Abstract

Introduction: Pityriasis Lichenoides et Varioliformis Acute (PLEVA) is a rare and benign inflammatory skin disease. This acute form is distinguished from the milder chronic condition, Pityriasis Lichenoides Chronica (PLC). The etiology of PLEVA itself remains unclear, although it has been associated with infection as a trigger.Case: Here, we present the case of a 34-year-old woman who presented with abrupt, generalized, pruritic skin eruptions for two months.Case Discussion: The eruptions started as red scaly papules and vesicles and resolved as hyperpigmentations. She denied having any previous dermatological diseases, routine medications, or vaccinations. On examination, she showed generalized multiple hyperpigmented macules, scaly papules, and vesicles over the trunk, extremities, and face, with erosions and crusts in several areas. Eosinophilia and asymptomatic bacteria were found in her urinalysis. A skin biopsy showed diffuse lymphocytic infiltration in the dermal/epidermal junction and perivascular region, with parakeratosis and lymphocyte exocytosis. While the pathogenesis of PLEVA itself remains unclear, a triggering factor of inflammatory reaction, such as infection, may contribute to the development of the disease.Conclusion: Asymptomatic bacteriuria is associated with inflammatory responses, and thus, it can be one of many possibilities. More studies are needed to confirm this association with PLEVA. Due to the lack of a clear pathogenesis and the benign nature of the disease, this rare entity may present difficulties in diagnosis and therapy

Abstract

Introduction: Muscles are the largest tissue group in the body and can be classified into various classifications, one of which is based on their morphological shape. Examination of muscles, especially skeletal muscles, can use electromyography. The purpose of this study was to determine whether muscles that have similar morphology have no different electromyogram.Method: This is a descriptive observational study using needle electromyography. The sample amounted to 5 with five times MUP (Motor Unit Potential) in each musculus deltoideus and musculusResult: The mean amplitude, duration, number of voltage phases and the number of voltage turns off the musculus deltoideus and musculus gluteus medius in the study subjects were similar (p-value greater than 0.05).Conclusion: Muscles that have similar morphology in each parameter of amplitude, duration, number of voltage phases, and number of voltage turns do not have a significant difference in electromyogram.

Abstract

Introduction: Chlorhexidine is widely used in health centers as an antiseptic and disinfectant. Several cases of immediate and delayed hypersensitivity to chlorhexidine have been reported. Case report: We reported a case of a 26-year-old nurse working in Hospital X that came with a complaint of itch in several areas of the body that started four days before presenting at the hospital. The itch was continuously felt, and it started to form multiple large red bumps. Redness accompanied by dry skin was present between the fingers and palms. Cetirizine and dexamethasone did not relieve the symptoms. The patient has been working for six years in the inpatient unit and six months in the COVID-19 ward. Since the pandemic, she had been using alcohol-based hand rubs more frequently. The hospital uses chlorhexidine as an alcohol-based hand rub, which may cause hypersensitivity reactions to exposed health workers. The patient was finally diagnosed with acute urticaria. Based on the seven steps of occupational diagnosis of disease, it was still inconclusive whether the urticaria is an occupational disease because there was no data regarding the causal relationship between chlorhexidine exposure and the incidence of urticaria. Conclusion: Hence, it is necessary to carry out further diagnostic test with a puncture test. The hospital is still required to implement control measures toward chlorhexidine exposure.

Abstract

Introduction. COVID-19 patient not only experienced respiratory manifestation, but they also generate generalized muscle weakness. This manifestation results in deterioration of physical capacity. Methods. This cross-sectional study identified the characters of COVID-19 patient based on their age, gender, severity of disease and whether they have comorbidities or not. This study also described the functional capacity using a modified 30-Second Sit-to-Stand Test (m30STS) and chest expansion measured at three levels (axillary, nipple, and xiphisternum). Result. M30STS was one of reliable measure that used for physical or functional capacity. Chest wall expansion determines lung volume and functional capacity as well. Eighty-five subjects in isolation ward were enrolled. This study shows there is reduction of functional capacity in COVID-19 patients. Majority of subjects tend to reached lower number of repetitions in m30STS or about 10.37 times in average. Clonclusion. The thoracic expansion was reduced below normal range in all subjects.

