Abstract

Chemerin is an adipokine involved in inflammation, adipogenesis, angiogenesis, and energy metabolism. Diabetes Mellitus (DM) is a group of metabolic diseases with hyperglycemia characteristics that occur due to abnormalities in insulin secretion, insulin action, or both. This study aimed to determine serum chemerin levels in patients with Type 2 Diabetes Mellitus (T2DM) between controlled and uncontrolled groups. This study was an analytical observational study with a cross-sectional approach. The study samples were all patients diagnosed with T2DM who had HbA1c test results and met the inclusion and exclusion criteria. Serum chemerin was measured using the ELISA method. The Kolmogorov-Smirnov, Chi-Square, Pearson, and independent T-test statistically analyzed the data. The subjects were 60 controlled and uncontrolled DM patients, 29 males and 31 females. There was a difference in serum chemerin levels in the controlled and uncontrolled DM group (5.73±2.3 ng/mL with 6.87±1.7 ng/mL, p=0.40). There was a positive correlation between chemerin levels and HbA1c (r=0.266; p=0.040). Serum chemerin levels of patients with uncontrolled DM were higher than in patients with controlled DM. Serum chemerin will increase along with the increase of HbA1c level.

Abstract

The pathogenesis of Lupus Nephritis (LN) is still unclear, causing difficulties in diagnosis and therapeutic management. Recent studies have suggested that microRNAs (miRNAs) may play an important role in the pathogenesis of LN. However, the role of miRNA-451a in LN has not been widely studied and known, and there is controversy in the results of studies that have been done. This study aimed to analyze the expression of miRNA-451a in LN and lupus without nephritis and to evaluate its correlation with hemoglobin levels, leukocyte count, leukocyturia, and erythrocyturia. Samples from 45 lupus participants were collected, consisting of 22 with LN and 23 without nephritis. The expression of microRNA 451a was determined by reverse transcriptase real-time PCR. Renal disorder criteria based on ACR criteria. Statistical analysis using Mann-Whitney, Spearman correlation, ROC curve, and table 2X2. MiRNA-451a expression in LN (median 0.31; range 0.06-7.21) was lower than in lupus without nephritis (median 1.31; range 0.07-13.26) with a significant difference (p=0.041). The cut-off value for comparing the two groups was 0.7531 with an accuracy of 75%. The correlation between miRNA 451a expression and hemoglobin levels, blood leukocyte count, erythrocyturia, and leukocyturia showed no significant correlation (p > 0.05). MicroRNA-451a expression in lupus nephritis was lower than in lupus without nephritis.

Abstract

The primary pathophysiology of preeclampsia is abnormal trophoblast invasion, vasospasm, and endothelial dysfunction occurring, as well as platelet activation, increased platelet consumption, and the stimulation of the release of new platelets, which affects the platelet indices and immature platelets. This study compared the platelet indices (Mean Platelet Volume=MPV, Platelets Distribution Width=PDW, Platelet Large Cell Ratio=PLCR, and Immature Platelet Fraction (IPF) between preeclampsia and normal pregnancy. This research was an unpaired comparative analytic observational study with a cross-sectional design conducted in the Tangerang District General Hospital from February to August 2020. The platelet indices (MPV, PDW, PLCR) and IPF of preeclampsia subjects (n=33) at Tangerang District General Hospital and normal pregnant subjects (n=38) at Annisa Hospital Tangerang were examined using the Sysmex XN-1000 automatic hematology analyzer. Comparative platelet indices and IPF analysis were carried out by independent T-test or Man-Whitney test. Receiver Operating Characteristics (ROC) analysis was performed to obtain the cut-off value of the platelet indices and IPF to differentiate preeclampsia and normal pregnancy. The platelet indices and IPF were significantly higher in preeclampsia. The mean (SD) MPV was 10.86 (0.9) fL and 10. (0.89) fL, p<0.001; PDW 12.7 (8.7-19.1) fL and 10.85 (8.5-15.7) fL, p<0.001; PLCR 31.96 (7.02) % and 25.06 (6.92) %, p<0.001; and IPF 8.50 (3.10-20.40)% and 2.75 (1.2-9.4)%, p<0.001, respectively in preeclampsia and normal pregnancy. ROC analysis showed that an IPF with an AUC of 0.88 (95% confidence interval (CI) 0.78-0.95) with an IPF cut-off value of >6.5% was a good marker, while the MPV cut-off >10.2 fL, AUC 0.75(95% CI 0.63-0.84), PDW cut-off >11.3fL, AUC 0.77 (95%CI 0.65-0.86) and PLCR cut-off >26.1%, AUC 0.76 (95% CI 0.65-0.85) were good enough markers to differentiate preeclampsia from normal pregnancy. Platelet indices and IPF were statistically significantly higher in preeclampsia. The IPF parameter is a good marker, while the platelet indices can be a pretty good marker to distinguish preeclampsia and normal pregnancy. The IPF parameter and platelet indices can be proposed as a routine examination in pregnancy. Changes in IPF and platelet indices can be early signs of preeclampsia or the development of preeclampsia.

