Abstract

Introduction: A gummy smile becomes one of the esthetic problems for patients, especially when smiling or laughing. Etiology-based treatment should be considered to fulfill the patient’s needs. In this case report, we performed treatment focused on both extraoral and intraoral etiology: lip repositioning with smile elevator muscle sutures and crown lengthening respectively. Case: A 27 year old female patient presented with the chief complaint of excessive appearance of gingiva when especially laughing. The patient has a gummy smile and a disharmonious gingival line on the anterior maxilla. The diagnosis in this case was excessive gingival display et causa hyperactive upper lip and altered passive eruption. Case treatment: To achieve the esthetic goal, crown lengthening was performed with ostectomy to correct the gingival line and zenith, followed by lip repositioning with partial thickness flap with smile elevator muscle suturing. Six months postoperative follow-up showed pink gingiva with harmonious gingival line contour. Discussion: Lip repositioning has a high risk of relapse in cases of hyperactive upper lip. Lip reposition surgery with sutures on the smile elevator muscle along with a crown lengthening procedure is a predictable alternative treatment in this case. Conclusion: The surgical procedure for handling gummy smile cases provides good treatment results, especially in cases of multiple etiologies of hyperactive upper lip and altered passive eruption.

Abstract

Introduction: A charming smile is formed from good harmonization of teeth and gums. Short teeth clinical crown, dark gingival color, diastema between the maxillary central incisors can interfere the aesthetic of the smile. This case report aims to describe the treatment of periodontal plastic surgery in form of aesthetic crown lengthening, gingival depigmentation and frenectomy to improve function and aesthetic in one visit using scalpel and laser. Case: A 23 year old male patient complained that the maxillary front teeth spaced, short and small, and the gum of the upper jaw is dark, which caused aesthetic discomfort. Objective examination showed high frenal attachment reaching the interdental papilla to palatal, diastema of the maxillary central incisors, short clinical crown and brownish maxillary gingiva. aberrant frenum labialis superior, altered passive eruption and gingival hyperpigmentation diagnoses was made in this case. Case Treatment: Aesthetic crown lengthening treatment with scalpel, gingival depigmentation using scrapping technique and frenectomy using combination of laser and scalpel surgical technique was carried out in one visit. Discussions: The advantages performing three periodontal plastic surgeries in one visit is require just once preparation of surgical materials and instruments, once surgical trauma, once prescription and less number of control visits. Conclusions and Suggestions: Treatment of crown lengthening, gingival depigmentation and frenectomy in one visit are considered effective and efficient based on time, trauma, and costs required. The periodontal plastic surgery technique can be choosing based on clinical finding on patient, patient’s needed and clinical skills of operator.

Abstract

Introduction: Crown lengthening is a surgical procedure to increase the height of the clinical crown for aesthetic and restorative purposes, or combination. The indications include treatment of subgingival caries and altered passive eruption. Functional crown lengthening for restorative purposes in the esthetic zone should consider the concept of biologic width and the ideal proportion of the dentogingival complex. Case: A 39-year-old male patient was referred from the Department of Conservative Dentistry, RSGM UGM Prof. Soedomo to expose the maxillary anterior teeth crowns for full crown and veneer restorations. Clinical examination showed 11, 12, and 12 post root canal treatment with 3-4 mm remaining crowns. Teeth 13, 21, 22, and 23 appear short, excessive gingival display of 3 mm, with a diagnosis of altered passive eruption type 1B. Treatment: Functional crown lengthening was performed by gingivectomy followed by flap elevation, ostectomy, and osteoplasty. A fenestration was found at the apical of 22 and treated with regenerative procedure. Full crowns and veneers insertion was done 3 months after surgery. Crown lengthening as a pre-restorative procedure is a multidisciplinary treatment that considers the need for restoration by re-establishing the biologic width, to prevent inflammation and bone loss. In addition, adequate post- surgical tissue healing time prior to restoration is necessary to avoid gingival margin rebound and to assure long-term success of the restoration. Conclusionn : Functional crown lengthening as a pre-restorative procedure in the esthetic zone can gives satisfactory and stable results, with careful examination and correct surgical procedure.

Abstract

Introduction: Crown lengthening is a widely recognized dental procedure with both aesthetic and functional purposes. It involves the reduction of periodontal hard and soft tissues to obtain supracrestal tooth length, resulting in longer clinical crowns and adequate supracrestal tissue attachment. Laser therapy has emerged as a promising alternative that is currently developing in the field of periodontics and offers several advantages. This review aims to synthesize the current research on the clinical efficacy of laser-assisted crown lengthening procedures and highlight their advantages over conventional methods. Review: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, a comprehensive search was conducted in electronic databases PubMed, Scopus, Embase, and Google Scholar for human studies published from 2017 to April 2023. Out of 213 identified and screened studies, three original articles were included in the analysis. Two clinical trial studies used Er,Cr:YSGG while one study used Er:YAG for crown lengthening procedures involving gingiva and alveolar bone reduction, with a 3-month follow-up period. Compared to conventional crown lengthening, the laser-assisted approach showed comparable increases in clinical crown length. However, the laser-assisted group demonstrated better gingival margin stability, improved soft tissue healing, and minimal damage to adjacent tissues. Furthermore, there was a statistically significant lower bacterial load and less post-surgery pain with laser-assisted crown lengthening compared to the conventional method. Conclusion: In cases where both gingival and bone reduction are required, laser-assisted crown lengthening using Er,Cr:YSGG and Er:YAG can be considered a minimally invasive alternative with several advantages over conventional procedures.