Abstract

Introduction: The lack of experts in forensic medicine will become a serious problem globally in the future. In Indonesia, the unique position of forensic medicine at the intersection between medicine and law itself means that the role of forensic doctors can still expand widely. The future development of forensic medicine in Indonesia is determined mainly by forensic doctors and prospective forensic doctors themselves. Therefore, this study aims to determine the motivation of doctors in choosing the specialty of forensic medicine and their views on work as forensic doctors. Methods: This is a cross-sectional descriptive observational research using mixed methods, with forensic doctors and residents as respondents. Result: The results show that 17.8% chose forensic medicine because of their interests and 11.1% because they considered forensic medicine to be a challenging specialty. The absence of scholarships, the lack of institutional and family support, the lack of number and variety of cases in various educational centers, and the limited supporting facilities and financing of forensic medical services are the main obstacles for forensic doctors. Conclusion: Regeneration is needed to meet the need of forensic doctors in various regions in Indonesia by spreading information about the importance of forensic medicine. Good coordination between forensic experts, the Indonesian Forensic Doctors Association (PDFI), other professional organizations, and relevant stakeholders is needed to overcome various obstacles and ensure the growth of forensic medicine in Indonesia.

Abstract

Introduction: Cataract in workers need to be distinguished whether it is an occupational disease or not. Palm oil plantation workers are one of the high-risk jobs for potential hazards in the workplace that have an impact on health, one of which is cataract. Case report: A 38-year-old male oil palm plantation worker with complaints of blurred eyes since 1.5 years ago, sensitivity with lights and glare, and clouded. He was exposed ultraviolet (UV) rays 5 hours every day for 18 years without UV protective eyewear. To on examination revealed visual acuity in the right eye was 6/60 and the left eye was 6/9, both posterior subcapsular lenses opacities, vitreous papillae on the right were blurred.Discussion: Occupational cataract in oil palm plantation workers can be caused by prolonged exposure of UV rays, without wearing UV protective eyewear. The examination revealed decreased vision, posterior subcapsular lens opacities and blurred papillae. Determination of occupational cataract refers to the Seven Steps Occupational Diagnosis method issued by PERDOKI (Indonesian Association of Occupational Medicine Specialists).Conclusion: Occupational cataract can be diagnosed by the Seven Steps Occupational Diagnosis method and prevented by administrative controls and the use of UV protective eyewear.

Abstract

Tumor necrosis factor-α (TNF-α) merupakan sitokin pleiotropik penting pada patogenesis berbagai kelainan inflamasi dalam bidang dermatologi. Beberapa contoh penyakit tersebut adalah psoriasis, hidradenitis supuratif, dermatosis neutrofilik, sarkoidosis, granuloma anulare, toxic epidermal necrolysis, penyakit Behçet, pitiriasis rubra pilaris, skleroderma, sklerosis sistemik, dan eritema nodosum leprosum. Penggunaan anti TNF-α menjadi strategi terapi untuk menghambat progresivitas berbagai penyakit tersebut. Terdapat lima anti TNF-α yang telah diuji secara klinis, yaitu etanercept, adalimumab, certolizumab pegol, infliximab, dan golimumab. Masing-masing obat memiliki profil farmakologi, indikasi, serta efikasi yang berbeda untuk setiap penyakit. Penggunaan anti TNF-α juga berisiko menimbulkan berbagai efek samping, baik bersifat lokal maupun sistemik, termasuk secara paradoks meningkatkan risiko psoriasis.

