Abstract

Introduction: Cost Benefit Analysis is an economic analysis used to evaluate the use of economic resources so that scarce economic resources can be used efficiently. In 2023, SK Clinic proposes opening a new poly clinic which aims to improve the health services provided by SK Clinic. Not all of these proposals can be proposed in their entirety, due to the limited funds that the SK clinic has. The aim of this research is to calculate the Cost Benefit Analysis between the proposal to open a Dental Clinic or Children's Physiotherapy Clinic. Material and Methods: This research used a quantitative descriptive type of research by comparing the costs and benefits between opening a Dental Clinic or Pediatric Physiotherapy Clinic at the SK clinic in Sidoarjo as the unit of analysis in this research. Cost analysis methods used Cost Benefit Analysis (CBA) and Benefit Cost Ratio (BCR) were also carried out to determine alternative actions to be taken. Results and Discussions: Based on the calculation results of each step of the Cost Benefit Analysis, it was found that each proposal had a different Benefit Cost Ratio (BCR) value, where the BCR value of the proposal to open a dental clinic was 1,48 smaller than the BCR value of the proposal to open a Children's Physiotherapy Clinic, which was equal to 1,97. Conclusion: Based on the BCR value, the proposal that was prioritized first was the proposal to open a Children's Physiotherapy Clinic so that this proposal will bring greater benefits to the SK Clinic for children.

Abstract

Introduction: The frenulum is a fold of mucosa that attaches the lips and cheeks to the alveolar mucosa, gingiva, and underlying periosteum. If the attachment is too high and thick to approach the gingival margin (papilla penetrating type) it causes a central diastema on the upper incisor or recurrence after orthodontic treatment, besides causing oral hygiene problems and gingival recession as well as denture stability. An abnormal frenulum should be removed by frenectomy. History and Clinical Finding: An 18-year-old female patient with a tall and thick maxillary labial frenulum as a cause of central diastema on teeth 11, 21. The patient had been treated with removable orthodontics starting 2 years ago. Since 4-6 months ago the diastema has relapse. The maxillary labial frenulum appears to be attached to the incisive papilla (penetration of the papillae). The Blanch test shows movement of the interdental papillae and blanching (ischemia) of the incisor papillae. Case Management: Frenectomy using a scalpel until all the papillae and fibrous fibers are removed. Control 14 days postoperative, the wound was healed. During the blanching test, there was no movement of the papillae and the color was normally. Discussions: The high maxillary labial frenulum causes the two central incisors to erupt far apart. Orthodontic treatment  must be accompanied by removal of the etiological factor by frenectomy for the treatment to be successful. Conclusions: Penetrating papilla type frenectomy of the maxillary labial frenulum with the conventional technique of using a scalpel effectively eliminates the causes of central diastema.

Abstract

Introduction: Many school children suffered from dental caries, which causes children to skip school because of toothache. The involvement of teachers in schools is considered important. The teacher is a model for children who can provide motivation for proper dental health behavior. The purpose of this study is to provide training to elementary school teachers, to increase their dental health knowledge, in order to educate their students. Materials and Methods: This is a pre-experimental design study, which provides dental health training to teachers. This activity involved 4 dentists as trainers, who had previously performed calibration. There are 2 schools involved, namely Saraswati 1 Elementary School and Saraswati 2 Elementary School in Denpasar Bali. A total of 31 teachers who signed the informed consent were included as samples. Dental health education materials are in the form of power points, animated educational videos, dental models, fluoridated toothpaste and toothbrush for children. The questionnaires were used as an instrument. The educational theme is "prevent cavities by brushing your teeth morning and night. Visit the dentist before it's too late". Data were analyzed using Wilcoxon signed rank test. \ Results and Discussion: At the beginning of the study, it was found that 25.81% of teachers had the poor knowledge category, and 74.19% were in the middle category. There was no teacher with good knowledge category. However, at the end of the study, 100% of the teachers had good dental health knowledge. There are differences in knowledge of teachers before and after receiving training. Conclusion: Dental and oral health training method for elementary school teachers can improve dental health knowledge, so that teachers are worthy to teach dental health to their students.