Abstract

Chronic Kidney Disease (CKD) is a condition characterized by kidney damage for > 3 months, as well as structural or functional abnormalities with or without a decrease in Glomerular Filtration Rate (GFR) < 60 mL/minute/1.73. This damage leads to increased urea levels in the blood, also known as uremia, which can increase the risk of inflammation in CKD patients undergoing hemodialysis (HD). Previous studies revealed that increased urea and Neutrophil-Lymphocyte Ratio (NLR) can be used as inflammatory biomarkers to replace CRP, IL-6, and other indicators. This study aimed to determine the correlation between NLR and serum urea levels in pre- and post-HD CKD patients at Dr. Doris Sylvanus Hospital. An analytical survey method was used with a retrospective design.The process started by taking medical records of patients currently undergoing HD who were selected as respondents. The sample population consisted of 50 (70.4%) males and 21 (29.6%) females. Based on the age group, 43.7% of patients were aged 51-60. The results showed that the average NLR pre- and post-HD were 5.24±4.88 and 10.41±12.31, respectively. The average urea pre- and post-HD were 195.97±77.88 and 120.97±47.4, respectively. The bivariate analysis showed a significant weak correlation between NLR and serum urea level pre-HD (p=0.004 with r =0.338) and post-HD (p=0.039 with r =0.246) in patients.

Abstract

In COVID-19 patients, abnormal blood clotting is common, characterized by elevated D-dimer and fibrinogen levels, reduced platelets, and prolonged clotting times. The second week of infection can trigger a cytokine storm, marked by heightened proinflammatory Interleukin-6 (IL-6) levels, associated with Acute Respiratory Distress Syndrome (ARDS) and organ failure. This study compared hematological biomarkers, D-dimer, and IL-6 in moderate and severe COVID-19 cases. In a cross-sectional study, 81 patients meeting inclusion criteria were examined at a leading private COVID-19 referral hospital in Malang Regency. Data from clinical records and lab results encompassing blood counts, D-dimer, and IL-6 levels were collected. D-dimer was assessed through immunoturbidimetry (STA-Procoag-PPL, Diagnostica Stago S.A.S.), while IL-6 was measured using a chemiluminescent immunoassay (Cobas e411 Elecsys, Roche). Data distribution normality was assessed using Kolmogorov-Smirnov and Shapiro-Wilk tests. Non-normally distributed data were analyzed using the Mann-Whitney U test for numerical data and the Fisher exact test for comorbidity-severity correlation. Moderate COVID-19 cases disproportionately affected females, while severe cases had an even gender distribution. The median age was comparable, but mild cases were typically younger. Hemoglobin, hematocrit, leukocyte, neutrophil, platelet, and procalcitonin levels were normal in both groups, with lowered lymphocyte counts. Severe cases displayed a higher Neutrophil-to-Lymphocyte Ratio (NLR). D-dimer and IL-6 were significantly elevated in extreme cases. This study underscores potential gender and age-related discrepancies in COVID-19 severity, emphasizing the significance of monitoring specific blood parameters for disease progression indicators. Further investigation is vital to unveil underlying mechanisms and clinical implications, aiding the management of COVID-19 patients.

Abstract

Obesity is the accumulation of excessive fat resulting from an imbalance in energy intake compared to energy used for a long period. Vaspin is secreted in adipose tissue and may exhibit compensatory responses related to obesity, insulin resistance, and type 2 diabetes mellitus. This study aimed to determine vaspin levels in obese and non-obese subjects.  An observational research design with a cross-sectional approach was used in this study. A total of 80 research subjects were included in this study. The research was conducted at the Clinical Pathology Laboratory at the Department of Clinical Pathology, Hasanuddin University Hospital in January 2022. Kolmogorov-Smirnov test was used to determine the normality of the data. Chi-Square, unpaired T-test, Mann-Whitney test, and Spearman test were used as statistical tests. This study showed a significant difference in anthropometric parameters between the obese and non-obese groups (p < 0.001). There was a significant difference in the mean levels of vaspin between the obese and non-obese groups (707.68±774.47 ng/mL vs. 510.60±460.08 ng/mL, p=0.004). There was a positive correlation between vaspin levels and waist circumference (r = 0.245, p=0.004) and between vaspin levels and BMI (r = 0.322, p=0.028). The serum levels of vaspin tended to be higher in obese subjects compared to non-obese subjects. The positive correlation between vaspin levels and BMI and waist circumference indicated that a higher BMI and/or waist circumference led to a higher vaspin level.

