Abstract

Introduction: Nasopharyngeal swab followed by RT-PCR has become the gold standard in diagnosing COVID-19. This method is difficult to perform because the procedure is inconvenient for the patient. There is another diagnostic method using saliva as an RT-PCR specimen.Objective: To determine the diagnostic accuracy of saliva specimen RT-PCR in diagnosing COVID-19 compared to nasopharyngeal swab specimens as the gold standard.Methods: Literature searching was performed by searching literature through MEDLINE, EBSCO, and Scopus. The articles selected were diagnostic or cross-sectional studies containing sensitivity, specificity, prevalence data, and appropriate to our PICO and eligibility criteria. A critical appraisal was performed after obtaining the selected articles.Results: Six articles were critically appraised. All articles showed that the saliva sample used for RT-PCR in diagnosing COVID-19 had good diagnostic accuracy.Conclusion: Saliva specimen to diagnose COVID-19 can replace nasopharyngeal swab specimen because it has comparable diagnostic accuracy with the gold standard. It is also a non invasive method which is more convenient for patients.

Abstract

Oklusi vena retina atau Retinal Vein Occlusion (RVO) adalah sumbatan yang terjadi pada vena retina sehingga mengakibatkan gangguan perdarahan di dalam bola mata. Oklusi vena dapat terjadi pada vena retina cabang atau Branch Retinal Vein Occlusion (BRVO) maupun vena retina sentral atau Central Retinal Vein Occlusion (CRVO). Penyakit ini berpotensi untuk menimbulkan kebutaan.

Abstract

Introduction: There has been a decrease in access to antiretroviral (ARV) services due to concerns of PLHIV infected with COVID-19, recommendations for quarantine by the government and health facility functions that are focused on dealing with the COVID-19 emergency. This study aims to determine the relationship between social factors of people living with HIV/AIDS and access to ARV services during the COVID-19 pandemic in Kediri Regency/City. Method: This study used a cross-sectional design with 107 respondents. Variables of social factors are relationship status, social support, residence status and peer community participation. The variable of access to ARV services is the act of visiting ARV services independently during a pandemic. Data were collected used snowball sampling techniques by online questionnaires and analyzed by chi-square test. Result: The majority of the respondents were 26-35 years old (57.9%), male (56.1%), and heterosexual (74.8%). Variables that had a significant relationship with access to ARV services included relationship status (p less than 0.05), social support (p less than 0.05), and residence status (p less than 0.05). Meanwhile, no significant relationship was found between the peer community participation variable and access to ARV services (p = 0.69). Conclusion: There was a relationship between social factors including relationship status, social support, and residence status of people living with HIV/AIDS with access to ARV services during the pandemic. It is necessary to engage and strengthen the social role of the community, especially families, by providing training and education related to understanding about HIV/AIDS, the needs and motivations of PLWHA

Abstract

Introductions: Acute appendicitis is an inflammation that occurs in the Vermiformis appendix. Inflammatory response to acute appendicitis is characterized by the activation of cellular and molecular effectors, including recruitment and activation of neutrophils. The Neutrophil Lymphocytes Ratio (NLR) is a comparison of neutrophil counts with lymphocytes used as a simple benchmark. Methods: we analysed with observational diagnostic test analytical design of retrospective cross sectional studies. The study subjects were acute appendicitis patients at Zainoel Abidin General Hospital in 2018-2019. Subjects upon arrival performed a blood test, and then calculated NLR, the result of NLR count compared to post-appendectomy anatomical pathology examination. The data is analyzed with descriptive statistical analysis, ROC curve analysis and diagnostic tests.Results: There were 176 samples was came to emergency room with diagnosed appendicitis, the most age was 17-25 years (31.2%), 81 male (46%), 95 female (54%). From patological anatomy result were 129 acute appendicitis and 47 chronic appendicitis. From the AUC ROC 0.96 obtained cut off point NLR greater than 3,209 in acute appendicitis. Diagnostic tests obtained sensitivity 83,72% specificity 82,98%, positive guess value 97,71% and negative guess value 65,00% with 83,52% accuracy.Conclusion: we concluded NLR may predict diagnosis of acute appendicitis with NLR greater than 3,209, sensitivity scores of 83,72% specificity 82,98%.

Abstract

Introduction: Tamoxifen (selective estrogen receptor) is the standard hormonal therapy in women with breast cancer with positive estrogen receptor. The use of this regimen proved to reduce the incidence of relapse at local, collateral, or distant body area, and also decrease breast cancer mortality. One of the effects of tamoxifen is to induce endometrial proliferation, cause various abnormalities in endometrial tissue.Method: This study is an unpaired categorical comparative analytic study with a retrospective single cohort (internal comparison group) design. All breast cancer patients at dr. Zainoel Abidin Banda Aceh from 2017 - 2019 was involved in this research. Evaluation of the incidence of endometriosis was carried out based on the duration of tamoxifen using the Mann-Whitney U test. This study is an unpaired categorical comparative analytic study with a retrospective single cohort (internal comparison group) design. All breast cancer patients at dr. Zainoel Abidin Banda Aceh from 2017 - 2019 was involved in this research. Evaluation of the incidence of endometriosis was carried out based on the duration of tamoxifen using the Mann-Whitney U test.Result: A total of 114 patients (46.15%) out of a total of 247 breast cancer patients using tamoxifen reported experiencing endometriosis with a mean age of 49 years old. Most of the patients diagnosed with stage IIIB breast cancer of ductal cell carcinoma. A total of 111 people (97.37%) experienced endometriosis after taking tamoxifen for 30-36 months and the remaining 3 (2.63%) after 24-29 months. The mean duration of tamoxifen use associated with endometriosis was 34 months (p less than 0.001).Conclusion: Long-term use of tamoxifen in breast cancer patients increases the risk of endometriosis

