Abstract

Background Adenocarcinoma of the prostate is an invasive carcinoma, composed of neoplastic prostatic epithelial cells with differentiation and secretion arranged in various histomorphological patterns.Quantitative analysis of the elements that make up the stroma in casesprostate adenocarcinomahas been linked to prognostic factors and the Gleason score, which correlates with progression and metastasis, and may contribute to new prognostic feature approaches.   Method Descriptive analytic study with 33 paraffin block samples diagnosed as prostate adenocarcinoma at Haji Adam Malik General Hospital Medan.Assessment is done forsee the relationship between reactive stromal expression using Trichrome Masson and histopathological grading gleason score then determine the scoring score for the stroma around the prostate gland adenocarcinoma lesion stained blue with trichrome masson, a positive score of one (+): if 0-15% stroma is stained blue, positive two (++): if >15%-<50% stroma is stained and triple positive (+++): if >50% stroma is stained blue. The histopathological grading was categorized into grade I (well differentiated: Gleason score ≤6), grade II (moderately differentiated: Gleason score 7 (3+4)), and grade III (poorly differentiated: Gleason score 7 (4+3) and 8 -10).   Results: Reactive stromal expression the most found was positive three (+++) (69.7%) and the most common histopathological grading was grade III (69.7%). There was a significant correlation between reactive stromal expression and histopathological grading in prostate adenocarcinoma (p-value=0.044), namely the worse the cell differentiation, the higher the positive score of reactive stromal expression.   Conclusion Assessment of reactive stromal expression associated with histopathological grading can be used as an indication of prognosis in patients with prostate adenocarcinoma.

Abstract

Background Primary salivary gland malignancy is rare and has limited reports. Mucoepidermoid Carcinoma (MEC) is the most common malignant tumor of the salivary glands. However, epidemiological reports on salivary gland MEC are still limited, particularly in Indonesia. This study aimed to report the clinicopathological characteristics of Salivary gland MEC in dr Hasan Sadikin Central Hospital (RSHS) from January 2013 to December 2022. Method: This study is a descriptive study with a cross-sectional method. Total cases of salivary glands MEC that were histopathologically diagnosed and recorded in the RSHS Hospital Information System (SIRS) medical records from 2013 to 2022 were collected. It is included demographic, clinical, and histopathological data. Result A total of 86 MEC cases were diagnosed histopathologically between 2013 and 2022. The cases involved 44 (51,2%) females and 42 (48,8%) males. The mean age is 46,4 years old. The most common site affected is a parotid gland (37,2%). According histopathological grade, 51,2% were high grade, 25,6% were intermediate grade, and 23,3% were low grade. The result of clinical stage is Stage III has the highest percentage (n=32, 37,2%). The majority of patients receive surgical combined with radiotherapy treatment (n=53; 61,6%). Conclusion There are 86 cases of Salivary MEC in RSHS period 2013-2022. MEC is common in females, mean age 46,4 years old. Parotid gland is the most common site. The most frequent histopathological grading and clinical staging are high grade and stage III. Abstract

Background Ovarian carcinoma is a cancer with high mortality in women and although comprehensive management with surgery and chemotherapy at an advanced stage, the resistance rate is still low. GATA3 contributes to the progression of malignancy and its expression is one of the predictors in some malignancies, but the results are mixed in ovarian carcinoma. High GATA3 expression is associated with the aggressiveness of tumor growth and poor prognosis of ovarian carcinoma.   Methods This research is an cross-sectional descriptive-analytical study with 33 histological specimens diagnosed with ovarian carcinoma from medical records/archives at H. Adam Malik Hospital Medan. Each sample specimen was stain with GATA3, and several various histopathological subtypes of ovarian carcinoma.   Results From a total of 33 samples, 14 samples were serous carcinoma, 6 samples were mucinous carcinoma, 7 samples were endometrioid carcinoma, and 6 samples were clear cell carcinoma. GATA3 was expressed in 42.5% of serous carcinoma. Positive expression of GATA3 is mostly found in advanced ovarian carcinoma, older age, and histopathological type of serous carcinoma.    Conclusion Immunohistochemistry GATA3 expression was expressed in 42.4% of serous carcinoma, 21.2% in endometrioid carcinoma, 18.2% in clear cell carcinoma, and 18.2% in mucinous carcinoma.   Keywords: ovarian carcinoma, GATA3, immunohistochemistry

