Abstract

Hypertension is a direct cause of various cardiovascular diseases that can cause complications if not controlled. Drug Related Problems (DRPs) are treatment-related problems that can intervene in therapeutic outcomes, one of which is blood pressure. This study analyzed the relationship of factors that influence the case of DRPs in hypertensive patients. This research was an observational quantitative study with a cross-sectional study design conducted by direct interview method to 141 hypertensive prolanis patients in Semarang City Health Center. The instrument used was a questionnaire. The results showed there was a relationship between the number of drugs (p = 0.038), comorbidity (p = 0,000), and blood pressure (p = 0,000) with the case of DRPs. There was no relationship between the area of residence and the case of DRPs (p = 0.569). Multivariate models showed the most influential factors were comorbidity (p = 0,000, OR: 21,689) and blood pressure (p = 0,000, OR: 13,277). The conclusion of this study is comorbidity and blood pressure are the predictors that most influence the case of DRPs in hypertensive patients.

Abstract

A multidisciplinary team approach in an integrated practice unit has been proposed as part of value-based health care in hospital diabetes management strategy. The objective of this study is to assess the economic impact of integrated multidisciplinary team implementation compared to conventional care in the management of Type 2 Diabetic patients in a hospital setting.  This study is a systematic literature review that was conducted using the following databases: CINAHL, Medline (PubMed), ProQuest, EMBASE, and Cochrane library.  The outcome included economical aspects of multidisciplinary team implementation. Quality appraisal of the studies was undertaken using the CASP checklist.  From five included studies, there are four cost-effectiveness studies and one retrospective study using ABC costing method. All five studies indicated a positive trend toward the intervention groups. The cost of each QALY ranged from 2443 EUR – 5031 EUR (5993USD). The total cost spent for the implementation of a multidisciplinary team compared to the conventional method were varied among the studies. However, after the calculation projected to the cost of the complication models and QALY gained on each study, the implementation cost was found to be cost-effective and cost-saving in the treatment group. Therefore, the implementation of an integrated multidisciplinary team is potentially economically beneficial for type 2 diabetes patients management in hospitals.

Abstract

Service waiting time in an outpatient is an indicator of minimum hospital quality service standards. The amount of waiting time at the X hospital's polyclinic in 2018 was 84.75 minutes, exceeding the hospital quality service standard in 2009 at 60 minutes. The preliminary research shows that the waiting time for medical services by part-time medical specialists was 60.75 minutes. This study aims to identify factors that affect part-time medical specialists' tardiness at X Hospital's polyclinic. This study performed an analytical survey using a cross-sectional approach from May to June 2020. It collected data from 67 part-time medical specialists who conduct medical practices at the polyclinic. Univariate, bivariate Spearman correlation and multivariate multiple linear regression tests with the Backward method were performed to analyze data. The bivariate analysis shows a significant effect of biographical characteristics factor where practice hours are less the standard time of 60 minutes ( p=0.01) and extrinsic factor of the work environment (p= 0.019) on the tardiness of part-time medical specialists. Work environment was the most affecting factor in late medical practices (p=0.016, Exp B/OR -0.433). This study could be a reference for the hospital to manage the human resources in terms of proper practice hours and a supportive work environment for patient medical services.

Data collection was carried out by in-depth interviews, observation and documentation. The informants of this study were the Customer Service coordinator, the head of the household division, the head of the general division, the head of the outpatient services section, and doctors in the outpatient installation. Triangulation informants were the deputy director general and finance and deputy director of services at the Tugurejo Hospital. The results showed that the complaint management system of the tugurejo hospital in the managerial dimension, namely the strategic plan and organizational structure was good. In the leadership dimension, it was found that some leaders were not good communicators. In the staff dimension, educational backgrounds in the Customer Service unit vary. This is due to limited manpower and budget so that the hospital optimizes existing personnel. For staff training regarding hospital services it has been done but it is not specific about the handling of complaints. In the dimension of work culture, a culture of openness to complaints by doctors and not strict management is one of the consequences of recurring complaints about doctor services in outpatient installations. For the operational dimension, namely the implementation process for complaint handling has been going well. The management of change has been going well, although there are still many obstacles in its implementation. The output management needs to be improved for each unit, namely monitoring and evaluation. In the dimension of obstacles in the household subsection is the budget, for the service sector is openness to complaints and lack of firm management. For the technical dimension, the capability and information system of recording each section have not been implemented optimally.