Abstract

Introduction: Developmental regression is always an alarming symptom in children as it is an early sign of some genetic disorders, one of which is neuronal ceroid lipofuscinosis (NCL). NCL is a group of rare neurodegenerative disorder caused by accumulation of intracellular ceroid lipofuscin. Since 2017 an enzyme replacement therapy (ERT) has been approved by Food and Drug Administration (FDA) for this disease. The symptoms of NCL could be managed by ERT if detected early, and the child could live normally.Case: We present a case of a 6-year-and-5-month-old boy with developmental regression, speech delay, recurrent seizure, and visual impairment, who was diagnosed with NCL type 2 after genetic testing. Compound heterozygous mutations in tripeptidyl-peptidase 1 (TPP1) gene was revealed, consistent with very low level of TPP1 enzyme in this patient.Discussion: NCL is a fatal disease which is often misdiagnosed in early stage. Diagnostic delay of NCL often occurs due to lack of awareness which often leads to premature death.Conclusion: Knowledge regarding the disease is important for early detection and to slow down the disease progression.

Abstract

Pendahuluan: Trombosis vena dalam (TVD) merupakan salah satu penyebab kematian terbesar di dunia. TVD memiliki faktor risiko yang bermacam-macam sehingga presentasi pasien TVD dapat berbeda pada tiap kelompok usia, jenis kelamin, dan ras. Penelitian ini bertujuan untuk memperoleh informasi mengenai karakteristik pasien thrombosis vena dalam.Metode: Penelitian ini menggunakan studi pendekatan kualitatif yang merupakan studi pustaka yang menggunakan buku-buku dan literatur-literatur lainnya sebagai objek utama. Penelitian ini menggunakan mesin pencarian, seperti Pubmed, Google Scholar, dan Clinical Key untuk mendapatkan jurnal-jurnal yang berkaitan dengan karakteristik pasien trombosis vena dalam.Hasil: Dari 17 literatur, ditemukan bahwa karakteristik pasien TVD berbeda pada tiap kelompok usia, jenis kelamin, dan ras. Semakin tua usia pasien, derajat keparahan TVD meningkat karena adanya kondisi lain yang biasanya muncul di usia lanjut. Laki-laki lebih rentan mengalami TVD dari perempuan tanpa faktor risiko reproduktif, seperti kehamilan dan menopause. Orang Afrika memiliki presentasi TVD lebih parah dibandingkan dengan ras lain. Risiko paling rendah ditemukan pada orang Asia, meskipun tidak ada perbedaan mortalitas yang signifikan antar ras.Kesimpulan: Karakteristik pasien TVD (angka kejadian, faktor risiko, lokasi, dan tingkat keparahan) bervariasi pada tiap kelompok usia, jenis kelamin, dan ras.

Abstract

Introduction: Coronavirus disease 2019 (COVID-19) is an infectious respiratory disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV 2). The Indonesian government and the World Health Organization (WHO) have made some efforts in the fight against the spread of COVID-19, one of which is by implementing respiratory hygiene. Respiratory hygiene, as an on-pharmaceutical intervention (NPIs), is one of the prevention measures designed to limit the transmission of respiratory infection spread by airborne and droplet routes. Method: This study used a descriptive research method with a simple random sampling technique. One hundred six people from ten villages in Baiturrahman Sub-district were selected as the subject of the study. The data were collected using an online questionnaire (google form). The obtained data were analyzed through univariate analysis. Result: The results revealed that only 4 respondents (3,8%) had a high level of respiratory hygiene compliance, 57 respondents (53,8%) were fairly compliant with respiratory hygiene, and 45 respondents (42,5%) had a very low respiratory hygiene compliance level. The study also found that 50% of respondents had used non-medical masks when doing activities outside the home, and 8.5% had applied physical distancing. Conclusion: The compliance level of good respiratory hygiene in the Baiturrahman Sub-District in Banda Aceh is still low, and the most common COVID-19 precautions the community takes are the use of non-medical masks when doing activities outside the home.

Abstract

Background: Recurrent Clostridium difficile infection (RCDI) continues to increase in incidence and severity around the world. Fecal microbiota transplantation (FMT) is commonly used to treat RCDI in adults, but its efficacy and safety in the pediatric population are infrequently studied. Here, we performed a systematic review to evaluate the efficacy and safety of FMT for RCDI in children.Methods: Systematic literature search was performed using PubMed, Science Direct, Nature, Cochrane Library, and Springer to identify original studies in English language published from 2010-2020. The clinical resolution, bacterial resolution, and failure of therapy were calculated as indicators of efficacy of FMT. Adverse events were also calculated as indicators of FMT safety.Results: A total of 21 studies, with 498 pediatric patients were included in this review. There were 418/498 (84%) patients reported clinical and bacterial resolution. There were 67/498 (13%) patients who experienced FMT-related adverse events, with 97% of them had mild adverse event. There was one patient who reported FMT-related serious adverse events which were aspiration pneumonia and dehydration.Conclusion: We found evidence that supports the efficacy and safety of the use of FMT for treatment of recurrent C. difficile infection in children.
