Abstract

Objective: This case report discussed the treatment of an open apex using an MTA apical plug in a non-vital permanent tooth.Methode: A 39-year-old female visits the Dental Hospital of Hasanuddin University with the chief complaint of decay in her upper front tooth. The tooth was filled, and it fell out a few weeks ago. There is a history of gingival swelling a month before. Radiographic examination showed an open apex #11 with radiolucency in the apical area. Diagnosis was necrosis with a chronic apical abscess. The root canal was prepared using circumferential techniques. Then, dressing using calcium hydroxide was given. MTA was used to perform an apical plug a few weeks later.Results: Patient showed no complaints of pain, and the size of the radiolucency in the periapical area of the tooth was reduced. Following that, a post crown created on #11.Conclusion: MTA apical plugs are effective to treat non-vital permanent tooth with an open apex.

Abstract

Coronavirus disease 2019 (COVID-19) is an infectious disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) that presents with a multitude of clinical presentations in various body systems. In addition to hematological, radiological, and biochemical findings, oral manifestations of COVID-19, including but not limited to xerostomia, loss of taste, and oral lesions have been reported. To our knowledge, this is the first case to report on a series of multifactorial and concurrent viral, autoimmune, and fungal oral lesions following a COVID-19 infection, namely the reactivation of herpetic gingivostomatitis, herpes-associated erythema multiforme, and concomitant oral candidiasis. This study aims to present a case of compounded immunological involvements of COVID-19 and frame it as a possible predisposing factor to the development of oral lesions and symptoms due to COVID-19’s profound role in immune dysregulation. Further studies are needed to investigate the etiopathogenesis and pathophysiology of the recurrence of oral lesions following COVID-19 infections.

Abstract

Objective: This article aims to report interesting case of whole mouth pigmentation, including on patient full protheses.Methods: A 79-year old female came to dental clinic with a chief complaint of difficulty in swallowing. The patient felt no pain but is very uncomfortable due to impaired taste and a burning sensation in her mouth.  In addition, the patient's dentures were stained with a diffuse black-and-white coating indicating a long and chronic course event.  This complaint occurred for last 2 months, after a long history of being in and out from the hospital due to several illnesses.  Ongoing treatment of the patient include drug for chronic digestive diseases and hypertension. Results: The patients told to come back in 7 days with strict instructions to bring all hospital-related documents to rule out possible etiologic of the pigmentation.Conclusions: This interesting case concluded with working diagnosis of oral melanocarcinoma due to macula fast and massive spread and distribution. However, black-pigmentation on patient prothesis can lead to a strong suggestion that the trigger of those malignant possible triggered by a long-chronic un-treated candida infection.

Abstract

Unicystic ameloblastoma is rare and the least aggressive type of ameloblastoma tumor, it comprises 6% of all ameloblastoma types and more frequently affects the mandibular jaw.The clinical case reports a 21-years-old male who had a mandibular swelling at the right retromolar region. Panoramic radiograph and CBCT images revealed an unicystic radiolucency and sizing 2.0×1.5 cm. A provisional differential diagnosis of unicystic ameloblastoma was considered. The treatment steps were: conservative marsupialization treatment, monthly follow-up until cystic volume shrinkage, a complete cystic remnants removal by a bone curettage after 10 months, and an every 6-months follow-up including clinical and image examination. There was none sign of recurrence after 5 years of follow-up.

Abstract

Objective- Oral submucous fibrosis (OSMF) is a chronic and gradually progressing condition characterized by progressive fibrosis and restricted oral mucosal elasticity. It is prevalent in certain regions of the world, particularly South Asia, and is strongly linked to the habitual use of areca nut and tobacco. The present case report explores the combined therapeutic potential of antioxidants and photobiomodulation (PBM) in managing OSMF along with mouth opening exercises.Methods- A 31-year-old male patient presented with burning sensations in the cheeks with restricted mouth opening. The treatment protocol consisted of six sittings of PBM therapy at three-day intervals, coupled with antioxidant supplementation for 2 months, and mouth opening exercise.Result- The patient showed significant improvement in symptoms, with a marked increase in mouth opening from 3.0 cm to 3.5 cm.Conclusion- This case highlights the synergistic benefits of PBM and antioxidants along with mouth opening exercises as a promising non-surgical treatment for OSMF.Key words- Oral submucous fibrosis, reactive oxygen species, antioxidants, photo biomodulation

Abstract

Objective: This article aims to report an interesting rare case of recurrent oral ulceration with a high level of cobalamin.Case report: A 44-year-old female came to dental clinic with chief complaint of recalcitrant oral ulceration since half a year ago. Patients basically had history of periodic oral ulcer for quite long time with short occurrence and only one or two ulcer in one period. However, the ulcer become alarming after she was hospitalized due to extreme fatigue. Patients subsequently instructed for routine blood along with cobalamin and IgE total examination. The test came back with highly decreased level of red blood index indicates that the patients on severe anemic condition. Unexpectedly, the levels of cobalamin in blood increased by 70% from the referral score. Patients urgently refer for hospitalization to recover her Hb level. After blood transfusion, patients discharged from the hospital but under hematologic observation for the exact etiology of her anemia, including the high cobalamin level. After two months observation, patients Hb gained normal level, however the vitamin B12 level remain. Patients reported decreased in number of ulcers.Conclusion:  It is worthy to always check on cobalamin level for patients with recurrent oral ulcers since lower level can directly causing it or the high level can lead to some life-threatening diseases which can also trigger the recalcitrant oral ulcers.

