Abstract

Objectives: This report describes 2 cases diagnosed with minor RAS treated with LLLT to relieve pain and promote the healing of the ulcers.Methods: A 21-years old female presented of ulcer on labial mucosa for about 4 days with VAS=5. The ulcer always appears 1-2/month before or on her menstruation period. A 21-years-old female presented of ulcer on right buccal mucosa for about 2 days with VAS=8. The ulcer always appears once a month. The predisposing factor in this case were trauma and stress.Results: We treat the patient with LLLT and educated the patients for management the predisposing factor. After treatment with laser, the pain decreased immediately. The lesions healed completely within 4-5 days and scoring of the Ulcer Severity Scores (USS) also decreased.Conclusion: Low level laser therapy is recommended for patients with minor RAS since it can relieve pain, reduce the healing period and lesion size and recurrence of the lesions.

Abstract

Foreign body ingestions are rare, but do occasionally occur during dental treatment. It is estimated that the ingestion of endodontic instrument(s) occurs at an incidence of 0.12 per 100,000 root canal treatments.(1) Although reports of accidental foreign body ingestions exist, few have been able to follow the patients’ medical journey closely enough to capture photos demonstrating how extensive the surgical intervention necessary to manage such events may be. Perhaps this lack of visualization, and associated lack of awareness, is one of the reasons some clinicians still provide non-surgical root canal therapy (NSRCT) without a rubber dam. The case report outlines the medical treatment of a 30 year old male who ingested a K file during NSRCT. The patient was referred to a nearby hospital for subsequent evaluation and treatment, which consisted of numerous imaging studies, endoscopic evaluation, and surgical removal of the K file from his small intestine.

Abstract

Objective: The purpose of this paper is to report the success of root canal treatment (PSA) on the mandibular right first molar for the management of periapical abscess accompanied by cutaneous fistula ar mandible. Methods: This case report describes the management of dental abscess submandibular dextra et causa pulp necrosis of tooth 46 with root canal treatment of tooth 46. Emergency management of dental abscess submandibular dextra et causa pulp necrosis of tooth 46 was carried out with root canal treatment accompanied by the administration of antibiotics.Results: Pulp necrosis can be caused by various noxious irritant injuries to the pulp. Irritation to the pulp or periradicular tissues can cause inflammation. Microbial/Bacterial is the most common cause of inflammation. Microorganisms found in dental caries are the main source of irritation to the pulp and periradicular tissues. Microorganisms in caries produce 4 toxins that can penetrate the pulp through the dentinal tubules. This initial response results in a focal accumulation of chronic inflammatory cells such as macrophages, lymphocytes and plasma cells. Conclusion: There was no active swelling in the patient so it was indicated for root canal treatment.

Abstract

Objective: The aim of this case report is to describe treatment management restoration after endodontic reintervention treatment with indirect bonded restoration using morphology- driven preparation technique.Methods: The preparation technique that can be used is morphology driven, preparation design, while consider bio emulation criteria such as the height of contour, sigmoid nature of enamel dentin interface, and the direction of enamel rods with respect to axial and proximal surfaces. This preparation technique improves resistance by preserving dentin and enamel, also increases bond strength by improving the quality of bonding substrate, marginal integrity and aesthetics.Results: The tooth was inadequately treated about two months ago which made the clinician decided to do an endodontic reintervention. After that, an indirect restoration with lithium disilicate was planned for the patient. The preparation design is a long-wrap overlay, butt-join preparation in the interproximal box (1 mm – 1.2 mm) and rounded inside angles.Conclusion: Restoration after reintervention endodontic treatment with indirect bonded restoration using morphology-driven preparation techniques shows successful treatment with periapical tissue healing and good tissue response.

Abstract

Objective: This case report aimed to answer challenging in the treatment of chronic recalcitrant ulcers in elderly complicated by anemic condition. A 68 years old female patient came to Hasanuddin University Dental Hospital with chief complaint of recalcitrant painful canker sores affecting mouth and throat.Methods: Elder age could contribute in difficulty swallowing due to reduced saliva, slow eating habit, lost or caries teeth which lead to a less efficiency in the absorption of important nutrients such as iron and folic acid. This condition triggers a recalcitrant but chronic ulcer which in the end would add more difficulties for swallowing. A part from the pale conjunctiva of the eye, there was no other abnormality found extra orally. Drug therapy Bcom 150 mg with chlorhexidine gluconate 0.2% mouthwash and systemic corticosteroid.Results: Blood screening results demonstrated lower hemoglobin level with 11.6 and hematocrit 35.6 with referred value of 12 and 37, respectively. Long visit was needed to slowly adapted patient oral environment with local condition, as well as from systemic background improvement. Recalcitrant but chronic ulcers might be challenging in the treatment due to variabilities contributed factors such as age, unhealthy-eating habits, systemic background, and local oral hygiene and condition. One should be careful and be patient in changing the long behavior of the patient.Conclusion: Most importantly, patient been given advice to change her eating behaviors by consume more iron and folic acid food, and avoid irritant food. Local irritant of element 45 also been grinded.

