Abstract

Rheumatoid arthritis is a chronic autoimmune disease characterized by joint inflammation, pain, stiffness, and decreased mobility. Ankle pain is one of the common complaints that significantly affects patients' daily activities and quality of life. Non-pharmacological interventions such as warm water foot soaking therapy are considered effective in reducing pain through vasodilation and muscle relaxation mechanisms. This study aimed to determine the effectiveness of warm water foot soaking in reducing ankle pain in patients with rheumatoid arthritis. This research employed a descriptive case study design involving two patients at RS TK II Putri Hijau Medan. The intervention was conducted for three consecutive days using warm water with a temperature of 31–37°C for 15–20 minutes. Pain intensity was measured using the Numeric Rating Scale (NRS). The results showed a significant decrease in pain intensity from a scale of 5 (moderate pain) to 2 (mild pain) in both patients after the intervention. Patients also reported increased comfort and improved mobility. In conclusion, warm water foot soaking is effective as a non-pharmacological nursing intervention to reduce ankle pain in patients with rheumatoid arthritis. This therapy is simple, safe, cost-effective, and can be applied independently by patients.

Abstract

Patient satisfaction is one of the important indicators to measure the quality of services we provide and satisfaction is an asset to get more patients and to get loyal patients. Patient satisfaction standards in health services are set nationally by the Ministry of Health of the Republic of Indonesia, which is above 95%. This study aims to analyze the factors that influence the quality of life of pulmonary TB patients at UPTD.Special Lung Hospital Medan.  This type of research is quantitative with a cross-sectional study design. From the satisfaction survey data of RSU Mitra Medika Bandar Klippa for 2023, it shows that the patient satisfaction rate is 87%. When the researchers conducted an initial survey on October 12, 2021, the initial survey data obtained showed that 7 patients expressed dissatisfaction with the services provided in the outpatient room. The purpose of this study was to analyze services to patient satisfaction in the outpatient room of RSU Mitra Medika Bandar Klippa. This research is a descriptive qualitative study conducted in the outpatient room of RSU Mitra Medika Bandar Klippa in June 2022. Where the informants in this study were the Head of the Outpatient Room, Five Patients, Specialist Doctors, Customer Service and Cleaning Officers. Data were analyzed using theme analysis. The results showed that health worker communication is closely related to patient satisfaction. Responsiveness of health workers is closely related to patient satisfaction. Cleanliness and comfort are closely related to patient satisfaction. Information before the patient goes home is closely related to patient satisfaction. Based on the conclusions, it is recommended that the results of this study be input material and become material in evaluating communication in providing services to all hospital visitors, by increasing effective communication, increasing the responsiveness of all hospital staff, maintaining the cleanliness and comfort of the outpatient room and health workers always provide information before patients leave the service unit. It is recommended that the results of this study can be an applicable concept for health workers to be able to improve health service efforts for pulmonary TB patients so that they can apply the rules that have been set, patients comply with undergoing treatment regularly and are good at taking care of themselves so that their disease is not transmitted to others.

Abstract

Throughout the course of the disease, TB patients will experience several symptoms that interfere with their lives. Quality of life is a condition where a person gets satisfaction or enjoys their daily life. This study aims to analyze the factors that influence the quality of life of pulmonary TB patients at UPTD.Special Lung Hospital Medan.  This type of research is quantitative with a cross-sectional study design. This research was conducted at UPTD.Special Lung Hospital Medan in August-September 2024. The population in this study were all patients with pulmonary TB and were not compliant with treatment at UPTD.Special Lung Hospital Medan with a total of 558 people. The sample was determined by accidental sampling of 85 people. Data were analyzed univariately, bivariately and multivariate.  The results of the study showed that there was an influence of age (p=0.000), gender (p=0.000), compliance with taking medication (p=0.000), medical history (p=0.000), family support (p=0.000), smoking dependence (p=0.000) on the quality of pulmonary TB patients at the UPTD.RS Khusus Lung Medan. The most dominant factor was family support.  The conclusion is that there is an influence of age, gender, compliance with taking medication, medical history, family support, smoking dependence on the quality of pulmonary TB patients at the UPTD.RS Khusus Lung Medan. It is recommended that the results of this study can be an applicable concept for health workers to be able to improve health service efforts for pulmonary TB patients so that they can apply the rules that have been set, patients comply with undergoing treatment regularly and are good at taking care of themselves so that their disease is not transmitted to others.

Abstract

Lung disease is a disease characterized by persistent respiratory disorders and limitations caused by abnormalities of the airways or alveolar tract due to exposure to harmful particles and gases. RSU ER. Mitra Medika Bandar Klippa has certain indicators to maintain quality and patient safety, in this case the indicator is the Accuracy of Diagnosis of Priority Lung Diseases by Emergency Room Doctors. The number of priority lung patients from May - November 2021 was 1,309 patients and the number of correct diagnoses was 928 patients, the remaining 381 patients received incorrect diagnoses. The aim of this research is to analyze the performance factors that cause discrepancies in the initial diagnoses of emergency room doctors and the final diagnoses of specialist doctors in diagnosing priority lung diseases. This type of research uses a qualitative method with a descriptive approach which was carried out in August – September 2022, with a total of 8 informants, 2 (two) people namely Specialist Doctors, namely 5 (five) emergency room doctors, 1 (one) person namely Service Manager Patient (MPP) at RSU. Klippa Bandar Medika Partners. From the results of the research and discussion, it was concluded that from the performance factors (Work Quantity, Knowledge, Cooperation and Work Initiative) only 2 (two) performance factors caused a discrepancy in the initial diagnosis of the ER doctor and the final diagnosis of the Lung Specialist Doctor, namely, Work Quantity and Knowledge. Based on the research results, it is recommended to RSU Management. Mitra Medika Bandar Klippa needs to propose training related to lung disease for all General Practitioners who work at RSU.