Abstract

Introduction: Periodontal regenerative treatment with periodontal flap surgery followed by application of bone graft combined with platelet-rich fibrin (PRF) is a treatment using the concept of guided bone regeneration which is required to stimulate bone and periodontal tissue regeneration in chronic periodontitis with radiographic sign of alveolar bone vertical defect. This case report will describe the flap surgery with bone graft (Ti-OSS®, South Korea) and PRF treatment for correction of alveolar bone vertical defect in tooth with chronic periodontitis. Case: A 45-years old female complained of swollen and suppurated gingiva in the upper right area. Clinical finding showed deep periodontal pockets in tooth 13 and 15 and radiographic finding showed apical third alveolar bone loss in tooth 13 and 15. Case Treatment: Initial treatments included scaling and root planing, occlusal adjustment, and antibiotic prescription. Treatments were followed by periodontal flap surgery for debridement and elimination of granulation tissue on the root and alveolar bone surface of tooth 13 to 15, application of Ti-OSS® combined with PRF and pericardium membrane. Discussion: The follow-up evaluation 2 months after treatment showed clinical finding of decreased Clincial Attachment Level (CAL) and radiographic finding showed increased alveolar bone height around defect area of tooth 13 and 15. Conclusion and Suggestions: Periodontal flap surgery combined with application of Ti-OSS® and PRF may able to stimulate regeneration of alveolar bone and periodontal tissue in chronic periodontitis

Abstract

Introduction: Connective Tissue Graft (CTG) is one of the techniques in gingival recession treatment, but considering the morbidity of using CTG, biomaterials such as Platelet Rich Fibrin (PRF) can be an alternative in gingival recession closure treatment. This systematic review aims to compare and evaluate the use of PRF, CTG and their combination in gingival recession closure. Review: An electronic literature search in Medline (PubMed), Google Scholar, and Elsevier (Sciencedirect) was conducted to identify Randomized Clinical Trial (RCT) reporting on the treatment of gingival recession using PRF, CTG or a combination of both from 2018 to 2023 in English. This systematic review was conducted following the Preferred Reporting Items for Systematic review (PRISMA) guidelines. The parameters assessed in the RCTs were: recession depth (RD), clinical attachment level (CAL), keratinized gingival width and height (KTW, KTH), probing depth (PD), gingival thickness (GT) and patient discomfort after surgery assessed by VAS (Visual Analog Scale). Based on the 1,152 articles retrieved, there were only five RCTs eligible for systematic review. Four articles compared the use of PRF with CTG, and one article compared the use of CTG with the combination of CTG + PRF in gingival recession closure. Conclusion: The use of PRF, CTG, and a combination of both were effective in closing gingival recession. The use of CTG gave better results in increasing gingival thickness and keratinized gingival width compared to the use of PRF, but the combination of CTG and PRF gave significant results in keratinized gingival height and decreasing recession depth compared to the use of CTG alone. Thus, the combination of CTG and PRF may be a more promising gingival recession closure treatment option.

Abstract

Introduction: Surface roughness is a contributing factor to the accumulation of plaque and the development of secondary caries, also can determine the aesthetic and long-term durability of dental restoration materials, such as nanohybrid composite resin. The use of mouthwash is one of the factors that cause the surface roughness of nanohybrid composite resin. Material and Methods: The research method used was laboratory experimental research with a pre-test and post-test design. Thirty samples of nanohybrid composite resin were randomly divided into three treatment groups (n=10) according to the types of mouthwash (alcohol-containing, alcohol-free, and herbal), and then were immersed for 36 hours. Surface roughness measurements were performed with a surface roughness tester before and after treatment. Results and Discussions: One-way ANOVA statistical analysis showed a significant difference (p<0,05) in all treatment groups. Conclusion: The highest surface roughness was found in alcohol-containing mouthwash, followed by alcohol-free mouthwash and herbal mouthwash.

Abstract

Introduction: Fixed orthodontic treatment is a type of dental treatment that is very popular throughout the world, not only to improve function but also lifestyle. However, it has an impact on the condition of the oral cavity, namely the flow rate and pH of salivary, which can affect dental and oral health. This study aims to determine the differences in flow rate and saliva pH between users and non-users of fixed orthodontic devices. Materials and Methods: This type of research is analytical observational, with a cross sectional approach. The population in this study were students class 2020, 2021, and 2022, the sample in this study was 116 students. Data collection by observation, interviews and examination. The method for measuring saliva flow rate with drainase and saliva pH using the Hanna HI98107 pH meter. Univariate and bivariate data analysis (independent sample t test). Results and Discussions: The results of the study showed that the there was a difference in saliva flow rate in users and non-users of fixed orthodontic appliances (p-value 0.001), there is a difference in saliva pH between users and non-users (p-value 0.000). Conclusion: The use of fixed orthodontic can affect saliva flow rate and salivary pH.

Abstract

Introduction: : Teeth and mouth are parts of the body as a whole that have a function to support and enable humans to function essentially. The mouth is also a fundamental feature in a person's identity. The existence of diseases of the teeth and mouth such as caries can cause disruption of oral health conditions and reduce a person's quality of life. Therefore, this research was conducted to find out the relationship between the level of dental caries experienced and the quality of life of students at the Teaching Faculty of Mulawarman University. Material and Methods: This research is an analytical observational study with a cross-sectional study design. This research involved 96 students from the 2018-2021 class who were selected using a stratified random sampling technique. This study used the OHIP-14 questionnaire to measure quality of life and oral photographs to measure the level of caries. Research data was analyzed using the Spearman test. Results and Discussions: The research results showed that the level of dental caries among Mulawarman University students was 2.43 in the low category and the majority of students' quality of life level was good at 54%. The results of the relationship analysis show p = 0.000 (p < 0.05); rho = 0.703. Conclusion: There is a relationship between the level of caries and the quality of life in students at the Teaching Faculty at Mulawarman University with strong strength and a positive direction of the relationship.