Abstract

Central Giant Cell Granuloma (CGCG) is a benign, intraosseous osteolytic lesion which often affects the mandible and its’ pathophysiology is not fully understood. Histopathology is still the dominant diagnostic method for this disorder. Clinically invasive lesions can lead to premature injury and may require invasive treatment to prevent recurrence. In this study, a case of invasive CGCG of maxillary bone in a 9-year-old boy with diagnostic and therapeutic challenges was presented.

Abstract

Introduction : Gingival Enlargement can happen due to variable reasons, one of them being occlusion trauma (biting), this is due to the fact that the enlargement happened as a trauma response from chronic irritation. One of the main concerns of epulis in the anterior region is the potential of exacerbated conditions due to repeated contact, thus causing further inflammation in which will cause bone damage, furthermore aesthetic is also a concern as it discernible as it is located in the smile area of the patient. Case Report : A 30 year old female patient was presented to Hasanuddin Dental Hospital due to swollen gum in the lower front teeth, patient also complained that it bleeds easily. Patient stated that the swelling happened when the patient was eating an apple and the patient’s gum was torn from biting and was exacerbated when the incident happened again when eating, during which the swelling happened. Patient first visited a health facility before being referred and was given antibiotics, during which the gum is still bleeding and even bled during normal conversation. Patient routinely visited the dentist and last visited 3 months ago for Scalling, furthermore patients denied any systemic diseases and allergies. Conclusion : Epulis should be handled immediately with excisional surgery, furthermore, upon finding bone defects in the overgrown area, surgical debridement should be done as to prevent further damage to the alveolar structure in order to circumvent potential tooth mobility

Abstract

Objective: This case report aimed to describe the restoration of endodontically treated mandibular molar tooth with semi-indirect bonded restoration using single shade composite resin.Methods: Endodontic treatment for tooth #36 with asymptomatic irreversible pulpitis. After root canal obturation, treatment was continued with semi-indirect bonded restoration using single shade composite resin. An impression was taken using alginate and then poured with silicone material (Silicone Die, Voco) to produce the model. Single shade composite resin (Omnichroma, Tokoyama Dental) was used to build up the restoration layer by layer in 2 mm increments up to the occlusal surface.Results: After 1 month of control, objective and subjective examinations were carried out on the patient, there were no complaints, indicating the tooth is functioning well and the patient is satisfied with the restorationConclusion: Restoration of endodontically treated mandibular molar tooth with semi-indirect bonded restoration using single shade composite resin in this case gives satisfactory results to restore function and esthetics

Abstract

Objective: The management of facial fractures varies due to variety of reasons in the pediatric age groups. The clinicians should therefore atune themselves in managing pediatric patients and the variations thereof. Every modality mentioned in the literature has its own advantages and disadvantages. The right choice of treatment by clinicians depends on their knowledge of pediatric anatomy, physiology, age dependent bone growth, type and extend of fractures.Methods: Patients reporting to the OPD of Oral and Maxillofacial department falling under the age group of upto 13 years of age and seeking treatment for mandibular fractures were studied.Results: Three different modalities of treatment in the form of circummandibular wiring, intermaxillary fixation and open reduction and internal fixation were rendered to the patients considering various factors such as the age, location of fracture line, status of occlusion and dentition of the patient. All the three modalities showed good results in the form of adequate reduction and no post-op complications.Conclusion: Pediatric trauma are a challenging entity for the surgeon. The choice of ideal treatment modalityultimately depends on presence of tooth buds and maintanence of uninterupted growth of the jaw bone. Thus a well thought-of and customised treatment plan should be devised for each individual case.

Abstract

Objective: This article proposes a case report of Invasive cervical resorption (ICR) and its management. The prominent features were a substantial cervical resorptive defect with localized fibrous in-growth located on the cervicolabial aspect of the maxillary canine involving the enamel and dentin and pulp.Methods: A 25-year-old, male patient came to the Department of Conservative Dentistry and Endodontics with the chief complaint of bleeding gums, mild pain and small cavitation in maxillary right front tooth region since few months. The clinical diagnosis was asymptomatic irreversible pulpitis with class 3 ICR.Results: Root canal treatment with non-surgical debridement, obturation using single cone technique and restoration of the resorptive lacuna using Glass ionomer cement was done. Long term recall showed satisfactory gingival healing. Eighteen -month follow-up of the cased not reveal any recurrenceConclusion: ICR can be arrested using the "Heithersay approach" (i.e., mechanical debridement, treatment with TCA, and reconstitution).