Abstract

Introduction: Dental trauma is damage that affects the hard tissue of the teeth and/or periodontal due to mechanical causes. One of the effects of trauma to the teeth is intrusive luxation, namely the movement of teeth into the alveolar bone, which can cause damage or fracture of the alveolar socket. Intrusive luxation causes the tooth crown to appear shorter. Case: An 11 year old male patient who was involved in a traffic accident came to the emergency department with complaints of being unable to walk, swollen lips and broken teeth. Case Management: The patient experienced intrusive luxation on tooth 21 and subluxation on teeth 11 and 22. The treatment carried out in this case was repositioning of tooth 21. Then splinting was carried out using wire on teeth 12, 11, 21, 22 for 1 month. Discussion: Immediate repositioning of luxated teeth is very important to accelerate tissue healing. Treatment in acute cases can be supplemented with acute management, namely administering antibiotics. In this case, antibiotics, anti-inflammatories and calcium are given to speed up tissue healing and reduce the risk of complications in the tissue. Conclusions and Suggestions: immediate repositioning, accompanied by wire splints and administration of antibiotics provide good results in the treatment of cases of intrusive luxation due to accidental trauma.

Abstract

Introduction: Periodontal tissue condition plays an important role as an adjunctive in the success of orthodontic therapy. One of prominent esthetic problem is midline diastema case, which was caused by high and aberrant frenulum attachment. This clinical case aims to report the frenectomy procedures with electrosurgery through multidiscipline perio- orthodontic approach to close the midline diastema. Case: : A female 19 years old patient was referred from orthodontic clinic with chief complaint there was a gap between her central insisif in upper jaw. The frenulum showed the Blanch test positive and there was no clinical attachment loss. Case Treatment: Frenectomy with electrosurgery was done to eliminate the aberrant frenulum attachment. This procedure followed with fixed orthodontic therapy to close the midline diastema and prevent any relapse. Discussions: Frenectomy can be achieved with electrosurgery which has many advantages such as coagulation, a better visualization, blood-less area, bacterimial risk reduction, an uneventful healing process, less post operative discomforts, and minimal scar tissue formation. Fixed orthodontic appliance was bonded to close the gap between central incisors. The intraoral examination showed that there was no any relaps in midline after one year follow up debonding of fixed orthodontic. Conclusion and Suggestions: a functional and esthetic outcome can be achieved through multidiscipline perio-orthodontic approach. The combination of frenectomy and orthodontic therapy showed an optimal result to close the midline diastema. Abstract

Introduction. Halitosis is a dental and oral illness that is common in society. Steptococcus salivarius is the bacteria that causes halitosis. The purpose of this study was to determine whether lime  peel extract toothpaste possesses any bioactive antibacterial properties against the strain of bacteria Streptococcus salivarius  and can brighten teeth. Materials and Methods. The research method used was True Experimental Design. This research design used Post Test Only Control Design. Where there are three groups, namely the treatment group, positive control group, and negative control group. The data obtained will be tested statistically using the one-way ANOVA analysis with a confidence level of 95% and followed by the Least Significance test. Dental lightening research is carried out on teeth that have been extracted. Brush the teeth with lime peel paste for 7 days and do it 2 times a day. Tooth color was recorded before treatment and after treatment with shade guide Chromascop Ivoclar Vivedent. Result and Discussion. The results of this study showed that toothpaste with lime peel extract was able to inhibit the growth of Streptococcus salivarius bacteria. The respective concentrations of lime extract in toothpaste F0 to F4 are 0%, 2.5%, 5%, 7.5%, and 10%. K+ about using toothpaste on the market, with an average inhibition zone of F1 (8.7), F2 (9.86), F3 (11.58), F4 (13.4), K+ (19.65), and K- (0), so it can be stated that the more the extract content, the higher the inhibitory power for the growth of Streptococcus salivarius. Besides that, lime peel toothpaste can brighten teeth several levels Conclusion: Lime peel extract toothpaste is able to inhibit the growth of Streptococcus salivarius bacteria and is able to brighten teeth several levels.

Abstract

Introduction: The health of pregnant women is influenced by what they consume. Consuming good food but having infections or abnormalities in the mouth and teeth will cause the incoming food to be contaminated and will have an impact on the baby's growth and development. Based on this, dental and oral health in pregnant women plays a very important role for pregnant women and children. This study aims to find out how analysts promote dental and oral health in pregnant women. The method used is a systematic literature review using the PRISMA diagram by identifying P (Population, patients), Intervention (Intervention or treatment), Prognostic Factors, or Exposure, Comparison (comparison with control if any), Outcome (outcome) according to the research to be conducted, and Study (type of research). From the results of the reduction of selected journals, there are 19 scientific articles that will be analyzed. Review: In this research, there are various health promotions that can be carried out to provide education to the public, including social media, consultations, hospitals, training, providing video demonstrations and explanations via flip charts Conclusions: The conclusion of this research is that there are various health promotions that can be carried out to provide education to the public, including social media, consultations, hospitals, training, providing video demonstrations and explanations via flip charts. The suggestion from this research is the need for intensive health promotion and health education for pregnant women. Promotion and education can be carried out by residents in collaboration with community health centers or in collaboration with hospitals or can also be done through social media.

