Abstract

Introduction: Large artery atherosclerosis is the cause of ischemic stroke. Intracranial atherosclerosis (ICAS) is the main vascular lesion in patients with ischemic stroke in Asia, extracranial atherosclerosis (ECAS) also contributes to mortality or morbidity in 20% to 30% of the cases. Until now there are differences in the determinants of atherosclerosis in each country. Method: A retrospective cross-sectional study, the sample of ischemic stroke patients from 2017-2020, acute onset and post-acute period and had undergone digital subtraction angiography examination, found atherosclerosis and met the inclusion and exclusion criteria. Result: This study sample was 98 patients, consisting of 54 men and 44 women. The majority of ICAD was found in ischemic stroke patients 41%, combination (EICAS) 34%, and ECAS 26%, (p=0.633). In the circulation that had atherosclerosis, the majority was anterior circulation 62%, anterior-posterior 29%, and posterior circulation 9%, (p= less than 0.001). The age difference was more significant for cerebral artery circulation with stenosis (p=0.047). Based on the results of regression analysis, it was found that the age determination affected extracranial atherosclerosis in the posterior circulation by 1.68 times (p=0.030). Conclusion: Age is the determinant of extracranial blood vessel atherosclerosis in the posterior circulation in ischemic stroke patients at Prof. Dr. R.D Kandou Manado; meanwhile, the determinants of atherosclerosis of intracranial was inconsistent.

Abstract

Pandemi COVID-19 dan Produktifitas Pandemi COVID-19 telah berdampak merugikan masyarakat, tidak hanya kesakitan dan kematian tetapi juga pada berbagai bidang sosial dan ekonomi. Efek pandemi dengan berbagai situasi yang berhubungan seperti pembatasan kegiatan, pembatasan perdagangan, pembatasan pergerakan, dan penguncian daerah menyebabkan terbatasnya kegiatan berbagai sektor produksi. Pada bulletin Word Food Program COVID-19: Economic and Food Security Implications (4th Edition)1 merilis data tentang situasi Indonesia setelah 9 bulan mengalami masa pandemi. Dari data yang disampaikan didapatkan bahwa Product Domestic Bruto (PDB) Indonesia terus menyusut selama kuartal kedua berturut-turut karena pandemi, dengan tingkat inflasi tahunan meningkat menjadi 1,6% pada November 2020. Harapan optimis dengan gambaran secara berangsur adanya pertumbuhan positif di semua sektor ekonomi.

Abstract

Introduction: The implementation of distance learning due to the COVID-19 pandemic has impacted not only the students but also parents especially mothers. This study was aimed to provide an overview of mothers’ mental health status (stress and anxiety) in Jabodetabek and its association with mother’s education level and occupation status, number of children, and number of children doing distance learning during the COVID-19 pandemic.Method: This was a cross-sectional study using an online questionnaire targeted to married women who have greater than 1 children in elementary school-high school doing distance learning in Jabodetabek. The data was collected in the period of 2-20 December 2020. Instruments used to measure stress and anxiety levels were PSS-10 and GAD-7.Results: There were 311 respondents aged between 27-61 years old. The average respondents had moderate stress and mild anxiety. The risk of severe stress increased significantly in mothers with elementary-middle education level (crude RR 2,1; 95%CI 1,04-4,4) and increased risk of moderate-severe anxiety in mothers with greater than 3 children (crude RR 1,9; 95%CI 1,1-3,1).Conclusion: More than 50% of mothers with children in elementary school-high school in Jabodetabek had moderate stress and 31% showed signs of mild anxiety. The mother’s sociodemographic factor associated with the risk of severe stress was education level. The factor associated with moderate-severe anxiety risk was the number of children living under one roof.