Abstract

Introduction: Several studies have reported that in patients with Coronavirus Disease 2019 (COVID-19), in addition to manifesting abnormalities in the respiratory tract, it can also cause several neurological manifestations, including acute cerebrovascular disease. Acute cerebrovascular disease in COVID-19 is associated with worse manifestation and outcome; therefore, this study wanted to describe the characteristic of acute cerebrovascular disease in COVID-19 patients. The study aims to describe the characteristic of acute cerebrovascular disease in COVID-19 patients.Method: This study was a retrospective observational single-centre study by reviewing the electronic or physical medical records of confirmed COVID-19 patients admitted to Mardi Rahayu Hospital, Kudus from 1 March 2020 to 31 March 2021. The extracted data would be analyzed using the χ2 dan Kolmogorov-Smirnov test for categorical variables and the Mann-Whitney test for numerical variables.Result: From 1053 cases of confirmed COVID-19 that were studied, there were 50(4.7%) cases of acute cerebrovascular disease. Acute cerebrovascular disease patients had older age (57.5 (34-78) years vs 52 (1-88) years, p0.02), were more likely to have hypertension (39(78%) vs 357(35.5%), p less than 0.01), diabetes 31 (62%) vs 383 (38.2%), p=0.01), and prior cerebrovascular disease (11(22%) vs 8(0.8%), p=0.028), had more severe clinical manifestation (32(64%) vs 379(37.8%), p less than 0.01), and had worse outcome (22(44%) vs 218(21.7%), p=0.01). Conclusion: We found that cerebrovascular disease in COVID-19 patients had worse outcomes and clinical manifestation; therefore, management of these cases is better done more cautiously.

Abstract

Introduction: The off pump coronary artery bypass surgery (OPCAB) technique is expected to be able to eliminate the side effects and complications caused by the use of a cardiopulmonary bypass (CPB) machine. However, this technique is not completely free from side effects or complications. One of the effects that can be caused is an increase in the systemic inflammatory response, so that the duration of hospitalization becomes longer. This is aim of this study to conduct a study of the administration of dexamethasone for reduced the length of hospital stay in patients undergoing OPCAB surgery. Methods: This was an analytic observational study with sixty patients who underwent OPCAB surgery and were randomized to either dexamethasone or placebo groups of 30 people each. Clinical results were analyzed. Results: The length of stay in the dexamethasone group compared to the placebo group was consecutive (5 (5-8) days vs 6.5 (5-30) days; p=0.04) so that there was a significant difference. Other clinical outcomes in the dexamethasone group were better than the placebo group, in terms of duration of mechanical ventilation (p=0.03) and intensive care unit stay (p=0.03). There were significant differences in inflammatory markers between the two groups: interleukin-6 (p=0.0001), procalcitonin (p=0.0001), and C-reactive protein (p=0.0001) were lower in the dexamethasone group. Conclusion: Preoperative dexamethasone in patients undergoing off-pump coronary artery bypass is effective in reducing the length of hospital stay and controlling postoperative inflammatory reactions.

Abstract

Introduction : Cervical cancer is most common cancer in women in Asia Oceania and worldwide. VIA(Visual Inspection after Acetic acid) is being used as an alternative in low- and middle-income countries due to the minimal equipment required. Data from recent systematic reviews and multicentre trials show that VIA to be moderately sensitive for precancerous lesions, as compared with Pap testing. This study aim to determine the incidence of positive VIA, LBC, HPV DNA testing and colposcopy findings obtained from positive VIA cases in Dr. Cipto Mangunkusumo General Hospital.Methods: This was a cross-sectional study. Data was collected from cervical cancer screening by VIA examination at Dr. Cipto Mangunkusumo General Hospital, Jakarta, Indonesia in 2017. Age, VIA results were recorded. The results showed abnormal and predictive cancer lesions (acetowhite lesion) were defined as a “positive test.” The positive VIA result followed by LBC, HPV DNA testing, and Colposcopy. Only 25 performed LBC and HPV DNA test, and 30 of the VIA positive were performed Colposcopy. Results: A total of 1.960 subjects were recruited for this study. The mean age of the subjects was 34.5 years, and 908 (46 %) subjects belong to the 30-40 years age group. Negative VIA result were found in 1893 subjects (96,58%) and 67 subjects (3,42%) had positive results. From 25 VIA positive subject whose gone through LBC and HPV test, 9 subjects (36%) had abnormal LBC, 3 subjects (12%) had positive HPV DNA test. From 30 positive VIA whose got colposcopy examination 17 subjects (56,6 %) had abnormal colposcopy.Conclusions: The number of VIA screening participants in RSCM, Jakarta, Indonesia with positive VIA results is low. Further studies regarding the validation of VIA are required to evaluate its diagnostic value.
