Abstract

Indonesia is an endemic area of Dengue Hemorrhagic Fever (DHF). The clinical symptoms of this disease varies; it can beasymptomatic or atypical fever. Therefore, the early detection of Dengue virus is important to reduce the incidence of new victims fromhis infection. The aim of this study was to find out the haematological features of DHF suspected patients indicated for hospitalization. Aross-sectional study was done on DHF suspected patients indicated for hospitalization. The data were obtained from the Medical Recordat Wahidin Sudirohusodo Hospital from January-July 2009. The data were presented in the form of table. The results showed that themean leukocyte counts was 5.138 103/µL ranging from 2.10–16.70 103/µL, the mean erythrocyte count was 5.136 106/µL rangingrom 3.2–6.40 106/µL, the mean platelet count was 111.27 103/µL ranging from 11.40–260 103/µL. The DHF patients at admissionwho had leucositopenia, thrombocytopenia, and monocytosis were 54.30%, 71.40%, and 51.40%, respectively. Based on the result itan be concluded, that the most common haematological abnormality in DHF suspected patients whom indicated for hospitalizationwere thrombo cytopenia, leucocytopenia and monocytosis.

Abstract

Diabetes Mellitus (DM) belongs to the group of metabolic diseases. The long-term complications of diabetes such as: diabeticretinopathy, neuropathy, and nephropathy, can be prevented or reduced by controlling the blood glucose levels. Impaired glucose tolerance,hypertension and dyslipidemia can decrease the disappearance from proximal tubules resulting in an increase in serum uric acid. The aimof the study to know the ratio of uric acid urine/serum in DMT2 with controls. This research study used a cross sectional study was andconducted on 35 samples of type 2 diabetes mellitus and 35 control samples of In- and Out-patient Clinics at the Dr. Wahidin SudirohusodoHospital, Makassar. The blood glucose, uric acid levels and urine uric acid were examined by ABX Pentra 400 (colorimetric method). Thenumber of DMT2 male patients was higher than females. Most of their age were between 50−54 years. The ratio of the urinary levels ofurine/serum UA was positively correlated to: age, sex, urine UA in DMT2 and control. Their data were analyzed with unpaired t test (p =0.00). This research showed that the ratio of urine/serum UA in DMT2 was 1.5 times than the controls. The results of this study showedan increase in the ratio of urinary levels of serum uric acid as 1.5 times higher than the controls.

Abstract

Hepatitis C (HCV) infection could be spread by blood transfusion. Screening of HCV in donor blood could prevent HCV infection to therecipient. HCV antibody test using rapid test of multiple antibody detection by immunochromatography method is an easy and rapid testthat could detect four HCV antibodies separately. The aim of this study was to evaluate the diagnostic value of antibody HCV using multipleantibody detection rapid test in diagnosing HCV infection. This was an analytical observational study with a cross sectional design. Thesamples consisted of 42 donors’ blood serum from the Surabaya Branch of the Indonesian Red Cross which underwent HCV infectiontest using ELISA method. The samples were then tested using PCR HCV RNA as the gold standard and antibody HCV multiple antibodydetection rapid test The diagnostic value of HCV antibody test using multiple antibody detection rapid test by immunochromatographymethod showed a diagnostic sensitivity of 100%, diagnostic specificity of 75%, positive predictive value of 66.7% and negative predictivevalue of 100%, a diagnostic efficiency of 83.3%, with a positive probability ratio of 4 times. The most often positive antibody patternwas four (4) positive antibodies (core protein, NS3, NS4 and NS5). Core protein (CP) and NS3 were the most often positive antibodies.Based on this study result, the HCV antibody test using multiple antibody detection rapid test by immunochromatography method hasa good diagnostic value.

