Abstract

Critical congenital heart disease (CHD) is a type of CHD that requires early intervention in the first year of life to survive. Morbidity and mortality increases significantly if newborns with critical CHD experience delay in the initial diagnosis and management. The infants may develop cyanosis, systemic hypoperfusion, or respiratory distress as the main manifestations of critical CHD. Pulse oximetry screening for early detection of critical CHD must be performed in newborns after 24 hours of age or before discharge from hospital. Generally, infants with critical CHD require patency of the ductus arteriosus with infusion of prostaglandin to maintain pulmonary or systemic blood flow. After initial management, the infants must be immediately referred to tertiary care center for definitive intervention. Abstract

Introduction. This study aims to determine the role of self-stigma towardspsychological well-being to the people who have had leprosy in South Sumatera. Thestudy hypothesis that there is a role for self-stigma towards psychological well-beingviewed from psychological distress in people who have had leprosy in SouthSumatera. Methods. The participants of this study were people who have hadleprosy who finished their medical treatment. The research sample was 112 peoplewho have had leprosy. This study used the consecutive sampling technique.Psychological well-being and self-stigma were measured by the scale. The researchused simple regression analysis. Results. The result obtained by the value of R=0.442, R square= 0.178, F= 11.821, and P= 0,000 (p<0,05). It was proved that self-stigma has a significant role in 17.8% towards psychological well-being. Conclusion.There is no difference in psychological distress based on the role of self-stigma andpsychological well-being of people who have had leprosy in South Sumatera.

Abstract

Infantile hemangioma represents the most common benign tumour arising ininfancy. The natural history of infantile hemangioma is characterized by aproliferative phase during the first few months of life, followed by slower involutionover the years. Evolution of infantile hemangioma has four stages, contain nascent,proliferation, involution, and involuted. Most infantile hemangioma do not requireactive treatment and involute without leaving significant residual. However, someinfantile hemangioma is problematic due to complications such as ulceration,interference with a vital function, or potential disfigurement. The managementapproach for IH contains active non-intervention and active intervention. Thedecision of treatment is based on many factors, including size and location,psychosocial implication, risks and benefits of the proposed therapy. Earlyrecognition of infantile hemangioma is essential for optimal management andpreventing complication.

Abstract

Introduction. Indonesia ranks third in tuberculosis cases, with 23,000 new dualdrug-resistant tuberculosis patients. The delay in predicting tuberculosis resistanceoccurs because sputum tools are not yet available. This study aimed to determinethe differences in chest x-rays for drug-sensitive multiple drug-resistanttuberculosis in Palembang. Method. A case-control design to compare theradiological characteristics of multiple drug resistance with drug-sensitive atPalembang during January-July 2020. One hundred thirty-eight patients consistedof cases of drug-sensitive tuberculosis and multidrug-resistant tuberculosis basedon the rapid molecular test that met the inclusion criteria, analyzed forposteroanterior chest X-rays by a radiologist without knowing the diagnosis—comparative chi-square using SPSS 22. Result. Multidrug resistance tuberculosishad more extensive or moderate lesions than drug-sensitive tuberculosis 89.8% vs72.4%. The multidrug-resistant group has more infiltrate detected, namely 84.1% vs69.6% and cavity 37.6% vs 30.4%. Atelectasis and pleural effusion are common indrug-resistant tuberculosis. Bivariate analysis showed that multidrug-resistanttuberculosis lesion infiltrates compared to drug-sensitive tuberculosis with p =0.025; OR 2,6 (CI 95% 1,1-6,0) sensitivity 85.5%, specificity 30.4%. Multivariateanalysis showed the presence of infiltrates p = 0.028; OR 2.58 (CI 95% 1.1-6.003).Consolidation, nodules, cavities, fibrosis, atelectasis, pleural effusion were notsignificant differences in the two groups. Conclusion. Multidrug-resistanttuberculosis has a more expansive lesion, the presence of infiltrates and cavities thatis more dominant than drug-sensitive tuberculosis.

Abstract

Introduction. Vitamin D plays a role in health overall, but hypovitaminosis D stilloccurs throughout the world. HIV/AIDS patients are prone to suffer fromhypovitaminosis D because of the infection itself and the side effects of antiretroviraltherapy. Various effort have been tried to improve the immune status of HIV/AIDSpatients, one of them is by adding vitamin D. Vitamin D acts as an antiinflammatoryso that it can prevent apoptosis of CD4 T cells and increase CD4 cell count.Methods. This is a randomized control trial add on a study that aims to determinethe effect of vitamin D to increase in CD4 counts of HIV / AIDS patients who havereceived antiretroviral drugs. Subjects were HIV / AIDS patients who had receivedantiretroviral drugs. A total of 20 subjects were divided randomly into two groups;one group received vitamin D (calcitriol 0.5 mcg per day) for eight weeks, and theother group that received a placebo. Each group was measured of CD4 cell countbefore and after treatment. Results. There was a significant increase in the CD4 cellcount of the vitamin D group (p = 0.046), but not in the CD4 cell count of bothgroups (p = 0.985). The comparison of mean CD4 cell counts between groups beforetreatment was not significantly different (p = 0.057), but after treatment, it becamesignificantly different (p = 0.040). Conclusion. Vitamin D has been successful inincreasing CD4 cell count in the vitamin D group, and it is recommended to giveHIV / AIDS patients to increase CD4 cell count.