Abstract

Ovarian cancer is one of the gynecological malignancies with a bad outcome. Epidemiologically, it is the fifth cause of death in females worldwide. Patients have various complaints, and examinations available for early detection are inadequate, causing high mortality rates and late diagnosis. Prepare a more appropriate examination to establish an earlier diagnosis, especially in females with risk factors requiring screening. Laboratory examinations combined with other supporting examinations can provide better results to confirm the diagnosis of ovarian cancer and lower the morbidity rate. Knowing the updated examination to support the diagnosis, such as examination of ovarian cancer biological markers specific to HE4, CA-125, CEA, CA 19-9. A combination of examinations is needed for early detection of ovarian cancer.

Abstract

World Health Organization recommends rapid results of Human Immunodeficiency Virus (HIV) viral load by using Point of Care (POC) testing, delivering faster decision-making to prevent transmission and suppress drug resistance and greater likelihood of starting therapy if the clinic provides care and treatment services. The m-PIMATM HIV-1/2VL cartridge is an RT-PCR POC test that detects HIV-1 and HIV-2 RNA viral load, recognizing the conserved-part 5'-UTR of the HIV genome. The standard, Roche Cobas®4800 system HIV-1, targets gag and LTR regions. This study aimed to determine the correlation, sensitivity, and specificity of Abbott m-PIMATM HIV-1/2VL compared to Roche Cobas®4800 system HIV-1. A cross-sectional study at Dharmais Hospital Laboratory. EDTA plasma obtained from 200 HIV patients, consisted of 80 high-viral-load samples (>1000 copies/mL), 80 low-viral-load samples (<1000 copies/mL), and 40 undetectable-viral-load samples (<14.2 copies/mL). Kappa Cohen was used to measure the categorical correlation. Sensitivity and specificity were calculated with diagnostic test. Dharmais Ethic Committee released ethical clearance No.0132/KEPK/IX/2020. The categorical correlation of Abbott m-PIMATM HIV-1/2 VL in determining the group of high or low viral load samples was 0.948 (very strong correlation). The categorical correlation deciding whether the HIV viral load sample was high, low, or undetectable was 0.714 (strong correlation). The categorical sensitivity of Abbott m-PIMATM HIV-1/2 VL compared to Roche Cobas®4800 system HIV-1 in categorizing HIV viral load was 97.5%, specificity of 97.5%. Abbott m-PIMATM HIV-1/2 VL performance in determining HIV RNA viral load group samples has a strong correlation compared to Roche Cobas®4800 system HIV-1. A larger study is required.

Abstract

Obesity is defined as an increase in total body mass due to excess fat, as measured by the body mass index. Obesity affects metabolism through factors such as insulin resistance, which triggers the development of chronic diseases. Lipid and ApoB/ApoA-I ratios can be considered additional assessments, especially in patients at high risk of cardiovascular diseases such as diabetes. This study aims to determine the relationship between lipid and ApoB/ApoA-I ratios with Homa-IR in in non-diabetes mellitus adult subjects. The method used in this study was a cross-sectional study involving a total of 80 non-diabetic adult subjects, consisting of 38 male and 42 female subjects. The results showed that there were significant differences between the ratios of CHOL/HDL, TG/HDL, ApoB/ApoA-I, and HOMA-IR in non-obese and obese non-DM adult subjects (4.64 ± 2.18 vs 5.23 ± 1.87, p = 0.045; 2.38 ± 1.20 vs. 3.18±1.77, p=0.013;0.58±0.17 vs. 0.66±0.17, p=0.035 and 1.47±0.80 vs. 3.34±4.37, p = 0.000), and there was no significant difference between LDL/HDL (p = 0.117). The correlation test results between the ratios of CHOL/HDL to HOMA-IR, LDL/HDL to HOMA-IR, TG/HDL to HOMA-IR, and ApoB/ApoA-I to HOMA-IR in non-DM adult subjects (p=0.05, r=0.308; p=0.037, r=0.233; p=0.000, r=0.402; p=0.19, r=0.261). This study concluded that there was a significant correlation between lipid and ApoB/ApoA-I ratios with Homa-IR in non-diabetes mellitus adult subjects.
