Abstract

Introduction: During the COVID-19 pandemic, where the disease might spread in a medical facility setting, the common problems found in every country is the shortage of personal protective equipment (PPE) for medical personnel – especially the disposable N95 respirators. Thus, a higher amplitude to disinfect and reuse N95 is urgently needed. In this study, we designed an effective and safe disinfection methods through an Ultraviolet Germicidal Irradiation (UVGI) chamber in Dr. Cipto Mangunkusumo Hospital to control the shortage of PPE by disinfecting and reusing disposable N95 respirators. Purpose: To evaluate the dosage and effectiveness of UV-C radiation for disposable N95 respirators disinfection in our designated UVGI chamber. Methods: This study used a cross-sectional design to determine the dose and the effectiveness of UV-C radiation in eradicating SARS-CoV 2 on disposable N95 respirators. Results: Using two different distances from the light source, we confirmed the inverse square law of UV-C radiation power. Irradiation for 2 hours with a radiation dose of 1080 mJ/cm2 resulted in undetected SARS-CoV-2 gene based on PCR examination in 10 out of 10 samples. Conclusion: This UVGI chamber is a potential solution for hospitals or medical facilities to overcome the limitations that occurred in the pandemic by disinfecting PPE.

Abstract

Introduction: Ferritin is a routine parameter used in the evaluation of iron overload in thalassemia patients. Excess iron causes the formation of ROS, resulting in an oxidative reaction that triggers inflammation. This study aims to determine the correlation of ferritin levels with proinflammatory cytokines in beta thalassemia. Methods: A cross-sectional analytical study was conducted on 33 beta thalassemia patients who received routine blood transfusions at I.A. Moeis Samarinda. Spearman rank correlation test was used to determine the correlation between ferritin and TNF-α and IL-6. Results: The results of this study showed that there was no relationship between ferritin and TNF- (p=0.580) and IL-6 (p=0.924). Meanwhile, there was a significant relationship between TNF- and IL-6 (p less than 0.0001) which had a strong positive correlation (r=0.621). Conclusion: This study showed that there was no correlation between ferritin levels with TNF-α and IL-6. Meanwhile, proinflammatory cytokines showed a correlation between TNF-α and IL-6 in thalassemia patients in Samarinda City.

Abstract

Introduction: Chronic kidney disease (CKD) is a global health problem with consistently increasing prevalence and incidence, poor prognosis, and high cost. CKD ranks third in funding in Indonesia, reaching IDR 2.3 trillion/year in 2018. Integrated Guidance Post (Posbindu) program affiliated to PHC (Puskesmas) with community cadres has been launched as a community empowerment effort to suppress CKD incidence. This study aimed to strengthen the capacity of Posbindu cadres as the health driving force about CKD to encourage public awareness of CKD risk factors. Method: This quasi-experimental study with one group pretest-posttest design compared four education models for Posbindu cadres in four PHC in Yogyakarta to improve their knowledge of CKD. All education models used the smart module with models 3 and 4 explicitly involving health-worker collaboration. Model 1 used posters, model 2 used leaflets, while model 4 also used posters and leaflets. Knowledge of CKD was assessed through a previously-validated questionnaire with four knowledge domains. Result: All of the 86 cadres were women aged 43.7±8.6 years, mostly with at least high-school education and no risk factors for CKD predisposition. The results showed that model 4 significantly improved knowledge of CKD risk factors, symptoms and examination, and prevention behavior. Meanwhile, improvement in the last domain about CKD treatment was likely affected by model 3. Conclusion: This study confirmed health-worker collaboration’s significance in increasing public knowledge of CKD. Adding various printed education media effectively increased knowledge of CKD, particularly in cadres with higher educational attainment.

Abstract

Laringectomee may have problems on their lung compliance as a result of the diaphragm’s increasing resistance. These can cause an increase on CO2 end-tidal. Non-invasive ventilator (NIV) is theorized to improve lung compliance with the end result on CO2 end-tidal correction. This case series was made to report the effect of NIV on several lung function parameters. Three cases of laringectomees were reported. Each history, present complain was noted. Diaphragm movement at inspiration and expiration were measured using ultrasonography. Peak huff flow and CO2 end tidal were also measure before and after NIV. All patients showed improvement on diaphragm movement and CO2 end tidal. One patient showed a decrease on his peak huff flow measurement. Clinically NIV showed a good result on diaphragm compliance and CO2 end-tidal. Effect of NIV on peak huff flow was not consistent in this case series. A clinical trial with sufficient numbers of patients and a proper method is needed to give evidence whether NIV can provide significant positive effect for laringectomees.

Abstract

Breast cancer is the most common cancer among women in Indonesia. The cure rate can be increased by early detection and appropriate therapy. The early detection modality recommended by WHO is screening mammography. The aim of this study was to determine the role and advantages and disadvantages of mammography compared to other modalities through narrative literature review with 30 literature that met the inclusion and exclusion criteria. The advantages of screening mammography are reducing mortality, increasing early treatment, and improving the quality of diagnostics as a follow-up to the results of screening. The disadvantages of this modality are overdiagnosis, false alarm, interval cancer, and radiation risk. Screening mammography can be combined with other modalities to increase accuracy such as MRI in high-risk women and ultrasound in women with denser breasts. The recommendation to initiate routine screening mammography at one year is 40 years. Barriers that often occur in lower middle-income countries are lack of knowledge and awareness, unavailable infrastructure and resources, accessibility, inadequate costs and the persistence of fear and shame. Therefore, the benefits of screening outweigh the risks and can be combined with other modalities to increase accuracy.

Abstract

Introduction: Malaria is an infectious disease that continues to be a public health issue worldwide, including in Indonesia. This study aims to detect the presence of Plasmodium parasites among immigrants from malaria endemic areas. Method: The research was conducted from September until November 2019 in three regency/cities in South Sulawesi, namely Makassar City, Tana Toraja and North Toraja. The population in this study were immigrants from malaria endemic areas (native Papua and non-Papua) who lived in the area for at least 2 years before visiting the cities of Makassar, Tana Toraja, or North Toraja. All samples were examined for malaria parasites using Polymerase Chain Reaction (PCR). Results: The results of the PCR test on 256 samples of migrants (native Papuan and non-Papuan) detected 19.53% positive for malaria. This research showed that a high prevalence of malaria parasites was found among asymptomatic immigrants, both native Papuans and non-Papuans in South Sulawesi. Conclusion: It can be concluded that the asymptomatic immigrants from endemic areas such as Papua need to be screened early because they may still have malaria parasites in their blood and become carriers, leading potential to cause local transmission.
