Abstract

Constipation has a negative impact, both for the child and their family, because it can reduce the quality of life, as well as impact on health care costs. The diagnosis of functional constipation is based on Rome IV criteria. The prevalence of functional constipation is related to geography, dietary habits and types, and exposure to stressful life events for children. Although most constipation in children is functional, an organic disorder must be excluded by looking for any alarm signs.

Abstract

Introduction: Gait analysis is the study about how to move. However, the normal gait reference which is currently used is the normal gait of Europeans. The body proportions, leg length, and height between Europeans and Indonesians are different.Methods: The samples were 44 Indonesian aged 18 – 26 years which was divided into 22 males and 22 females. Samples used legging and socks that had been marked with a color at a certain point. The research sample walked on a treadmill at low, medium and high speeds and recorded from the back and sides. Based on the video, the angles of the hip joint, knee join was assessed.Results: In male subjects, the joint angles of low, medium and high speed walking: maximal thigh joint (21.30o, 21.77o, 22.67o), minimal thigh joint (-7.05o, -7.74o, 8.66o), maximal knee joint (55.25o, 57.00o, 60.04o), minimal knee joints (9.63o, 10.13o, 10.50o). In female subjects, the joint angles of low, medium and high speed walking: maximal thigh joint (24.02o, 24.43o, 24.99o), minimum thigh joint (-3.25o, -3.86o, -4.22o), maximal knee joint (53.60o, 56.80o, 58.99o), minimal knee joints (11.12o, 12.94o, 12.83o).Conclusion: The faster walking speed, the greater angle of thigh joint and knee joint

Abstract

Introduction: Anosmia is one of the clinical characteristics of COVID-19 patients. Anosmia was reported as the first symptom before other symptoms were discovered. Prior studies have shown anosmia implications as a favorable prognostic factor in COVID-19 patients. The research aim is to describe the characteristics of COVID-19 patients with anosmia and its correlation with disease severity.Methods: This study was a descriptive, cross-sectional, retrospective study with a total sample of 149 patients confirmed COVID-19. This research was conducted on hospitalized patients from March 1 to July 31, 2020, at the Primasatya Husada Citra Hospital Surabaya. Our study classified patients into two groups, with and without anosmia. The patient’s clinical characteristics were described in a table and odds ratio was calculated to determine the risk factors incidence of severe degree in COVID-19 patients.Results: The mean age of patients with anosmia was 32 years old, the patient’s gender was dominated with the female (63.2%). Patients with anosmia had lower severity, the odds ratio of having severe degree is 0.479. Patients with anosmia also had other symptoms such as cough, rhinorrhea, sore throat, tightness, headache, and muscle aches (p less than 0.05).Conclusion: COVID-19 patients with anosmia had lower severity than patients without anosmia.

Abstract

Snakebite is declared as neglected tropical disease by WHO in 2009. Limited report and collection data about the exact number of snakebite national incidents shows that this case lacking of attention. Moreover, control program, management, also proper treatment have not been understood by public and health workers. In fact, snakebite is a medical emergency that can lead to permanent disability, limb amputation, and even death. For this reason, effort are needed to reduce morbidity and mortality that arise through preventive, curative, including comprehensive understanding of related proper, safe, and effective case management.

Abstract

Background: Ischemic stroke in Indonesia is one of the main causes of disability and mortality in elderly. Aging is related with the increase of comorbidities. The comorbidities in elderly increase the potential for polypharmacy prescribing which increases the incidence of drug side effects. This study’s purpose is to know the demographic characteristic, treatment pattern, and side effect of the treatment in elderly patients with ischemic stroke. Methods: This study is a descriptive study with a cross-sectional design which was conducted throughout September–October 2020. This study used secondary data from ischemic stroke patients’ medical records admitted in Dr. Hasan Sadikin Central General Hospital’s in 2015–2019, with simple random sampling.Result: Result of this study shows that most elderly patients with ischemic stroke were young elderly aged 60-69 years, male, with hypertension as the major risk factor. Polytherapy (2-4 drugs) was found in 79 patients and polypharmacy (greater than 5 drugs) was found in 21 patients. Six patients with polytherapy and four patients with polypharmacy experienced drug side effects. Drug side effects of treatment observed were stress ulcer, upper gastrointestinal bleeding, and anemia.Conclusions: The treatment patterns were dominated by polytherapy (2-4 drugs). The percentage of elderly patients with polypharmacy (greater than 5 drugs) experiencing side effects was more than those with polytherapy. Drug side effects were dominated by stress ulcer.

Abstract

Introduction: Displaced supracondylar fracture in children is a challenging injury that may result in impaired functional and cosmetic outcome if not well-treated. Utilization of Closed Reduction and Percutaneus Pinning (CRPP) increased for this pathology, some authors believe ORIF results better anatomical reduction and lower rate of loss of reduction. Study aims to compare CRPP and ORIF for pediatric supracondylar humerus fracture. Method: Systematic review was conducted based on PRISMA guideline. Inclusion criteria were age less than 18 years old, comparing CRPP and ORIF for Supracondylar Humerus Fractures Gartland Type II, II.Studies of one surgical technique, Gartland type I, case reports were excluded. For meta-analysis, 6 studies were included and fixed effect model used to pool the result. In each study, mean difference (MD) with 95% confidence interval (CI) was calculated for dichotomous outcomes using Review Manager. Result: Total of 252 patients aged 0-15 years old were included. CRPP more often performed than ORIF. Satisfactory outcomes measured by Flynn’s criteria were achieved in 87.74% in CRPP and 86.73% in ORIF patient group, indicating significant difference (Heterogeneity, I2 = 23%; WMD, 1.26; 0.58 to 2.73; P =0.56). Conclusion: Current systematic review and meta-analysis suggest that for displaced supracondylar humerus fractures, ORIF offers a comparable functional and cosmetic outcome compared to CRPP.

