Abstract

Introduction: Pregnancy is a risk factor for periodontal disease due to an increase in hormones which causes a change in response of periodontal tissue to the accumulation of plaque and calculus that can increase the occurrence of inflammation of the gingiva and periodontal disease. Pregnant women in coastal areas have a high risk of periodontal disease because the majority consume seafood that contains high calcium and phosphorus, which allows for increased calculus formation. Periodontal disease during pregnancy has an adverse impact on the health of the fetus. Therefore, it is important to pay attention to the health of the periodontal tissues of pregnant women. This study aims to examine the description of health and periodontal treatment needs in pregnant women in the coastal area of Puger, Jember. Material and Methods: This type of research is descriptive observational with a cross sectional study on 70 subjects selected by purposive sampling technique. Data were obtained based on the results of the Community Periodontal Index of Treatment Needs (CPITN) examinations and questionnaire interviews regarding oral health. Results and Discussions: Health status of periodontal tissues of pregnant women in the coastal area of Puger Beach mostly (75,71%) had supra or subgingival calculus (CPITN score 2), there were periodontal pockets and there were no pregnant women with healthy periodontal tissue conditions. Types of periodontal treatment needs that are needed by all pregnant women are scaling and root planing and improving oral hygiene (Treatment Needs/TN 2).  Conclusion: Pregnant women in the coastal area of Puger Beach all experience periodontal disease so they need periodontal treatment.

Abstract

Introduction: Allopurinol is a xanthine oxidase inhibitor and has become established as the drug of choice for preventing and treating chronic gout. Erythema multiforme (EM) is an immune-mediated reaction that involves the skin and sometimes the mucosa. Many cases of EM are caused by drugs, one of which is allopurinol. The purpose of the case study is to explain the identification of allopurinol as a predisposing factor for EM and how it is treated. Case. A 56-year-old woman came with complaints of scabs on her lips for 1 week ago, and had been treated with corticosteroid ointment for 3 days. Extraoral examination showed hemorrhagic crusts on the upper and lower lips with erosive surrounded by erythematous margins. On the extremity found a target lesion on the right hand. Based on the examination, the patient was diagnosed with Erythema Multiforme with a differential diagnosis of herpes labialis and herpes associated erythema multiforme. Case Management. The therapy in this case was giving antihistamines for 1 week, NaCl 0.9% compresses on the lips for 30 minutes 3 times a day, after 2 weeks of treatment added with the application of 1% hydrocortisone cream as an anti-inflammatory and petroleum jelly on the lips as a moisturizer. Conclusion. Identification of the etiology of this EM case was carried out through anamnesis and history of drugs consumed as well as administration of topical corticosteroids as anti-inflammatories and oral antihistamines to reduce allergic reactions.

Abstract

Introduction: Improper management of condyle fractures can lead to changes in the structure of the temporomandibular joint, which can impact the patient's oral function. Corrective surgery is necessary to restore the patient's function and quality of life. This case report aims to discuss the procedure of bilateral condylectomy in neglected condylar fracture, as well as the risks and post-operative complication management. Case: A 26-year-old female patient presented with mandibular hypomobility and occasional temporomandibular pain. The patient had history of a traffic accident around four months ago, and was managed in a public hospital but symptoms persisted. Case Treatment: After thorough examinations, the patient was diagnosed with neglected complete bilateral condylar fracture with displacement and managed via bilateral condylectomy. Post-operative, the patient presented with anterior open-bite and facial paralysis on the right side, and was treated accordingly. During 4-month follow-up, complications and chief complaints had all subsided. Discussions: Mismanagement of condylar fracture could result in malunion of fragments, which is difficult to reposition due to the changes that have already occurred in the temporomandibular structure. Condylectomy is often necessary to relieve symptoms, although bilateral condylectomy poses certain neurological risks. Prompt recognition and intervention of nerve injury is important to prevent permanent complication. Conclusion and Suggestions: Condylectomy could properly treat neglected bilateral condylar fracture in adult patients. Operators must know the associated risk and management of post-operative complications.

Abstract

Introduction: Dry socket is post-tooth extraction pain characterized by partial or complete loss of blood clots in the alveolar socket. One of the management of dry socket is by using antibiotic therapy. Inappropriate use of antibiotics in addition to being economically wasteful can also cause bacterial resistance to antibiotics. The use of herbal ingredients such as Cemcem (Spondiaspinnata (L.f) Kurz) is one solution. Cemcem leaves are thought to have antibacterial effects because they contain flavonoids, tannins and saponins. This study aims to determine the inhibition of cemcem leaf extract (Spondias pinnata (L.f) Kurz) against the growth of Streptococcus mutans. Materials and methods: The research method used is the Kirby Bauer method which is a sensitivity test with the agar diffusion method using the disc diffusion technique. In the Kirby Bauer sensitivity test using selective media, namely Muller Hinton Blood Agar media, the inhibition zone created is characterized by a clear zone around the disc. The data obtained were analyzed using the Kruskall Wallis test to determine whether or not there was a significant difference then continued with the Mann Whitney U - Test. Results: Cemcem leaf extract (Spondias pinnata (L.f) Kurz) with a concentration of 60% and 70% can inhibit the growth of Streptococcus mutans better than positive control and 80% concentration. Conclusion: 60% concentration of cemcem leaf extract is more effective in inhibiting the growth of Streptococcus mutans than 70% and 80% concentrations.

Abstract

Introduction: Diabetes mellitus (DM) is a systemic disease that affects many human populations in the world. One of DM’s oral complications is periodontitis. The pro-inflammatory cytokine that plays a major role in the relationship between these two diseases is TNF-α. Review: Diabetes mellitus is a systemic disease characterized by high blood glucose levels (hyperglycemia) due to inadequate insulin secretion, insulin resistance or both. Metabolic disorders caused by hyperglycemia can cause endothelial dysfunction in blood vessels. Endothelial dysfunction that occurs due to hyperglycemia can trigger an inflammatory response that can increase the progression of periodontitis. Periodontitis is a chronic inflammatory response in periodontal tissue to pathological microorganisms in dental biofilm. The periodontitis process begins with gingival inflammatory response. The initial phase of gingival inflammation is characterized by redness and bleeding on probing. Worsening inflammation will cause loss of periodontal tissue attachment and alveolar bone resorption which can lead to tooth loss. The inflammatory process that occurs in periodontitis involves pro-inflammatory cytokines that can influence insulin resistance, such as Tumor necrosis factor-alpha (TNF-α). Tumor necrosis factor-alpha is also a cytokine that plays a major role in the pathogenesis of diabetes mellitus, so that periodontitis and diabetes mellitus are interrelated diseases Conclusion: The TNF-α cytokine is a potential therapy target for periodontitis and diabetes mellitus.