Abstract

Introduction: There is no susceptibility data of E. coli and K. aeromobilis in Indonesia, even data regarding minimal inhibitory concentration (MIC)-based susceptibility of E. coli and K. aeromobilis towards single antibiotic or combination of fosfomycin (FOS) and sulbactam-cepoferazone (SUL-CPZ) is very scarce, even though the data is required by clinicians. Methods: A descriptive observational study was carried out at the Microbiology Clinical Laboratory of the Faculty of Medicine, Universitas Indonesia. Thirty strains each of clinical isolates of E. coli and K. aeromobilis were subjected to MIC determination against FOS and SUL-CPZ. For susceptibility criteria, we adopted the Eucast guideline. The synergism of the combined antibiotics was determined by checkerboard titration. One strain of E. coli and K. aeromobilis showing a synergistic and independent effect against the combined antibiotics was subjected to a time-kill assay. The post-antibiotic effect (PAE) was determined on a strain of E. coli showing synergism against the combined antibiotics. Results: The MIC level of all strains decreased when the bacteria were exposed to the combined antibiotics. Synergism was observed in 53.3% of E. coli and 56.8% of K. aeromobilis. No antagonism was observed. Higher bacterial death during the first four hours occurred with the isolate, showing synergism compared to the isolate showing an independent effect. The PAE of E. coli was longer when exposed to combined antibiotics. Conclusion: In vitro synergism of FOS and SUL-CPZ was observed in the majority of isolates and could be used as the basis for further research on empirical treatment

Abstract

Background. Ultrasound evaluation on the median nerve in carpal tunnel syndrome (CTS) patients by measuring cross-sectional area (CSA) is widely known. However, studies have shown that CSA of the median nerve could vary in different demographical factors. Therefore, the cut-off CSA value for CTS in the Indonesian population could be significantly different. Purpose. To compare mean CSA of the median nerve between normal and CTS populations and establish a cut-off point of CSA to diagnose CTS. Materials and Methods. A comparative cross-sectional study was conducted involving 40 people divided equally into CTS and normal groups. Normal and CTS subjects are determined by Electrodiagnostic study (EDx) using EMG and Nerve Conduction Study (NCS). The CSA measurement was performed at the level of the carpal tunnel inlet. Results. The mean CSA value at the level of tunnel inlet of the normal group was 8.3 ± 1.4 mm2 and for the CTS group is 15.4 ± 4.4 mm2. The receiver operating characteristics (ROC) analysis showed 10.6 mm2 as the cut-off point with 95% sensitivity and 95% specificity (P less than 0.001). Conclusion. The median nerve CSA at the tunnel inlet is significantly larger in the CTS population and the cut-off point can be recommended in detecting CTS in the Indonesian population.

Abstract

A febrile seizure is a seizure associated with an increase of body temperature of more than 38°C. This condition is not caused by the central nervous system (CNS) infection and electrolyte or metabolic imbalance. A febrile seizure can affect children from the age of 6 months to 6 years. A febrile seizure is the common cause of seizure in children with benign clinical course dan good prognosis which will disappear at six years old. The diagnosis is achieved based on clinical manifestation, and the clinician should exclude CNS infection, especially in complex febrile seizures. Based on the literature, the indication of supporting examination and therapy has changed significantly. Laboratory examination, electroencephalography, and imaging only indicated if the patient’s condition was in line with the guideline. Management of the febrile seizure consists of symptomatic and prophylaxis therapy. The recommendation of the febrile seizure treatment includes management during the seizure, intermittent prophylaxis therapy, and long-time maintenance therapy. The recommendation of the prophylaxis therapy was formed based on Indonesia’s condition. A febrile seizure is often the cause of a patient’s over-anxiety. Therefore, education about the good prognosis, clinical course, risk, and benefit of prophylaxis therapy is needed. General practitioners, pediatricians, and neurologic pediatricians are expected to implement the recommendation to minimalize the over-use of supporting examination and treatment.