Abstract

Introduction: To address the limitations resulting from the implementation of health protocols in providing healthcare services to patients, a solution was found in the utilization of information and communication technology to support healthcare services for patients in the form of telehealth, often known as telemedicine. Review: According to Article 1, Paragraph 21 of Law Number 17 of 2023 regarding Health, telemedicine is defined as the provision and facilitation of healthcare services, including public health, health information services, and self-care services, through telecommunications and digital communication technology. One of the aspects of telemedicine in the field of dentistry is teledentistry, which can be a solution for optimizing dental and oral healthcare services for individuals and the community. However, there are various issues in its implementation, especially related to legal issues such as licensing, patient electronic medical record privacy and confidentiality, clinical guidelines, and others that are highly vulnerable to data discrepancies, falsification, or digital patient data leakage. Conclusion: These issues can potentially lead to medical disputes that may enter the legal realm. On the other hand, teledentistry services also play a crucial role in improving public health, especially in dental and oral health in Indonesia, particularly during the COVID-19 pandemic. Therefore, the development of teledentistry services needs to be continuously improved, both in terms of systems and regulations, to provide security and legal certainty for the Indonesian population.

Abstract

Introduction: Dentoalveolar fractures can be treated in a simple non-surgical procedure by repositioning the fractured fragments and immobilizing them. One of the factors affecting the success of dentoalveolar fracture treatment is the choice of material and immobilization technique. Case: A 16-year-old girl came to the Emergency Department with complaints of bleeding from the mouth due to a traffic accident 5 hours ago. Her lip hit the handlebar of a motorcycle. There was no history of fainting, vomiting, and bleeding from the ears and nose. Extraoral examination revealed an asymmetrical face. Edema and laceration on the upper lip. Intraoral examination revealed gingival laceration of teeth 12-22 and avulsion of teeth 12,22. Panoramic x-ray revealed supra position of teeth 11, 21 and avulsion of teeth 12, 22. Case Management: Wound debridement, indirect root canal treatment of teeth 12, 22, repositioning of teeth 12-22, fixation with wire and composite splinting, selective grinding, antibiotics, and analgesics were performed. Post-operative instructions included maintaining oral hygiene, a soft diet, and a return for splinting control. Discussion: The goal of dentoalveolar trauma treatment is to restore the normal function and shape of the masticatory organs. The treatment in this case uses flexible splinting because it has ability to resemble the physiological mobility condition of the teeth which can help the healing of the periodontal ligament. The treatment results showed soft and hard tissue healing after one month of treatment. Conclusion: Successful treatment of dentoalveolar fractures will be achieved if performed promptly and if accurate immobilization and accurate tools are used. Simple tools and techniques make flexible splinting a good treatment for simple dentoalveolar fractures.

Abstract

Introduction: Artificial intelligence (AI) involves the creation of computer systems that imitate human actions, and it is progressively adopted as a supportive tool in aiding clinicians with disease diagnosis and treatment. One prevalent global ailment is periodontitis, which leads to the degradation and loss of the tooth-supporting tissues. The aim of this  review is to evaluate existing literature that delineates the influence of AI on diagnosing and studying the prevalence of this condition. Review: A Pubmed advanced search with narrative review was conducted of the past ten years using several search term such as “artificial Intelligences” and “periodontics”. Thorough searches were conducted on Pubmed in June 2023, encompassing studies where AI functioned as the independent variable for assessing, diagnosing, or treating patients with periodontitis. After eliminating duplicates, a total of 100 articles were recognized for preliminary abstract scrutiny. Of these, 76 documents were excluded, resulting in 24 texts for comprehensive evaluation. Conclusion: The development of artificial intelligence in the field of dentistry requires more systematic reviews and meta-analyses to enhance the knowledge and scope of artificial intelligence applications. AI models for periodontal applications are still under development and in the future, they have the potential to support diagnostic accuracy capability.