Abstract

Aktivasi sistem imun memegang peran yang sangat penting di infeksi HIV. Hal ini dapat diketahui dengan salah satu pengukuranneopterin dalam cairan tubuh manusia sebagai pemeriksaan untuk pemantauan aktivitas imun seluler, yang dapat dikerjakan denganmudah dan peka. Neopterin merupakan hasil katabolik guanosine triphosphate (GTP), yaitu nukleotida purin tertentu, yang memilikigolongan kimiawi yang dikenal sebagai pteridin. Tujuan penelitian ini adalah untuk mengetahui keberadaan kenasaban antara kadarneopterin dan jumlah limfosit T-CD4+ darah di pasien HIV. Penelitian bersifat analitik pengamatan dengan rancangan potong lintang.Sampel terdiri dari 32 pasien yang terinfeksi HIV stadium I yang datang di Unit Perawatan Intermediit Penyakit Infeksi RSUD Dr.Soetomo Surabaya antara bulan Juli−September 2014. Pemeriksaan neopterin dengan metode ELISA dan memeriksa jumlah limfositT-CD4+ menggunakan metode flowcytometry (BD FACSCaliburTM). Hasil menganalisis secara statistik menggunakan uji kenasaban dariPearson dan dilanjutkan dengan uji regresi. Kadar neopterin penderita yang terinfeksi HIV cenderung meningkat dengan rerata 14,74nmol/L sedangkan jumlah limfosit T-CD4+ menurun dengan rerata 231,81 sel/μL. Keberadaan kenasaban negatif antara neopterindan limfosit T-CD4+ darah di infeksi HIV stadium I. Penurunan limfosit T-CD4+ disertai peningkatan kadar neopterin di pasien yangterinfeksi HIV stadium I.

Abstract

Thalassemia patients who receive repeated blood transfusions are at risk of iron overload and hepatitis C infection. Iron overload cancause iron deposit in many organs, including the liver. Iron deposits in the liver and hepatitis C infection can cause chronic inflammationof the liver and induce hepatic stellate cells to produce Extra Cellular Matrix (ECM) causing liver fibrosis. Laboratory diagnosis of liverfibrosis is based on direct and indirect markers. Collagen IV is a direct marker reflecting ECM degradation in liver fibrosis. AlanineTransaminase (ALT), Aspartate Transaminase (AST) and AST/ALT ratio are indirect markers reflecting liver cell damage due to liverfibrosis. The aim of this study was to investigate the difference of Collagen IV in liver damage and hepatitis C infection in thalassemiapatients with iron overload. Collagen IV was measured using ELISA, while ALT and AST were measured by enzymatic colorimetric assay.Fifty eight thalassemia patients with iron overload, 29 with hepatitis C and 29 without hepatitis C were studied. This study showed nosignificant difference in Collagen IV level, ALT, AST activity and AST/ALT ratio between subjects with and without hepatitis C(p 0.131,0.243, 0.256 and 0.726) and no significant correlation was found between collagen IV level and ALT activity, and between collagen IVand AST/ALT ratio (p 0.160 and 0.509). These findings indicate that Collagen IV showed no correlation with liver damage and hepatitisC infection in thalassemia patients with iron overload.

Abstract

The Absolute Neutrophil Count (ANC) is the absolute number of neutrophil derived from the multiplication of the relative number ofleukocytes to the total neutrophil count. Neutrophils can be disturbed due to myelosupressive effects of chemotherapy. Fever is a commonsymptom in neutropenia patients due to chemotherapy. This fever is an emergency requiring rapid handling of Oncology and needsadministration of appropriate antibiotics. Therefore, the data on the occurence of neutropenia and its related fever in patients whomaccept chemotherapy after breast cancer it is important to be noted. This study is carried on to know the Absolute Neutrophil Count(ANC) in breast cancer patients with chemotherapy by analyzing them. The study was performed retrospectively by taking medical recorddata from January up to December 2012 in Dr. Wahidin Sudirohusodo Hospital Makassar. The absolute Neutrophil Count pre and afterchemotherapy analysis was done to determine neutropenia, as well as the febrile neutropenia and the correlation between ANC withpatients age and stage of breast cancer. A total of 55 samples with the majority of patients (85.45%) were in the age group of 41–60years old. The ANC after chemotherapy occurred in 50 patients were decreased (90.91%). There are five (5) patients (9.09%) with feverand two (2) persons (patients) (3.63%) suffered febrile neutropenia. The correlation analysis between decreased ANC with patient ageand stage of breast cancer are p=0.054 and 0.070 respectively. Based on this study it can be concluded, that decreased ANC occurredin most patients after accepting chemotherapy but only few patients suffered febrile neutropenia. There was no significant correlationbetween the decreased ANC with patient age and stage of breast cancer.