Abstract

Introduction: Body fat percentage (%BF) can be easily and accurately predicted by measuring skinfold thickness. So that the distribution, relationship, and prediction with the hematoma volume of patients with intracerebral hemorrhagic (ICH) stroke are interesting to study.Method: An analytic cross-sectional study of subjects with ICH stroke at Prof. Dr. R.D Kandou General Hospital, Manado, Indonesia who met research criteria consecutively measured the thickness of body skinfolds then converted to %BF and estimated hematoma volume using the ellipse method was examined by linear regression analysis.Result: A total of 165 samples (87 men) with a mean age of 53 years. 65% sample had %BF greater than 25% (median 27.8%). greater than 50% men had %BF greater than 25%, 60% women had %BF greater than 30% (p less than 0.001). The median ICH stroke hematoma volume was 20.5mL (IQR 8.6−43.2mL). There is an increase in the volume of ICH stroke hematoma, especially in men with %BF greater than 30%. In this group, each increase in %BF would increase the volume of ICH stroke hematoma by 22.34mL (95% CI 5.15−39.52mL; p=0.011).Conclusion: There was a significant relationship between %BF assessed with skinfold thickness and the volume of ICH stroke hematoma in this study, volume increase was evident especially in men with %BF greater than 30%.

Abstract

Introduction: Several training techniques are used to improve the performance of taekwondo athletes, one of which is sprint running. Sprinting is one of the branches of athletic sports which requires strength, muscle power, agility and speed. Dollyo chagi kicks is most often used both in attack and defense in taekwondo competition. To be a profesional athlete, they must have sport components such as muscle strengh, muscle power, speed, flexibility, agility, and endurance. The winner of the taekwondo competition is determined by the number of points earned. The author choose the speed an power of dollyo chagi because it is reliable to get points taekwondo matches.Methods: This is a quasi-experimental study with one-group pre-test and post-test design. Using purposive sampling techniques, seventeen junior taekwondo athletes (9 males and 8 female) were recruited. The athletes were trained with 50-meter sprint run for 4 weeks with intensity of 3 sets and 4 times a week. Result: Speed of dollyo chagi kicks increase significantly 17.74% (p less than 0.05) and power of dollyo chagi kicks increase significantly 23.06% (p less than 0.05). Conclusion: 50m sprint training can increase the speed and power of dollyo chagi kicks in taekwondo athletes.

Abstract

Introduction: Sheehan syndrome may arise after giving birth, which is caused by reduced pituitary perfusion due to low blood pressure during or after parturition. It has varied and unspecific symptoms. Early diagnosis and treatment is important to prevent death. Case report: We reported a case of 39 years old female with recurring hyponatremia, myositis and secondary amenorrhea, which were consistent with sheehan syndrome.Discussion: Patient had history of severe postpartum bleeding. She presented with hyponatremia, hypokalemia, and hypokalemia. This electrolyte imbalance might be caused by anterior pituitary hormones deficit, possible posterior pituitary hormones deficit, disturbance of adrenocortical and estrogen hormone. Increased creatinine kinase showed myositis, and possibly caused by herbal medication or other condition.Conclusion: In this case, the patient was getting well after given adequate electrolyte and hormone replacement, without steroid administration.

Abstract

Introduction: Relationship between serum magnesium (Mg) level and short-term outcome of ischemic stroke (IS) has not been studied frequently and therefore interesting to investigate. Methods: A prospective cohort study of subjects with acute IS was conducted at Prof. Dr. R.D Kandou General Hospital, Manado, Indonesia between October 2019 and April 2020. Mg serum level of eligible subjects were examined and the National Institute of Health Stroke Scale (NIHSS) scores periodically assessed for three weeks. Relationship between serum Mg levels and all NIHSS measurements was assessed through bivariate analysis continued with linear regression. Results: There were 65 subjects (33 male) with a mean age of 57,9 years. The median onset of SI was 24 hours before hospital admission. Median systolic blood pressure was 150mmHg. The mean serum Mg level was 1.96mg/dl, with 8 subjects were hypomagnesemia. An increase in serum Mg increases baseline NIHSS score but not statistically significant (p=0,06). This value also correlates with the magnitude of changes in the initial and week III NIHSS scores (p=0,04). Conclusion: IS patients generally have normal serum Mg levels. Higher the serum Mg level, greater change between baseline NIHSS score and week III as of worsening the outcome.