Abstract

Background Colorectal cancer ranks third highest of all malignancies worldwide and ranks fourth out of the 10 most common cancers encountered in Indonesia. More than 90% of colorectal cancers are adenocarcinoma type. The mechanism of autophagy has a dual role in tumorigenesis, namely as a tumor suppressor and as a tumor promoter. Microtubule-associated protein 1 light chain 3B (LC3B) is one of the widely used autophagy markers. The role of autophagy requires further research so that LC3B can be considered as one of the predictors of prognosis, especially in colorectal adenocarcinoma. The aim of this research is to determine the correlation of Light Chain 3B immunohistochemical expression with histopathological grading of colorectal adenocarcinoma. Methods Cross-sectional analytic study of 41 samples diagnosed with colorectal carcinoma at H. Adam Malik General Hospital Medan. Staining was performed using a polyclonal antibody rabbit Light Chain 3B immunohistochemistry (GeneTex) which would appear positive in the cytoplasm of tumor cells at a dilution of 1: 500. LC3B expression was assessed semiquantitatively with low and high expression categories. Data analysis was tested with the Spearman correlation test. Results LC3B immunohistochemical expression has a very strong correlation with the histopathological grading of colorectal adenocarcinoma and in a positive direction where the higher the colorectal adenocarcinoma grade, the higher the LC3B immunohistochemical expression, and vice versa (p=0,0001; r=0,763). Conclusion There was a significant relationship between LC3B immunohistochemical expression and histopathological grading of colorectal adenocarcinoma.

Abstract

Background Cervical cancer is a neoplasm in the cervix due to abnormal cells that damage the surrounding tissue. Cervical cancer from squamous cells is called squamous cell carcinoma cervix NOS and that from glands is called adenocarcinoma cervix NOS. The prognostic of both can be assessed by tumor budding and Ki67 immunohistochemistry, tumor budding is a tumor bud which is ≤ 5 cells in a cluster that grows in front of the parent tumor and immunohistochemistry Ki67 is a labile nonhistone nuclear protein which is expressed in the G1, S, G2 and M phases of the cycle cell.   Method This research is an analytical type of research that aims to find out the relationship between Ki67 expression and the peritumoral budding tumor index compared to cases of squamous cell carcinoma cervix NOS and adenocarcinoma cervix NOS with 36 paraffin block samples diagnosed with squamous cell carcinoma NOS and adenocarcinoma NOS at HAM Hospital. Medan. Assessment method of Ki67 expression where low expression (<20%) and high expression (≥20%). Assessment of peritumoral budding tumors in both cases was said to be low buds if < 5 buds and high buds if ≥ 5 buds. Results This study provides a significant appearance (p-value 0.0001) which means that peritumoral budding tumors with HE staining can be accepted as a measuring tool for assessing cervical cancer in addition to assessment using immunohistochemistry Ki67 which is generally used in the prognostic assessment of cervical cancer. Conclusion Assessment in this case can consider peritumoral budding tumor assessment to determine prognostic in cases of squamous cell carcinoma NOS and cervical adenocarcinoma NOS, if Ki67 immunohistochemistry is not available.

Abstract

Background Matrix metalloproteinase-9 (MMP-9) is a member of the metalloproteinase enzyme that is often found in colorectal adenocarcinoma. MMP-9 is able to destroy basement membrane proteins and extracellular matrix which play a role in tumor cell invasion.   Method Cross-sectional analytic study with 30 paraffin block samples diagnosed as colorectal adenocarcinoma NOS at H. Adam Malik General Hospital Medan. MMP-9 expression was assessed using immunohistochemical staining in the cytoplasm of tumor cells and cells in the stroma. Expression was assessed using the Quick Score method and was declared weak if the total value was 0-5 and strong if the total value was ≥6. Statistical analysis between MMP-9 expression and depth of invasion was carried out using Fisher's Exact and Mann-Whitney U tests.   Results MMP-9 expression was significantly related to stage (p=0.01), depth of invasion (p=0.03), and stromal Tils (p=0.03) and not significantly related to gender (p=0.28) , age (p=0.82), tumor location (p=0.69), grading (p=0.26), lymph node involvement (p=0.53), metastasis (p=0.53), perineural invasion (p=0.25), lymphatic invasion (p=1.00), IMVI (p=0.531.00), EMVI (p=1.00), tumor budding (0.25).   Conclusion MMP-9 expression can be used as an indication of prognosis and also helps clinicians, especially in the management of patients with colorectal adenocarcinoma NOS.

Abstract

Endometrial carcinoma is a malignant proliferation of the endometrium cells. This particular type of carcinoma is a world problem since it was ranked the second most common gynecology malignancy, and is the most common malignancy found in the uterus. This research is conducted to identify the sociodemographic and clinicopathologic characteristics of endometrial carcinoma patients at RSUP Dr. Mohammad Hoesin Palembang. This is an observational study with a cross-sectional design. Of all 37 samples collected, the majority are women of ages 20-50 (81,1%), married (83,8%), housewives (40,5%), and live within the city of Palembang (73%). Histopathologically, the most common subtypes found are endometrioid adenocarcinoma (62,2%), and of grade G3. The majority of the samples are within stage 1 cancer (27%), and with no metastases (83,8%). It can be concluded that, socio-demographically, the majority of endometrial carcinoma patients are aged 20-59 years, are married, work as housewives, and live in the city of Palembang, while the histopathological characteristics that are commonly found are the endometrioid adenocarcinoma subtype, grade G3 carcinoma, stage 1 carcinoma, and carcinoma without metastases.