Abstract

Objectives:  This article aimed to report RAS with severe clinical form due to hormonal fluctuant triggered by Intra Uterine Device.Methods: A 26-year-old female patient came to Hasanuddin University Dental Hospital with complaints of continuous mouth ulcers since 1 year ago. Deep anamnesis revealed that the initial trigger of the unhealed ulcers incidentally happened at the same time after the patient installed an Intra Uterine Device (IUD) which also led to continuous menstruation in the past month leading to self-decision to have her IUD removed. Due to her clinically anemic condition, the patient was referred for routine blood examination with the result of lower red blood cells and hemoglobin which also contributed to the ulcer severity. Since the main cause of hormonal imbalance has been removed, supportive treatment uses vitamins to support the patient’s systemic condition which gives a good response.Conclusion: Recalcitrant and severe intraoral ulceration may be triggered by hormonal imbalance caused by IUD. Removal of these devices can help reduce oral complaints if the patient is on optimum health, otherwise, the background systemic needs to be improved.

Abstract

Objective: This case report discusses orthodontic treatment of malocclusion cases with teeth after root canal treatment.Method: A 24 year old woman came to the clinic with the main complaint of crowded teeth. Intra-oral examination revealed tooth rotation, palatoversion and edge to edge bite. Radiographic examination revealed teeth that had been treated with root canal treatment. Diagnosis is dental class I malocclusion.Result: Crowded teeth, rotation of teeth, palatoversion of teeth and edge to edge bite have been corrected. Interdigitation of the maxillary and mandibular teeth has been achieved.Conclusion: In this case the tooth after root canal treatment can be adjusted orthodontically

Abstract

Objective: Ascorbic acid has an inhibitory effect on tyrosinase enzyme which is the key enzyme in melanogenesis. This case reported the effect of two application techniques of ascorbic acid following surgical gingival depigmentation on the recurrence rate of pigments. Methods:  A 24-year-old healthy non-smoker girl was seeking non-expensive cosmetic treatment for unesthetic dark appearance of her gums for the last 5 years, but the possible recurrence of those pigments was her main concern. On examination, diffuse brown hyperpigmented patches was observed in the maxillary and mandibular anterior attached gingiva and diagnosed as physiological gingival moderate pigmentation with score 2. The surgical scalpel scraping procedure using number 15 blade was performed to remove all visible pigments followed by intra mucosal field injection of 1ml ascorbic acid using insulin syringe weekly for 1 month, then monthly for 5 months in maxillary labial gingiva. While, ascorbic acid gel was applied topically twice weekly for 6 months on mandibular labial gingiva in split mouth design. The long-term effect of the two application techniques was followed for one year. Results: No pigment recurrence was observed at 6 months but mild pigmentations were seen at 12 months for both techniques. The patient was more compliant to topical than injectable vitamin C treatment protocol, however she reported perception of slight recurring pigment after one year with topical vitamin C. Conclusion: Both adjunctive intramucosal injected or topical vitamin C were effective in delaying recurrence following surgical depigmentation up to 1 year.

Abstract

Introduction: Endo-periodontal lesions are characterized as bacterial infection conditions that cause widespread periodontal tissue destruction and inflammation or pulp necrosis conditions. These lesions occur simultaneously in the periodontal and periapical tissues of a single tooth. Treatment of these lesions requires a multidisciplinary approach that integrates endodontic and periodontal therapy. Knowledge of the pulp and periodontal relationship, the complexity of the root canal system is essential in the management of these cases. The use of periapical radiographs and CBCT are crucial for the success of this treatment.Case:  A second mandibular right molar was diagnosed as a primary endodontic with secondary periodontal lesion, and treated with a combination of endodontic and periodontal therapy. Tooth 47 with Vertucci type II root canal configuration was subjected to multi-visit endodontic treatment, beginning with placement of root canal dressings between visits and final composite resin restoration. Subsequent procedure with scaling, root planning and curettage from Periodontics department. A post-treatment control showed good results.Discussion: Endo-perio lesions are clinical conditions that involve inflammatory conditions in the pulp and periodontal tissues simultaneously. This relationship can occur through anatomical pathways such as the apical foramen and lateral canals, as well as non-physiological pathways due to trauma or improper procedures. Treatment of these lesions requires a multidisciplinary approach to achieve optimal results.Conclusion: The treatment of endo-perio lesions accompanied by Vertucci type II in molar teeth can be completed well, because the dentist is able to perform the examination appropriately and involve multidisciplinary treatment.