Abstract

Background. Phyllanthus niruri (local name : meniran) extract is utilized in treating kidney and uterus disease. Effects of ethanol extract of P. niruri on kidney and uterus of 2% NaCl triggered fibrotic rats had been studied. Aim of Study. Aim of this study to explore the effect of leafflower (Phyllanthus niruri Linn.) treatment on kidney and uterus in 2% NaCl - induced fibrotic rats. Methods. Wistar rats were challenged with 2% NaCl in drinking water to induce fibrotic kidney and uterus. Phyllanthus niruri extract (200 mg/kg) was given orally for 4 weeks. Matrix extracellular abundance was determined by HE Staining and measured by METAVIR Fibrosis Score. The results of this study were assayed by SPSS 16. Results. P. niruri extract was potent to reduce the matrix extracellular in 2% NaCl-induced rats compared to control group (p<0.05). Conclusion. P. niruri extract was potent to decrease the matrix extracellular in the kidney and uterus of the rats.

Abstract

Platelets are an important component in the process of hemostasis and coagulation. It’snow known that high platelets count closely related to poor prognosis of patients withcancer, due to their role in the hematogenous spread of cancer cells. Platelet can beactivated by cancer cells into tumor educated platelet and then cause thrombosis throughtumor induced platelet aggregation. Platelet also protect cancer cells in the blood circulationfrom natural killer cells and help the transition of cancer cells from epithelial tomesenchymal and vice versa, resulting in the process of metastasis. In the next stage ofmetastasis, platelets trigger extravasation of cancer cells from primary cancer and helpadhesion of cancer cells to distant organs.

Abstract

A B S T R A C TIntroduction: Recursive Partitioning Analysis is one of prognostic scores, has beenvalidated to any different setting. Objective: To identify the concordance of RecursivePartitioning Analysis stratification in survival with brain metastases patients.Methods: Retrospective study was performed on brain metastases patients fromJanuary 2017 until Desember 2019 based on medical record. The follow up timestarted from the first diagnosis of brain metastases to death or last follow up. TheKaplan Meier was used to plot survival curves and the log-rank test was used toanalyse differences between groups Results: Mean overall survival time was 4,67months with 1,14 months for median survival for all patients. According to scoring,mostly (80,8%) patients were in group 3. The median survival time was 7 monthsand 2 months for group 2 and 3. Conclusion: It has shown relatively congruitysurvival in BM patients with stratification of Recursive Partitioning Analysis in ourinstitution.

Abstract

A B S T R A C TIntroduction. Acute kidney injury (AKI) is defined as sudden decline in theglomerular filtration rate, resulting in the retention of nitrogenous wastes, such asurea and creatinine in plasma. Cinnamomum burmannii is known as a anti-inflammatory renoprotective agent, although the precise mechanism is not wellunderstood. This study aimed to elucidate the effectiveness of Cinnamomumburmannii extract in decreasing creatinine level of acute kidney injury Ischemiareperfusion (IR) model. Method. We performed Ischemia reperfusion (IR) in maleWistar rat to induce acute kidney injury. The rat (n=30) were divided into six groups:IR, 1 group treated with methylprednisolone as a control (IR+M), 3 groups treatedwith different oral Cinnamomum burmannii extract doses (50mg/kg (IR+EKM1),100mg/kg (IR+EKM2), and 200 mg/kg (IR+EKM3), and a Sham operation (SO)group. AKI stage reduction based on serum creatinine levels, before and aftermodeling, before and after the cinnamon extract intervention. Creatinine levels werequntified by spectrophotometry and analyzed by SPSS. Result. Cinnamomumburmannii extract lowers creatinine levels; significant (P <0.05). 200 mg / kgbb isthe effective dose of lowering creatinine levels in the IR model. Conclusion.Cinnamomum burmannii extract reduced serum creatinine levels associated withdecreased acute renal staging in the IR model.

Abstract

A B S T R A C TIntroduction Diabetes mellitus is a degenerative disease that has globally increasedprevalence annually 1 . Impaired functional capacity due to poor blood sugar controland presence of cardiovascular autonomic dysfunction. Six minute walk test is amethod that is widely used in the assessment of functional capacity in patients withType 2 Diabetes Mellitus.ObjectiveTo evaluate factors that are associated withfunctional capacity in Type 2 Diabetes Mellitus.Methods This is an analyticobservational study with a cross sectional design. Fourty patients who had beendiagnosed with diabetes mellitus was assessed by six minute walk test. Chi squareand logistic regression analysis was perform by using SPSS 25. Results Six minutewalk test correlated significantly with Ancle Brachial Index scores (p = 0.016) andBody Mass Index (p = 0.03) Conclusion Ancle Brachial Index Score and Body MassIndex are factors associated with functional capacity in Type 2 Diabetes Mellitus
