidly after injury and reaches peak levels three days after injury, which coincides with the peak of the inflammatory phase of wound healing. Decreased expression of TGF β1 is associated with impaired wound healing. Aloe Vera has long been known in Indonesia as a healing plant. Aloe Vera extract contains a variety of bioactive compounds that play a role in stimulating wound healing. The administration of Aloe Vera extract on thermal burns aims to reduce inflammation, increase collagen content, and increase the rate of re-epithelization. The administration of Aloe Vera gel extract allows faster healing of thermal burns and rebuilds the vascularity of the wound tissue. Objective of this study is to determine the difference between the expression of Transforming Growth Factor Beta 1 (TGF β1) in a rat model of thermal burn injury grade II of the eyelids treated with topical Aloe Vera gel extract with the concentrations of 20%, 40%, Balanced Salt Sodium (BSS) and a control group that was not given burn intervention. An experimental study with the post-test-only approach with a control group design. 24 Rats have induced grade II eyelid thermal burns, divided into four groups: Normal (without treatment), negative control (BSS), given Aloe vera 20%, 40%. Then TGF β1expression was quantified using the Image J application. The Topical application of Aloe vera gel can increase the expression of TGF along with the increase in dose. The highest rise in TGF expression was found in the group with 40% Aloe vera gel, followed by 20% Aloe vera gel compared to the negative control (BSS Balanced Salt Solution). Aloe Vera can increase TGF expression in a rat model of grade II palpebral burn injury and is dose-dependent.

Abstract

The use of nuclear technology is increasingly high in various aspects, one of which is medicine, especially in the radiology section. X-rays have a very high penetrating power against the material they pass. So that X-rays can be used as a diagnostic and therapeutic tool in medicine. However, the risk of possible harm to radiation workers is that it can cause hematopoietic damage (blood disorders), such as anemia, leukemia, and leukopenia. Therefore, radiation protection and safety measures are indispensable. Knowledge of the risk of radiation hazards is one of the things that can prevent the impact of radiation. Objective: This study aims to determine the relationship between Radiation Hazard Risk Knowledge and the Level of Compliance with the Use of PPE for Radiation Workers in the Radiology Section of Hospitals in Palembang. Method: This study is an observational analytical, quantitative research design using a cross-sectional approach on 25 respondents’ radiation workers in the radiology department of RSUP Rivai Abdullah, and RSUD Palembang Bari were taken using total sampling. The instrument uses questionnaires on the level of knowledge of radiation hazards and the level of compliance. Results: Fisher's analysis obtained a value of p = 1000, meaning there is no relationship between knowledge and compliance. Conclusion: The results of this study show no relationship between radiation hazard risk knowledge and the level of compliance with the use of PPE for radiation workers in the radiology department of hospitals in Palembang.

Abstract

Pneumothorax is an accumulation of air between the lungs and the chest wall that can occur either spontaneously or in conjunction with an underlying lung disease (primary spontaneous pneumothorax; secondary spontaneous pneumothorax, respectively). The primary manifestations of this ailment are chest discomfort and dyspnea. Secondary spontaneous pneumothorax is a common and potentially life-threatening consequence of pulmonary tuberculosis (TB) that frequently occurs in clinical settings and requires prompt and effective treatment. This case report seeks to comprehend the underlying mechanisms of spontaneous pneumothorax resulting from tuberculosis and effectively diagnose and treat this condition. A 37-year-old male presented to the emergency department at Abdoel Moeloek Regional General Hospital in Lampung Province with worsening dyspnea, followed by coughing and right-sided chest pain. By doing a thorough analysis of the patient's medical history, performing a physical examination, and conducting other tests, it was determined that the patient is suffering from a right secondary spontaneous pneumothorax, which is a result of pulmonary tuberculosis. The patient had WSD installation management and thereafter received hospitalization for symptomatic therapy and antibiotic treatment.

Abstract

Paracetamol is a widely utilized medication globally and is the primary cause of poisoning incidents in high-income countries. Paracetamol induces hepatotoxicity that is depending on the dosage when taken in excessive amounts. The Moringa oleifera (MO), belonging to the Moringaceae family, is one of the medicinal plants investigated for its potential in treating hepatotoxicity. The MO tree extracts have been documented to possess inhibitory effects, primarily against hepatitis drug-induced hepatotoxicity, owing to their bioactive components. An experimental laboratory enrolled using 36 samples of male white rats (Rattus norvegicus), which were divided into six groups: standard control group (K), positive control group (K1) given paracetamol 3000mg/kg BW on 15th days; antioxidant control group (K2) given vitamin c 500mg/kg BW for 14 days; treatment group 1 (P1), treatment group 2 (P2) and treatment group 3 (P3) each given moringa oleifera leaves extract 200mg/kg BW, 400mg/kg BW and 800mg/kg BW for 14 days. Each group, except the standard control group, was given paracetamol 3000mg/Kg BW on the 15th day. On the 16th day, the mice were euthanized, and then their livers were made into histological preparations and examined using a microscope at 40 times magnification. The level of liver cellular damage was categorized into scoring based on the level of damage. Score 1 if no damage, score 2 if parenchymatous degeneration was found, score 3 if hydropic degeneration was found, and score 4 if necrosis was found. The average scoring per group was calculated as K: 1.1; K1: 3.17; K2: 1.57; P1: 1.5; P2: 1.73; P3: 1.9. K1 appeared necrotic. Inflammation and hydropic degeneration were found in K2, P1, P2, and P3. Data were analyzed using the Kruskal-Wallis Test, obtained p = 0.000 (p < 0.05). Moringa leaf ethanol extract affected hepatic induction by paracetamol because significant differences were found between groups P1, P2, and P3 with K1 (p < 0.05). There were no significant differences between groups K2 and P1 (p > 0.05), showing that the effect of giving 500mg/kg BW vitamin C was equivalent to providing 200mg/kg BW Moringa leaves. It was concluded that giving ethanol extract of Moringa oleifera leaves protected the acute histopathological damage of the liver of male white rats of the Sprague Dawley strain induced by paracetamol.