Abstract

Virus SARS-CoV-2 yang terbilang baru ini mencetuskan berbagai permasalahan. Ketidakpahaman masyarakat terhadap penyakit ini menjadi salah satu celah sumber permasalahan yang harus dihadapi dan segera diperbaiki. Ketiadaan pemahaman yang benar mengenai pasien COVID-19 dapat menimbulkan stigma sosial. Stigma sosial dalam konteks wabah adalah pemberian label, stereotip, diskriminasi, perlakuan terpisah, dan / atau penghilangan status karena seseorang/sekelompok orang dianggap berkaitan dengan suatu penyakit. Stigma sosial yang timbul pada pandemi COVID-19 memicu perilaku diskriminatif terhadap individu yang terjangkit maupun terhadap individu yang diduga pernah melakukan kontak dengan virus ini, termasuk tenaga Kesehatan. Hal ini dapat menghambat upaya pemutusan rantai penularan Covid-19.

Abstract

Introduction: One of the main health problems in Indonesia is respiratory disease. Respiratory disease can be caused by fungus, so-called lung mycosis. The fungi species that cause respiratory infections in humans range from Candida sp. to more dangerous species such as Aspergillus fumigatus. Culture methods using respiratory specimens, especially sputum, can be used to identify species of fungi that cause pulmonary mycosis. Aim this study was conducted to compare the culture results between conventional culture methods on sloped agar and high volume culture method on plate.Method: This study used a cross-sectional design in which the data of culture results were obtained from different culture methods.Result: High volume culture method grew more colonies including Aspergillus fumigatus than conventional culture method which grew no Aspergillus fumigatus. The correlation of data between culture methods and culture results were analyzed with McNemar test and it showed p=0.000.Conclusion: The relationship between the culture method and culture results has a significant correlation (p less than 0.05). Therefore, high volume culture was a better method to grow more fungal colonies including Aspergillus fumigatus than conventional culture methods.

Abstract

Background: Tuberculoma are tumor-like space occupying lesions dispersed among various sites. Unlike pulmonary tuberculosis (TB), diagnosis and treatment of tuberculoma have received little consideration, especially those affected during pregnancy.Case: A 22-years-old women with two gestations, one parity, one abortion, and one alive child was admitted due to several syncope episodes. It was found that she had cerebral tuberculoma and received appropriate medication. Following her discharge, she was pregnant and had another symptoms. Holistic and multidiscipline approach was provided to the patient.Discussion: Cerebral tuberculoma is one of the most serious clinical forms of TB. There are some interactions between pregnancy and cerebral tuberculoma. Diagnosis and management on biopsychosocial aspect of cerebral tuberculoma in pregnancy was needed for patients, couples, and families affected.Conclusion: Tuberculoma in pregnancy is one of the challenging problem in pregnancy. Limited resource of references combined with both the mother and baby condition. It is important that healthcare providers assess the patient’s need and provide comprehensive and holistic management.

Abstract

Introduction: There is an increasing attention on post stroke cognitive impairment. The post stroke cognitive impairment often diagnosed late. There were only few studies that measured the predictive factors for cognitive impairment post-acute ischemic stroke in Indonesia. The aim of this study is measure the predictive factors for the post stroke cognitive impairment in acute ischemic stroke.Method: This was a cohort study. The patients included were more than 18 years old, and diagnosed with acute ischemic stroke. The subjects were measured with Mini Mental State Examination (MMSE) and Clock Drawing Test (CDT) examination at day-30 at Bethesda Hospital Yogyakarta. Patients who had a history of cognitive impairment were excluded. The result of MSSE and CDT at day-30 were the outcome of this study. To determine the relationship between the independent variable and the dependent variable, it was analysed using Chi-square method followed by multivariate logistic regression. Result: A total of 140 subjects were included in the study with an average age of 62,8 years, 86 (61,4%) male and 54 (38,6%) female. Ninety-one subjects, which is 65% from total subjects, experienced post stroke cognitive impairment. The analysis of multivariate showed age greater than 70 years, education level ≤ 6 years, modified Rankin score greater than 3 at diagnosis, Barhtel Index (BI) score ≤ 4 at diagnosis, the number of multiple lesions and the location of lesion in the cortex was an independent predictor factor affecting cognitive impairment 30 days after acute ischemic stroke. Conclusion: Age greater than 70 years, education level ≤ 6 years, modified Rankin score greater than 3 at diagnosis, Barhtel Index score ≤ 4 at diagnosis, the number of multiple lesions and the location of lesion in the cortex were independent predictive factors affecting cognitive impairment 30 days after acute ischemic stroke.