Abstract

Patients with Type 2 Diabetes Mellitus (T2DM) have an increased prevalence of dyslipidemia, which contributes to ahigher risk of dyslipidemia- related complications in T2DM such as cardiovascular disease and stroke. This study aimed todetermine the correlation between TG and VLDL-C towards HbA1c levels in a person with T2DM. A retrospective study of 74outpatients with T2DM at Sanglah General Hospital, Denpasar, who examined serum HbA1c and lipid profiles were tracedfor serum TG. From the obtained TG profile, a secondary calculation of VLDL was carried out using the Friedewald equation(TG/5). A correlation test was used to determine the relationship between TG and VLDL-C towards HbA1c levels. Serum TG(212.95±147.46 mg/dL) and VLDL (36.69±23.54 mg/dL) were found to be higher in the group with poor glycemic control(HbA1c > 7 mg/dL) compared to serum TG (111.00±39.56 mg/dL) and VLDL (21.05±6.13 mg/dL) in the group with goodglycemic control (HbA1c ≤ 7 mg/dL) (p < 0.05). A positive correlation between serum TG (r=0.512; p < 0.001) and VLDL(r=0.18; p <0.001) towards HbA1c levels in T2DM patient was found. Insulin resistance increases the production of VLDL andApoC-III in the liver and increased chylomicron absorption in the gastrointestinal tract, causing prolonged postprandiallipemia and disruption of VLDL and TG clearance, thereby resulting in increased TG and VLDL in circulation. There is asignificant positive correlation between serum TG and VLDL towards HbA1c levels in a patient with T2DM.

Abstract

Evaluating the degree of liver fibrosis degree is invasive as well as uncomfortable, therefore, non invasive examinations such as liverfunction tests and elastography (Fibro Scan) as a predictor‘s device of liver fibrosis degree are necessary. The aim of this study was toknow the differences of liver function parameters based on the fibrosis degree in patients with chronic liver disease. This study was a crosssectional design using data from chronic liver disease patients treated at the Dr. Wahidin Sudirohusodo Hospital. The elasticity of the liverwas measured using a fibro scan device during June 2010–July 2011. The analysis was carried out by ANOVA test on various parametersof liver function particularly on the fibrosis degree in chronic liver disease. In this study PT, albumin, total bilirubin and platelet countshowed a significant difference of 0.019, 0.009, 0.017 and 0.000 respectively. The mean values of PT and total bilirubin were significantlyhigher in the high degree of fibrosis compared to those with medium and low degree of fibrosis in the chronic liver disease patients. Basedon this study, the mean albumin levels and platelet count were significantly lower in the high degree of fibrosis compared with the mediumand low degree of fibrosis, however, no significant differences in AST, ALT, APTT and GGT were found.

Abstract

In December 2019, an outbreak of acute pneumonia occurred in Wuhan, China. The disease was transmitted betweenhumans through droplets (coughing or sneezing) of infected patients, causing this outbreak to spread rapidly in variouscountries in the world, including Indonesia. On February 11, 2020, WHO announced the pneumonia was caused byCoronavirus Disease 2019 (COVID-19), which was caused by a new type of Coronavirus, the SARS-CoV-2. A rapid andaccurate diagnosis is critical for the control of the COVID-19 outbreak. The widely used test is a serology-based test thatdetects the presence of SARS-CoV-2 IgM/IgG antibodies in the patient's body. One of the methods used for this test isChemiluminescent Immunoassay (CLIA). This study aimed to determine the reliability of CLIA. The study was conductedfrom August to September 2020. The number of samples was 63 patients' serum. Polymerase chain reaction examination atHusada Utama Hospital, Surabaya, revealed that 21 patients were confirmed positive for COVID-19 with positive PCRresults, and 42 patients were healthy with negative COVID-19 results. The results showed that IgM had a diagnosticsensitivity of 85.7%, diagnostic specificity of 92.8%, a positive predictive value of 85.7%, a negative predictive value of 92.8%,and accuracy of 90.4%. In comparison, IgG had a diagnostic sensitivity of 90.4%, diagnostic specificity of 90.4%, a positivepredictive value of 82.6%, a negative predictive value of 90.5%, and accuracy of 90.4%. In conclusion, IgG has a highersensitivity than IgM, while IgM had higher specificity, positive predictive value, and negative predictive value than IgG.However, the positive, negative predictive value and efficiency values were the same for IgM and IgG.
